[Is oncological surgery contraindicated in the older patient?].

Almost three-quarters of patients with cancer of the gastrointestinal tract are more than 65 years of age. In the operative treatment of esophageal pancreatic, hepatocellular and colorectal carcinomas, studies show comparable risks for patients under, and those over, 70 years of age. In terms of morbidity, mortality and 5-year mortality rates, attendant risks in the latter do not generally appear to be greater. An increase in the rate of complications, however, has been observed for palliative interventions in colorectal cancer patients. In particular cardiovascular risk factors play a major role in the over-70-year-olds.

[A case report--Efficacy of combination therapy to unresectable advanced gall bladder carcinoma--palliative operation, hepatic arterial infusion therapy, and radiation therapy].

A patient of advanced gall bladder carcinoma with liver metastases and direct invasions to the duodenum and liver underwent a palliative operation, 3 hepatic arterial infusion (HAI) therapies, and radiation therapy at the obstructive common biliary duct. (Palliative operation was a partial resection of duodenum and transverse colon, HAI therapy with 5-FU (1 g/day) was given as a continuous infusion for 6 days, radiation therapy was given 2 Gy/day for 20 times) After the combination therapy, the main tumor of gall bladder and hepatic metastases were decreased and tumor markers were normalized. (CEA 15.1 ng/ml, CA19-9 93 U/ml to CEA 4.4 ng/ml, CA19-9 29 U/ml) Then, an expandable metallic stent (EMS) could be inserted to the stenotic common biliary duct after radiation therapy. Although para-aotic lymph nodes were existent, systemic chemotherapy (UFT 300 mg/day p.o., MMC 2 mg/week div) has been performed as an outpatient with a good quality of life.

[The clinical picture and diagnosis of mechanical jaundice during tumor growth].

Studies on the clinical course of the lesion in 525 patients with malignant tumors of the bilio-pancreato-duodenal system organs have evidenced that jaundice is not an early sign of the affection. In developed jaundice it is rational to differentiate two main diagnostic stages: presurgical which main task is differentiation between mechanic jaundice and hepatocellular jaundice, the greatest importance being attached to clinical investigation of serum enzymes and clinical data. The second main diagnostic stage is an operation during which all feasible methods of ultimate diagnosis should be employed, including direct cholangiography and urgent microscopy of a tumor.

Bile Duct Neoplasms↗

[Diagnostic value of computed tomography for the detection of invasion of the caudate bile duct branch in carcinoma of the hepatic hilum].

The visualization of the caudate bile duct branch (B1) in computed tomography (CT-scan) with a high dose of contrast medium was evaluated in 71 patients with carcinoma of the liver, biliary tract and pancreas, preoperatively. The patients were classified into four groups: Group A, 22 patients (twelve hepatomas, six gall bladder cancers and four cancers of the pancreas body or tail) without abnormal findings in the biliary tract; Group B, two patients (cancer of the pancreas head and the common bile duct) with obstructive jaundice whose CT-scans were taken before percutaneous transhepatic cholangio-drainage (PTCD); Group C, 22 patients (16 cancers of the pancreas head and six common bile duct cancers) whose CT-scans were taken after release of jaundice by PTCD; Group D, 25 patients with carcinoma of the hepatic hilum whose CT-scans were taken after release of jaundice by PTCD. The results were as follows. 1) In Group A, B1 was invisible in all the patients. 2) In Group B, B1 was clearly visible in all the patients. But in Group C, B1 was visible only in one patient. 3) In Group D, B1 was visible in 19 out of 25 patients and in 18 patients out of these 19 patients, cancer invasion toward B1 was histopathologically confirmed. In contrast, invasion was revealed only in one out of six patients whose B1s were invisible. From these results, it is concluded that in carcinoma of the hepatic hilum the visualization of B1 in CT-scan after release of jaundice by PTCD strongly suggests the cancer invasion on B1, and requests the caudate lobe resection.

[Uni- or bilateral hepatoenterostomy in central bile duct occlusion?].

With the help of different microscopic and macroscopic techniques 100 livers have been investigated to find out whether there are primary and secondary intrahepatic communications between the bile ducts of the right and the left lobe. The results of these investigations are compared with eight case reports. To find the best surgical approach to extensive tumorous blockage of the hilus it is basically necessary to know the anatomical situation in detail.
