Use of free peritoneal patch in reenforcing alimentary tract anastomosis.

The use of free peritoneal patch was evaluated for the effect on protection against intestinal anastomotic leakage in dogs. Free peritoneal patch was found to be effective in preventing the anastomotic leakage of the alimentary tract in borderline situation. Free peritoneal patch was more effective when applied with the 'smooth side out' than with the "smooth side in'. The usefulness of free peritoneal patch was also demonstrated in the treatment of the already established fistula of the cervical esophagus.

Late complications of incisional hernias following prosthetic mesh repair.

BACKGROUND: Incisional hernias still continue to be a serious problem for surgeons. In this study, we aimed to investigate the effects of polypropylene mesh and mersilene mesh, which are frequently employed in incisional hernias, as well as the effects of their application techniques on late complications. METHODS: Two-hundred-sixty-four open abdominal hernia repairs were performed between 1986--2000, using prosthetic materials: polypropylene mesh and mersilene mesh were used in hernia repair. Mesh was placed as onlay, underlay and sandwich techniques. Follow-up data were obtained from medical records. RESULTS: Comparisons were made with respect to surgical techniques and to late complications such as recurrence, enterocutaneous fistula, intestinal obstruction, and infections. Enterocutaneous fistulas developed in two patients. Recurrence occurred in 6.4 per cent. Chronic infection and wound sinus formation occurred in 5 per cent. The causes of recurrence included smoking, cellulitis, chronic infection/sinus tract, upper abdominal localization, and obstruction. Fistula formation occurred in patients with no peritoneal prevention, which was statistically significant (p=0.012). Chronic infection/sinus tract was high in patients for whom mersilene mesh was used, and enterocutaneous fistula occurred in 2 patients. CONCLUSION: To prevent late complications, it is necessary to avoid the contact of mesh with bowel.

[Sub-phrenic abscess: 28 cases, one death (author's transl)].

28 patients were operated on for subphrenic abscess, one patient died. The author emphasises the disadvantages of excessive antibiotics. The diagnosis should be made early in order to avoid severe infective complications. The diagnosis is not difficult. It is nowadays facilitated by echotomography and scannography, which permit one to localise precisely the collection of pus and to detect multiple abscesses. The incision should carefully avoid crossing the peritoneal or pleural cavities. The quality and the permanence of the drainage are the key to success. One should therefore leave in position numerous aspiration drains. One should generally abstain from any suture or digestive anastomosis. It is sufficient to direct the digestive fistula, when present, towards the skin surface. But the best policy is to avoid this operative complication the origin of which is almost always a mistake in the design or installation of the drain at the end of various operations on the abdomen.

Duodenal obstruction by gallstones (Bouveret's syndrome): a review of the literature.

Bouveret's syndrome, first described in 1896, is gastric obstruction by a gallstone following a cholecystoduodenal fistula. Endoscopy is the mainstay of diagnosis, but radiographic examination such as upper gastrointestinal contrast series and abdominal radiography can also contribute to the diagnosis. Diagnosis by computed tomography and ultrasonography has also been described. The syndrome can be diagnosed and treated endoscopically, with stone extraction or mechanical lithotripsy. Extracorporeal shockwave lithotripsy has also been used successfully. Surgery is required in over 90% of cases, with mortality rates ranging from 19% to 24%. One-stage and two-stage procedures have been described, including enterolithotomy, cholecystectomy, and fistula repair, no convincing data are available to show which of these two approaches provides a better outcome. Although the condition is rare, Bouveret's syndrome should be considered in elderly patients with a history of chronic cholecystitis who present with pain, vomiting or haematemesis.

Endorectal flap advancement repair is an effective treatment for selected patients with anorectal fistulas.

Although the majority of anorectal fistulas may be treated with fistulotomy, patients with higher-lying fistulas or women with anterior fistulas in whom simple fistulotomy may threaten continence present a management dilemma. Endorectal flap advancement repair is one potential solution. Chart review was performed of consecutive patients receiving endorectal advancement flaps during a 5-year period (1999-2003). Twenty-nine patients received endorectal advancement flap repair. In 21 (72%) patients, an etiology of the fistula could not be clearly identified, and they were considered to be cryptoglandular in origin. Eight (24%) patients had pathology presumed to be related to prior obstetrical injury. There were 18 (62%) women, and the majority (72%) had anterior fistulas and flaps. Primary success was defined as resolution of symptoms for at least a 3-month period without an additional surgical procedure. If resolution of symptoms after flap placement was achieved after an additional surgical procedure, it was considered a secondary success. Primary success was achieved in 20 (69%) patients, and a secondary success in four (44%) of the remaining nine patients, for a total success of 83 per cent. Anal advancement flaps demonstrate a reasonable success rate when utilized in this group of patients with challenging anorectal fistulas.

[Reno-colic fistula].

Nephro-intestinal fistulas involve the GI tract and the kidney or pyelocalyceal system. The main causes may be urologic or digestive, the site diverse. Reno-colic fistulas, although rare, are more frequently observed. We describe a case of left reno-colic fistula secondary to lithiasic pyonephrosis that presented as sepsis. Diagnosis was confirmed by retrograde pyelography. Treatment was by surgery (left nephrectomy, closure of the fistula, and colonorhaphy).

