Visual field progression: comparison of Humphrey Statpac2 and pointwise linear regression analysis.

BACKGROUND: Humphrey Statpac2 "glaucoma change probability analysis' is a widely available analysis technique to aid the clinician in the diagnosis of glaucomatous visual field deterioration. A comparison of this technique with the more recently described pointwise linear regression analysis (PROGRESSOR) is given. METHODS: Series of visual field data from a group of nine eyes of nine patients with normal-tension glaucoma were selected. Each series had 16 fields with mean follow-up of 5.7 years (SD 0.6 years). Statpac2 "glaucoma change probability analysis' was used to define test locations that had unequivocally deteriorated in the last three fields of each series. The accuracy of both Statpac2 and PROGRESSOR in providing early detection of these deteriorated locations was assessed. RESULTS: The sensitivity and specificity of the two techniques in predicting deteriorated locations were similar when a rate of luminance sensitivity loss of faster than 1 dB/year (2 dB/year for outer locations beyond 15 deg of eccentricity) with a slope significance of P < 0.10 was used as the regression definition of deterioration. The difficulties of comparing two techniques in the early diagnosis of field progression without a true external standard for field loss are illustrated. CONCLUSIONS: PROGRESSOR closely emulates the performance of Statpac2 in detecting sensitivity deterioration at individual test locations. This new technique, which uses all available data in a field series and gives the rate of sensitivity loss at each location, may provide a clinically useful method for detecting field progression in glaucoma.

Regression analysis of recent changes in cardiovascular morbidity and mortality in The Netherlands.

OBJECTIVES: To test whether recent declines in mortality from coronary heart disease were associated with increased mortality from other cardiovascular diseases. DESIGN: Poisson regression analysis of national data on causes of death and hospital discharges. SETTING AND SUBJECTS: Population of the Netherlands, 1969-93. MAIN OUTCOME MEASURES: Annual changes in mortality from coronary heart disease, stroke, and other cardiovascular diseases and annual changes in hospital discharge rates for acute coronary events, stroke, and congestive heart failures. RESULTS: Patterns of cardiovascular mortality changed abruptly in 1987-93. Annual decline in mortality from coronary heart disease increased sharply for women and men: from -1.9% (95% confidence interval -2.2% to -1.6%) and -1.7% (-1.9% to -1.4%) respectively in 1979-86 to -3.1% (-3.5% to -2.6%) and -4.2% (-4.6% to -3.9%) in 1987-93. The longstanding decline in mortality from stroke levelled off: from annual change of -3.3% (-3.7% to -2.8%) and -3.2% (-3.7% to -2.8%) in 1979-86 to -0.1% (-0.7% to 0.4%) and -1.1% (-1.7% to -0.5%) in 1987-93. Mortality from other cardiovascular diseases, however, started to increase: from -2.0% (-2.4% to -1.6%) and -0.2% (-0.5% to 0.2%) in 1979-86 to 1.5% (1.0% to 2.0%) and 1.9% (1.5% to 2.3%) in 1987-93. Hospital discharge rates for acute coronary heart disease, congestive heart failure, and stroke increased during 1980-6. During 1987-93 discharge rates for stroke and coronary heart disease stabilised but rates for congestive heart failure increased. CONCLUSION: Improved management of coronary heart disease seems to have reduced mortality, but some of the gains are lost to deaths from stroke and other cardiovascular diseases. The increasing numbers of patients with coronary heart disease who survive will increase demands on health services for long term care.

Electronic spreadsheet program for estimating two-compartment intravenous pharmacokinetic parameters by least squares linear regression analysis.

A microcomputer program using an electronic spreadsheet program was developed to calculate pharmacokinetic values of thiacetarsamide sodium, a drug used to kill the adult heartworm (Dirofilaria immitis) parasite of the dog. A least squares, semilogarithmic regression analysis was done on data using a two compartment intravenous model. The data entered includes the time after injection and the drug concentration at that time. A graph of the points can be viewed or plotted, with the time on the x-axis, and the natural log of the drug concentration on the y-axis. The program was developed on a Televideo (512 kbytes) microcomputer. The spreadsheet used was Lotus 123 with the pharmacokinetics program occupying 16 kbytes of memory (720 cells) out of the 400 kbytes available with Lotus.

