Failure of endodontic treatment due to a palatal gingival groove in a maxillary lateral incisor with talon cusp and two root canals.

A case is presented in which endodontic treatment of a maxillary lateral incisor with a talon cusp and two root canals failed following a mistaken diagnosis. What was first diagnosed as an endodontic lesion was, in all probability, a primary periodontal lesion caused by the advance of bacteria from the gingival crevice to the apex along the radicular groove between the main tooth and the talon cusp.

Detection and treatment of multiple canals in mandibular premolars.

The endodontic treatment of mandibular premolars may not be successful because of failure to recognize and/or treat multiple canals when they are present in these teeth. Factors such as an awareness of data pertaining to the number of canals, knowledge of canal morphology, correct radiographic interpretation, and tactile examination of canal walls are important in detecting the presence of multiple canals. These factors, as well as guidelines for subsequent canal preparation and obturation, are provided.

Incidence of four-rooted maxillary second molars: literature review and radiographic survey of 1,200 teeth.

A study of 1,200 teeth was carried out in order to determine the occurrence of four-rooted maxillary second molars. One-thousand untreated teeth were examined radiographically. Two-hundred teeth were evaluated radiographically following endodontic treatment. The results indicated that only 0.4% of the teeth presented with four separate roots. Endodontic therapy in such a tooth is described.

A maxillary central incisor having two root canals geminated with a supernumerary tooth.

This article reports the treatment of a maxillary left central incisor geminated with a supernumerary tooth. A patient having labial swelling around the maxillary incisors was referred to this hospital for diagnosis and endodontic treatment of the maxillary left central incisor. Clinical and radiographic examinations revealed a tooth having two roots and three canals. Examination of a rubber base impression of the cleaned canals showed there was neither perforation of, nor communication with, the radicular groove in the tooth.

Pulp management in dens evaginatus.

A 12-yr-old Guamanian girl presented with seven premolars displaying dens evaginatus. The pulp of one tooth was necrotic secondary to fracture of the evagination. Calcium hydroxide treatment was used to induce closure of this root apex, and subsequent routine canal obturation resulted in clinical success. Three teeth had fractured evaginations and compromised coronal pulps, but the radicular pulp tissue was vital. Coronal calcium hydroxide pulpotomy procedures were performed, and root development continued on all three teeth. In one of these teeth, endodontic treatment was completed, and in the other two the radicular pulp remained vital. Three teeth had intact evaginations and vital pulps. In an attempt to protect these evaginations from trauma, composite reinforcements were placed around the evaginations. These pulps remained vital and the roots completed normal development. Early diagnosis and treatment should be sought in cases of dens evaginatus to provide the greatest opportunity for complete root development.

Oral-dental disease in cats. A feline practitioner's perspective.

The impact and relevance of dentistry in a general feline practice is detailed. Hospital policy and protocol are described for client education, preanesthetic evaluation, dental procedures, and recall programs. A discussion of commonly observed feline dental problems is included.
