MR imaging in children with ectopic pituitary gland and anterior hypopituitarism.

Posterior pituitary ectopia refers to an absent normal posterior pituitary bright spot within the sella with ectopic bright signal at another site (such as the median eminence) on a weighted magnetic resonance. We describe two children with idiopathic anterior hypopituitarism who showed an ectopic posterior pituitary and absent pituitary stalk on imaging. We emphasize the association of the absent pituitary stalk in ectopic pituitary gland and low growth hormone levels.

ACTH content of pituitary glands of arteriosclerotic breeder vs. non-arteriosclerotic virgin rats.

Acid extracts were made of the pituitary glands of non-arteriosclerotic male and female virgin rats and arteriosclerotic, male and female breeder rats. The ACTH content of these pituitary glands was determined by measuring the amount of corticosterone produced by the adrenal glands of healthy but hypophysectomized rats when challenged by the various ACTH acid extracts. The pituitary glands of the arteriosclerotic animals were significantly heavier than non-arteriosclerotic rats and contained decreased amounts of ACTH commensurate with the severity of the pituitary donor's arteriosclerosis. Similarly, arteriosclerotic donors had heavier adrenal glands but contained considerably less corticosterone than non-arteriosclerotic virgin rats. Male breeder rats developed microscopic aortic lesions involving intimal mesenchymal cell and ground substance alterations whereas the female breeder lesions progressed from intimal ground substance and connective tissue changes to medial elastolytic degeneration and eventual medial cartilaginous and osseous metaplasia. Histopathologic examination of the pituitary and adrenal glands demonstrated hyperplasia of basophil and other cytologic elements of the pituitary gland including colloid-filled cysts and lipid depletion of the zona glomerulosa as well as hemorrhage and thrombosis of the adrenal cortex of the arteriosclerotic breeder rats. The hypothesis is made that repeated breeding leads to abnormal hypothalamic regulation of pituitary gland synthesis and release of ACTH, which, in turn, contributes to abnormal adrenal glandular function conditioning the aortic wall toward abnormal changes in mesenchymal cell activity and metabolism of connective tissue ground substance.

Function of prolactin cells in the individual rat pituitary gland is location dependent.

Anterior pituitary glands from individual ovariectomized (ovx) or ovx-estrogen (E2) treated rats were sectioned into 1/8 cubes. Each section was incubated for four consecutive 15 min periods in order to measure the release of immunoreactive and bioactive prolactin (PRL); each individual section was then trypsinized into a single cell suspension for determination of PRL cell numbers in that section. Hormone release (ng PRL/1000 PRL cells) was not uniform throughout the gland; the consistency of the secretory patterns demonstrated that the amount of PRL release from the gland was location-dependent. Statistical analysis of the data showed that the most active cells were in the gland's left lobe, while the least active were in the right lobe. Within these lobes, the dorsal-caudal and ventral-rostral left lobe areas released the most hormone in vitro while those in the dorsal-rostral, dorsal-caudal and ventral-rostral right lobe areas were least active. Injection of ovx rats with E2 for 2 days altered these secretory patterns. This sectioning procedure should prove useful in future studies addressing issues of cell-cell interaction and geographic location as they relate to pituitary cell function.

Pituitary gland enlargement secondary to dural arteriovenous fistula in the cavernous sinus: appearance at MR imaging.

PURPOSE: To evaluate pituitary gland size in cases of dural arteriovenous (AV) fistula in the cavernous sinus and to correlate the size with the degree of AV fistula. MATERIALS AND METHODS: Magnetic resonance images in 21 patients with angiographically proved dural AV fistula of the cavernous sinus were retrospectively reviewed. In four patients, findings obtained before and after embolization therapy were compared. RESULTS: The 21 patients were divided into group 1 (severe AV fistula) and group 2 (moderate or mild AV fistula). The superior contour of the pituitary gland was convex or flat in group 1 and flat or concave in group 2. The mean height of the pituitary gland was 9.4 mm +/- 1.5 (standard deviation) in group 1 and 6.7 mm +/- 1.2 in group 2 (P = .0002). After embolization therapy, pituitary gland height decreased and signal intensity voids improved in the four patients and no AV fistula was observed in three. CONCLUSION: In cases of severe dural AV fistula in the cavernous sinus, the pituitary gland is enlarged, which should not be misdiagnosed as pathologic.

Aerobic glycolysis by the pituitary gland in vivo.

