Primary angioplasty in acute inferior myocardial infarction with anomalous-origin right coronary arteries as infarct-related arteries: focus on anatomic and clinical features, outcomes, selection of guiding catheters and management.

BACKGROUND: Inferior wall myocardial infarction caused by obstruction of an anomalous-origin right coronary artery (RCA) is a rare angiographic finding; primary angioplasty to an anomalous-origin RCA has never been reported. METHODS: In 185 patients with acute inferior wall myocardial infarction resulting from RCA occlusion who underwent primary angioplasty, eight patients (4.3%) had anomalous-origin RCAs. RESULTS: Coronary angiography showed that all 8 patients had a dominant RCA. Six patients (75%) had an anomalous-origin RCA arising from the anterior aspect of the ascending aorta above the sinotubular line and the other 2 patients (25%) had an anomalous-origin RCA arising from the left sinus of Valsalva with a separate ostium from the left main coronary artery. The standard Judkins right guiding catheter did not offer adequate support in these patients. In the group of 6 patients, an Amplatz guiding catheter offered good support, while a standard Judkins left guiding catheter was adequate in the other 2 patients. Obstruction of the proximal RCA occurred in 6 patients (75%). Successful reperfusion was achieved in 6 patients (75%), resulting in an uneventful clinical course and long-term survival (mean follow-up, 24.9 +/- 16.5 months). Two patients (25%) had unsuccessful reperfusion and died from cardiogenic shock. CONCLUSIONS: In this small series, anomalous-origin RCAs were the dominant artery and predisposed to atherosclerosis at the proximal portions. We suggest that appropriate guide catheter selection and careful manipulation are essential for the success of revascularization. Complete reperfusion results in an excellent clinical and long-term outcome in patients with anomalous-origin RCAs.

[Level of development of selected somatic features of children at the age of 6 from the Swietokrzyskie region depending on their biological state in their infancy].

This material presents the results of a long-term research on 384 children born in 1982. The aim of this research was to state the changes in height and body mass--depending on the influence of some of the paragenetic factors. The same children were examined in different periods of their development, this means at the age of 0, 6, 10, 14 and 18 years. The article presents the results of research concerning children at the age of 6. The obtained results confirm the influence of especially the age of mother and the sequence of pregnancies on the physical development of children at the age of 6. This coherence is greater concerning boys rather than girls.

[Open globe injuries--selected epidemiologic features in own material].

PURPOSE: To evaluate chosen epidemiological factors in perforating eye injuries in own material MATERIAL AND METHOD: Retrospective case review over period from 2001 to 2003 hospitalised in the Department of Ophthalmology in Bydgoszcz. The group studied comprised 78 patients (79 eyes) aged from 4 to 77 years. Analyzed were following data: age, gender, the extend of the injury, visual acuity at presentation, the presence of - and intraocular foreign body nature, type of injury, professional activity, and other factors (alcohol use, glasses). RESULTS: The group studied involved 89.7% men. Mean age of all the patients was 35.6 years (SD=17.1), male: 35 years (D=17.0), female: 41 (SD=18.5). Severe wounds, including corneal and scleral laceration totaled 32.9%. In 41.8% cases initial visual acuity was less than 0.001, including 42.4% of light perception. There was no light perception in 18.2% in this group. Intraocular foreign bodies were found in 39.2% of studied eyes (75.8% involved ferromagnet metal). Domestic activities accounted for 50% of injuries, and 33.3% occurred at worksite. In the group studied there were 28.2% of unemployed, the pensioners were affected in 9%. CONCLUSIONS: Most of the penetrating eye injuries affected young males aged mean 35 and happened at home. Almost one half of the patients experienced severe injuries leading to severe visual loss. Ferromagnet metal predominated intraocular foreign bodies.

Selected epidemiological features of human brucellosis in Yazd, Islamic Republic of Iran: 1993-1998.

Brucellosis is a significant health problem in countries where control of zoonoses is inadequate. During 1993-98, we analysed sera and cultures from 792 suspected brucellosis patients who presented with histories of fever, chills, night sweating, weakness, malaise and headache to the referral hospital in Yazd. Cases were investigated by tube agglutination test (TAT) and 2-mercaptoethanol test (2-MET) and a questionnaire was completed for each.TAT titre was > or = 1:1 60 for 745 patients (94.1%) and 2-MET was positive for 42 (5.3%). Of 745 confirmed cases, 460 were from 1996-1997. Prevalence was highest in summer (39.5%) and more common males than among females. Prevalence was highest among those aged 10-19 years (27.7%). Most patients had a history of infected cheese, milk and milk product consumption (98%).

Modeling clinical judgment and implicit guideline compliance in the diagnosis of melanomas using machine learning.

We explore several machine learning techniques to model clinical decision making of 6 dermatologists in the clinical task of melanoma diagnosis of 177 pigmented skin lesions (76 malignant, 101 benign). In particular we apply Support Vector Machine (SVM) classifiers to model clinician judgments, Markov Blanket and SVM feature selection to eliminate clinical features that are effectively ignored by the dermatologists, and a novel explanation technique whereby regression tree induction is run on the reduced SVM model's output to explain the physicians' implicit patterns of decision making. Our main findings include: (a) clinician judgments can be accurately predicted, (b) subtle decision making rules are revealed enabling the explanation of differences of opinion among physicians, and (c) physician judgment is non-compliant with the diagnostic guidelines that physicians self-report as guiding their decision making.

[Influence of selected clinical features on detection of sentinel node in patients with skin melanoma].

