[Recovery of facial nerve function using sural nerve transplantation after its injury in acoustic nerve surgery].

INTRODUCTION: Statoacoustic n. neurinoma is a benign, slow-growing and usually unilateral tumor. During its growth the tumor exerts pressure on the surrounding anatomic forms within the pontocerebellar angle: cranial nerves, pons, cerebellum. Therefore the first clinical symptoms are ear buzzing and deafness, vision disorders, occipital headache or walking difficulties. The diagnosis of such conditions must be precise, whereas CT (computerized tomography) and MRI (magnetic resonance imaging) are the methods of choice. Surgical tumor removal is the only therapy, but during surgery facial nerve injury occurs. The objective of this paper is a case report of a facial nerve injury and sural nerve transplantation during acoustic neurinoma surgery and recovery of facial nerve function. CASE REPORT: A 23-year-old male patient suffered from ear buzzing in the right ear for a year and a half with gradual development of deafness. Due to frequent headaches and after ophthalmologic examination, the patient was urgently hospitalized at the Neurology Clinic of the Faculty of Medicine in Novi Sad where MRI of the endocranium was performed revealing a tumor of the pontocerebellar right angle, 3 x 3.5 cm in size. The patient has undergone surgery at the Neurosurgery Clinic of the Faculty of Medicine in Pees in Hungary, with suboccipital craniotomy and tumor ablation. During surgery facial nerve injury occurred in the premeatal segment and intraoperative transplantation of sural nerve grafts from the left leg to the distal parts of the facial nerve was performed. The histopathologic finding revealed an acoustic neurinoma (Schwannoma). After surgery a control CT was performed revealing a complete tumor removal. The wound healed per primam intentionem and the patient was released from hospital two weeks later. During the postoperative period physical therapy was performed in the aim of rehabilitation of the facial nerve due to peripheral paralysis. After electrodiagnostic tests using GALVOMED 20 apparatus, massage was performed in the periorbital and perioral regions. Kinesitherapy was also done in front of a mirror several times a day. 6 months after surgery an EMG (electromyography) of m. frontalis dx., m. orbicularis oculi dx. and m. orbicularis oris dx. were performed. The EMG revealed evident reinnervation possibilities. A year after surgery the control MR finding of the endocranium was regular, as well as the control MR two years after surgery (postoperative cyst without signs of recurrence of the removed neurinoma). Control EMG of the m. frontalis dx., m. orbicularis oculi dx., m. orbicularis oris dx. showed signs of reinnervation. DISCUSSION: Tumors of the pontocerebellar angle are usually acoustic nerve neurinomas. 8% of intracranial tumors are Schwannomas. They originate from neurilemmal cells, by rule they grow slowly and are benign tumors. Therefore, for years the only signs pointing to them are ear buzzing and gradual development of deafness. That is why these anamnestic data are important for diagnosis. Headaches, walking difficulties, vision disorders are the usual difficulties due to which patients seek doctor's help. Computerized tomography and magnetic resonance imaging represent the diagnostic methods of choice in establishing the diagnosis. In this case MRI was performed on time. Surgery is the only therapy, but during tumor ablation the facial nerve was injured in the premeatal region. Intraoperative transplantation of grafts taken from sural nerve to proximal and distal parts of the facial nerve provides possibilities for injured nerve regeneration. The process of regeneration of such a nerve is long-term and often permanent 40% axon loss occurs. This is the reason to perform a control EMG two years after surgery. During peripheral paralysis rehabilitation is performed in the aim of preventing contractures. It is achieved by passive exercises in front of a mirror a few times a day. (ABSTRACT TRUNCATED)

Modelling sensitivity losses in ocular disorders: colour vision anomalies following intense blue-light exposure in monkeys.

The effects of prolonged exposure to intense, short-wavelength light were studied in monkeys through the measurement of increment-threshold spectral sensitivity (ITSS) and threshold-versus-intensity (TVI) functions using a behavioural method. The long-term effect of intense blue-light exposure was to induce a short-wavelength (SW) sensitivity loss which did not depend on the intensity or chromatic composition of the adapting field. The TVI curves for short wavelength stimuli revealed an increase in test threshold without changes in field sensitivity. Since this SW sensitivity loss may generalize to characteristic colour vision defects found in many outer retinal diseases, models of acquired alterations of colour vision mechanisms are considered. These models describe probable changes in ITSS functions and TVI curves in diseases affecting the inner or outer retina as well as changes in dark adaptation.