Regression analysis of catecholamine utilization in discrete hypothalamic and forebrain regions of the male rat: effects of thyroidectomy.

The effects of thyroidectomy (4 weeks) on dopamine (DA) and noradrenaline (NA) turnover rates were determined by means of regression analysis. The disappearance of catecholamine (CA) fluorescence (using quantitative histofluorimetry) after tyrosine hydroxylase inhibition (alpha-methyl-DL-p-tyrosine methyl ester) has been investigated in discrete hypothalamic and forebrain DA and NA nerve terminal systems of the male rat. A time-dependent monophasic CA fluorescence disappearance was observed in all CA nerve terminal systems of the sham-operated and thyroidectomized rats. In the thyroidectomized rat, DA turnover in the anterior nucleus accumbens and in the medial and lateral palisade zones of the median eminence (ME) was reduced while DA turnover in the posterior nucleus accumbens was increased as compared to control rats. Furthermore, NA turnover was increased in the paraventricular hypothalamic nucleus (PA) and reduced in the dorsomedial hypothalamic nucleus (DM) and in the 'border zone' (lateral hypothalamus). Radioimmunoassay of hormones in serum demonstrated marked increases in TSH levels and reduced concentrations of GH, prolactin, corticosterone, triiodothyronine and thyroxine. The reduced DA turnover in the external layer of the ME and the increased NA turnover in the PA may indicate an inhibitory dopaminergic mechanism in the ME and a facilitatory noradrenergic mechanism in the PA in the regulation of TSH secretion. These mechanisms seem to interact with thyroid hormones. The reduced NA turnover demonstrated in the DM and in the border zone may be related to the lowering of growth hormone levels and pulsatility caused by thyroidectomy. Finally, the DA nerve terminal systems in the anterior and posterior parts of the nucleus accumbens are differently regulated by changes in the brain-pituitary-thyroid axis.

[Regression analysis of corneal endothelium after nonmechanical penetrating keratoplasty].

PURPOSE: The corneal endothelial cell density is a crucial parameter for the pump function and the transparency of grafts after penetrating keratoplasty (PK). The purpose of this study was to assess corneal endothelial cell density with different regression models after nonmechanical penetrating keratoplasty and to check for differences between diagnoses and two different storage methods. PATIENTS AND METHODS: Two-hundred ninety-six eyes (195 keratoconus, 101 Fuchs' dystrophies, 148 each with short-term preserved and organ-cultured donor corneas) of 268 patients were included in this prospective study. Donor and recipient trephination was performed using nonmechanical trephination technique with the excimer laser 193 nm along metal aperture masks from the epithelial side. The time course of the endothelial cell density (specular microscope EM 1100, TOMEY, Erlangen) after PK was assessed. Endothelial cell density was first analyzed in a cross sectional manner at the 3, 6, 12, 18 and 24 months follow-up and, secondly in a longitudinal manner with linear, polynomial and exponential regression models in the sense of minimizing the residuum (distance between observed and predicted endothelial cell count). The mean donor postmortem time was 10.3 +/- 6.8 hours for short-term-preserved and 19.6 +/- 9.5 hours for organ-cultured corneas (p < 0.0001). The storage time was 63 +/- 49 and 19 +/- 7 days (p < 0.0001), respectively. RESULTS: In a cross section, overall mean endothelial cell density decreased from 2145 +/- 599 cells/mm2 at the 3 months to 1751 +/- 605 cells/mm2 at 2 years follow-up (p > 0.05). Cell density did not differ significantly between different diagnoses or storage methods at any postoperative stage. In a longitudinal section, the linear regression model estimated an annual decrease of 214 cells/mm2. In a polynomial model the decrease expressed by a tangent to the regression line at 24 months was 175 cells/mm2. The exponential regression model yielded a relative decrease of 9.5% annually. The so-called residuum as a measure for the validity of the regression model was maximal in the linear and minimal in the exponential estimate. With keratoconus and short-term preserved donor material the endothelial cell loss was less in the regression analysis. CONCLUSION: During the first two years after nonmechanical trephination in PK, a non-significant decrease in endothelial cell density was observed. The exponential regression model seems to be predestinated for analysis of the time course of corneal endothelium in a longitudinal manner. The annual cell loss ranged around 9.5% without significant differences between diagnoses and storage methods.

