[Treatment of pseudarthrosis of the tibia by the method of traction].

The report is based on the experience with treatment, using the method of dosaged distraction of 39 pseudarthroses and unhealing fractures of the tibia in 38 patients. Twenty three patients showed osteomyelitis, 32-shortening of crural bones and 27-their deformation. With the aim of distraction the G. A. Ilizarov apparatus was employed. Up to data, the treatment is termined in all 38 patients. Healing was gained after 36 operations in 35 patients. No union was obtained after 3 operations of distraction the osteosynthesis in avascular forms of pseudarthrosis. Some regularities in formation of the regenerate and its reoconstruction during the process of lasting fixation were studied in 25 patients.

Treatment of rigid hypertrophic posttraumatic pseudarthrosis of the tibia in children using distraction osteogenesis.

Posttraumatic tibial pseudarthrosis is a relatively uncommon complication of tibia fractures in children. Although reported as a successful method of treating tibial nonunions in adults, distraction osteogenesis through a nonunion site via ring external fixation has not been described in children. The authors report three consecutive cases of distraction through an angulated, shortened, hypertrophic, posttraumatic nonunion to achieve successful union and concurrent correction of deformity. Distraction was carried out using a ring fixator with computer-guided correction. Union was achieved in each of the three patients with complete correction of deformity and length. Lengthening of 8 to 31 mm was achieved. The length of time in the external fixator ranged from 7 to 27 weeks. Pin tract infections developed in all patients; they were treated successfully using oral antibiotics. No deep infections or other significant complications developed. Each patient was followed up for at least 1 year.

A retrospective review of congenital pseudarthrosis of the tibia.

Thirty-three children with congenital pseudarthrosis of the tibia seen from 1970 to 1997 at the Shriners Hospitals for Children in Lexington, KY were retrospectively reviewed. Twelve children were excluded due to follow-up of less than 2 years. Data was collected on age, sex, side and associated neurofibromatosis from the medical records of the remaining 21 children. Lesions were classified as to status at birth. The number of surgical attempts for union was recorded. Presence of bony union, deformity, and limb length discrepancy was noted. The mean age at last follow-up was 13 years. Union was reported in 10 children (48%). There was no union after more than 3 surgical attempts. The 11 children without union had an average of 4 surgical attempts (range 2-6). Amputation was performed in 7 of the 11 children without union. No predictor for nonunion was identified. Amputation was frequently needed.

[Treatment of pseudarthrosis of the lower third of the femur].

The results of surgical for pseudarthrosis of the lower third of the femur in 11 patients, using a modified King technic, are reported. To improve fixation of a distal fragment the authors suggest to employ two CITO pins with diverging ends at the level of femoral condyles with an additional compression by two rings of the Ilizarov apparatus. Plaster cast immobilization was not utilized. This method of fragments fixation provides an early motion of the knee joint. Roentgenologically and clinically the consolidation of fragments was noted after 4--5 months, the knee joint function being restored.

The transarticular graft for infantile pseudarthrosis of the tibia. A new technique.

Fourteen cases of pseudarthrosis of the tibia in childhood presented at a hospital in Burma over a period of eight years. The ages of the patients ranged from one month to seventeen years. Nine were treated by a pointed graft driven into the medullary cavity of the distal tibia, and usually across the ankle joint into the body of the talus, before fixation to the proximal tibia. In six of the nine union was secured, but one case required a second grafting. Transarticular segments of graft showed a marked tendency to undergo absorption. No significant deformity was observed to follow central penetration of the growth plate and epiphysis of the lower tibia.

Pseudarthrosis and pseudopseudarthrosis of ankylosing spondylitis-report of 2 cases.

Pseudarthrosis is a well known, late complication of ankylosing spondylitis associated with extensive discovertebral junction destruction and fracture of the posterior elements. In contrast, the term "Pseudopseudarthrosis" is published in the recent literature to portray similar roentgenologic findings of discovertebral destruction but without bony break of the posterior arch. In this article, each complication is presented by a single case using the demonstration of plain film and computed tomography studies to compare their differences in the posterior spinal elements.

Congenital pseudarthrosis.

Five cases of congenital pseudarthrosis of the clavicle were treated with open reduction and fixation with appropriate-sized contoured plates and screws. Iliac bone grafts were used to bridge the gap in four patients; local bone graft was used in the fifth patient. Consolidation was noted an average of 3 months postoperatively in all five cases. The plate and screws were removed in all patients at an average of 16 months postoperatively. Average length of follow-up for the five patients is 4 years. Roentgenograms demonstrate that the clavicle has continued to grow normally. All five patients have pain-free full range of motion and are engaged in unrestricted activities.

Use of an intramedullary rod for the treatment of congenital pseudarthrosis of the tibia.

