The impact of words on children with ADHD and DAMP. Consequences for psychoanalytic technique.

Children with attention-deficit/hyperactivity disorder (ADHD) and disorder of attention, motility control, and perception (DAMP) are often sensitive to the analyst's interventions. This is not always due to the literal import of the intervention. The children sometimes react as if the words were dangerous concrete objects, which they must physically fend off. The author traces this phenomenon to the child's unstable internal situation. A bad, un-containing internal object is easily awakened and threatens to expel the analyst's words independently of their content. This results in violent clinical situations. Infant research and psychoanalytic work with infants and mothers evince how a complex semiotic process develops between mother and baby. The prerequisite for this process to get started and maintained is a secure external object, which gradually is internalized. Findings from developmental research and clinical infant work are used to illuminate analytic work with children with ADHD and DAMP. Vignettes demonstrate how important it is for the analyst to phrase interpretations after having gauged the state of the analysand's internal object as well as his/her own countertransference. If this is overlooked, the psychoanalytic dialogue easily capsizes. The author provides some technical recommendations on the psychoanalysis of these children. As part of the theoretical discussion he raises the general question of how the representations, which the baby forms in interaction with the mother, and the analysand forms in interaction with the analyst, should be classified. Rather than dividing them into bipartite thing- or word-presentations (Freud), the author suggests C. S. Peirce's tripartite semiotic classification in that the baby forms representations of icons, indices, and symbols.

The physical mechanisms of the perception of dampness in fabrics.

The detection of dampness in hygroscopic materials has been investigated both by subjective tests and by the application of a model of the physical mechanisms involved. Subjects were asked to rate the degree of dampness of a range of materials of different moisture contents after a short period of contact with the inner forearm. Skin and fabric inner surface temperatures were recorded. It was found that highly hygroscopic wool fabrics were perceived as being dryer and maintained a higher temperature at the skin surface than polyester, a less hygroscopic fabric, during fabric-skin contact. A physical model of the sorption/desorption process in hygroscopic materials has been developed from knowledge of fibre sorption kinetics and used to study the physical processes which take place at the skin-fabric interface during transient contact. These predictions agree well with the subjective responses and the measured temperatures.

Doppler ultrasound recognition of preclinical changes in arterial wall in diabetic subjects: compliance and pulse-wave damping.

Doppler ultrasound was used to detect early changes in arteries of the legs by two independent techniques. Pulse-wave velocity was measured to calculate arterial wall compliance and Fourier analysis was used to measure damping of the pulse-wave forms. Ten non-insulin-dependent diabetic men with no clinical evidence of peripheral arterial disease had significantly lower compliance and greater pulse-wave damping than 10 matched nondiabetic control subjects. There was a good correlation between the results for the two different techniques.

Gross electrocortical activity as a linear wave phenomenon with variable temporal damping, regulated by ascending catecholamine neurones.

We report critical tests for a theory of electrocortical wave processes, in which telencephalic dendritic potentials reflect the mass action of coupled oscillatory circuits exhibiting complicated and unspecified non-linearities, the whole system being driven by active cell firing. Specific assumptions were: stochastic independence for instantaneous coupling parameters in the system, an individual central tendency to the cycle time for each circuit, and the maintenance of steady state conditions. Application of the central limit theorem to the state transition matrix shows that the gross electrocortical waves should be linear waves, exhibiting a multitude of invariant resonant modes, with the natural frequencies of all the modes being clustered about a smaller number of center values. Ascending brain-stem neurones of at least the dopaminergic and noradrenergic classes should regulate both the power of noise-like signals driving the telencephalic resonant patterns, and the temporal damping of each resonance. We devised tests which involved between hemisphere comparisons of electrocortical spectra, before and after unilateral lesion of transhypothalamic ascending fibres, thus obtaining ratio power changes attributable to post lesion asymmetry of damping and driving, in modes of equivalent left-right center-frequencies. These ratio spectra were curve-fitted to an approximate theoretical expression, and the parameters obtained enabled tests of several specific predictions. Estimates of the center values for resonant mode frequencies, comparison of the relative changes in left/right phase with that expected from the ratio changes in power, and estimates of the surface-to-signal transformation of left and right signals made by a back-calculation, all conform to expectation from the theory, and are consistent across lesion of different types of ascending neurone.

