Findings of enterotoxigenic Staphylococcus aureus strains in scarlet fever.

From 48 out of 143 scarlet fever patients in the age range of 3-15 years, Staphylococcus aureus strains were isolated in the course of hospitalization or convalescence (isolation rate 33.6%). Staphylococcal enterotoxin (SE) was produced by 19 (39.6%) of these isolates. The most frequent SE types detected were A (47.4%) and C (36.8%); type B was produced by 2 strains, type E by 1, while type D, or a combination of SE types, was not detected at all (by means of immunodiffusion in a micromodification). So far no significant correlation has been found between the clinical course of scarlet fever or accompanying complaints and presence of SE-positive S. aureus strains. The significance of these findings is discussed.

[Stage-by-stage rehabilitation of patients with myocardial infarct].

Continuous stage-wise rehabilitation was used in 102 patients who survived acute macrofocal myocardial infarction that was of a transmural type in nearly 60% of them. While hospitalized in a clinic, the patients underwent a 5-week physical activization course. Depending on the severity of the lesion the average duration of their hospitalization ranged from 34.2 to 43.1 days. Seventy patients (68.8%) were referred to the local cardiological sanatorium directly from the hospital. Subsequently all these patients were followed-up (up to 2 1/2 years) and treated on an out-patient basis in the cardiological service of the clinic. Among 88 patients who had worked before infarction, 72 persons (81.8%) returned to their professional activities after 4.9 months, on the average, following the development of myocardial infarction. Professional rehabilitation proved more effective in those who had received the sanatorium treatment after hospital, 87.1% of them returning to their jobs, and nearly 1 month sooner than those who had had no sanatorium treatment. Among the main clinical and instrumental methods of control of the state of myocardial infarction cases during the rehabilitation period the test with portioned physical exercises was also used. The examinations were repeated in 80 patients, in 50 of them -- on the 30th--35th day of the disease. The obtained data indicate that the threshold tolerance of the patients increases due to the stage-wise rehabilitation measures.

Post-acute service use following acute myocardial infarction in the elderly.

This exploratory study examined the extent to which factors beyond characteristics of the patient, such as discharging hospital attributes and State factors, contributed to variations in post-acute services use (PASU) in a cohort of elderly Medicare patients following acute myocardial infarction (AMI). Thirty-seven percent of this cohort received PAS within 30 days of discharge and home health care was the most common type of service used. Patient severity of illness at hospital discharge, for-profit ownership of the discharging hospital, and discharging hospital provision of home health services were shown to be important predictors of PASU. After adjusting for many patient and hospital characteristics, however, variation in PASU remained across States.
