Primary molars in severe infraocclusion: a retrospective study.

UNLABELLED: The term severe infraocclusion is used to describe teeth located at the level of or below the alveolar crest and is rare. AIM: The aims of the study were to evaluate the influence of age of diagnosis and treatment on the outcome of the successors of primary molars with severe infraocclusion and to propose a treatment protocol based on the age of diagnosis. METHODS: A retrospective study of patients with primary molars in severe infraocclusion (PMSI) in the period 1987-2001 was carried out. Parameters assessed were age, sex, degree of infraocclusion based on radiographs, altered position of adjacent and successor teeth and treatment outcome. RESULTS: The sample comprised 19 patients with 23 cases of PMSI, all were second primary molars (47.8% maxillary and 52.2% mandibular). Migration of the neighbouring tooth was present in 51.5% of cases. The PMSI treatment was by extraction in all cases, a space maintainer was fitted and remained in place until the eruption of the successor. The outcome was favourable in 82.6% of cases. CONCLUSION: Successful treatment of PMSI depends on prompt diagnosis and appropriate treatment and follow-up of the cases. The earlier the age of detection, the more favourable the outcome.

[Observations on prosthetic restoration in children].

In young children there are two frequent factors, that cause edentation: the syndrome of the sugar "baby's bottle" or the "honey dummy" and tooth-loss by trauma. Multiple extractions can be programmed in children for reason of general condition, in the major concern to remove present or potential infectious foci. Still less frequently, sometimes lately diagnosed anomalies in number or structure oblige to replacement of missing or deficient tooth. The complexity of these restorations can be related to technical problems, less frequent, in contradiction to the current opinion, to psychological problems. They have to be integrated in structures in growth, the key character of each treatment in children or adolescents. Consequently it is necessary to take account of all this factors, before to attempt this charge, and to foresee adaptations, regular control, succession of the case and interception.

[Replacement of deciduous incisors in children: psychological aspects].

The absence of the temporary incisors could be to a genetic illness or to some multiple premature extractions. These extractions are the aftermaths of the carious lesions or some traumatisms underwent by the temporary incisors. Beyond measure the loss of the space, the premature loss some temporary incisors very often assign the relational development of the child and disturb its psychological development and the aesthetic function. Across some cases clinics, the authors show that the replacement of the temporary incisors is the therapeutic ideal solution. Indeed, the child prosthesis, replacing the temporary absent incisors, solves the psychological, aesthetic and relational problems of the child.

Glass fiber reinforced composite resin space maintainer: case reports.

PURPOSE: Glass fiber reinforced composite resins (GFRCR) are new to the pediatric dental market and can be an alternative to conventional space maintenainers. GFRCRs have been developed for dental applications during the past several years. This preliminary clinical study described the treatment of patients whose missing primary molars were restored with everStick, a semimanufactured product made of glass fibers, thermoplastic polymer, and lightcuring resin matrix for reinforcing dental polymer. METHODS: everStick was applied as a space maintainer when there was a loss of 1 or 2 teeth in the maxillary or mandibular arch. RESULTS: This study showed that the GFRCR space maintainers functioned well during a short period. The clinical advantages of the GFRCR space maintainers were that they: provided cost and time savings; did not require a cast model; did not require a second visit; were easy to apply; provided reliable adhesive bonding; provided long-term retention; were used when there was an indication of metal allergy; were easy to clean; had a natural feel and were esthetic. CONCLUSIONS: GFRCR space maintainers can be a new alternative to the conventional space maintainers used in pediatric dentistry.

Glass-fiber reinforced composite in management of avulsed central incisor: a case report.

Reimplantation failure of avulsed anterior tooth in an adolescent patient requires removal of the failed tooth and consideration of restorative options. These options may include a removable partial denture, conventional 3-unit fixed partial denture, implant, or a resin-bonded appliance with a metal substructure (Maryland bridge). The glass-fiber reinforced composite material (everSTICK, StickTech Ltd, Turku, Finland) offers a restorative solution that is conservative and esthetic when compared to other restorations. Advantages include reduction of cost compared to conventional bridges, saving of time, elimination of second visit, ease of application, absence of metal allergy, ease of cleaning, and naturalness of feel. Its limitations include occlusal factors, and the presence of unsuitable abutment teeth. Another traditional contraindication is the presence of diastemas, which may limit the potential esthetic gains. This case of an 11-year-old girl, addresses the indications, preparation guidelines, and restorative procedures for an everSTICK bridge.

Everyday uses of adult orthodontics.

Adults are now much more in favor of receiving orthodontic treatment than in the past. The baby boomer generation has a deep desire to keep their "youngness" and is willing to invest in such. Along with this attitude has come the ability to treat malocclusions and other clinical deficiencies with new products that decrease the treatment time. Nickel titanium wire has revolutionized the mechanics of treatment, so that often only one wire need be used throughout treatment, and the time of care has been reduced. Invisalign has resulted in higher acceptance rates for treatment that was passed up before. Although Invisalign has its limitations, for most basic alignment, it can provide a nice result. Patients who might come to an office for Invisalign can be open to braces if Invisalign will not correct their problem. In this article, several uses of adult orthodontics have been shown. It behooves the general practitioner to refer those cases that can benefit from the multiple uses of orthodontics. Should the GP desire to learn to perform any or all of the tooth movements necessary to create a more desired outcome, there are several marketed orthodontic courses that will provide the knowledge necessary. Finally, I encourage those who would enjoy treating patients with orthodontic needs. I have found it to be very rewarding to work toward a shared outcome that is often a less invasive treatment. Patients really enjoy the results.

Band and loop space maintainer--made easy.

Space maintainers have been used in Pediatric Dentistry for many years. The use of these appliances, however, in terms of indications, contraindications, design and construction has gained little attention from researchers. This paper highlights the construction of a new technique of fabrication of band and loop space maintainer. The advantages of this new Direct technique or Single sitting technique over the conventional technique are discussed.

Clinical evaluation of simple fixed space maintainers bonded with flow composite resin.

The aim of this study was to evaluate the clinical performance of simple fixed space maintainers bonded by using a flow composite resin (Tetric Flow) to prevent space resulting from early extracted primary teeth. For that reason, 64 fixed space maintainers (34 in the lower jaw and 30 in the upper jaw) were applied to 45 patients. The patients followed up for 12 to 18 months. Survival rate, prevention ability of that space, and whether damage to the abutment teeth occurred were evaluated. Five percent of space maintainers were determined to be unsuccessful at the end of the control period. During this period, loss of space among the abutment teeth was found to be statistically insignificant (P > .05). Finally, it was observed that the use of simple fixed space maintainers was successful due to operator experience and the choosing of favorable patient groups.