Prognostic factors in advanced nonseminomatous testicular cancer. A multivariate logistic regression analysis.

In order to define prognostic factors for advanced stage of nonseminomatous germ cell tumors (NSGCT) of the testis, the authors reviewed 84 patients treated from 1978 through 1985. The survival rate was 51% at 3 years. Patients with elevated seric levels of human chorionic gonadotropin (HCG) and/or alpha-fetoprotein (AFP), or the presence of an abdominal mass had significantly worse survival. Only HCG and AFP levels retained their significance when multivariate Cox analysis was performed. The probability that a patient achieves a complete remission (CR) was assessed by a function of certain patient characteristics using a multivariate logistic regression analysis. The significant variables were a function of HCG and AFP values. Since both variables are related to the CR rate and survival the authors define the obtention of a CR as a unique outcome of interest. The probability of a CR greater than 70% adequately separates the patients into two prognostic subgroups. This model currently is being used to enrole NSGCT patients in a prospective modulated clinical trial according to these prognostic factors.

Prognosis of acute acoustic trauma: a retrospective study using multiple logistic regression analysis.

OBJECTIVE: The prognosis for acute acoustic trauma (AAT) is difficult to predict. Multiple logistic regression analysis was used in this study to determine which factors were most strongly related to outcome for patients with AAT. METHODS: The study group was comprised of 52 patients (52 ears). Separate analyses were performed on the no change and partial recovery groups, and on the no change and full recovery groups. The following eight factors were examined as explanatory variables, age; number of days before the start of treatment; use of earplugs; drug therapy (adrenocortical hormones, low molecular weight dextrans, and vitamin B12), mean hearing levels at 0.5, 1, and 2 kHz (pure tone average, PTA), and mean hearing levels at 4 and 8 kHz (high tone average, HTA). RESULTS: Factors determined to be most strongly related to outcome were the number of days before the start of treatment, PTA, and HTA. No significant relation to outcome was determined for the five following factors, age; use of earplugs; and drug therapy (adrenocortical hormones, low molecular weight dextrans, and vitamin B12). CONCLUSION: Our results will increase the ability to prognosticate the outcome for AAT.

Reliability and uncertainty in the estimation of pKa by least squares nonlinear regression analysis of multiwavelength spectrophotometric pH titration data.

When drugs are poorly soluble then, instead of the potentiometric determination of dissociation constants, pH-spectrophotometric titration can be used along with nonlinear regression of the absorbance response surface data. Generally, regression models are extremely useful for extracting the essential features from a multiwavelength set of data. Regression diagnostics represent procedures for examining the regression triplet (data, model, method) in order to check (a) the data quality for a proposed model; (b) the model quality for a given set of data; and (c) that all of the assumptions used for least squares hold. In the interactive, PC-assisted diagnosis of data, models and estimation methods, the examination of data quality involves the detection of influential points, outliers and high leverages, that cause many problems when regression fitting the absorbance response hyperplane. All graphically oriented techniques are suitable for the rapid estimation of influential points. The reliability of the dissociation constants for the acid drug silybin may be proven with goodness-of-fit tests of the multiwavelength spectrophotometric pH-titration data. The uncertainty in the measurement of the pK (a) of a weak acid obtained by the least squares nonlinear regression analysis of absorption spectra is calculated. The procedure takes into account the drift in pH measurement, the drift in spectral measurement, and all of the drifts in analytical operations, as well as the relative importance of each source of uncertainty. The most important source of uncertainty in the experimental set-up for the example is the uncertainty in the pH measurement. The influences of various sources of uncertainty on the accuracy and precision are discussed using the example of the mixed dissociation constants of silybin, obtained using the SQUAD(84) and SPECFIT/32 regression programs.

Correlating sensory attributes to gas chromatography-mass spectrometry profiles and e-nose responses using partial least squares regression analysis.

Sensory scores for 15 attributes identified in soy sauce aroma by quantitative descriptive analysis were correlated with purge and trap gas chromatography-mass spectrometry (GC-MS) profiles and electronic nose (e-nose) responses using partial least squares (PLS) regression analysis. Highly predictive PLS models were obtained for every attribute based on whole GC-MS profiles. However, the predictability has been greatly improved in the models calculated from 20 selected peaks that showed higher contribution to each attribute in the first PLS analysis. Contrarily, except for alcoholic and fishy notes, predictability of PLS models calculated from e-nose responses was poor. The correlation between GC-MS profiles and e-nose responses was unsatisfactory due to high similarity in sensor responses.

