Satisfaction with complete immediate dentures and complete immediate overdentures. A 1 year survey.

The aim of this study was to evaluate denture satisfaction over a 1 year period after extraction of the last teeth. Seventy-four patients who required immediate denture therapy in the mandible were randomly treated, (1) with complete immediate dentures (22); (2) with immediate overdentures on two lower canines without attachments (26); or (3) with immediate overdentures on two lower canines provided with Dyna direct magnetic attachments, inserted on the abutments 9 months after treatment (26). Denture satisfaction was estimated by means of questionnaires, filled out by each patient at certain periods, starting before treatment and ending 1 year after denture insertion. After 1 year approximately 85% of the patients in the three groups were satisfied with their dentures, had got used to them and could eat well. This phenomenon was constant over the whole first year period. No differences in denture satisfaction were found between the three groups. It can be concluded that although patients who receive immediate complete dentures might experience a lot of discomfort during the first year of edentulousness, they are satisfied, in general, with their dentures independent of the treatment procedure.

[Care of edentulous mandible with implant stabilized telescope complete denture].

In comparison to the present junction elements of the implantologic-prosthetic treatment of the toothless lower jaw the dental implant stabilized overdenture is a good alternative. The movable positioning of the resilient telescopes needs the observance of the prosthetic criteria. Having only a guiding function the number of the implants can be reduced to two implants.

[Statistical analysis of a dental care program for homebound elderly patients in Shinjuku-ku].

A dental care service program for homebound elderly patients has been operating since 1988 in Shinjuku-ku, Tokyo. The purpose of this study was to clarify the characteristics of patients who received this dental care service. Based on a survey questionnaire and treatment card from April 1995 to March 1999, we analyzed the characteristics of patients. The following conclusions were obtained. 1. We studied 301 patients (males, 124; females, 177). The average age of the patients was 81.2 +/- 9.0 years. Eighty-four percent of the patients had a medical history. 2. Oral hygiene was practiced by 68% of the subjects: 78% by themselves, and 22% by caregivers. Half of the patients (53%) brushed their teeth every day, and one-third (32%) did not. 3. The mean number of DMF teeth was 25.4 (DT, 1.6; MT, 20.5; FT, 3.3). 4. About eighty percent (79%) of the patients had removable dentures, and one-third of them were upper and lower complete dentures. 5. The chief complaint of homebound elderly was the denture problem (73%). Therefore, the care provided at the bedside was mainly denture-related treatments. 6. Average times for visiting were 5.1. 7. Sixty-eight percent of the patients requested home dentist's visitation for dental care service, and 80% of the requested dentists performed such treatment. 8. Seventy-eight percent of the elderly patients were satisfied with the service program. This dental care program for homebound elderly patients seemed to work well. However, to meet needs for the expanding number of aged persons and to make it coincide with the new health and welfare scheme, reform of the program might be necessary.

Incidence of occlusal contacts with complete dentures during mastication using a 6-channel telemetry system: preliminary measurements.

The purpose of this study was to observe the incidence of occlusal contact with complete dentures during mastication using a 6-channel telemetry system whose accuracy had been improved. The telemetry system consisted of the transmitter unit and the receiver unit. The transmitter unit was embedded in the lower complete denture. This system detects occlusal contacts by the shift of frequency in 100 kHz. The transmitter allowed 6 channels for the occlusal contacts. The data was telemetered by a time division multiplex communication method. A special switch was designed to measure excursive movement on the occlusal surface. After careful occlusal adjustment, the special switches were inserted in the lower first molars. On the basis of preliminary measurements on a 64-year-old edentulous male, it appears that the width of the gliding on the occlusal surface at the lower first molar was mostly 0.2-1.2 mm and the majority of the excursive movements ended at the centric occlusal position.

Implant-stabilized complete mandibular denture for a patient with cerebral palsy.

Sufferers of cerebral palsy often have difficulty in chewing, speaking and swallowing because of the involuntary muscle spasms caused by their condition. This problem also makes it very difficult or even impossible for them to wear complete mandibular dentures. This article describes a cerebral palsy sufferer who was successfully fitted with a complete lower denture stabilized by osseointegrated implants. It may be possible to apply the techniques used for people with similar problems.

