Immediate changes of the electroencephalograms after intravenous injection of clonazepam and their relation to its effect on clinical fits in children with minor seizures.

Immediate EEG changes after intravenous administration of clonazepam and a correlation between the EEG changes and the effect of oral administration of the drug were studied in 21 children with minor seizures whose interictal EEG showed a paroxysmal abnormality. In 13 cases of infantile spasms whose EEG showed hypasrhythmia, paroxysmal discharges were completely or remarkably suppressed in 4 cases, partially suppressed in 3 cases, but not improved in 6 cases. Suppression bursts pattern was less improved. In 5 cases of Lennox syndrome, paroxysmal discharges were markedly improved in 3 cases. In a case of petit mal absence, parxoxysmal discharges were completely suppressed. In all 5 cases whose EEGs were completely improved, paroxysmal discharges reappeared 7 to 30 min after the intravenous injection. In 2 out of the 5 cases, paroxysmal discharges became severer at reappearance than before the injection. Among 12 cases whose EEG showed an improvement after the intravenous injection, their clinical seizures were improved in 9 cases, but the clinical effect was mostly transient. In the majority of the cases whose EEGs were not improved, no clinical effect was observed. There was a highly significant correlation between immediate EEG changes and clinical effect of clonazepam (p less than 0.02 by the chi-square test).

Elimination kinetics of circulating antigens and immune complexes. I. A simple multicompartment model for the analysis of blood elimination of intravenously injected antigens and immune complexes.

A multi-compartment model designed to analyse the blood elimination kinetics of intravenously injected radiolabelled antigens in experimental animals is presented. The model was fitted to experimental data on blood radioactivity levels at different times following intravenous injection of radiolabelled antigen. Numerical values were obtained for tissue association, degradation, and urinary recovery of degradation products. The model may prove useful as a test for reticuloendothelial function.

Enhancement of Doppler flow signals in the left heart chambers by intravenous injection of sonicated albumin.

OBJECTIVES: The objective of this study was to evaluate the effect of a transpulmonary contrast agent on Doppler flow signals in the left heart chambers. BACKGROUND: Echo contrast agents are good ultrasound reflectors and could be used as Doppler signal enhancers. Sonicated albumin microbubbles are transpulmonary echo contrast agents and could enhance left heart Doppler signals after peripheral venous injection. METHODS: Thirty-one patients with various heart diseases without intracardiac shunts were assessed with Doppler echocardiography before and after injection of sonicated albumin. RESULTS: After an intravenous injection, pulsed Doppler signals of transmitral flow became more intense in all 16 patients examined, although flow velocity itself was not changed. In Doppler color flow imaging, the maximal mitral regurgitant signal area increased from 312 +/- 405 mm2 to 434 +/- 465 mm2, an average increase of 59 +/- 40% in all 17 patients with mitral regurgitation (p < 0.01). These effects were considered to be due to improvement of signal to noise ratio by the enhancement of Doppler flow signals. The duration of enhancement of pulsed Doppler transmitral flow signals was significantly longer than that of the left ventricular echocardiographic opacification (44 +/- 11 s vs. 17 +/- 7 s, p < 0.01). CONCLUSIONS: Intravenous injection of sonicated albumin can enhance the Doppler flow signals in the left heart chambers. This effect may be useful to improve the sensitivity of the Doppler system for detecting abnormalities of left heart blood flow such as mitral regurgitation.

Concentration change of chemotherapeutic agents in plasma and tissue after intraarterial and intravenous injection.

OBJECTIVE: To study the concentration change of chemotherapeutic agents in plasma and tissue after intraarterial and intravenous injection. METHODS: Ten mature female New Zealand rabbits were divided randomly into two groups. Fluorouracil, etopiside, and cisplatin were injected into the rabbits through the ear vein in one group and through the internal iliac artery in the other group. Blood samples and the uterus tissue specimens were collected at various time points after injection. Drug concentration in plasma and tissue was determined by high performance liquid chromatography (HPLC) method. The data were analyzed by the pharmacokinetic program 3P97. RESULTS: Regular concentration change of the three drugs in plasma and tissue was observed after the intravenous and intraarterial injection, which met the two - compartment model. The pharmacokinetic parameters of the three drugs after intravenous and intraarterial injection were different. The peak concentration in plasma after intraarterial injection was lower than that after intravenous injection and the peak concentration and area under curve (AUC) value in tissue after intraarterial injections were higher than those after intravenous injection. CONCLUSION: Intraarterial chemotherapy has advantages to intravenous chemotherapy in fluorouracil, etopiside and cisplatin. These advantages depend on the drug pharmacological properties.

