The HNK-1 epitope and the elastic fiber system of the human ciliary body. An immunoelectron microscopic study.

PURPOSE: To localize at the electron microscopic level the cell adhesion-related HNK-1 carbohydrate epitope in the inner connective tissue layer (ICTL) of the human ciliary body. METHODS: Seven specimens representing the pars plicata (age range, 12 to 86 years) and three specimens representing the pars plana (age range, 29 to 71 years) of the ciliary body were sampled at death from eight normal human eyes. Three additional specimens from the pars plicata were taken from three eyes with exfoliation syndrome (age range, 78 to 81 years). All specimens were embedded in LR white resin and studied by postembedding immunogold labeling using two monoclonal antibodies (mAbs) HNK-1 and NC-1 in detecting the HNK-1 epitope. RESULTS: In the ICTL of the pars plicata and the pars plana, mAbs HNK-1 and NC-1 constantly bound to the surface of fibroblast-like cells present in the subepithelial connective tissue matrix. The immunoreaction was localized along the cell membrane, both around the cell body and around its long, slender cytoplasmic processes. Long microfibrillar bundles, which consisted of approximately 10-nm thick microfibrils in close association with these subepithelial matrix cells and elastic fibers, were also labeled. The periphery of elastic fibers was likewise immunolabeled for the HNK-1 epitope. The reaction pattern was essentially the same, regardless of age and presence or absence of exfoliation syndrome. CONCLUSIONS: At the ultrastructural level, the HNK-1 epitope in the ICTL is a common denominator to the subepithelial matrix cells, microfibrillar bundles, and elastic fibers. This suggests that the subepithelial matrix cells secrete this epitope, and that molecules hearing it may be involved in joining these connective tissue elements that structurally stabilize the ciliary body.

[Three-dimensional structures of the nonpigmented epithelium of the dog ciliary body--application of sodium hydroxide digestion to scanning electron microscopic observation of the ciliary epithelium].

The nonpigmented epithelium (NPE) of the dog ciliary body was three-dimensionally observed by scanning electron microscopy (SEM) using sodium hydroxide digestion. The inner limiting membrane of the NPE and the zonular fibers covering the NPE were completely dissolved in 6N sodium hydroxide for 10-15 minutes at 60 degrees C, although the surface structures of the plasma membrane were remarkably preserved despite the rather rigorous treatment. Thus, the entire surface of the NPE was exposed for SEM. The surface of the NPE at the anterior part of the pars plicata was flat, but many complex processes were observed in the surfaces of the NPE as lateral walls of the middle and posterior parts of the pars plicata, to which the zonular fibers had adhered. The processes were more prominent in the middle portion and gradually flattened near the posterior portion. Many pit-like structures and drumstick-like processes of the plasma membrane were localized at the apex of the ciliary process.

Unusual ciliary body tumor. Mesectodermal leiomyoma.

A 23-year-old white woman presented with a pigmented ciliary body mass in the left eye. The tumor appeared to transilluminate when observed with indirect illumination. Clinically the tumor was considered to represent either a malignant melanoma or a cyst and was excised by iridocyclectomy. Electron microscopic examination established the smooth muscle nature of the tumor. The literature on this unusual tumor is reviewed. Furthermore, this report serves to emphasize the value of iridocyclectomy in the diagnosis and treatment of ciliary body tumors.

[Surgical management of ciliary body melanoma].

The aim was to report my own experiences of the surgical treatment of ciliary body melanoma. 42 patients were treated by trasnscleral resection of melanoma between January 1995 and June 2001. Additional rutenium brachytherapy was performed in 29 cases. Intraoperative complications included bleeding (3 cases), vitreous loss (4) and retinal detachment (2). Residual tumor in one case was the reason of enucleation. Cataract surgery and intraocular lens implantation was necessary in 7 cases. Postoperative filtration bleb appeared in 6 eyes. The evaluation of functional results has shown no impairment of visual acuity in 56% of cases and did not change during follow up. These observations show that transscleral resection with additional brachytherapy is a method of choice in the management of ciliary body melanoma.

Cavitary melanoma of the ciliary body. A study of eight cases.