Denture mobility with six degrees of freedom during function.

The purpose of this study was (1) to assemble and verify a system to measure the three-dimensional (3-D) movement of the upper and lower complete dentures and the movement of the mandible simultaneously, and (2) to analyse the relation between denture movements and the path of closure of the mandible during function. A 3-D motion capture system with four infrared TV cameras was used for this purpose. The relation between the dentures and the mandibular movements was analysed through the change of the inner product of normal vectors of the denture occlusal planes and mandibular planes. The mandibular movements were classified into two types; the normal stroke (the path of closure was on the ipsilateral side of mastication) and the reverse stroke (on the contralateral side). The results showed that the system could measure the denture mobility within a 0.3 mm error. The mobility of the upper dentures had a correlation to the path of closure of the mandible regardless of the working side or nonworking side, and the lower dentures had a tendency to move toward the working side.

[Six channels telemetry system for recording masticatory forces of complete denture wearers].

A six channels telemetry system for recording masticatory force of complete denture wearers has been developed. This system is consisted of three blocks, the transmission block, the receiver block and the data storage and correction block. The transmission block is embedded in a lower complete denture. System has six load sensors and a standard resistor to correct sensor data. Data from six load sensors and standard resistor as well as standard zero are multiplexed using a division multiplex communication method. The sampling frequency is 666 Hz. And the signal is transmitted by FM modulation. The carrier wave frequency is 64 MHz. Receive data are decoded using start-stop system. The wave form noise is reduced using a signal averaging technique at an integrator. The data are input into the computer with an A/D converter. In this system, the sensor data is corrected at the computer with the standard resistor data. This method could reduce the influence of battery power changes and thermal drift significantly. For the load of 10 kg or less, the total linearity of the system remained at +/- 4% of the calibrated value. The noise level is 300 g.

[A study on evaluation methods of masticatory ability for complete denture wearers--correlation between evaluation using a questionnaire and masticatory performance].

The purpose of this study was to investigate the relation between evaluation by a questionnaire on masticatory aspects of 20 foods, and masticatory performance obtained using the sieving method, previously used in many studies. After replacement of upper and lower complete dentures made newly, 28 edentulous patients voluntarily participated in this study. The basal seat area of each patient was scored and divided into two groups, and masticatory ability was evaluated by two methods: using a questionnaire and the sieving method. The following conclusions were drawn from the data: 1. Evaluation of masticatory ability by both methods used in this study were not associated with gender, age, and evaluation of the basal seat area. 2. Evaluation using a questionnaire and masticatory performance showed significant correlation, and Spearman's correlation coefficient was 0.628 (p < 0.001).

[A fundamental study on the influence of posterior artificial tooth forms on denture movements during mastication].

The form of posterior artificial tooth is an essential factor related to denture stability during mastication and to masticatory ability. Generally, cusp teeth are selected out of consideration for balance during eccentric occlusion. From another viewpoint, cuspless teeth are selected to ensure stability and to prevent the occurrence of lateral vectors caused by the inclination of the occlusal facet. It has been reported, however, that the counterbalance phenomenon occurring between occlusal facets decreases the lateral vector. Moreover, cusp teeth are useful in increasing the possibility of establishing bilateral balance. Guidelines for selecting posterior artificial teeth in the production of functional dentures are diverse and cannot be said to have been firmly established yet. This model experiment, which employed complete lower dentures, was designed to provide basic information for discussing the influence of occlusal facets of cusp teeth on denture movements during mastication and to arrive at some clinical suggestions for selecting posterior artificial teeth. Artificial teeth with cusp angles of 0 degrees, 10 degrees, 20 degrees, and 30 degrees were made by adjusting setting conditions on a condylar type articulator. Four kinds of experimental dentures were made and used in measuring horizontal movements of lower dentures during the crushing of experimental food. Final phase of chewing stroke was taken into consideration in establishing two types of loading direction. One type was at 70 degrees (DL 70 degrees) to the occlusal plane on the frontal plane and from the outer side (the working side). The other type was 90 degrees (DL 90 degrees) to the occlusal plane. Maximum load was 15kg and the denture displacement was analyzed at 2kg, 4kg, 8kg and 15kg. Experimental foods were smoked cuttlefish (sectile food), raisins (food flattened under pressure), and peanuts (crushable food). Analyses were made of experimental denture movement patterns and denture displacement amounts. Results 1. Experimental denture movement patterns differed according to the nature of the experimental food. The pattern in the case of smoked cuttlefish was mainly of the C type; that is, primarily motion first to the balancing side and then to the working side. The pattern for raisins was mainly of the S type; that is, primarily motion first to the working side, then to the balancing side, and then to the working side again. Several patterns appeared in the case of peanuts.(ABSTRACT TRUNCATED AT 400 WORDS)

