Liver carcinoma and liver fluke infection.

The neoplastic transformation of bile duct changes induced by infections due to Clonorchis sinensis or Opisthorchis viverrini has been persistently debated because of the difficulty to establish the causative relation between those trematode infections and the development of primary carcinoma of the liver (PCAL) which has been found in Hong Kong, China, Korea and Thailand. The following observations, however, clearly support the hypothesis that clonorchiasis and opisthorchiasis are basic etiological factors of the pathogenesis of human cholangio-carcinoma (CC): C. sinensis and O. viverrini infections lead to quite similar tissue reactions in bile ducts, starting with desquamation of epithelial linings with subsequent development of adenomatous hyperplasia and periductal fibrosis besides inflammatory cellular exudation and goblet cell metaplasia in both man and experimental animals. The majority of PCAL are of the hepatocellular type but as to CC, the prevalence rate is much higher in those areas where C. sinensis and O. viverrini infections are endemic. Histologically, the neoplastic transformation from adenomatous hyperplasia in bile ducts to CC through dysplastic changes of lining cells or from cholangiofibrosis is clearly demonstrable. Furthermore, mucin-producing activity is also a frequent feature to reflect the neoplastic transformation of the metaplastic goblet cell change of the bile duct linings. Application of various carcinogens to liver fluke infected animals cause a higher incidence of CC than is the case in non-infected controls. These epidemiological and histopathologic characteristics seem to indicate that C. sinensis and O. viverrini are predisposing conditions for the pathogenesis of CC in Southeast and East Asian countries.

[Hepatic calcification in children. Review apropos of 4 personal cases].

The authors describe the main patterns of the calcifications of the liver and present the different causes which are numerous. Value of the radiological procedures, mainly ultrasound and computer tomography is emphasized. They are necessary for the diagnosis of the type of calcifications and of the disease of the patient.

Peritoneoscopy in a developing country.

Peritoneoscopy in medical cases remains largely unpopular in the USA and the UK. In many developing countries there is often a heavy demand on medical facilities and the rapid diagnosis of intra-abdominal problems is of major importance. We present a series of 193 patients in whom peritoneoscopy was carried out during a 2 1/2-year period. Eighty-six per cent (166) of examinations yielded information of positive diagnostic value. The most common diagnoses were hepatocellular carcinoma, liver cirrhosis, bilharzial fibrosis and peritoneal tuberculosis. The peritoneoscopic appearances in the various lesions are illustrated and discussed. Peritoneoscopy is a safe procedure and its value in a developing country is great, both in terms of economy of man-hours employed and, more particularly, of diagnostic yield.

[Liver surgery in Switzerland].

In 1981, the surgical departments of the 5 Universities of Switzerland and 5 departments of Cantonal Hospitals and City Hospitals performed a total of 77 liver resections. The most prominent indication was liver metastases (especially from colorectal carcinomas), and lobectomy (hemihepatectomy) was the most commonly performed operative procedure. These data are in agreement with the experience of Zurich University Hospital from 1961-1981: 26 out of 45 liver resections were indicated by liver metastases. The overall postoperative lethality was 6.6%; lethality for resection of metastases was zero.

[ELISA in schistosomiasis. Limits. Possibility of application (author's transl)].

Sera from patients with schistosomiasis and various infections were examined by an enzyme-linked immunosorbant assay technique (ELISA) using soluble antigens prepared from adult worms and eggs of Schistosoma mansoni. Marked false positive reactions were observed in cases of certain parasitic (hydatidosis) and non parasitic diseases (liver cirrhosis). Equivalent results with the techniques of immunofluorescence and immunoenzymology (done on adult sections) were obtained with adult worm antigen read at a higher optic-density limit. At present, this technique might be useful in seroepidemiological surveys; however, further purification of the antigen will increase its sensibility and specificity.

[Hepatic schistosomiasis. X-ray computed tomographic aspects. Apropos of 97 cases].

Ninety seven patients with liver cirrhosis of schistosomiasis were collected. Its CT findings and features were analysed on the clinicopathological basis including (1) the high percentage of the hepatomegaly and of the enlargement of the left lobe; (2) occurrence of deposition and calcification of eggs in large bowel wall (5/5), the portal vein system (22/97), the root of the mesentery (9/97) and the liver (93/97); (3) the variety and characteristics of the appearances of the intrahepatic calcification; (4) widened and fibrotic changes in the portal canal region. It was also noted that high percentage of hepatocellular carcinoma associated with this disease in the present series (25/97).

Duodenal and hepatic toxoplasmosis in a patient with HIV infection: review of the literature.

We report a case of watery diarrhea due to duodenal toxoplasmosis in a patient with the acquired immunodeficiency syndrome. Treatment with pyrimethamine, clindamycin, and folinic acid decreased the diarrhea as well as the duodenal toxoplasma cyst load. Hepatic toxoplasmosis was also present, associated with an elevated serum alkaline phosphatase activity and a minimally elevated lactate dehydrogenase level.