[Risk assessment expanded accident insurance for children].

Disability is a well known and tragic event for children. While adults are an established group for specific disability insurance cover, children were often neglected in the past. Although parents, organizations and paediatricans are aware of the risk, children specific incidence rates for disability are hardly available. The only sufficient source for some statistical data are the accident statistics because they represent a substantial group of specific cause related disability for children. Incidence rates for disease related chronic severe impairment or disability in children are either derived by single disease research or actuarial calculation of the German Social Disability Registration. Based on this statistical background, an extended accident insurance for children was introduced in Germany covering both accidents and disabling diseases. The key limitation for all variations of this insurance are exclusion clauses for congential diseases and mental disorders. This insurance requires a new approach in underwriting of the health risks. Because of the substantial number of impaired children, a simple decline of substandard cases are unacceptable. The early experience or medical underwriting shows predominantly health impairments of the following types: allergies, bronchial asthma, ectopic eczema (neurodermitis), disorders of speech and articulation, vision disorders and mental impairments. The suggested solution for underwriting of substandard risks is the predetermination of the possible future maximum degree of disability. The need for underwriting guidelines is supported by the market impact of the new disability cover with thousands of insurance policies issued in the first month after introduction.

Social handicaps of multiple sclerosis and their relation to neurological alterations.

The social conditions of 73 patients with multiple sclerosis (MS) were studied in relation to the most common neurological disorders observed among them. The Environmental Status Scale (ESS) and Kurtzke's Neurological Functional System (NFS)--both included within the Minimal Record of Disability (MRD)--were used. Pyramidal alterations were the more frequent neurological findings, whereas the need for personal care was the most common social handicap. The chi 2 test applied to contrast each of the dicotomic ESS and NFS variables showed a significant relationship between pyramidal alteration (weakness) and the need for personal care (p < 0.001), the use of community services (p < 0.01), and transportation problems (p < 0.05). Cerebellar disorders were significantly related to the need for community services (p < 0.01) and transportation problems (p < 0.01). Vision disorders were significantly related to the need for personal care (p < 0.05), transportation problems (p < 0.05), and social activity (p < 0.05). In turn, spasticity was significantly related to the need for personal care (p < 0.001), financial (p < 0.001) and transportation problems (p < 0.001), laboral situation (p < 0.05), and the use of community services (p < 0.05). Contrasting the four NFS and 7 ESS nondicotomic variables showed a marked correlation between the pyramidal alterations and personal care (r = 0.71) and transport (r = 0.62). Pyramidal alterations influence the social situation of MS patients and may be the foremost alterations to be considered for the optimal social integration of these patients.

Screening for amblyopia in preverbal children with photoscreening photographs: IV. Interobserver variability in photograph grading: origin and method of reduction.

BACKGROUND: Earlier detection of childhood vision disorders is a prominent goal of vision screening. The Medical Technology and Innovation (MTI) PhotoScreener addresses this objective. Use of this camera does not require verbal feedback and may be administered early in a child's development. Decreasing the variability in photograph grading results will boost the utility of any photoscreening system. This report aims to understand and to decrease intra- and inter-observer variability in grading photoscreening photographs. METHODS: We dissected the photograph grading process and quantified the intra- and inter-observer agreement using intraclass plot correlation coefficients. We evaluated the outcome of a two grader verification system vs. adjudicated measurements with Receiver Operator Characteristic (ROC) curves. PARTICIPANTS: Data on 955 children under 5 years of age, normal except for refractive error, each with complete photoscreening and eye examination data, culled from two previous studies. MAIN OUTCOME MEASURES: Intra- and inter-observer agreement in measuring bright crescent dimensions and pupillary diameters. Sensitivity and specificity of detection of hyperopia. RESULTS: Measurements of bright crescents are associated with greater variability than are measurements of pupillary diameters. Recognition and omission of light "rim" measurements from photograph grading will result in superior inter-observer agreement. Photograph independent errors increase variability and may be corrected by remeasurement. A verification system in which the most discrepant 5% of measurements are redone results in ROC curves similar to adjudicated dimension. CONCLUSIONS: We conclude: 1) two novices grading photographs can do as well as one expert in most cases; 2) the proposed grading methodology has undergone statistical validation and can be used in other areas of ophthalmology and medicine; and 3) inter-observer variability, one of the limitations of photoscreening photograph grading, can be reduced. For 95% of the photographs, two novices achieve similar true positive and true negative values with or without adjudication.

