Free vascularised osteocutaneous filet flap for covering, that permitted sensitive terminal weight-bearing by a thigh stump after transfemoral amputation.

We covered a transfemoral amputation stump with a modified free filet flap including the calcaneus. The flap survived, resulting in lengthening of the femur, improved distal weight-bearing, preserved sensitivity of the stump, and the design of a prosthesis with terminal loading of the femur rather than an ischial contained prosthesis.

An instrument for monitoring stump oedema and shrinkage in amputees.

A new system for measuring the cross-sectional area profiles of amputation stumps and whole limbs has been designed at the Amputee Research Centre. The instrument consists of a cylindrical tank supported on an elevator. The tank is raised to the height of the amputation stump and filled with water. A graph of the cross-sectional area profile of the amputation stump is generated by a mini-computer as the elevator descends. The cross-sectional area (A) is calculated from the expression: formula: (see text) where Hw = height of water in the tank He = height of the elevator Ac = a constant, related to the size of the measuring tank. This paper describes the instrument, which may find application in many other areas where there is a need to study shape.

Morphological changes during early trans-tibial prosthetic fitting.

Morphological changes in the amputation stump may have serious implications regarding the suspension and fit of the prosthetic socket. In an earlier study (Lilja and Oberg, 1997) the authors have shown that the volume of the trans-tibial amputation stump decreases according to a negative power function after amputation, and that the stump volume does not stabilise until four months after the operation. In the present study, Magnetic Resonance Imaging (MRI) technique was used to examine morphological changes in the amputation stump after trans-tibial amputation in a small number of cases. The authors expected to find a decrease in the cross-sectional area of the stump and of the separate muscles similar to the findings in earlier studies. However, two different patterns were found. The cross-sectional area of the entire stump as well as that of the medial muscle group changed according to the authors' hypothesis, i.e. an initial fast decrease, followed by a more moderate decrease of the area. In the lateral muscle group another pattern was found. After an initial rapid decrease the area increased, sometimes to a magnitude larger than the initial value. After the amputation the lateral muscle group may acquire a new function, contributing to the suspension of the socket. Despite the limited number of patients, this study presents findings which may be important in the clinical fitting of trans-tibial prostheses.

Modification of Krukenberg's kineplastic operation.

Little attention has been paid lately to the kineplastic operations of amputation stumps. But Krukenberg's procedure, the forcipisation of forearm amputation stumps introduced in 1917 has, nevertheless survived. The authors have modified the operation. The skin of the forearm is split with a U-shaped incision. All muscles are saved. The interdigital commissure is covered with two triangular flaps. The remaining skin defect is left on the dorsum of the radial branch and covered with a free split skin graft. The skin graft is not used to cover the contact surface. In the Orthopaedic Hospital of the Invalid Foundation of Helsinki nine hands of seven patients have been operated during the years 1969--73 using this method.

Advanced prosthetic techniques for below knee amputations.

Recent advances in the evaluation of the amputation stump, the materials that are available for prosthetic application, techniques of improving socket fit, and prosthetic finishings promise to dramatically improve amputee function. Precision casting techniques for providing optimal fit of the amputation stump using materials such as alginate are described. The advantages of transparent check sockets for fitting the complicated amputation stump are described. Advances in research that promise to provide more functional prosthetic feet and faster and more reliable socket molding are the use of CAD-CAM (computer aided design-computer aided manufacturing) and the use of gait analysis techniques to aid in the alignment of the prosthesis after socket fitting. Finishing techniques to provide a more natural appearing prosthesis are described. These advances will gradually spread to the entire prosthetic profession.

[Hyperbaric oxygenation therapy for chronic occlusive arterial diseases of the extremities].

Therapeutic effects of hyperbaric oxygen therapy (HBO) in 50 patients with chronic occlusive arterial diseases were studied with determination of the transcutaneous oxygen pressure (TcPO2), plasma lipid peroxide level, and plasma superoxide dismutase (SOD) level. Necrosis or ulceration was present in 30 patients, rest pain without tissue loss in 6, infection and necrosis in 2, infection of the amputated stump in 2, delayed healing of the amputated stump wound in 8, and delayed union of bone fractures in 2. HBOs were carried out in 2-3 absolute atmospheres for 60 min for 3-40 times (mean, 12.7 times). In combination with HBO, sympathetic denervation was performed in 41 patients, and PGE1 infusions were administered in 46. Of patients with necrosis or ulceration, 16 were healed, 13 were improved, and one was unchanged. Of patients with rest pain, 5 had relief and one was unchanged. All patients with infection were cured. Of patients with delayed healing of amputation wounds, 7 were healed and one required reamputation. All patients with bone fractures obtained bone union. The TcPO2 markedly increased during HBO and remained at a high level for some time after HBO. The lipid peroxide and SOD levels were not changed significantly by HBO.