Viscoelasticity of fibrinogen solution and of blood during coagulation studied by a new damped oscillation rheometer.

The behavior of a newly developed damped oscillation type rheometer was analyzed for fibrinogen solution and blood during coagulation. This rheometer consists of a cylindrical tube suspended from a torsion wire, that is filled with liquid to be tested. The logarithmic damping factor (LDF) during coagulation for blood and fibrinogen solution was obtained by this rheometer, which was closely related to the changes of viscosity and/or viscoelasticity of the blood sample. The slight increase of LDF prior to the rapid decrease was observed for blood. The increase of LDF would be reflected in the formation of the aggregation structure of red blood cells (rouleaux network) prior to the formation of fibrin network. The value of LDF for fibrinogen solution sharply increased and then decreased through a maximum value with the progress of coagulation, although the change of LDF was remarkably dependent on the fibrinogen concentration. The initial increase in LDF for fibrinogen solution was considered to be due to the formation of small clots in the solution. The decrease in LDF after attaining a maximum value is ascribed to the formation of fully developed fibrin network. The maximum value of LDF during coagulation for fibrinogen solution is higher than that for blood. The behavior was compared with that for non-biological fluids such as viscosity standard liquids and polyvinyl alcohol solution. From those data, it was concluded that the higher value of LDF than that for Newtonian liquids was due to the formation of aggregation structure or inhomogeneous fine clots in the liquid, which was accompanied with the appearance of the elasticity.

[Effects of traditional Chinese medicine for invigorating spleen to resolve dampness and dredging collaterals on patients with rheumatoid arthritis and anemia].

OBJECTIVE: To observe the clinical effects of Xinfeng Capsules (XFC), a traditional Chinese medicine for invigorating the spleen to resolve dampness and dredging collaterals, on patients with rheumatoid arthritis (RA) and anemia, and to investigate its mechanism. METHODS: Forty patients with RA and anemia were divided into three groups: XFC-treated group (n=20), Tripterygium glycosides Tablets (TGT)-treated group (n=10) and methotrexate (MTX)-treated group (n=10). The patients in each group took corresponding medicine for three months. The response rates of the three groups were evaluated after treatment. The general symptoms and specific signs and symptoms of RA were observed before and after the treatment. The indexes of blood routine examination and some other laboratory indexes such as erythrocyte sedimentation rate (ESR), and blood levels of rheumatoid factor (RF), iron, C-reaction protein (CRP), immunoglobins (Ig), complements 3 and 4, tumor necrosis factor-alpha (TNF-alpha), interleukin-10 (IL-10) and erythropoietin (EPO) were all examined and compared before and after treatment. RESULTS: The total response rate in the XFC-treated group was similar to those both in the TGT-treated and MTX-treated groups. The effects on relieving specific symptoms of RA in the three groups were also similar. The ESR and serum levels of RF, CRP, IgG, IgA and IgM were all decreased after treatment as compared with those before treatment in the three groups, and there were no significant differences among those laboratory indexes in the three groups after treatment. The XFC displayed more obvious effects on improving the general symptoms of patients with RA and anemia, increasing the blood levels of hemoglobin, iron and IL-10, while decreasing the serum level of TNF-alpha and regulating the serum EPO level, as compared with those in the TGT-treated and MTX-treated groups. CONCLUSION: The XFC for invigorating the spleen to resolve dampness and dredging collaterals was developed on the basic principle of regulating spleen. It can obviously improve the symptoms and laboratory indicators of RA. Such effects may be related to maintaining the balance of cytokines, regulating the serum level of EPO and increasing the serum iron level in patients with RA and anemia.

[DAMP and ADHD as "hidden" functional disabilities. Carefully prepared care of children at risk is beneficial for an optimal development].

A current study of seven-year-olds in Karlstad showed the incidence of DAMP (deficits in attention, motor function and perception) to be 1.7 per cent, and that of ADHD (attention deficit hyperactivity disorder) to be 2.0 per cent, figures consistent with those obtained in previous studies. A further 4.9 per cent were found to have milder forms of DAMP. Research during the past few decades has yielded comprehensive knowledge of the effect of such disability on the child's development. Manifestation of the child's problems is dependent on how the child is treated by others, and on the understanding and help deriving from the family and from the preschool or school. It is essential to evolve a well considered management programme of health care for which the school or preschool is responsible. The programme should include methods for early detection, access to a competent investigatory team, support and treatment facilities for the child and the parents, and assistance at school or preschool.