The use of an intramedullary rod as described by Williams, combined with implantation of an autogenous bone graft, resulted in union of an established congenital pseudarthrosis of the tibia in nine of ten patients. One patient needed additional bone-grafting before union occurred. The average age at the time of the operation was five years and three months. A rod of the appropriate length was inserted at the site of the non-union, antegrade through the distal part of the tibia and the hindfoot and then retrograde through the proximal fragment. This resulted in splinting of the tibia, ankle, and subtalar joints. Solid osseous union occurred an average of six months after the procedure in all ten patients. Five patients had a refracture of the tibia after the initial consolidation. Three of the five needed one or more additional operative procedures; one was managed with a cast; and one patient, who had been followed for four years before the refracture, did not return for treatment of the refracture. As is the plan with this method of treatment, the distal part of the tibia grew off the rod and the distal tip of the rod was located proximal to the foot and ankle, or it was located more proximally than it had been at the operation, in six patients. The rod was removed from three patients. At an average of six years, all ten patients were able to walk without pain.

[Pseudarthrosis of the humeral shaft. Apropos of 58 cases].

We report 58 cases of humeral shaft non union following 28 times an orthopaedic treatment where persisted a space between the two fragments and 30 times an osteosynthesis but only 6 where satisfying. The sery contains 2 infected non union, dried in several months and 3 pseudarthrosis we did not operate because they were not inconvenienced. Among the 53 aseptic pseudoarthrosis the union has been to obtained after one operation for 42, two operations for 6 and three operations for 5. We analyse the consecutive mistakes having brought to the failures. We confirm that only an osteosynthesis not sufficient and that an associate graft is necessary. It could be an osteoperiosteum decortication, a spongious graft or a tibial graft (this last one is more safe but reserved to the iterative cases of the one with a big defect). The long evolution (more than a year for half at the cases) explains the fragment repercussions on shoulder or elbow.

[Considerations concerning the treatment of pseudarthrosis of the humerus].

A humeral shaft fracture results in a nonunion in less than 10% of cases, regardless of the technique used. The treatment of such cases must be different from the initial treatment. In a pseudarthrosis of the medial shaft of the humerus with a transverse or short oblique line, a suitable procedure is medullary filling with Hackethal nails or a Küntscher nail. If the fracture line is long and oblique, spiral, fragmented or distant from the narrow part of the humeral shaft, a modified interlocking intramedullary nail is recommended.

[Internal osteosynthesis in pseudarthrosis without inter-fracture tissue dissection].

UNLABELLED: In the paper is presented the experience of operative treatment of 149 patients with false joints of bones, which were subjected to internal osteosynthesis with preservation of inter-fracture tissues. The operation was carried out in the absence of real danger of suppuration. The most frequently operated were the patients with tibia pseudarthrosis (72.5%), fixed by means of a plate. Adhesion has been achieved with 96.7% of patients. COMPLICATIONS: osteomyelitis (1), skin necrosis (2), fractured fragment dislocation under the action of contractor compression application (2). In the process of treatment of 439 patients by means of internal osteosynthesis with dissection of inter-fracture tissues, separation of bone fractured fragments and medullar cavity opening, adhesion has been achieved with 87.2% of patients.

[Congenital crural pseudarthrosis in a hemophiliac].

A case of congenital crural pseudarthrosis is reported in which three operations were done with full success. Mild haemophilia was recognized at the age of 11 years, after completion of surgical treatment.

Transscaphoid perilunate fracture dislocation and pseudarthrosis of the scaphoid.

The results of conservative treatment of a perilunate fracture dislocation were retrospectively studied. In six patients, the treatment consisted of closed reduction and immobilization in a below the elbow cast for 12 to 16 weeks. Five patients had a compromised healing of the scaphoid fracture. The frail vascularization of the scaphoid, extensive ligamentous injury and possible mid-carpal instability in case of perilunate fracture dislocation as well as a non-anatomical reduction of a scaphoid fracture are causative factors in ensuing scaphoid pseudarthrosis. If anatomical reduction of a perilunate fracture dislocation cannot be obtained, open reduction and internal fixation should be considered. This will offer the scaphoid the best chance at fracture healing and will restore the stabilizing function on the mid carpus.

[Fractured Monteggia pseudarthrosis. An unusual case].

The diagnosis of an ulnar fracture is easily made in Monteggia lesions, but dislocation of the radial head can occasionally be overlooked. In the case presented luxation persisted and ulnar pseudarthrosis developed. In spite of functional disorders, however, the patient still managed to work for 36 years as a lorry driver. An unusual feature of this case is that 40 years after the primary event traumatic fracture of the pseudarthrotic ulna developed and was managed by bone grafting, with good functional results.

Vascularized fibular grafts in the treatment of congenital pseudarthrosis of the tibia.

Free vascularized fibular bone grafts were used in nineteen children, seen consecutively, who had congenital pseudarthrosis of the tibia. The average age was 5.1 years (range, 1.4 to 11.4 years). Sixteen of the patients had been treated with electrical stimulation for at least one year, and the tibia had not united. All but four patients had had at least one previous operative procedure. At an average follow-up of 6.3 years (range, 2.0 to 11.0 years), eighteen (95 per cent) of the nineteen pseudarthroses had healed. The leg-length discrepancy averaged 1.6 centimeters (range, 0 to 4.0 centimeters), but ten tibiae had residual or progressive valgus or anteroposterior malalignment despite bracing. There was minimum morbidity at the donor site.