Preferential accumulation of 11C in human brain tumors after intravenous injection of 11C-1-pyruvate.

Regional cerebral turnover of 11C following intravenous injection of 11C-1-pyruvate was investigated in 12 patients with brain tumors. The positron emission tomography (PET) image taken at 10-15 min after the injection always demonstrated a high concentration of 11C in tumor, and thus visualized precise tumor contours. This phenomenon was not influenced qualitatively by the variation of blood flow in the tumor. Brain edema associated with the tumor was not visualized. We conclude that the localization of brain tumors would be well determined by the PET scan, using 11C-1-pyruvate.

Clinical studies on ampicillin-cloxacillin for intravenous injection (Rectocillin 'Kyowa') (author's transl).

(1) Ampicillin-cloxacillin for intravenous injection (Rectocillin 'Kyowa') was administered to 9 patients 1 g or 2 g daily for protection of postoperative infections, and good result was obtained in all cases. Eight out of 9 cases were with renal insufficiency. No side effect was encountered. (2) Rectocillin was administered to 14 patients with infections. The treatment was remarkably effective in 4 cases, and effective in 6 cases. In the cases with renal insufficiency, a daily dose of 1 g was effective. The dose administered to the patients with normal renal function was 2 g or 4 g daily. Neither clinical nor laboratory findings showed evidence of side effect.

Metabolism of lithocholate in healthy man. III. Plasma disappearance of radioactivity after intravenous injection of labeled lithocholate and its derivatives.

The plasma disappearance of intravenously injected radiolabeled lithocholate, lithocholylglycine, and their respective 3alpha-sulfates was defined in healthy subjects. Binding of these bile acids to serum protein in vitro was measured by ultrafiltration. Lithocholate and lithocholylglycine rapidly disappeared from plasma (t1/2 less than 3 min), showing a behavior similar to that of the other major primary bile acids present in man. However, after 15 min, when 99% of the radioactivity had been cleared, the disappearance curve flattened, and chromatography suggested that the chemical form of circulating radioactivity had become sulfolithocholylglycine, the end product of hepatic metabolism of lithocholate in man. Injected sulfolithocholate and sulfolithocholylglycine were cleared about one-fourth as rapidly. Lithocholate and its derivatives were tightly bound to serum proteins (87 to 99%); for a given steroid moiety and mode of conjugation, binding was altered slightly by sulfation, but all sulfated derivatives were tightly bound (greater than 95%) to serum protein. The results indicate that lithocholate, which enters the plasma compartment, whether unconjugated, conjugated, or conjugated and sulfated, will be protein-bound but nonetheless will be rapidly cleared from plasma.

[The main pharmacokinetic parameters of p-tyrosol upon intravenous injection in rats].

The main pharmacokinetic parameters ofp-tyrosol upon intravenous injection have been experimentally studied in rats. The drug concentration in the blood was determined by means of spectrofluorimetry. It is established thatp-tyrosol rapidly penetrates into organs with high blood supply. The drug is rapidly eliminated from rat organism, with T1/2 approximately 70 min.

The role of macrophages in the immunoadjuvant action of liposomes: effects of elimination of splenic macrophages on the immune response against intravenously injected liposome-associated albumin antigen.