[Fixation of complete lower prostheses with intra-alveolar implants (case report)].

The author describes a female patient in whom complete lower prosthesis had to be made for the first time. For improving the stabilization of the prosthesis had to be made for the first time. For improving the stabilization of the prosthesis and facilitate the adaptation it was planned to carry out intra-alveolar implants by Wierzyński's method inserted into freshly emptied alveoli. The immediate prosthesis was a specially adapted prosthesis which had been worn previously by the patient. After healing of the post-extraction wound and epithelialization of alveoli final prosthetic replacement was prepared, i.e. upper partial prosthesis and lower complete prosthesis with intra-alveolar implant.

Masticatory function and patient satisfaction with implant-supported mandibular overdentures: a prospective 5-year study.

PURPOSE: The aim of this study was to assess the outcome of treatment with implant-supported mandibular overdentures in terms of biting and chewing, in entirely satisfied and not fully satisfied patients. MATERIALS AND METHODS: Twelve edentulous patients who had worn dentures for at least 5 years participated. They were in good health but had retention problems with their mandibular dentures. First, all patients received new dentures. After 3 months, two Astra Tech implants were placed in the anterior part of the mandible, and 6 months later the abutments were connected. Patient assessment (questionnaire) and functional recordings (chewing ability, bite force, electromyographic activity) were performed with the new dentures, and again 3 months, 1 year, and 5 years after overdenture treatment. RESULTS: After treatment, all patients were able to comminute hard and tough food, the maximum bite force and the chewing activity increased in parallel, and the duration of the chewing cycle was reduced. Every patient felt improved function and reduction of chewing pain. However, the seven patients not fully satisfied with the function of the implant-supported mandibular overdentures were characterized by lower muscle activity, even before implant placement, than the entirely satisfied patients. CONCLUSION: Implant-supported mandibular overdenture treatment permits better biting and chewing function than conventional complete dentures.

A study of the reproducibility of the functional denture space with a dynamic impression technique.

A study was made on four patients to determine the reproducibility of the outer form of the buccal and lingual flanges of a complete lower denture as developed by functional movements of the surrounding tissues. 1. Two dentists participated in the study to find out the operator effect when making impressions for the functional form of the flanges. 2. Two tissue-conditioning impression materials were used to determine if the characteristics of the impression material had any effect on the final form. 3. The impression materials were applied to the denture flanges by two different methods. 4. The impressions were placed in a mitre box, stabilized with plaster, and sectioned in the molar, premolar, and incisor regions. This provided five cut surfaces for measuring. 5. Each section was measured at five to six representative distances. 6. The differences in the flange form measurement and the distances among the impressions made with the same technique and material were small, and the reproducibility can be considered clinically acceptable. 7. There were statistically significant differences in the flange form measurement distances among the different materials and method of application of the material.

[Implant-retained overdentures compared with complete dentures with or without preprosthetic surgery. Satisfaction and subjective chewing capabilities].

The aim of this prospective randomized clinical trial was to evaluate and compare a set of clinical items and satisfaction of a group of edentulous patients during a 5-year follow-up. They were treated according to one of the following modalities: 61 patients with a mandibular overdenture on two implants (IMP-group), 60 patients with a conventional complete denture (VP-group) and 28 patients with a complete denture after preprosthetic surgery (MVP-group). It can be concluded that endosseous implants, serving as retention for a mandibular overdenture, have a high survival rate after 5 years of follow-up (93%). The mean satisfaction score of the VP-group was lower than of the IMP-group. The mean satisfaction score of the MVP-group is lower than of the IMP-group.