PURPOSE: The authors present the unique clinical features of cavitary uveal melanoma. DESIGN: Retrospective chart review. PARTICIPANTS: Eight patients with cavitary uveal melanoma. MAIN OUTCOME MEASURES: The clinical, ultrasonographic, and histopathologic features of eight patients with cavitary melanoma of the ciliary body were studied. RESULTS: In all eyes there was a brown ciliary body mass that blocked transmission of light on trans-scleral transillumination. Ocular ultrasonography revealed a large, single hollow cavity (unilocular "pseudocyst") in five cases and multiple hollow cavities (multilocular "pseudocyst") in three cases. The cavity occupied a mean of 55% of the entire mass thickness (range, 31%-79%). In five cases, a basal uveal mass was noted on ultrasonography. Four patients underwent tumor resection; one had enucleation, and three had 125I radioactive plaque treatment. In the five cases confirmed histopathologically, the cavitation was empty, contained erythrocytes, serous fluid, and/or pigment-laden macrophages. In no case was the cavity lined by necrotic tumor, endothelial cells, or epithelial cells. CONCLUSION: Ciliary body melanoma can develop an intralesional cavity resembling an intraocular cyst. The presence of a solid mass at the base and a thick wall surrounding the cavity can assist in the differentiation of cavitary melanoma from benign cyst.

Specific receptor sites for PAF in iris and ciliary body of the rabbit eye.

The existence of specific binding sites by using tritiated 1-O-alkyl-2-O-acetyl-sn-glycero-3-phosphocholine (3H-PAF) was investigated on iris and ciliary body tissue of pigmented rabbit eyes. The binding was saturable, specific, time-dependent and reversible. Scatchard's analysis indicated the presence of two types of binding sites with a Kd1 4.90 +/- 0.47 nM, a Kd2 11.60 +/- 0.33 nM, a Bmax1 3.17 +/- 0.50 pmol/mg protein and a Bmax2 12.45 +/- 2.30 pmol/mg protein for iris tissue, and a Kd1 5.71 +/- 0.09 nM, a K2d 24.40 +/- 0.91 nM, a Bmax1 3.41 +/- 1.00 pmol/mg protein and a Bmax2 16.60 +/- 0.51 pmol/mg protein for ciliary body. The binding was fully displaced by unlabelled PAF in both iris and ciliary body preparations, and partially inhibited by the PAF antagonist BN 52021 in iris tissue.

Distribution of ascorbic acid in the ciliary body of albino rabbit, guinea pig and rat.

The distribution of ascorbic acid in the ciliary body has been demonstrated by means of a silver nitrate technique. It appeared that the basal cells in the pars plana region and in the valleys between the ciliary processes contain great amounts of ascorbic acid, in contrast to the basal cells over the processes proper which showed only traces. Rabbit and guinea pig revealed mainly the same findings, and quantitative differences occurred before and after intracardial injection of ascorbic acid. On the other hand, ascorbic acid was absent in the ciliary body of untreated rats, whereas only traces were found after intracardial injection of this substance. These species differences are in keeping with previous reports showing high concentration of ascorbic acid in the aqueous humour from rabbit/guinea pig, and no ascorbic acid at all in aqueous humour from rats.

Schwartz-Matsuo syndrome in retinal detachment with tears of the nonpigmented epithelium of the ciliary body.

PURPOSE: To elucidate clinical features of Schwartz-Matsuo syndrome (photoreceptor outer segment glaucoma) occurring in retinal detachment with tears of the nonpigmented epithelium of the pars plana or plicata of the ciliary body. METHODS: We reviewed retrospectively medical records of 17 consecutive patients (18 eyes) with retinal detachment caused by tears of the nonpigmented epithelium of pars plana (10 eyes) and pars plicata (8 eyes) of the ciliary body. RESULTS: Aqueous cells, ranging from 1+ to 3+, were found in 15 eyes, in contrast with no cells in the other 3 eyes. Eleven of the 15 eyes with aqueous cells showed circadian fluctuation of the intraocular pressure with a range, 4 mmHg or wider compared with the range in the fellow eye, but only 4 of these 11 eyes had a maximum pressure of 21 mmHg or higher. Photoreceptor outer segments were detected in the aqueous humor of 8 eyes out of 14 eyes with aqueous cells in which aqueous tap was done. The remaining 6 eyes had too small a number of aqueous cells for electron microscopy. CONCLUSION: Marked fluctuation of the intraocular pressure compared with the fellow eye, which was not necessarily associated with its rise, was found in retinal detachment with tears of the nonpigmented epithelium of the ciliary body, and should be included as one of key features for diagnosis of Schwartz-Matsuo syndrome.

Schwannoma of the ciliary body treated by block excision.

