Remodelling in slipped capital femoral epiphysis.

In-situ pinning of a slipped capital femoral epiphysis is the most common form of treatment for this problem. To find out if remodelling of the upper femur after pinning occurs often enough to justify this form of treatment and whether secondary osteotomies are required for residual deformity, 82 patients with slipped capital femoral epiphyses were reviewed. X-ray films were studied sequentially, noting remodelling changes in the femoral head and neck. Sixty-eight percent of moderate to severe slips that could be properly assessed showed signs of remodelling. The authors recognized two distinct remodelling processes--one, occurring early, affected the neck adjacent to the growth plate, the second appeared later, more distally along the femoral neck. Very few osteotomies were done for residual deformity. Pinning in situ of a slipped capital femoral epiphysis represents a simple, rapid and effective method of treatment, allowing remodelling even in more severe slips.

An osteoid osteoma in an open distal femoral epiphysis. A case report.

An osteoid osteoma occurred in the distal femoral epiphysis of an 11-year-old boy. Only eight other cases of this tumor in an open epiphysis have been reported. An image intensifier was used to place the tip of a wire in the center lesion. Removal through a small incision with a bone trephine placed over a guiding Kirschner wire was curative. Bone peripheral to the lesion was replaced, allowing early mobilization without support.

The association of femoral retroversion with slipped capital femoral epiphysis.

We examined twenty-five patients who had a unilateral or bilateral slip of the capital femoral epiphysis and determined the degree of anteversion of the thirty-nine involved hips with computerized axial tomography. Thirteen patients (eighteen hips) were seen at the time of the original diagnosis (Group I), and twelve patients (twenty-one hips) were seen one to seven years after operative treatment (Group II). The mean amount of anteversion for all of the involved hips was +1.0 +/- 8.2 degrees. The mean amount of anteversion for the Group-I hips was -0.7 +/- 7.4 degrees and the mean amount for the Group-II hips was 2.5 +/- 8.7 degrees. The mean amount of anteversion for the hips in both Groups I and II was less than the predicted mean amount for individuals of the same age. The mean amount of anteversion of the unaffected hips of our patients who had a unilateral slip was +6.3 +/- 8.2 degrees. The amount of internal rotation of the hip in extension exceeded the amount when it was in flexion in all of the patients. A decreased angle of femoral anteversion appears to be specifically associated with the development of slipped capital femoral epiphysis. The mechanical forces that act across the proximal femoral physis may be altered by this rotational abnormality, and this may lead to an increased shear stress that ultimately causes failure of the growth plate.

Slipped capital femoral epiphysis in southern Sweden. Long-term results after femoral neck osteotomy.

Opinions differ concerning the treatment of choice for severe slipped capital femoral epiphysis, probably due to the lack of long-term follow-up evaluations on the different methods of treatment. A series of 33 patients with severe slipped capital femoral epiphysis, treated per primam with wedge osteotomy of the femoral neck, were radiographically and clinically reexamined an average of 28 years (range, 16-32 years) after the operation. Segmental collapse and/or chondrolysis developed in ten patients. Nine of these patients were available for reexamination and all had severe arthrosis with poor function. Arthrosis developed in nine of the 19 patients without signs of segmental collapse or chondrolysis; these patients had a satisfactory joint function. This series was compared with another series (from the same orthopedic departments) of patients with severe slip without any primary treatment. The long-term results in these two groups were similar; consequently, the value of realignment by wedge osteotomy of the femoral neck is questionable.

[Anatomo-histologic characteristics of the epiphysis of the female silver fox outside the breeding season].

The epiphysis of mature relatively wild silver foxes of various age living in the fur-farming farm have been studied during November--December. The organ mass (2.72 +/- 0.14 mg) has rather essential individual differences and its statistically significant changes with age are not revealed. There are not any connective septa deviding parenchyma of the organ into lobules. There is also not any definite division between the cortex and medulla. Under all histological methods of treatment the borders of the parenchymal cells are poorly defined. Nuclei of the pinealocytes can be divided into light and dark according to their coloration. The average diameter of the nuclei in the pinealocytes does not practically change with age, nor does the linear density of cellular elements arrangement. A definite individual variability is noted in amount of the cells including secretory granules. The phenomena mentioned demonstrate that the fox epiphysis maintains its constant level of physiological activity during the whole reproductive period of life.

Fractures of the proximal tibial epiphysis.

Fractures of the proximal tibial epiphysis are rare. A series of twenty-eight fractures classified according to the Salter-Harris method showed that nine were Type II and eight, Type IV. Lawn-mower injuries, a previously unreported mode of injury for this fracture, caused five of the eight Type-IV fractures and were associated with the worst prognosis by far. Two Type-V fractures in the proximal tibial epiphysis, previously unreported, are described.

Little league shoulder: osteochondrosis of the proximal humeral epiphysis in boy baseball pitchers.

Roentgenographic changes consistent with osteochondrosis of the proximal humeral epiphysis were observed in five young baseball pitchers complaining of shoulder pain in the throwing arm. The symptoms and findings were quite similar to the previously reported involvement of the medial epicondylar epiphysis or "Little Leaguer's elbow."The act of throwing a baseball hard is an abnormal whip-like action which places a forceful repetitious traction strain on the shoulder joint. Shoulder pain in youngsters engaged in organized competitive swimming programs can also be explained in this way. Since these entities became evident with the establishment of organized baseball programs for boys in this age group, better medical supervision and rule changes to limit the amount of pitching until the epiphyses close, are urgently needed.

[Effect of deficiency and excess of thyroid hormones in the body on indolamine metabolism in the rat epiphysis cerebri].