Dissociated vertical deviation: an exaggerated normal eye movement used to damp cyclovertical latent nystagmus.

PURPOSE: Dissociated vertical deviation (DVD) has eluded explanation for more than a century. The purpose of this study has been to elucidate the etiology and mechanism of DVD. METHODS: Eye movement recordings of six young adults with DVD were made with dual-coil scleral search coils under various conditions of fixation, illumination, and head tilt. Horizontal, vertical, and torsional eye movements were recorded for both eyes simultaneously. Analyses of the simultaneous vertical and torsional movements occurring during the DVD response were used to separate and identify the component vergence and version eye movements involved. RESULTS: Typically, both horizontal and cyclovertical latent nystagmus developed upon occlusion of either eye. A cycloversion then occurred, with the fixing eye intorting and tending to depress, the covered eye extorting and elevating. Simultaneously, upward versions occurred for the maintenance of fixation, consisting variously of saccades and smooth eye movements, leading to further elevation of the eye behind the cover. The cyclovertical component of the latent nystagmus became partially damped as the DVD developed. CONCLUSIONS: In patients with an early-onset defect of binocular function, the occlusion of one eye, or even concentration on fixing with one eye, produces unbalanced input to the vestibular system. This results in latent nystagmus, sometimes seen only with magnification. The cyclovertical component of the latent nystagmus, when present, is similar to normal vestibular nystagmus induced by dynamic head tilting about an oblique axis. Such vestibular nystagmus characteristically produces a hyperdeviation of the eyes. In the case of cyclovertical latent nystagmus, the analogous hyperdeviation will persist unless corrected by a vertical vergence. A normal, oblique-muscle-mediated, cycloversion/vertical vergence is called into play. This occurs in the proper direction to correct the hyperdeviation, but it occurs in an exaggerated form in the absence of binocular vision, probably as a learned response. The cycloversion/vertical vergence helps damp the cyclovertical nystagmus (a cyclovertical "nystagmus block-age" phenomenon), aiding vision in the fixing eye. But this mechanism also produces unavoidable and undesirable elevation and extortion of the fellow eye, which we call DVD.

[Diagnosis of attention deficit disorders, hyperactivity disorders and DAMP in children].

Children with attention deficits have been known since the beginning of the last century. Untreated children have a poor prognosis. This paper reviews the diagnostic criteria and the problems there still are with the diagnoses. There are two official diagnostic systems, ICD-10 and DSM-IV, both have descriptive and general diagnostic criteria. There are no independent valid tests, but the last two years have seen recommendations from the USA and UK for a more thorough diagnostic assessment. For the diagnosis of DAMP, as it is used in Sweden, there are no proper scientific studies. More specific criteria are described in a Danish reference programme for children with DAMP.

Biological control of soybean damping-off by antagonistic rhizobacteria.

Experiments were carried out with 133 bacterial isolates that were collected from soybean rhizosphere. These strains were used to investigate their biocontrol traits in vitro and their ability to suppress the soybean damping-off in vivo (soil and seed treatments). Three highly effective isolates were selected from these antagonists for subsequent studies. According to the biochemical, physiological and morphological tests, these isolates (B-2, B-12 and B-80) were identified as Bacillus spp. In soil treatment, the isolate B-3 with 70.8%, B-12 with 66.7%, B-80 with 54.2% had the highest effect on reducing the soybean damping-off. In seed treatment, the isolates B-43 with 62.5%, B-12 with 58.4 and B-80 with 45.8%, had the greatest effect on reducing the disease. These isolates produced volatile metabolites that inhibited mycelial growth of Phytophthora sojae.

[Pathogenic factor (Dampness-heat) of glomerulopathy].

Mesangial proliferative glomerulonephritis (MsPGN) induced by chronic serum sickness in rabbits coincide with the human chronic progressive glomerulonephritis resulted from repeated infection, which is similar to pathologic changes of Dampness-Heat Syndrome. The experimental model of MsPGN was treated by Abelmoschus manihot. which could remove the Dampness-Heat. The amount of proteinuria in treating and control group were 62.68mg/24hr and 121.94mg/24hr respectively (P < 0.05), the number of cells in glomeruli were 61.54 and 80.39 respectively (P < 0.01), and diameters of glomeruli were 102.43 microns and 121.13 microns respectively (P < 0.01). It suggested that the drug could alleviate circulating immune complex (CIC) mediated renal injury.

