Conditioning prostheses viewed from the standpoint of speech adaptation.

Subjects who had adapted to experimental alveolar-palatal prostheses for 2 weeks were able to speak naturally with them after a lapse of a month. Physiologic data on jaw motions and tongue-palate contacts during speech showed a rapid return to compensatory articulatory patterns. A single subject who had previously adapted to an experimental prosthesis over a period of a week made the same type of compensatory jaw adjustments when speaking with the prosthesis again 1 year later.

A flexible moulage for the fabrication of an orbital prosthesis.

A technique is disucssed by which a flexible three-piece moulage may be fabricated. This procedure is a useful aid in the modeling of an orbital prosthesis. It permits the continued transfer of a wax pattern from the moulage to the patient utilizing the tissue undercuts for the retention and stabilization of the pattern. The critical task of properly positioning the ocular prosthesis is simplified by the increased stability of the entire pattern. This is of the utmost importance since even a slight error in the alignment of the eye can entirely negate the effect of an otherwise accurate prosthesis. In addition, the retention of the wax pattern simplifies the carving procedure, and accurate detail is easily secured. The acrylic resin surface on which the final silicone prosthesis is cured has proven to be accurate. Well-adapted, thin margins of the facial prosthesis have been consistently obtained.

Magnetically connected removable sectional denture for a maxillary defect with severe undercut: a clinical report.

A removable sectional denture connected by a cobalt-samarium magnetic retention system ( Hicorex for a patient with a severe maxillary defect was described. After an observation period of 5 years, the obturator continues to function, with no deleterious effects resulting from the use of prosthetic components. This magnetic retention system is considered to be clinically useful for interrelating and retaining a sectional prosthesis parts.

Obturator prostheses for hemimaxillectomy patients.

Maxillary defects are created by surgical treatment of benign or malignant neoplasms, congenital malformation and by trauma. The size and location of the defects influence the degree of impairment and difficulty in prosthetic rehabilitation. Lack of support, retention, and stability are common prosthodontic treatment problems for patients who have had a maxillectomy. A prosthesis used to close a palatal defect in a dentate or edentulous mouth is referred to as an obturator. The obturator prosthesis is used to restore masticatory function and improve speech, deglutition and cosmetics for maxillary defect patients.

Construction of a maxillary prosthesis with a hollow obturator by the balloon technique and a case report.

The authors devised a new method for making a maxillary prosthesis with a hollow obturator, which was named the Balloon Technique. This new technique has a number of advantages over the conventional method in that it not only facilitates the molding of a soft plastic hollow obturator in a single process, but also simplifies the trial fitting of the completed obturator. Furthermore, it makes possible firm adhesion of the hollow obturator and the denture base by the hot-melt method while the denture base resin is cured. Finally, the completed maxillary prosthesis is extremely light.

Lip plumper prosthesis for a patient with a marginal mandibulectomy: a clinical report.

An intraoral prosthesis was fabricated to restore more normal contour to a patient's lower lip that was compromised following a marginal mandibulectomy performed to eradicate a malignant neoplasm. The surgical resection compromised the lower lip on the affected side because the marginal mandibular branch of the facial nerve was damaged during the surgical resection. The prosthesis successfully restored the lost lip support, reduced the incidence of lip biting, and improved the patient's oral competency. This clinical report describes the procedure for making an intraoral lip plumper prosthesis to improve patient esthetics and oral function.

A maxillary lip prosthesis retained by an obturator with attachments: A clinical report.

This article describes the prosthetic rehabilitation of small maxillary lip and maxilla defects with a lip prosthesis attached to an obturator. The definitive obturator prosthesis was designed with 3 plastic ball attachments on the labial surface that provided adequate retention for the lip prosthesis. The thin margin of the lip prosthesis ensured that it adapted well to functional movements.

Direct observation of tongue positions in speech--a patient study.

Tongue position for four selected consonants, /s/, /k/, /l/, and /t/, was observed and photographed through an opening in the cheek of a patient. The cheek had been resected to remove a tumor. On comparison of the tongue positions with those described in the literature, it was found that the variations in tongue position were negligible. Although the patient had a large facial defect, speech clarity was also not affected. As reconstructive surgery was not immediately feasible, a cheek prosthesis was fabricated for the facial aspect.

In practice evaluation of a denture adhesive using a gnathometer.

PURPOSE: This study measured the extent to which the performance of a maxillary complete denture can be improved with the use of a denture adhesive. This in-practice evaluation assessed both a quantitative measure (force until dislodgement), without and with the adhesive, and patients' perception regarding the use of adhesive. MATERIALS AND METHODS: A total of 194 patients (77 men, 117 women) who wore maxillary complete dentures were tested for denture performance both without and with denture adhesive. A simple gnathometer was used. Patient perceptions were also assessed by a questionnaire and analyzed for denture performance as well as the effects on speaking and chewing, fit and comfort, and perceived confidence. RESULTS: There was a 63.4% improvement in bite force dislodgement with the use of a denture adhesive. A total of 79.2% of the patients perceived better denture performance (bite force dislodgement) with adhesive use. A total of 55.7% perceived an improvement in speaking and chewing. A total of 56.2% noticed the fit and comfort were better. A total of 63.9% responded that they had improved confidence. CONCLUSIONS: Use of a denture adhesive can improve the incisal (or protrusive) biting force for patients who wear a maxillary complete denture. Subjectively, the patients reported an improved perception of denture performance and that the use of adhesive provided greater confidence when using the prosthesis.

Use of palatal lift and palatal augmentation prostheses to improve dysarthria in patients with amyotrophic lateral sclerosis: a case series.

STATEMENT OF PROBLEM: Amyotrophic Lateral Sclerosis (ALS) is a progressive, adult onset neurodegenerative disorder manifesting itself as a loss of motor capabilities and untimely death. The dysarthria seen in patients with ALS who have bulbar symptoms causes severe problems with communication. The struggle to be understood comes at a time when progressive cumulative disabilities make communication with family, friends, and healthcare workers vital. The use of palatal lift/augmentation prostheses for dysarthria in ALS is not a frequently requested procedure by neurologists. PURPOSE: The purpose of this retrospective outcomes study was to evaluate the effectiveness of this treatment on improving speech function and intelligibility in this group of patients. This study also reviews the history, incidence, pathogenesis, and speech characteristics of the patient with ALS. METHODS: A retrospective study of 25 patients treated with a prosthesis was performed using chart reviews and phone/office interviews to evaluate the efficacy of a palatal lift and/or augmentation prosthesis to improve speech in ALS patients. RESULTS: Twenty-one patients (84%) treated with a palatal lift demonstrated improvement in their dysarthria, specifically in reduction of hypernasality, with 19 (76%) benefiting at least moderately for 6 months. Of the 10 patients treated with a combination palatal lift and augmentation prosthesis, 6 (60%) demonstrated improvement in articulation. A majority of patients indicated it was easier to speak with less effort involved when wearing the prosthesis. CONCLUSION: On the basis of this preliminary retrospective study, the use of a palatal lift/augmentation prosthesis should be considered in ALS patients with dysarthria.