Thyroid hormones were demonstrated to play a role in serotonin metabolism control in pinealocytes. Saturation of the body with T4 shifts the process of serotonin utilization toward acetylation and subsequent ortho-methylation with primary formation of N-acetylserotonin and melatonin while total thyroidectomy blocks this process to a considerable degree. Under deficiency of thyroid hormones the pathway of oxidative serotonin deamination with primary formation of 5-hydroxyindole-acetic acid is the predominant one. Excess and deficiency of thyroid hormones produce opposite effects in the epiphysis. The data obtained not only support the presence of the negative feed-back in the epiphysis-thyroid system but also emphasize that the process of formation of epiphyseal methoxyindoles is the most sensitive one to the action of thyroid hormones.

Complications after cuneiform osteotomy for moderately or severely slipped capital femoral epiphysis.

Wedge osteotomy of the femoral neck for slipped capital femoral epiphysis was performed in seventy-seven hips of seventy-one children at Gillette Children's Hospital during the period 1938 to 1973. The major complications were avascular necrosis (28.5 per cent) and cartilage necrosis (37.6 per cent). This high incidence of severe complications led to the use of an osteotomy through the base of the neck for the treatment of severely slipped capital femoral epiphysis which has given satisfactory results in six hips to date.

Cuneiform osteotomy of the femoral neck in the treatment of slipped capital femoral epiphysis.

I reviewed the cases of eighty-two patients (106 hips) with slipped capital femoral epiphysis who were treated during a twenty-four-year period. Of these, forty-two hips had a sufficiently severe displacement to require surgical correction by means of a cuneiform osteotomy of the neck of the femur just distal to the physis. The purpose was to restore the normal anatomical relationship of the proximal capital femoral epiphysis to the neck of the femur. Follow-up of these patients ranged from two to twenty-two years, with an average of nine years and nine months. Aseptic necrosis developed in one femoral head and osteoarthritis developed in one hip. The remaining forty hips were graded as having an excellent result.

Slipped epiphysis associated with hypothyroidism.

Seven cases of slipped capital femoral epiphysis appearing concurrently with juvenile hypothyroidism are reported. This association seems to occur more commonly than the general incidence of slipped capital femoral epiphysis or of juvenile hypothyroidism would suggest, and the thyroid hormone deficiency and a diminished strength of the epiphyseal growth plate of the upper femur appear to be associated. Although concrete scientific data are as yet not available to define clearly the effect of thyroid hormone on the growth plate, the possible pathophysiology is discussed in light of available data.

[Role of polypeptide factors of the hypothalamus and epiphysis in regulating the function of stem cell precursors of granulomonopoiesis].

The claster- and colony-formation capacity of the offls--precursors of granulomonopoiesis in 29 normal subjects was studied in a bilayer agar system. Parallel cultures were used to investigate the effects of polypeptide factors of the hypothalamus and epiphysis. It was found that polypeptide factors of the hypothalamus and epiphysis have a regulatory influence on granulomonopoiesis at the level of precursor cells.

[Electron microscopic study of the epiphysis in the hen in different age periods].

Electron microscopic investigation of the epiphysis in 1 and 2 years old hens revealed that pineocytes are rich in mitochondria, ribosomes, granular endoplasmic network, Golgi apparatus, synapses. In contrast to 1-year hens, large amounts of lipofuscin granules and lysosomes were found in older animals. The data obtained reveal intensive hormone formation and oxidation-reduction processes in the epiphysis of birds.

Fractures of the distal tibial epiphysis in adolescence.

We studied the cases of nine adolescent patients with a Salter-Harris Type-III fracture of the lateral portion of the distal tibial epiphysis (sometimes called the juvenile Tillaux fracture) and eight patients with a triplane fracture of the distal tibial epiphysis. The mechanism of both injuries was postulated to be external rotation of the foot on the leg. There were both two-fragment and three-fragment triplane fractures in the series. The three-fragment triplane fractures occurred in younger patients, and surgical treatment was more frequently administered. The pattern of closure of the distal tibial growth plate in the adolescent strongly influenced the type of fracture.

Slipped capital femoral epiphysis following radiation and chemotherapy.

Patients who received radiation to the proximal femoral epiphysis and chemotherapy in childhood appear to have an increased risk of subsequently developing epiphyseolysis. Every effort should be made to exclude the proximal femoral epiphysis from the radiation port whenever possible. If the epiphyseal plate is widened and irregular and the adjacent bone is sclerotic, prophylactic pinning may be indicated even in the absence of a grossly visible slip.

Slipped capital femoral epiphysis in a 35-year-old man (a case report).

This is a case report of a 35-year-old man with multiple open epiphyses and a slipping of the right capital femoral epiphysis. An endocrine evaluation revealed primary hypothyroidism. The patient was placed on hormonal therapy and an in situ hip pinning was performed to prevent further slip. The operation was successful. The symptoms disappeared and all of the epiphyses were closed within one year. In an adult patient presenting with slipped capital femoral epiphysis, endocrine dysfunction such as hypothyroidism should be suspected.

Fracture-separation of the distal humeral epiphysis.

Sixteen patients were seen with fracture-separation of the distal humeral epiphysis. The difficulty that may be encountered in making the diagnosis is thought to be partly responsible for the relative paucity of previous reports of this condition. All sixteen patients revealed posteromedial displacement of the distal humeral epiphysis on the initial roentgenogram. Child-abuse was documented or suspected as a cause in six of the sixteen fractures. Treatment consisted of closed reduction and immobilization with the elbow in 90 degrees of flexion and the forearm pronated, for three weeks. Post-injury cubitus varus, which developed in three patients and was thought to be due to inadequate reduction, did not progress.