[Physiological mechanism of rice seedlings in low temperature-and damping-off disease resistance induced by calcium application into soil].

Applying calcium oxide into soil could improve the ability of rice seedlings in low temperature-and damping-off disease resistance. After treated with calcium, the active oxygen eliminating enzyme activities of rice seedlings increased significantly. Compared with control, the SOD activity in seedling's root and shoot increased. The POD activity in rice seedling's root also increased, but decreased in shoot. The CAT activities decreased at the early stage, but increased at late stage. The soluble protein content in seedling's root and shoot was higher than that of the control. In the PAGE pattern of isoperoxidase, the band number of shoot increased, but that of root decreased. AU these changes were part of reasons that applying calcium oxide into soil improved the resistance of rice seedlings to cold and damping-off disease.

[Technique of storing Coptis chinensis seeds in damp sand under shed and fine cultivation of seedling].

OBJECTIVE: To develop technique of storing Coptis chinensis seeds in damp sand under shed and fine cultivation of seedling and to enhance the cultivation seedling rate from seeds of Coptis chinensis. METHODS: With the old technique of seedling in woods as control, we screened the new method for storing seeds and cultivating seedlings. RESULTS: Seeds were picked up at May and stored in damp sand under shed until November, and then planted with technique of fine cultivation of seedling. The germination rate of seeds was up to 98%. One hundred and forty thousands seedlings could be getting from per kilogram seeds. CONCLUSION: Compared to the old method of cultivating seedling in woods, the technique of fine cultivation of seedling significantly increased the rate of cultivation seedlings by 8 times. This technique has been widely applied.

[Clinical study on treatment of non-alcoholic fatty liver of damp-heat syndrome type by danning tablet].

OBJECTIVE: To evaluate the therapeutic effect of Danning Tablet (DNT) on patients with non-alcoholic fatty liver (NAFL) of damp-heat Syndrome type. METHODS: Multi-center randomized double-blinded positive medicine parallel controlled method was adopted. One hundred and two patients were treated with DNT and 33 patients treated with Ursodeoxycholic acid (UDCA) as control. Indexes including body mass index (MBI), principal symptom, liver function, blood lipids, iconographic parameters and comprehensive efficacy were measured before and after treatment in the two groups respectively. RESULTS: DNT and UDCA had the effect in improving BMI, distress in hepatic region, fatigue, anorexia, liver function, blood lipids and iconographic parameters, etc. DNT showed effects in improving BMI and distress in hepatic region better than those of UDCA. Histological examination also showed that DNT had good therapeutic effect. CONCLUSION: DNT is an effective medicine for NAFL patients of damp-heat Syndrome type, and its efficacy is better than that of UDCA.

[Clinical study on effect of bushen granule combined with marine injection in treating chronic hepatitis B of Gan-shen deficiency with damp-heat syndrome type].

OBJECTIVE: To evaluate the antiviral efficacy of Bushen Granule (BSG) combined with Marine Injection (MI) in treating patients with chronic hepatitis B of Gan-shen deficiency with Damp-Heat (GSD) syndrome type. METHODS: A total of 90 patients, who were HBV DNA, HBsAg, and HBeAg positive and of GS-DD type, were enrolled, and they were randomly assigned into 2 groups. The 49 patients in the treated group were treated with the combined therapy of BSG and MI and the 41 were administered with lamivudine, the therapeutic course for both groups was 1 year. The negative conversion rate of HBeAg and HBV DNA and the changes in HBV DNA titre, liver function, symptoms and physical signs were observed in the two groups before and after treatment. RESULTS: In the treated group, the negative conversion rate of HBV DNA was 42.6% (20/ 47), insignificantly different to that in the control group (61.0%, 25/41, P > 0.05); the negative conversion rate of HBeAg and HBeAg/anti-HBe sero-conversion rate was 42.6% and 36.2%, respectively, significantly higher than those in the control group (22.0% and 17.0%, P < 0.05); the ALT normalizing rate was 74.4%, higher than that in the control group (51.4%, P < 0.05); the improvement of liver function (ALB and GLB) and clinical symptoms, especially the dizziness, soreness of waist, hypochondrial distending pain, and yellowish urine, were significantly superior to those in the control group(P < 0.01 or P < 0.05); and the partial response rate was 29.8%, insignificant different to that in the control group (14.6%, P > 0.05). CONCLUSIONS: Combined treatment of BSG and MI has an ideal short-term effect in treating patients with chronic hepatitis B of Gan-shen deficiency with Damp-Heat syndrome type, it can inhibit HBV replication and improve patients' liver function and physical signs.