Regression analysis of visual field progression in low tension glaucoma.

Eighty four patients (168 eyes) with low tension glaucoma were retrospectively reviewed (mean follow-up was 28 months). The mean age was 66 years, and 69% were females. Regression analysis of their automated Humphrey fields (a mean of eight fields per eye) showed progression in 50% of patients and in 37% of eyes. There was no statistically significant difference between patients with progression and non-progression with respect to age (p less than 0.05) or intraocular pressure (p less than 0.5). Visual field defects were located most frequently in the superior hemifield in both groups of patients. A considerable proportion of patients had advanced field loss at the time of diagnosis.

Single-strain regression analysis for determination of interpretive breakpoints for cefoperazone disk diffusion susceptibility testing.

A novel approach for setting interpretive breakpoints in disk diffusion antibiotic susceptibility testing according to determined minimum inhibitory concentration (MIC) limits is described, using the method of single-strain regression analysis. The procedure was tested on reference strains Staphylococcus aureus (ATCC 25923), Streptococcus faecalis (ATCC 29212), Escherichia coli (ATCC 25922), and Pseudomonas aeruginosa (ATCC 27853), using published results from cefoperazone disk diffusion experiments. The correlation between logarithm of the disk content and inhibition zone diameter squared was linear, excluding three endpoint values. When constants A and B in the new regression line equation were calculated for the four strains, all four showed different regression lines. Zone diameters corresponding to various MICs were calculated for a disk content of 75 micrograms. The values obtained for the four strains were 20.1, 20.9, 24.9, and 25.8 mm, respectively, for an MIC of 16 micrograms/ml, and 15.7, 15.7, 22.3, and 17.9 mm, respectively, for an MIC of 64 micrograms/ml. The following zone diameter breakpoints were determined for the "I" (intermediate) category, using a 75-micrograms disk: S. aureus, 18 to 15 mm; S. faecalis, 23 to 13 mm; E. coli, 20 to 17 mm; and P. aeruginosa, 20 to 17 mm.

Regression analysis of relationship between liver weight and body weight after partial hepatectomy in rats.

A method for evaluation of the initial weight of the liver is developed for calculating the regeneration coefficient after partial hepatectomy on the basis of linear regression analysis. Experiments on rats showed that the model reflecting the relationship between liver weight and body weight for calculation of regeneration coefficient after partial resection of the liver expressed by the equation P0=0.033 x M can be used for screening of potential hepatoprotectors for quantitative evaluation of the regeneration processes.

A regression analysis of the Waterlow score in pressure ulcer risk assessment.

OBJECTIVE: To explore the predictive value of the Waterlow score, and the subscores of age and gender. DESIGN: Logistic regression analysis was conducted on the two subscores of the Waterlow score, and the residual Waterlow score with gender and age removed. Receiver operating characteristic (ROC) analysis gave a quantitative measure of the classification ability of the Waterlow score. SETTING: Burton, UK. SUBJECTS: All admissions over a five-year period to the District General Hospital, a total of 150,015 admissions of 82,691 patients. INTERVENTIONS: None. MAIN OUTCOME MEASURES: Area under the ROC curve for significant (as determined by logistic regression) variables. RESULTS: Data were inaccurate in at least 44.7% of the records, and analysis was conducted on the 43,735 records for which no errors were apparent. Nine hundred and fifty-four patients had a pressure ulcer on admission (2.1%); 277 developed a pressure ulcer (0.6%). The Waterlow score was predictive of pressure ulcers. Age was predictive, and gender was not found to be a significant predictor. CONCLUSIONS: The Waterlow score may be improved and simplified by removing gender from the scoring system.

Simulation of the 13C nuclear magnetic resonance spectra of trisaccharides using multiple linear regression analysis and neural networks.