Arteriovenous differences of glucose, lactate, and pyruvate were measured across the pituitary glands of overnight-fasted female pigs to determine whether net uptake of glucose occurred, and if so, whether it was oxidized or converted to lactate. Arteriovenous differences were also measured across a portion of the cerebral cortex as a control. Cerebral cortex oxidized 84% of the glucose taken up, but this was not true in the pituitary gland, where glucose was almost completely converted to lactate. The arteriovenous difference across the pituitary gland was 0.61 mumol/ml for glucose while the venoarterial differences of lactate and pyruvate were 1.0 and 0.07 mumol/ml, respectively. The results indicated that little net oxidation of glucose either by the Krebs cycle or the pentose phosphate pathway occurs in the pituitary gland in vivo. Estimates of the amount of energy released from aerobic glycolysis indicate that, unless the requirements of the neurohypophysis are very different from those in the rest of brain, very little energy could be derived from the metabolism of glucose.

Granulated folliculo-stellate cells and growth hormone cells immunostained with anti-S 100 protein serum in the pituitary glands of the goat.

Goat pituitary glands were immunohistochemically studied with antisera for bovine S-100 protein, rat LH beta, FSH, TSH beta, prolactin, ovine GH, and porcine ACTH1-39 by use of the superimposition technique on adjacent sections. Folliculo-stellate (F-S) cells were divided into two categories on the basis of ultrastructural properties: One consisted of a mass of agranular cells in which the pseudolumina were equipped with microvilli and cilia. Elongate gap junctions were often observed among these cells. The other was a group of granulated cells with or without pseudolumina. In this group the gap junctions were shown to be disintegrated. The dense granules 150-250 nm in diameter began to accumulate in the cells. However, neither type of these F-S cells was immunostained for S-100 protein. On the other hand, numerous polygonal, elongate, irregular or stellate cells containing S-100 protein were distributed throughout the gland. Most of them were immunohistochemically identical with the GH cells laden with the secretory granules 250-450 nm in diameter, but some of them were identical to TSH and prolactin cells which immunostained faintly for S-100 protein. This appears to be the first demonstration of GH cells intensely immunostained for S-100 protein.

Transmitter mediated arginine vasopressin release from superfused hypothalamus and pituitary gland.

The study was designed to investigate the effect of various neurotransmitters on the hypothalamus and pituitary gland to determine the sites of their action. Superfused isolated rat hypothalami and pituitary glands demonstrated basal secretion of arginine vasopressin (AVP) and repeated response to stimulation thus showing the viability of the preparation. Acetylcholine and histamine stimulated the release of AVP at the hypothalamic and pituitary levels; dopamine and norepinephrine released AVP in a dose related manner only from the hypothalamus; angiotensin II released AVP in the same fashion only from the pituitary gland. AVP secretion stimulated by dopamine and norepinephrine may represent synaptic inputs which are localized at the hypothalamus and must be distinguished from the site of action at the pituitary gland of angiotensin II.

Purification of a growth factor for ovarian cells from bovine pituitary glands.

The purification of a growth factor from bovine pituitary glands that controls the division of an ovarian cell line maintained in tissue culture is reported. Extraction of bovine pituitary glands at pH 4.5 followed by ammonium sulfate precipitation, chromatography on carboxymethylcellulose, and gel filtration on Sephadex G-50 gives yields of ovarian growth factor of about 7 mg/kg of glands. Contamination by other pituitary hormones was not detected. The preparation obtained has been characterized with respect to biological activity, molecular weight, and amino-acid composition. The implication of this growth factor in the development of ovarian tumors is discussed.

Regional distribution of chromogranin B 420-493-like immunoreactivity in the pituitary gland and central nervous system of man, guinea-pig and rat.

The distribution of chromogranin B 420-493-like immunoreactivity was investigated in the pituitary gland and central nervous system of man, guinea-pig and rat, by using two different antibodies. In man, highest chromogranin B 420-493-like immunoreactivity concentrations were found in the pituitary gland, chromogranin B 420-493 (1-17)-like immunoreactivity 74.3 +/- 8.5 (mean +/- S.E.M. pmol/g wet wt tissue) and chromogranin B 420-493 (20-38)-like immunoreactivity 2017.0 +/- 142.3, followed by the hypothalamus and substantia nigra. In the spinal cord, the highest concentration was found in the sacral dorsal area. Chromogranin B 420-493-like immunoreactivity was detected in human cerebrospinal fluid, chromogranin B 420-493 (1-17)-like immunoreactivity 376.7 +/- 77.9 and chromogranin B 420-493 (20-38)-like immunoreactivity 1174.7 +/- 259.3 pmol/l cerebrospinal fluid. Chromogranin B 420-493-like immunoreactivity was also found in high concentrations in the guinea-pig and rat pituitary glands, but very low chromogranin B 420-493-like immunoreactivity levels were present in different parts of the brain of both species. Gel permeation chromatography and fast protein liquid chromatography analysis of eight different regions of human CNS showed multiple chromogranin B 420-493-like immunoreactivity peaks. The profile of pituitary extracts was different from those of other parts of the CNS and the cerebrospinal fluid. The profiles of guinea-pig and rat pituitary on gel chromatography and fast protein liquid chromatography were different again from those of human CNS. Examination of subcellular fractionation of whole rat brain showed highest concentrations of chromogranin B 420-493-like immunoreactivity in the synaptosome fractions. Chromogranin B 420-493-like immunoreactivity release was stimulated from rat pituitary cells by high potassium ion concentrations. These findings show that (1) chromogranin B 420-493-like immunoreactivity is widely distributed throughout the pituitary gland and CNS of three mammalian species, with the highest concentration in the pituitary gland, and it is also present in human cerebrospinal fluid, and (2) the processing of chromogranin B in the pituitary gland may be different from that in the other area of the CNS, so that it is possible chromogranin B or its fragments may play a neuroeffector role in the mammalian CNS.

