[Clinical study of baoyuan dahuang decoction in the treatment of chronic renal failure].

UNLABELLED: The quality of life of 56 chronic renal failure (CRF) patients using two kinds of treatment has been observed. 36 of them were taken as Group A with BUN 21.62 +/- 5.96 mmol/L, Cr 528.63 +/- 176.3 mumol/L and Hb 65 +/- 13 g/L were treated with Baoyuan Dahuang Decoction consisted of Panax ginseng, Astragalus membranaceus, Cassia cinnamomi, Glycyrrhiza uralensis and Rheum palmatum. RESULT: Six symptoms were observed, that were: fatigue, lassitude in loin and legs, aversion of cold, anorexia, sexual dysfunction and mental depression. Five of them improved markedly, from 12.5 +/- 2.91 before treatment to 5.58 +/- 4.68 after treatment in terms of symptom scores, P < 0.001. Furthermore, dihydrothelin (E2), testosterone (T) and pregnendione (P) were regulated significantly, the elevated level of creatine phosphokinase (CPK) before treatment lowered to near normal. The other 20 patients, BUN 20.24 +/- 6.57 mmol/L, Cr 487.08 +/- 238.68 mumol/L and Hb 68 +/- 13g/L formed Group B using comprehensive treatment of Western medicine as control. The main drug was coated aldehyde oxystarch. After treatment, the symptoms didn't show marked improvement, symptom scores being 12.55 +/- 2.0 compared with 10.05 +/- 2.72 before treatment, P > 0.05. E2, T, P and CPK were of insignificant differences (P > 0.05); Hb remained almost unchanged, even though BUN and Cr lowered for 22% and 29% respectively. It is assumed that Baoyuan Dahuang decoction could improve CRF patients' quality of life, and the mechanism of which was not only to reduce the urea nitrogen, but also to adjust the anemia and regulate the membrane function.

[134 patients with adult primary nephrotic syndrome treated with combined traditional Chinese medicine and Western medicine].

This paper studied the method and effect of adult primary nephrotic syndrome (PNS) with the integrated traditional Chinese and Western medicine (TCM-WM) treatment. 134 patients were divided into WM group (66 cases) and TCM-WM group (68 cases). Both groups were mainly treated with corticosteroid. The latter group was treated with corticosteroid plus Chinese medicinal herbs. The results showed that total effective rate of WM group and TCM-WM group was 56.1% and 85.3% respectively, while the occurrence rate of side effect was 48% and 14.8% respectively. It indicated that corticosteroid plus Chinese medicinal herbs could enhance curative effect and reduce side effect.

[Clinical and experimental study of left ventricular diastolic function in heart qi insufficiency syndrome].

In order to assess the left ventricular diastolic function of Heart Qi Insufficiency Syndrome (HQIS), apexcardiogram, ECG, electro-cardio-phonogram, impedance cardiogram were recorded and computerized simultaneously. A/D, PEP/LEVT, A/H were increased, while TRTI, DATI, HI, SV, CI reduced in 64 patients, 34 of them with coronary heart disease. The change was statistically significant. This study showed that DATI was more sensitive than PEP/LEVT in patients with HQIS, while there was no significant difference between abnormal ratio of them, overlapping of the two indices was showed. Replenishing Heart Mixture attained its therapeutic effect through improving abnormal left ventricular function. After treatment A/H, A/D, PEP/LEVT were reduced and DATI, SV, CI increased (P < 0.05-0.01). In experiment, HQIS was produced by acute myocardial ischemia in open-chest anesthetized dogs by left anterior descending coronary artery occlusion. It has been shown that dp/dtmax and T were more sensitive than dp/dtmax. Result of the clinical and the experimental study were consistent.

[Effects of total coumarins, essential oil and water extracts of Cnidium monnieri on plasma prostaglandin and cyclic nucleotide in the rats of kidney-yang insufficiency].

Effects of total coumarins, essential oil and water extracts of Cnidium monnieri on plasma prostaglandin (PGE2 and PGF2 alpha) and cyclic nucleotide levels in rats of Kidney-Yang insufficiency were studied. 55 rats were divided randomly into 5 groups, Group I was administered orally with saline (normal group), group II was injected with intraperitonally hydrocortison acetate to induce Kidney-Yang insufficiency (control group), group III, group IV and group V (experimental groups) were injected with hydrocortison acetate, the same as group II, and administered orally with the total coumarins, essential oil and water extracts of Fructus Cnidii respectively. The levels of plasma PGE2, PG2 alpha and plasma cAMP, cGMP were measured. In group II, in comparing with those of group I, the levels of plasma PGE2, and PGF2 alpha decreased significantly (P < 0.01), and the value of cAMP/cGMP also lowered obviously (P < 0.01) due to the significant reduction of cAMP and insignificant change of cGMP. In group III and group V, the above-mentioned indices changed significantly (P < 0.01 or 0.05) compared with those of group II, and after treatment it normalized basically in comparing with those of group I. In group IV, those indices didn't change regularly and apparently as group III and group V did, compared with group II, and could not normalize satisfactorily. It is suggested that the coumarins in the fruit of Cnidium monnieri are probably the effective ingredients to invigorate Kidney and strengthen Yang, while the efficacy of essential oil remained unconfirmed.