Predictive models are developed for the 13C NMR chemical shifts of the carbon atoms comprising the central rings of 46 trisaccharide compounds. Thirty-nine trisaccharides are used as a training set for development of models using regression analysis and computational neural networks, and seven compounds are used as an external prediction set. The descriptors used in the models are developed directly from the molecular structures of the trisaccharides. Three different methods of descriptor selection are compared. The dependence of the models on the geometries of the trisaccharides is explored. The models developed with geometric descriptors are better than those developed without geometric descriptors, although the latter models are still of a comparable quality. Overall, the best model found is a neural network based on descriptors selected by multiple linear regression.

Factors that determine false recall: a multiple regression analysis.

In the Deese-Roediger-McDermott (DRM) paradigm, subjects study lists of words that are designed to elicit the recall of an associatively related critical item. The 55 lists we have developed provide levels of false recall ranging from .01 to .65, and understanding this variability should provide a key to understanding this memory illusion. Using a simultaneous multiple regression analysis, we assessed the contribution of seven factors in creating false recall of critical items in the DRM paradigm. This analysis accounted for approximately 68% of the variance in false recall, with two main predictors: associative connections from the study words to the critical item (r = +.73; semipartial r = +.60) and recallability of the lists (r = -.43; semipartial r = -.34). Taken together, the variance in false recall captured by these predictors accounted for 84% of the variance that can be explained, given the reliability of the false recall measures (r = .90). Therefore, the results of this analysis strongly constrain theories of false memory in this paradigm, suggesting that at least two factors determine the propensity of DRM lists to elicit false recall. The results fit well within the theoretical framework postulating that both semantic activation of the critical item and strategic monitoring processes influence the probability of false recall and false recognition in this paradigm.

[Evaluation on hemorrhage factors secondary to endoscopic sinus surgery with multiple stepwise regression analysis].

OBJECTIVE: To investigate the factors relating to bleeding secondary to endoscopic sinus surgery. METHOD: 193 patients suffered from nasal polyps, chronic sinusitis and underwent endoscopic sinus surgery between May 1995 and February 2001 were analyzed. These patients's state of operation affairs tallies with this study's selecting condition and their clinic data was complete. The factors relating to bleeding secondary to endoscopic sinus surgery were analyzed with multiple stepwise regression analysis with STATA31 statistic computer software. RESULT: 20 selected hemorrhage factors were analyzed with multiple stepwise regressions. With these factors progressively removed, 5 items as follows showed relation to the hemorrhage: X18, X19, X9, X11, X20. An optimal regression equation for predicting hemorrhage has been established. According to this equation, the blood loss of endoscopic sinus surgery could be estimated. CONCLUSION: The clinical signification of 5 factors were recognized: 1. X18: Anatomy mark was broken down in former operation and showed difficulty when identified. 2. X19: Sinus presented proliferation such as scar forming, blood vessel showed hyperemia, sinus wall showed hyperplasia, Aperture being narrow and small. 3. X9: Sinusitis showed late stages. 4. X11: Estimated blood loss was significantly greater under general anesthesia. 5. X20: Operation went on over 2 hours. If aforementioned condition appears, blood loss may exceed 200 ml.

Prediction of postoperative complications after elective aortic surgery using stepwise logistic regression analysis.

Perioperative risk factors that contribute to postoperative morbidity have been identified for various forms of vascular disease, but it is not clear to what degree each type of disease contributes to morbidity when these diseases are seen in combination. We used stepwise logistic regression analysis to determine the relative importance of 25 risk factors in predicting postoperative complications in 100 consecutive patients undergoing elective intra-abdominal aortic surgery (59% aneurysmal disease, 37% occlusive disease, 26% renal artery lesions). Thirty-one patients developed postoperative complications, including three deaths. The most common complication was deterioration in renal function (17 patients, 24% of those at risk) followed by the need for prolonged endotracheal intubation (> 48 hours, 8%). All other events occurred uncommonly (4% or less). Stepwise logistic regression demonstrated four criteria that were significantly associated with complications. In descending order of importance these were: renal artery occlusive disease, intraoperative blood replacement, the preoperative APACHE II score, and a history of heavy smoking. Since both physiologic status and comorbid conditions contribute to morbidity and costs of elective vascular surgery, outcome studies must be adjusted to account for these preoperative characteristics. Additionally, since two-thirds of patients who have renal artery occlusive disease develop complications that prolong hospitalization, our current methods of protecting the kidney during ischemia should be improved to potentially reduce this cause of morbidity.