Immunocytochemical evidence that luteinizing hormone (LH) and follicle stimulating hormone (FSH) are present in the same cell type in the rhesus monkey pituitary gland.

Antisera to oLHbeta subunits were used to identify the gonadotrophic cells in partes distalis and tuberalis of the rhesus monkey pituitary gland. The same cell type in pituitary glands obtained from male and female rhesus monkeys stained with both antisera. Immunocytochemical staining was observed in PAS positive cells that were randomly dispersed throughout the entire pars distalis. The gonadotrophs did not react with antisera to either hTSHbeta or oACTH. These observations indicate that only one gonadotrophic cell type is present in the primate pituitary gland.

Receptors and sites of synthesis and storage of gamma-aminobutyric acid in human pituitary glands and in growth hormone adenomas.

gamma-Aminobutyric acid (GABA) is an important regulatory factor of pituitary gland function, which in addition to hypothalamic neurons, can be derived from intrapituitary sources, ie, growth hormone (GH) cells of rat and monkey. We report that human pituitary glands also express 2 isoforms of the GABA-synthesizing enzyme glutamate decarboxylase (GAD 65; GAD 67), the vesicular GABA transporter (VGAT), and multiple subunits of GABA (A, B, and C) receptors. GABA production and storage occurs in GH cells, as demonstrated by cellular colocalization of immunoreactive GAD and VGAT in GH cells and by reverse transcription-polymerase chain reaction analysis of laser capture-microdissected immunostained GH cells. It is interesting that human pituitary GH adenomas share expression of VGAT and GABA receptors with normal pituitary glands but lack GAD 65. We propose that GABA, synthesized by GH cells, might act as a paracrine or autocrine regulating factor in the human pituitary gland and in human GH adenoma. Because many drugs interfere with GABA function, the identification of GABA system components might have clinical implications.

Neurotensin in the rat anterior pituitary gland: effects of endocrinological manipulations.

Neurotensin-like immunoreactivity was detected by radioimmunoassay in the anterior lobe of the pituitary gland of several mammalian species, including man. In the rat, pituitary stalk transection did not change the content of neurotensin-like immunoreactivity. Surgical and chemical thyroidectomy produced a drastic reduction of neurotensin-like immunoreactivity in the anterior pituitary gland, whereas adrenalectomy and gonadectomy were without an effect and the decrease consequent to thyroidectomy was restricted to the pituitary gland. These results suggest an interaction between neurotensin and the hypothalamus/pituitary/thyroid axis at the level of the anterior pituitary gland.

Electrophoretic separation of prolactin secreted from mouse pituitary glands in vitro.

Prolactin secreted from the mouse pituitary gland in organ culture was characterized after disc or SDS-polyacrylamide gel electrophoresis. Some pituitary glands were cultured with 3H-labelled leucine for 24 h to obtain radioactive prolactin. In disc electrophoresis, immunoreactive, receptor-bindable and 3H-incorporated prolactins formed a single band with the same relative mobility (0.5). Prolactin migrating at Rf = 0.5 was extracted and re-analysed by SDS electrophoresis. A single stained and radioactive band was observed at the same position with an apparent molecular weight of 23 000 regardless of denaturing in the presence of absence of dithiothreitol. No other band was detected. These results indicate that mouse prolactin synthesized and secreted in organ culture is homogeneous and that mouse prolactin is a single-chain molecule.

[High uptake on 11C methionine PET scan in the pituitary gland of a patient with cerebral glioma after surgical abortion].

