[Quantitative coronary angiography before and after PTCA. Rate of restenosis, analysis of modifying factors].

In 95 consecutive patients in whom 101 coronary artery stenoses were successfully dilated, angiography was performed before, immediately after and at an average of 6.6 months after the procedures with computer-derived stenosis measurements (based on three-dimensional vessel reconstruction). The results were referenced to an analysis of factors possibly exerting an influence on post-PTCA outcome. In 56 of the 101 dilated stenoses (minimal area prior to PTCA 0.83 +/- 0.57 mm2, after PTCA 4.09 +/- 1.7 mm2), the findings remained essentially unchanged throughout the six-month follow-up. 17 of the 56 dilated stenoses showed a further increase in minimal area as compared with the findings seen immediately after PTCA. A reduction of the minimal area of more than 1 mm2 from that achieved immediately after PTCA, associated with narrowing of the vascular lumen greater than 70% was found in 33 of the 101 stenoses (minimal area before PTCA 0.82 +/- 0.59 mm2, immediately after PTCA 3.92 +/- 1.55 mm2, six months post-PTCA 1.39 +/- 1.27 mm2). In twelve of the 101 stenoses, restenosis amounting to more than 1 mm2 was also observed but without critical compromise of the vascular lumen (minimal area before PTCA 0.81 +/- 0.39 mm2, immediately after PTCA 5.54 +/- 1.28, six months post-PTCA 3.22 +/- 1.43 mm2, corresponding to a mean luminal narrowing of 45 +/- 24%).(ABSTRACT TRUNCATED AT 250 WORDS)

Liver insufficiency as a factor modifying the pharmacokinetic characteristic of the preparation nabumetone.

The effectiveness of the nonsteroidal anti-inflammatory agent nabumetone is related to the formation of an active metabolite: 6-methoxy-2-naphthylacetic acid. The plasma concentrations of nabumetone and its active metabolite, after administration of 1,000 mg single dose p.o., are followed at 0, 0.5, 1, 2, 4, 6, 8, 12, 24, 30 and 48 h in 6 patients with laboratory evidence for functional liver insufficiency caused by liver cirrhosis, confirmed by biopsy. In the patients with liver insufficiency the mean values of Cmax and AUC0-24 h for 6-methoxy-2-naphthylacetic acid were 26.75 +/- 8.45 mg/l and 623.64 +/- 161.8 mg X h/l respectively. They did not differ significantly from the values observed in healthy volunteers [v. Schrader et al. 1983]. The Tmax-value was prolonged to 8 h, compared to 4 h Tmax-value of the volunteers. There is a tendency towards a reduced bioavailability of 6-methoxy-2-naphthylacetic acid after nabumetone administration in patients with a more severely expressed pathologic impairment, compared to patients with slight morphologic changes of the liver parenchyma.

Erythrocyte reinfusion and maximal aerobic power. An examination of modifying factors.

Induced erythrocythemia is associated with a variable increase in maximal oxygen uptake. To examine the roles of the magnitude of change in hemoglobin concentration and the individual's initial aerobic fitness in this effect, we combined individual data from our own research and three other studies. In each study freeze-preserved erythrocytes from the product of 2 units of blood were reinfused, and maximal oxygen uptake was measured within 24 to 72 hours after reinfusion. The 30 subjects had an initial aerobic power of 36 to 88 mL of oxygen per kilogram per minute. The combined results from these studies indicate that after erythrocyte reinfusion the increase in hemoglobin concentration is fairly homogeneous (mean +/- SD, 1.36 +/- 0.6 g/dL [13.6 +/- 6 g/L]), nearly all individuals demonstrate an increase in maximal oxygen uptake (0.357 +/- 0.216 L/min), the magnitude of increase in hemoglobin concentration is not related to the magnitude of increase in maximal oxygen uptake, and the magnitude of increase in maximal oxygen uptake is related to the individual's initial aerobic fitness. Individuals with an initial aerobic fitness between 50 and 65 mL X kg-1 X min-1 experience approximately twice the increase in maximal oxygen uptake after erythrocyte reinfusion of individuals with greater fitness and also of lesser fitness.

Occurrence and detection of natural mutagens and modifying factors in food products.

Various food products of plant origin were investigated for the occurrence of natural mutagens using the Salmonella/microsome assay. In general, food plants were freeze-dried and subsequently extracted with a number of solvents. Solvents were evaporated and the residues obtained were tested for mutagenicity. In addition to S9-mix, gut flora extracts were applied for metabolic activation. From bracken fern (Pteridium aquilinum) a novel mutagen, designated Aquilide A, was isolated and its chemical structure was identified. Aquilide A requires activation to become mutagenic. This activation occurs spontaneously at pH levels above 6-7. Activated Aquilide A was found to be genotoxic in cultured mammalian cells. Natural mutagens were detected in 4 out of 6 vegetables investigated. In addition, broad beans (Vicia faba) were found to be mutagenic after treatment with nitrite. All mutagenic vegetables showed marked intercultivar variations. From lettuce and string beans quercetin was isolated (after chemical hydrolysis) and in rhubarb emodin, an anthraquinon, was detected. The mutagenic activity of these two compounds was further investigated using cultured mammalian cells. Quercetin and emodin responded negative or weakly positive in the systems applied. The genotoxic properties of a number of pyrrolizidin alkaloids, which are reported to occur in various flowering plants and as a result occur in honey and some herbal preparations, were studied using a cocultivation system of V79 Chinese hamster cells and primary cultures of chick embryo hepatocytes (PCCEH/V79). All four pyrrolizidine alkaloids investigated were found to be potent inducers of SCEs in this test system. Anti-mutagenic effects of butylated hydroxyanisole (BHA) and indole-3-carbinol (I3C) were detected using the PCCEH/V79 cocultivation system. This indicates that the cocultivation system described can be a valuable tool for the screening of various products for potential anti-carcinogenic properties. Extracts of lettuce and string beans, and a number of natural chemicals were found to reduce the mutagenic activity of cigarette smoke condensate and benzo (a) pyrene (BaP) as detected in the Salmonella/microsome assay. Intercultivar variation with respect to the antimutagenic activity observed was less pronounced than the variation noted for the mutagenic activity of these vegetables. Measures which may result in a reduction of the exposure to a number of natural mutagens are discussed.