Fistulas following gastrectomy.

Postoperative fistula occurred in 16 of 761 cases in which subtotal gastrectomy was done. Twelve of the 16 patients died. In the majority of cases complications arose because of the anatomical location of the ulcer with surrounding inflammation. Abrupt and dramatic onset of symptoms following leakage occurred in only a few cases. Perforation occurred in a few cases after the patient had been discharged home well and asymptomatic. Obstruction of the proximal limb was not present in any of the cases in which postmortem examination was carried out. The exclusion operation with resection of antral mucosa may not be a safe procedure. For treatment of ulcer which cannot be removed, gastroenterostomy with vagotomy is advised.

Secondary aortoesophageal fistula after endoluminal exclusion because of thoracic aortic transection.

Secondary aortoesophageal fistula (AEF) is a rare but catastrophic complication that occurs after thoracic aortic reconstruction. Recently endoluminal stent grafts have been used in selected patients with a thoracic aortic aneurysm, dissection, or traumatic aortic transection. A 24-year-old woman had massive upper gastrointestinal tract bleeding 15 months after endoluminal stent graft placement because of traumatic descending thoracic aortic transection. Evaluation demonstrated an AEF from the mid-esophagus to the endoluminal stent graft. The endoluminal graft was explanted, with primary repair of the thoracic aortic defect and simultaneous primary repair of the esophageal injury. The patient is well 15 months after open repair of the AEF.

Prevention of anastomosis leakage: an artificial connective tissue.

Anastomosis leakage still causes considerable morbidity and mortality after digestive tract surgery. We report here our experience of the use of reconstituted connective tissue created from elastin, fibronectin and collagen in preventing anastomosis leakage and closing intestinal fistulas. In animal studies (56 rats), this connective tissue was used as a patch applied with fibrin sealant to the edges of a 1 cm diameter colonic defect. In a clinical evaluation, eight patients had an intestinal fistula closed by a simple sewn suture reinforced by a sealed patch and seven patients had a high risk anastomosis reinforced by a sealed patch, in most cases as an alternative to staged surgery. In animal studies, complete reconstitution of the three colonic layers was obtained without retraction and without inflammatory reaction within 40 days, while the patch was slowly resorbed. In clinical trials all 15 defects and anastomoses healed without complication. Three patients died from other causes and, at autopsy, the patch was found to have remained in place covering the suture line. If these results are confirmed by a prospective clinical trial, such biomaterial offers the possibility of reducing the occurrence of anastomosis leakage, especially in high risk circumstances.

Bouveret's syndrome: case report and review of the literature.

Bouveret's syndrome, or gallstone duodenal pyloric obstruction, almost always presents with abdominal pain or vomiting. It occurs more commonly in females (65%), with a median age of 68.6 years. The diagnosis is made by endoscopy (60%), upper gastrointestinal series (45%) or by direct abdominal x-ray (23%). The syndrome is mainly treated by surgery (93%), but recently, it has also been treated by endoscopy or extracorporeal shock wave lithotripsy. The mortality rate has improved from 33%, as was the case before 1968, to 12% in recent years. Herein we report the case of a 79-year-old female with Bouveret's syndrome.

Studies on the flow of zinc, cobalt, copper and manganese along the digestive tract of sheep given fresh perennial ryegrass, or white or red clover.

1. Sheep fitted with a rumen fistula and either a re-entrant cannula at the proximal duodenum or a re-entrant cannula at the terminal ileum were given twice daily 480-520 g dry matter as fresh perennial ryegrass, or white or red clover. Flows of digesta were corrected to 100% recovery of chronic oxide. 2. The quantities (g/24 h) of zince and cobalt leaving the stomach were significantly greater than those in the food. No significant change was found in the quantities of copper and manganese. Significantly smaller quantities of Co (all three diets) and Zn (all diets except red clover) left the small intestine than those which entered this region. No significant differences in the quantities of Cu and Mn entering and leaving the small intestine were found. Significantly smaller quantities of Zn, Co, Cu and Mn were excreted in the faeces than entered the large intestine. 3. From the flow results it was determined that there was a significant net secretion of Zn and Co in the stomach, and a significant net absorption of Zn (except with the red-clover diet) and Co from the small intestine, and of Zp Co Cu and Mn from the large intestine.

[Food conversion in the digestive system of juvenile bulls. 3. Rumen liquor investigations].

Two bulls with a permanent rumen fistula and a re-entrant cannula at the duodenum were fed various concentrates and straw (dry matter ration composition: 60% concentrate, 40% straw). Negative correlations were found between the pH value and total fatty acid concentration (r = -0.70). The animals were given the straw one hour before the concentrate. The pH value increased to not more than 7.3 one hour after they had received the straw. It did not drop below 6.4 after the concentrate with any of the rations. The addition of dried sugar beet pulp to the rations leads to a higher mean acetate concentration than the other rations and causes an unvarying high total fatty acid concentration.