[Effect of three kinds (tonifying kidney, invigorating spleen, promoting blood circulation) recipes on the hypothalamus-pituitary-adrenal-thymus (HPAT) axis and CRF gene expression].

OBJECTIVE: To observe the effect of tonifying Kidney (TK), invigorating Spleen (IS) and promoting blood circulation (PBC) recipes on the hypothalamus-pituitary-adrenal-thymus (HPAT) axis. METHODS: Reverse-transcription polymerase chain reaction, radioimmunoassay and cellular immunity techniques were used to observe the effects of TH, IS, PBC on the neuroendocrine and immune system of this experimental model which HPAT axis was inhibited by corticosterone. RESULTS: Only TK could avoid the depression of exogenous corticosterone, which by enhancing the expression of CRFmRNA in hypothalamus, and following the improvement of function of HPAT axis, that IS might have direct promotion on immune system, whereas without any effect of PBC on HPAT axis was observed. CONCLUSION: TK, IS and PBC recipes have different effects on HPAT axis.

[NK cell activity in patients with kidney insufficiency].

Natural killer cell (NKC) plays an important role in anti-cancer, anti-virus and the regulation of immune reaction in human body. NK cell activities from the peripheral blood were determined with 51Cr release method from the human erythroleukemia cell line K562 in 61 patients, who were selected in the light of the standard of deficiency syndrome in TCM with insufficiency of the Kidney in which 12 kinds of diseases in western medicine were included, and 60 healthy subjects as control group. The patients with insufficiency of the Kidney showed significantly decrease in NK cell activity compared to the control group (25.29 +/- 10.20 vs 56.27 +/- 16.72, P less than 0.001). NK cell activities in 20 out of 60 patients, with deficiency of Kidney-Yang, were lower than 21 with deficiency or Kidney-Yin (19.38 +/- 7.44 vs 26.60 +/- 8.95, P less than 0.01) and 20 with deficiency of Kidney-Qi (19.38 +/- 7.44 vs 29.38 +/- 11.30, P less than 0.01), respectively, while there was no difference between the deficiency of Kidney-Qi (P greater than 0.05). The authors also found that NK cell activities of healthy subjects were strongly inhibited by plasma from 43 out of 61 patients. These results suggested that decreased NK cell activity is one of the common characters in patients with insufficiency of the Kidney in TCM and there are some inhibitory substances against NK cell activity in plasma of ones. The fact that NK cell activity of patients with deficiency of Kidney-Yang was the lowest among three kinds patients with insufficiency of the Kidney.(ABSTRACT TRUNCATED AT 250 WORDS)

[Analysis of the correlations between immunological changes and syndrome groups in patients with immunological thrombocytopenic purpura (ITP)].

To study the relationship between the immunological changes and syndrome (Zheng,) groups by TCM of ITP, the T-lymphocyte subsets, B-lymphocyte, NK cell, platelet-associated IgG (PAIgG, PAIgA, PAIgM) and antiplatelet-autoantibodies (GPIIb, GPIIIa, GP I b) of 66 patients with ITP were assisted using APAAP and ELISA method separately. It was found that the T-lymphocyte subsets, PAIg and syndrome groups of ITP were closely related. From the group of blood-heat (Xuerewangxing) to the group of deficiency of both Qi and blood, the group of asthenia of both Spleen and Kidney, the group of deficiency of Liver-yin and Kidney-yin, and the group of deficiency Yin and Yang Ts lymphocyte successfully increased (from 29. 0 +/- 8.0% to 47.2 +/- 10.0%), Th/Ts ratio declined (from 1.35 +/- 0.60% to 0.69 +/- 10%), PAIg increased gradually except for PAIgM,PAIgG of the group of deficiency Yin and Yang. Only the Th of the group of asthenia of both Spleen and Kidney among 5 syndrome groups was decreased significantly and contrary to the group of deficiency of Liver-Yin and Kidney-Yin. These results indicated that every syndrome group has specific characteristics, and immunological changes of ITP could have prognostic value.