Positron emission tomography (PET) with various tracers provides physiologic and biochemical information of living organs. Since radiologic examinations are usually avoided in pregnant women, mainly because of the radiation risk to the fetus, little is known about the effect of pregnancy on cerebral blood flow and metabolism. This paper reports findings of a 11C methionine PET scan of the pituitary gland in a woman after an abortion. The patient was a 31-year-old woman who suffered a seizure in the 9th week of her second pregnancy. On admission, computed tomography showed an abnormal mass lesion in the right frontal lobe, and a brain tumor was suspected. The patient and her family asked that that pregnancy be terminated. Seven days after a surgical abortion, methionine PET was performed. The scan showed high methionine uptake in the pituitary gland as well as in the right frontal lobe tumor. We suspected that another tumor was present in the pituitary gland. The right frontal tumor was partially resected, and pathologic examination of the resected specimen showed an astrocytoma (grade 2). After the operation, the patient received 50 Gy irradiation and chemotherapy. Two months after the operation, we performed a second methionine PET scan, which showed high uptake in the residual right frontal tumor but not in the pituitary gland. Results of other radiologic studies of the pituitary gland were normal. These findings suggest that the transport of 11C methionine into the pituitary gland may increase during pregnancy. Moreover, the pituitary gland of pregnancy should be a part of the differential diagnosis of pituitary adenomas in PET scanning. The change in physiologic uptake by the female pituitary gland should be taken into account in the diagnosis of pituitary adenoma with methionine PET.

Sequence of enhancement of various portions of the pituitary gland on gadolinium-enhanced MR images: correlation with regional blood supply.

In an effort to study the blood supply of the pituitary gland, we determined the sequence in which various portions of the gland enhanced on MR images obtained after administration of gadopentetate dimeglumine in seven healthy volunteers. The pituitary gland has a unique blood supply: the pars nervosa is supplied by the inferior hypophyseal arteries; the infundibular stem is supplied by the superior hypophyseal arteries, which are more distal than the inferior hypophyseal arteries; and the pars distalis is supplied indirectly by portal systems from the infundibular stem. Therefore, the expected sequence of contrast enhancement of the pituitary gland is: the pars nervosa first, the infundibular stem second, and the pars distalis last. However, it is difficult to evaluate the temporal sequence of contrast enhancement with "slow" conventional MR techniques, such as routine enhanced images acquired minutes after contrast administration. Accordingly, we used rapid enhanced gradient-echo sequences (14 sec/image) and the region-of-interest method to determine contrast enhancement for various portions of the gland. Enhancement-time curves were then plotted for each portion and compared to determine the sequence of enhancement. Our results show that the timing of contrast enhancement of various parts of the pituitary gland as demonstrated on MR imaging correlates closely with the expected perfusion sequence based on knowledge of regional vascular anatomy. Normal enhancement-time curves of different portions of the pituitary gland were established. This technique and baseline data can be used to evaluate patients with hypopituitarism caused by ischemia.

Galanin gene expression is increased in the anterior pituitary gland of the human growth hormone-releasing hormone transgenic mouse.

The peptide galanin is synthesized within and secreted from specific cells of the anterior pituitary gland. Previous studies showed that GH-releasing hormone (GHRH) stimulates galanin release from pituitary cells in vitro. In the present study we used human (h) GHRH transgenic mice to examine the effects of high circulating levels of GHRH on pituitary galanin gene expression in vivo. Moreover, the hGHRH transgenic mice develop pituitary tumors and, thus, may be used as a model of estrogen-independent pituitary adenoma formation. We examined male hGHRH transgenic mice and nontransgenic siblings at 2, 4, 6, 8, and 10 months of age. Transgenic mice were identified using the polymerase chain reaction. Body weights and plasma mGH levels were higher in transgenic mice at all ages. Total protein contents in the anterior pituitary glands of transgenic mice were significantly greater at each age. Galanin peptide contents in the anterior pituitary gland of hGHRH mice were normalized for differences in total protein content and were significantly elevated at all ages examined. At 10 months of age, anterior pituitary galanin peptide concentrations were increased 7-fold. Hypothalamic concentrations of galanin peptide were also increased in hGHRH transgenic mice, but were not greater than those in nontransgenic siblings until 4 months of age. In contrast, no significant differences in galanin peptide concentrations of the neurointermediate lobes were evident. Galanin mRNA concentrations in the anterior pituitary of 6-month-old transgenic mice were increased 4-fold. In conclusion, 1) galanin peptide concentrations in the anterior pituitary gland and hypothalamus are increased in hGHRH transgenic mice compared to those in nontransgenic siblings, whereas galanin peptide concentrations in the neurointermediate lobe are not different; 2) pituitary galanin mRNA concentrations are increased 4-fold in 6-month-old transgenic mice; and 3) the development of pituitary hyperplasia is correlated to the increase in galanin mRNA and peptide concentrations.