[Effect of gegen qinlian decoction combined with short-term intensive insulin treatment on patients with type 2 diabetes mellitus of dampness-heat syndrome].

OBJECTIVE: To observe the effect of Gegen Qinlian Decoction (GQD) in combination with short-term intensive insulin treatment on type 2 diabetes mellitus (T2DM) of dampness-heat syndrome and its influence on dosage of insulin used. METHODS: The GQD group (n = 14) was treated by GQD and insulin, while the conventional group (n = 16) was given insulin intensive treatment alone. RESULTS: In the GQD group, the treatment was markedly effective in 5 patients, effective in 6 and ineffective in 3, the total effective rate being 78.6%, much better than that in the conventional group (2, 7, 7 and 56.3% respectively, u = 2.58, P < 0.01). And it took less time for controlling blood glucose (BG) in the GQD group (4.54 +/- 0.50 days) than that in the conventional group (5.31 +/- 0.57 days, P <0.01); furthermore, by the end of the treatment course, as compared with that at the time just after BG being controlled, the daily average insulin dosage used in the GQD group reduced by 9.07 +/- 6.51 U, while it was only 4.38 +/- 5.94 U in the conventional group, showing significant difference between them (P < 0.05). CONCLUSION: Based on short-term insulin intensive treatment, the combined using of GQD could reduce the dosage of insulin used and shows better clinical curative effect for patients with T2DM of dampness-heat syndrome.

Evaluation of soil microorganisms with inhibitory activity against Rhizoctonia solani causal agent of the damping-off of canola.

Pre- and post-emergence damping-off of canola seedlings caused by Rhizoctonia solani is a serious disease in Western Canada. Other fungi such as Fusarium spp. and Pythium spp. are also related to seedling damping-off. To-day, the search of soil bacteria is becoming a tool to use microorganisms as potential biocontrol agents for several plant diseases. The purpose of this research was to detect bacteria to biologically control R. solani, Pythium spp., and Fusarium spp. Soil samples were collected throughout Alberta during 1987 to isolate bacteria. Canola seedlings were also used to obtain bacteria from the same samples. Plant pathogenic fungi were tested to detect the antagonistic activity of the isolates. Tests were made with coated canola seeds, amendments and fresh of freeze-dried cells. Three hundred forty-one bacterial cultures were isolated. Only 16 inhibited fungal growth: 7 showed the same effects against R. solani and 9 showed uneven effects. Some isolates showed a weak action to Pythium spp. and Fusarium spp. Three isolates showed inhibitory effect on R. solani and Pythium spp. Isolate F1 improved by about 50% the germination of canola seeds in inoculated pots when compared with the inoculated control. Coated seeds had low germination and emergence was below the inoculated control. The emergence of canola seedlings was very much improved when isolate 147 was delivered as an amendment in inoculated pots. Identification showed that 3 bacterial belonged to Bacillus spp., 4 to green fluorescent Pseudomonas spp. and 2 were Streptomyces spp.

[Effect of the pacing interval and extrasystole on amplitude and damping of diastolic oscillations in the rabbit papillary muscle].

Diastolic mechanical oscillations of right ventricular rabbit papillary muscles investigated at 15 degrees C in the perfusion chamber were analysed using a relaxation model with 6 parameters. From this analysis follows that the first diastolic oscillation amplitude plotted against the driving interval ("interval amplitude curve" of the first diastolic oscillation) shows tow maxima thus differing from the shape of the interval strength curve of the preceding driven contraction. It is concluded that the amplitude of diastolic oscillations is less determined by the Ca-amount released from the sarcoplasmic reticulum during the action potential than by the frequency of processes which effect the Ca release and the Ca-sequestration. The damping ratio varies within the range of tested driving intervals (0.36 to 10 s). After an extrasystole damping ratio and period of diastolic oscillation are diminished compared with the values after regularly driven contractions.