BACKGROUND: Combined use of preoperative lymphoscintigraphy, intraoperative detection of gamma radiation and intraoperative blue-dye staining allow for identification of sentinel node in 94%-100% of skin melanoma patients. OBJECTIVES: To assess the influence of clinical factors on the progress of preoperative lympho-scintigraphy, intraoperative detection of gamma radiation and intraoperative blue-dye staining, during sentinel node biopsy procedure in skin melanoma patients. MATERIAL AND METHODS: From 1.12.1999 to 30.06.2001 in Department of Surgical Oncology, Medical University of Lódź, in 74 patients with skin melanoma, sentinel node biopsy was performed. Analysis comprised the influence of: age, sex, skin complexion of patients, diameter of primary tumor, presence of ulceration in primary tumor and location of primary tumor, on progress of parts of sentinel node biopsy. RESULTS: In subgroup of patients in whom time of sentinel node identification during preoperative lympho-scintigraphy was longer than 10 minutes, statistically significant relationship was found between time of identification and location of primary tumor (p < 0.05). The time was shortest in patients with primary tumor located on lower extremity and was the longest in patients with primary tumor located on head and neck. Statistically significant correlation was found between skin complexion and degree of sentinel node staining. In all red haired patients with light skin complexion full staining of sentinel node was observed. In patients with dark skin complexion full staining of sentinel node was observed only in 61.5% of cases. There was no statistical relationship between remaining clinical features and progress of preoperative lympho-scintigraphy, intraoperative detection of gamma radiation and intraoperative blue-dye staining. CONCLUSIONS: The time of identification of sentinel node during preoperative lympho-scintigraphy is related to location of primary tumor. The progress of intraoperative detection of gamma radiation is not related to studied clinical features. Degree of sentinel node staining is related to skin complexion of patient.

[Selected morphological features of the rat liver 2-97 days of life].

Examination of rat liver (Wistar strain) led us to distinguish 3 periods of its morphological development. The differences concerned the development hepatocyte cytoplasm and trabecular liver structure. The particularly essential period was observed between 21-42 days after birth; when cytoplasm capacity had the highest rate.

Structural and conformational features determining selective signal transduction in the beta 3-adrenergic receptor.

With respect to the beta 1- and beta 2-adrenergic receptors (ARs), the beta 3-AR induces specific physiological effects in a few target tissues and exhibits atypical pharmacological properties that distinguish it unambiguously from its counterparts. Therefore, the beta 3-AR represents a suitable model to study the molecular mechanism responsible for receptor subtype selectivity and specificity. Potent beta 3-AR ligands newly characterized in Chinese hamster ovary cells expressing the beta 3-AR were also evaluated in Chinese hamster ovary cells expressing beta 1- and beta 2-ARs and were classified into three groups according to their pharmacological properties. Among the beta 1/beta 2/beta 3 agonists BRL 37344 and LY 79771 exhibit beta 3 selectivity in stimulating adenylyl cyclase; among the beta 1/beta 2 antagonists displaying beta 3 agonistic effects ICI 201651 exhibits beta 3-AR binding selectivity, whereas among the beta 1/beta 2/beta 3 antagonist class bupranolol is the most efficient (but not selective) beta 3-AR antagonist. The structures of these ligands were simulated and compared using computer-generated molecular modeling. Structure-activity relationship analysis indicates that potent or selective beta 3-AR compounds, in addition to possessing a pharmacophore common to all beta-AR ligands, contain a long and bulky alkylamine substituent moiety, which is able to adopt and exchange extended and stacked conformations. Computerized three-dimensional models of the beta 1-, beta 2-, and beta 3-AR binding sites show that more bulky amino acid side chains point inside the groove of the beta 1 and beta 2 sites, compared with the beta 3 site, in a region implicated in signal processing. The long alkylamine chain of compounds behaving as beta 1/beta 2 antagonists and beta 3 agonists may thus adopt either a stacked conformation in the encumbered beta 1- and beta 2-AR sites, leading to antagonistic effects, or an extended conformation in the less encumbered beta 3 site, thus interacting with specific residues implicated in signal transduction.

Selected epidemiological features of herpes genitalis in Israel based on laboratory data.

The incidence in Israel of genital infection with herpes simplex virus (HSV) has been increasing steadily over the last two decades. During the years 1973-91, 1,508 patients had an HSV infection confirmed by viral culturing. The yearly incidence of new cases rose from sporadic cases in the seventies to 32.4 cases per 1,000 patients in 1990. The age distribution pattern did not change since 1980 and showed a peak incidence in subjects aged 20-40 years. The increase of HSV type 2 (HSV-2) infection rate in the Israeli population, as judged from specific geometric mean titers (GMT), rose from 15.25 in 1970 to 89.3 in 1991, while the GMT for HSV type 1 (HSV-1) did not increase significantly. The incidence of HSV-2 positive subjects (HSV-2/HSV-1 antibody ratio > or = 1) was low in the Jewish Israeli population, compared to other demographic areas. The predominant type of genital infection was HSV-2, although 21% of genital isolates were HSV-1.

Clinical and immunologic features of selective IgA deficiency.

Selective absence of serum and secretory IgA is probably the most common form of human immunodeficiency. High frequencies of recurrent sinusitis, otitis media, pneumonia, and atopy were noted among a group of 75 such patients, all but 4 of whom were Caucasian. Seven instances of familial absence of IgA were detected among 106 relatives of 34 of the group; in 1 family 1 member from each of 3 successive generations was affected. Two IgA-deficient children were later found to have normal amounts of serum IgA. Despite their humoral deficit, B lymphocytes bearing surface IgA were detected in 9/9 IgA-deficient patients in immunofluorescence studies of their peripheral blood lymphocytes. Although in vitro lymphocyte responses to 2 putative T-cell mitogens and to allogenic cells were normal, results of spontaneous rosette formation studies with sheep erythrocytes raise the possibility of a lymphocyte subpopulation deficit in this condition.