The primary antibody response to intravenously administered and liposome-associated human serum albumin (HSA) was studied in mice under conditions where no response could be detected against the non-liposome-associated form of the antigen. The positive response against the antigen, entrapped in and/or exposed on the surfaces of liposomes, thus resulted from the adjuvant action of the liposomes. In mice intravenously injected with dichloromethylene diphosphonate (C12MDP) also entrapped in liposomes, all red pulp macrophages, marginal metallophilic macrophages and marginal zone macrophages had disappeared from the spleen 2 days after administration. Twenty-two days after such a treatment red pulp macrophages and marginal metallophilic macrophages had reappeared, but marginal zone macrophages were still absent. In mice injected with liposome-associated HSA at 2 days after treatment with the C12MDP liposomes, anti-HSA responses were severely depressed, but administration of the liposome-associated antigen 22 days after C12MDP liposomes elicited a normal response. These results point to a role of splenic macrophages in the processing of liposome-associated antigens, but marginal zone macrophages, which are located close to the open ends of the white pulp capillaries and thus are the first macrophages to meet the antigens arriving in the marginal zone are not required.

Antagonizing effects of VA-045 on reduced activity of rat locus coeruleus neurons following head injury or intravenous injection of clonidine.

Based on the finding that VA-045, a novel apovincaminic acid derivative, had improved disturbance in consciousness, we examined the effects of the drug on the electrical activity of locus coeruleus (LC) neurons in animal models of consciousness disturbance. The animal models of consciousness disturbance used in this experiment were closed head injury (CHI) and intravenous injection of clonidine. CHI as well as clonidine injection reduced the spontaneous activity of LC neurons. The reduction of the spontaneus activity of LC neurons following CHI or clonidine injection was restored by intravenous injection of VA-045. The change of LC neuronal activity induced VA-045 preceded desynchronization of EEG. These results suggest that VA-045 exerts its ameliorating effect on consciousness disturbances, at least in part, by augmenting the spontaneous activity of noradrenergic LC neurons.

The efficacies of pure LICAM(C) and DTPA on the retention of plutonium-238 and americium-241 in rats after their inhalation as nitrate and intravenous injection as citrate.

The pure carboxylated catechoyl amide LICAM(C) and the calcium and zinc salts of diethylenetriaminepenta-acetic acid (DTPA), were tested for efficacy for removing 238Pu and 241Am from rats after inhalation of the nitrate or intravenous injection of the citrate. The results were compared with the efficacy of methylated LICAM(C) used in previous experiments. It was shown that: (1) after inhalation of 238Pu nitrate, DTPA was far superior to pure LICAM(C); (2) after intravenous injection of 238Pu citrate, the infusion of DTPA plus LICAM(C) was only marginally more effective than DTPA alone; and (3) after inhalation or intravenous injection of 238Pu plus 241Am, the efficacy of pure LICAM(C) was only marginally more effective than the methylated form and neither form was effective for the decorporation of 241Am. It was concluded that DTPA, at present, remains the chelating agent of choice for treating persons accidentally contaminated with transportable forms of Pu and Am.

Cycloheximide treatment modifies the pattern of "metastasis" following intravenous injection of Ehrlich ascites tumour cells.

Cyclohexamide treatment of recipient mice at various times in relation to an intravenous injection of Ehrlich ascites tumor cells alters the pattern of consequent "metastatic" disease as compared with the pattern in untreated, tumour-injected animals. Analysis of the results suggests the existence of a cycloheximide-sensitive host mechanism for eliminating arrested tumour cells. This mechanism could play an important role in determining the ultimate localisation pattern of neoplastic disease following an intravenous injection of Ehrlich tumour cells.

[Accidental administration of racemic adrenaline. Three life-threatening cases after intravenous injection in children].

The inhalation of racemic adrenalin is an important part of the treatment of inflammatory airway obstruction in children. In Norway during the last few years there have been several cases of adrenal solutions intended only for inhalation being accidentally administered as intravenous injections. The solution for inhalation contains an adrenalin concentration 110 times greater than the adrenalin intended for emergency use (0.1 mg/ml). The instant consequences of intravenous injections of inhalation adrenalin include arterial hypertension followed by hypotension, cardiac ischemia and cardiac insufficiency, pulmonary oedema, and respiratory failure and the need for artificial ventilation. The clinical picture in the three patients we describe was very dramatic. The injected doses were 0.16-1.1 mg l-adrenalin per kg body weight. All children survived without sequelae. In order to reduce the risk of accidentally administering intravenous injections of adrenalin intended for inhalation a set of guidelines is being proposed.