Nearpoint visual stress: clinical implications.

A physiological model of nearpoint stress, based on autonomic arousal, was presented in a companion paper. This paper deals with clinical implications of the nearpoint stress model, including clinical manifestations, adaptive responses to nearpoint stress, and management of nearpoint stress-induced vision disorders.

The relationship between vision and juvenile delinquency.

Vision disorders can be contributing causes of juvenile delinquency. Previous research is cited that illustrates this relationship. The author also reports his results obtained during a series of recent vision screenings of juvenile delinquents in a major metropolitan area. The results indicate a relatively high incidence of vision problems most of which dealt with poor binocularity. Excessive exophorias at near viewing distances were the most frequent reasons for failing the screening.

[Screening of the vision and hearing disorders in children from 0 to 3 years of age].

The screening of the visual and auditive sensory disorders is a significant stake of public health in France. Those are still too late and do not allow the assumption of responsibility early necessary. Between 0 and 3 years the issue is to avoid the two principal problems: irreversible amblyopia and language acquisition disorders by auditive deficit. It is thus advisable to find strategies which improve this situation. Pragmatic medical attitude and usual tests, usable in daily practice of the doctors who have really contact with the children, should be suggested.

The artist and defective sight (a book review and commentary).

BACKGROUND: Many authors have wondered if an artist "sees" things differently because of a vision disorder. For example, if a famous artist has a color deficiency, what would the effect be on his paintings? Would a painting be affected by a cataractous eye--or by presbyopia--or by diabetic retinopathy? Such questions have always interested the eye and vision specialist. METHODS/RESULTS: A 1970 book entitled The World Through Blunted Sight, written by British ophthalmologist Patrick Trevor-Roper, attempts to answer those questions. CONCLUSIONS: Interesting speculations are offered about the effects of myopia, astigmatism, presbyopia, and altered color vision on the works of such well-known artists as Monet, Holbein, Whistler, Gainsborough, and many others.

A Heidenhain variant of Creutzfeldt-Jakob disease: forensic implication.

To investigate whether typical clinical, diagnostic and neuropathological findings can be identified in a patient with a postmortem diagnosis of a Heidenhain variant of Creutzfeldt-Jakob disease (CJD). We report a new case of CJD in a rare variant. A man admitted to hospital with cefalea and vision disorder. Clinical and neurological examination showed headache, vision reduction, psychomotor anxiety and progressive torpor. The patient died 4 h after admission to hospital. The autopsy findings included marked encephalic vascular congestion. Hystoneurology examination showed no macroscopic anomaly. Microscopy findings included neuronal loss, gliosis in striate area with arachnoid cells and cerebellum microspongiosis. Creutzfeldt-Jakob disease is a rare neurodegenerative human disorder. The prion hypothesis as an explanatory model is currently favoured by majority of researchers. A disease course described by Heidenhain including the leading symptoms of a visual disorder and rapid progression. This report emphasize the multidisciplinary role (forensic, neurogenetic and neurohistologic) for diagnosis and to standardize a protocol to investigate.

Association between occupational asthenopia and psycho-physiological indicators of visual strain in workers using video display terminals.

BACKGROUND: There is a body of scientific literature examining functional changes in vision due to video display terminals (VDTs). The aim of our study was to determine the relationship of functional visual strain and symptoms of asthenopia and to evaluate the association between subjective and objective indicators of visual strain. MATERIAL/METHODS: Four hundred four office workers with and without involvement in VDT work were included in the study. To evaluate visual strain we used a questionnaire for subjective complaints, evaluated the main ophthalmologic indicators, and measured psycho-physiological indicators. RESULTS: The questionnaire data showed that 88.5% of the VDT workers complained of various vision disorders. VDT workers who complained of worsened vision, redness of the eyes, eye pain, and diplopia during work were found to show more significant changes in the psycho-physiological indicators objectively reflecting strain of the vision analyzer. In the group of people with symptoms of asthenopia, the differences in the indicators of visual sensomotoric reaction, constancy of clear vision, and changes in the periods of clear and unclear vision were statistically reliably greater than in people without symptoms of visual strain. CONCLUSIONS: The subjective perception of visual strain related to VDT work was confirmed by ophthalmologic and psycho-physiological measurements. Changes in ocular and psycho-physiological function before and at the end of the workday are a good objective index of visual and central nervous system strain.
