Reducing musculoskeletal burden through ergonomic program implementation in a large newspaper.

OBJECTIVES: To assess the impact of a workplace ergonomic program to reduce musculoskeletal burden among newspaper employees and to understand relationships among participation, risk factor changes and health status within an employee cohort. METHODS: We conducted repeat cross-sectional surveys, with 1,003 employees from all major departments in 1996 and 813 in 2001, generating a cohort of 433 participants in both surveys. Elements of the ergonomic program included employee RSI (repetitive strain injury) training, pro-active assessment of workstations and workstation modifications, and encouragement of early treatment through on-site physiotherapy. Potential risk factors included biomechanical and work organizational aspects of office work. Health status measures included pain intensity and the Work-Disability of the Arm, Shoulder, and Hand (DASH). Repeat cross-sectional analyses incorporated modifications for shared variance. For the cohort, a theory of change informed path analyses using MPLUS. RESULTS: Among respondents in 2001, 69% reported participation in RSI training and 56% had workstation assessments. Among those with pain, 57% had consulted a health practitioner, including the on-site physiotherapist. In repeat cross-sectional analyses, the proportion reporting moderate pain or worse, at least once per month or for longer than 1 week, declined from 20 to 16% (p=0.01). Among the cohort, pain intensity and work disability in 1996 were the strongest predictors of 2001 health status (both p<0.001). Stable or increased supervisor awareness and concern about RSI was associated with decreased pain in 2001(p<0.01). Participation in RSI training was associated with increases in decision latitude (p<0.05), which themselves were associated with decreased work disability in 2001 (p<0.05). Increased time mousing was associated with increases in work disability (p<0.05). CONCLUSIONS: Implementation of a worksite ergonomics program was associated with a reduction in frequent and severe pain in the workforce. Changes in work disability were affected by multiple factors.

The on-line study of sentence comprehension: an examination of dual task paradigms.

This paper presents three studies which examine the susceptibility of sentence comprehension to intrusion by extra-sentential probe words in two on-line dual-task techniques commonly used to study sentence processing: the cross-modal lexical priming paradigm and the unimodal all-visual lexical priming paradigm. It provides both a general review and a direct empirical examination of the effects of task-demand in the on-line study of sentence comprehension. In all three studies, sentential materials were presented to participants together with a target probe word which constituted either a better or a worse continuation of the sentence at a point at which it was presented. Materials were identical for all three studies. The manner of presentation of the sentence materials was, however, manipulated; presentation was either visual, auditory (normal rate) or auditory (slow rate). The results demonstrate that a technique in which a visual target probe interrupts ongoing sentence processing (such as occurs in unimodal visual presentation and in very slow auditory sentence presentation) encourages the integration of the probe word into the on-going sentence. Thus, when using such 'sentence interrupting' techniques, additional care to equate probes is necessary. Importantly, however, the results provide strong evidence that the standard use of fluent cross-modality sentence investigation methods are immune from such external probe word intrusions into ongoing sentence processing and are thus accurately reflect underlying comprehension processes.

Inferences on a life distribution by sampling from the ages or the ages at death.

Consider a system where units having independent and identically distributed lifetimes enter according to a nonhomogeneous Poisson process. After the unit's life in the system, the unit departs the system. For a fixed system time, this paper relates the units' common underlying life distribution with the distribution of the ages of units in the system, the distribution for the system life of units that departed the system and the distribution for the system life of units that have recently departed the system. Results can be used to estimate the underlying life distribution or a truncated version of that distribution based on the ages and/or most recent ages at death in both one sample and two sample situations. Results include a complete characterization of the possible distribution of the ages of those units in the system, how to estimate the underlying life distribution from the most recent ages at death, and how to test for an underlying monotone failure rate function based on independent samples from the ages and most recent ages at death. Two sample inferences that involve a likelihood ratio ordering make use of the results in Dykstra et al. (1995, J Amer Statisc Assoc 90(431):1030-1040), which provides the maximum likelihood estimators and a likelihood ratio test when the two distributions satisfy a likelihood ratio ordering. For the ages of the active units and the ages at death among the departed units, limits for their distributions and strong limiting results for their empirical distributions will be provided.

The case of a newspaper phobia in a 9-year-old child.

The case of E, a 9-year-old, male, with a newspaper phobia, is presented. The phobia developed over a 4-year period and gradually spread until it included many significant areas of E's life. Treatment took place over a 4-month period. It involved use of a token economy administered by E's parents. Cognitively, the treatment involved the concepts of the acquisition of perceived self-control as a consequence of the token economy agreement. This included volition, attention, and self-control by E as well as parental reinforcement.

Impact of the mass media on calls to the CDC National AIDS Hotline.

This paper considers new computer methodologies for assessing the impact of different types of public health information. The example used public service announcements (PSAs) and mass media news to predict the volume of attempts to call the CDC National AIDS Hotline from December 1992 through to the end of 1993. The analysis relied solely on data from electronic databases. Newspaper stories and television news transcripts were obtained from the NEXIS electronic database and were scored by machine for AIDS coverage. The PSA database was generated by computer monitoring of advertising distributed by the Centers for Disease Control and Prevention (CDC) and by others. The volume of call attempts was collected automatically by the public branch exchange (PBX) of the Hotline telephone system. The call attempts, the PSAs and the news story data were related to each other using both a standard time series method and the statistical model of ideodynamics. The analysis indicated that the only significant explanatory variable for the call attempts was PSAs produced by the CDC. One possible explanation was that these commercials all included the Hotline telephone number while the other information sources did not.

Mass media campaign--A Su Salud.

A mass media health promotion program directed toward reducing future cancer trends among Mexican Americans, the largest subgroup of Hispanics in the United States, by decreasing smoking and encouraging smoking prevention and other health practices is described. Included is an outline of the program design and its significant features and a discussion of social modeling, the theoretical approach which provides a framework for the program. The development of the program, including the role focus groups played in the identification of areas to be targeted by the program, and the production and implementation of the mass media campaign based upon the targeted program areas are also discussed.

[Violence and negligence to children and adolescents in institutions].

Institutional abuse comprises a structural aspect directly bound to the institutional area itself as well as direct abuses of all kinds which are committed by members of the staff. Such direct abuses can be felt by those who enact them or by observers as being legal in their context or as illicit and therefore concealed. Information about violence in institutions is rarely found in scientific literature. Some books, usually underground, and newspapers deal with the problem. Testimony from professionals working within institutions may not be justified or may be flawed by ulterior motive. However, when correctly decyphered and interpreted, it is absolutely necessary. Enquiries are difficult and not always conclusive. The paper presents a typology of "violent" institutions and proposes some recommendations to make this problem better known.