A 26-year-old man developed a non-pigmented ciliary body tumour of his right eye. A 7 mm block excision and tectonic corneoscleral graft were performed. The excised tissue was studied using histopathological, immunohistochemical, and electron microscopic techniques. The tumour revealed characteristic features of a Schwann cell neoplasm including Antoni A and B patterns, acid mucopolysaccharides, S-100, and vimentin positivity, and--by electron microscopy--Luse bodies. It was classified as a schwannoma. Although rare, schwannoma should be included in the clinical differential diagnosis of non-pigmented ciliary body tumours. Local excision should be considered to avoid over-treatment by enucleation.

Microcirculation patterns other than loops and networks in choroidal and ciliary body melanomas.

PURPOSE: To provide ophthalmologists and pathologists with expanded criteria for separating patients at high risk of metastatic melanoma from those at low risk on the basis of microcirculation patterns in choroidal and ciliary body melanomas. DESIGN: Tissue culture studies and observational case series. PARTICIPANTS: The pattern-forming ability of four uveal melanoma cell lines of varying degrees of aggressive behavior was studied in vitro. Histologic sections of 234 eyes removed for choroidal or ciliary body melanoma were studied for the presence of microcirculation patterns. METHODS: The study was divided into two phases: the study of histologic sections of eyes removed for choroidal and ciliary body melanomas and observations on the in vitro behavior of cultured melanoma cells of varying degrees of invasive behavior. The presence or absence of each of nine microcirculation patterns was recorded from tissue sections, and interrelationships between different patterns were explored statistically. In vitro reconstitution of patterns and a study of the interrelationships of patterns in histologic sections was carried out. In the in vitro studies, uveal melanoma cell lines of varying degrees of aggressive potential were cultured to observe the development of architectural patterns other than loops and networks. MAIN OUTCOME MEASURES: In histologic studies, the outcome measure was the conditional probability of detecting loops or networks given the presence or absence of other patterns positive for periodic acid-SCHIFF: For tissue culture studies, the outcome measure was either the development or lack of development of patterns of different shapes in vitro. RESULTS: Histologic studies disclosed that given the presence of arcs without or with branching in a tissue section, it is likely that loops or networks will be detected in the same section plane, suggesting that the production of these patterns by aggressive tumor cells reflects a spectrum of architectural potential. In vitro studies confirmed this hypothesis by revealing that highly aggressive and metastatic uveal melanoma cell lines, but not poorly aggressive tumor cell lines, generated parallel channels with and without crosslinking and arcs with and without branching as well as loops and networks. CONCLUSIONS: The criteria for separating patients into low- and high-risk categories for metastasis from uveal melanoma should be expanded to include patterns other than loops or networks. In both the pathology laboratory as well as in a clinical setting, the detection of arcs or arcs with branching and parallel channels should prompt a careful search for loops and networks and for crosslinking parallel channels, respectively.

Enzymes of the gamma-glutamyl cycle in the ciliary body and lens.

The enzymes of the gamma-glutamyl cycle have been found in rabbit ciliary body and, except for 5-oxoprolinase, also in the ocular lens. Histochemical studies show that gamma-glutamyl transpeptidase is localized mainly in the basal portions of the epithelial cells of the ciliary body; the findings are similar to those observed in the chloroid plexuses. The histochemical staining reaction in the ciliary epithelium is more intense than in the chloroid plexus, intestine, and kidney. gamma-Glutamyl transpeptidase staining activity in the epithelium of the intestinal and renal proximal convoluted tubules is confined to the microvillus border. Moderate transpeptidase activity was found in the cytoplasm of nonpigmented epithelial cells of the iris at the posterior pupillary margin. The histochemical and enzyme activity studies are consistent with the thesis that the gamma-glutamyl cycle functions in transport of amino acids across the blood-aqueous humor barrier.

[Metastasis of differentiated follicular thyroid carcinoma to the ciliary body].

A girl aged 20 years was suffering from a slowly growing tumor of the ciliary body of her left eye. The tumor was excised with iridocyclectomy supposed to be a melanoma. The histologic examination showed a well-differentiated follicular carcinoma of the thyroid gland. The primary tumor had been excised 16 years before, the diagnosis at that time was benign thyroid adenoma. After the successful removal of the ciliary body a follicular carcinoma of the thyroid gland was finally diagnosed and treated by total thyroidectomy and systemic 131I therapy.

Ciliary body schwannoma.

PURPOSE: Intraocular schwannomas are very rare, benign, peripheral nerve neoplasms. The authors report a case of ciliary body schwannoma. METHOD: A 39-year-old Korean woman presented with slowly decreasing visual acuity and proptosis of the right eye for 4 years. At the time of her visit, the eyeball protruded and deviated laterally. She could not sense light with her right eye. We enucleated the eyeball. The enucleated eyeball with tumor was examined histopathologically, immunohistochemically, and under electron microscope. RESULTS: Microscopic examination revealed spindle cells and characteristic Antoni type A and B areas with Verocay bodies. Immunohistochemical study showed tumor cell expressions of S-100 and vimentin, but other immunohistochemical studies were negative. Electron microscopic examination demonstrated Luse body. CONCLUSIONS: We diagnosed the tumor as a schwannoma arising from the ciliary body on the basis of the above mentioned results. Schwannomas are very rare intraocular neoplasms, but they are benign so we should differentiate from other intraocular neoplasms.

Ciliary body melanoma: a special challenge.

Although there is little doubt that the delayed recognition of ciliary body melanoma has a bearing on patient management and ultimate survival, the most compelling issues that face the clinician treating this neoplasm relate to the metastatic patterns and mechanisms of the disease. Several aspects of diagnosis and management of this tumour provide a unique challenge to the clinician. Ciliary body melanoma can remain clinically inapparent to the patient as well as to the clinician during its formative period. In management, tumour characteristics, including anterior and posterior margins, are more readily visualized with ultrasound biomicroscopy (UBM) than with other imaging techniques. UBM can provide valuable information when considering intervention, including biopsy, resection or plaque radiotherapy. Management depends on tumour size, intraocular involvement, patient preference and the presence or absence of systemic manifestations.

Three-dimensional reconstruction of the angioarchitecture of the ciliary body of the West Indian manatee (Trichechus manatus).

OBJECTIVE: To examine the angioarchitecture of the ciliary body in the West Indian manatee (Trichechus manatus), through the use of three-dimensional reconstruction. PROCEDURE: Specimens from West Indian manatee were preserved in 10% buffered formalin, embedded in paraffin, serial sectioned and stained by Masson trichrome for light microscopic three-dimensional reconstruction and evaluation. RESULTS: The network of blood vessels in the ciliary processes of the West Indian manatee is fed by the major arterial circle that lies mostly near the base of the iris. The branching arterioles give rise to a capillary-sinusoidal bed that extends internally along each process, emptying into two sets of veins, one being elevated. The elevated and nonelevated veins join posteriorly before emptying into the choroidal venous system. CONCLUSIONS: The angioarchitecture of the ciliary body of the West Indian manatee is clearly unique when compared to those previously examined in land mammals. Three-dimensional reconstruction of paraffin sections is an effective means to evaluate vascular patterns in ocular specimens, especially those unavailable for corrosion casting.

Higher proportions of type C than of types A and B natriuretic peptide receptors exist in the rat ciliary body.

We investigate the interaction of atrial natriuretic peptide (ANP) brain natriuretic peptide (BNP) and C-type natriuretic peptide (CNP) with their receptors (NPRA, NPRB and NPRC), as well as the proportion and localization of those receptors in the rat ciliary body. Binding assays and affinity cross-linking experiments demonstrated the presence of the NPRC receptor type. However, the three natriuretic peptides stimulate the guanylate cyclase activity in the ciliary body membranes suggesting the presence of the NPRA and NPRB receptor type. Microautoradiographic data show that the NPRs are localized in the whole ciliary body. Our results indicated that NPRC is the most prominent receptor type in this tissue.

Stimulation of endogenous cyclic AMP levels in ciliary body by SK&F 82526, a novel dopamine receptor agonist.

The effect of dopamine and SK&F 82526 on cyclic AMP metabolism in ocular tissues has been examined. The in vitro incubation of human and bovine ciliary body with these agonists produced a dose-dependent increase in endogenous levels of cyclic AMP. This stimulation was blocked by the selective DA1 receptor antagonist SK&F 83566, but not by the beta-receptor antagonist propranolol. The response to SK&F 82526 was stereoselective, with SK&F R-82526 being approximately 100 times more potent than SK&F S-82526 in this preparation. This stimulation of ciliary body cyclic AMP content was not observed in the rabbit or cat, nor was it seen in human, bovine or rabbit iris tissue. These data suggest that adenylate cyclase linked dopamine receptors are present in both human and bovine ciliary body.
