Synovial sarcoma of the soft palate in a child: a light and electron microscopic study.

An extremely rare case of synovial sarcoma localized in the soft palate of a 9-year-old boy is reported, describing all details noted during light and electron microscopic observation. All features characteristic of this mesenchymal tumour were evident in the light microscopy. Electron microscopy also confirmed the diagnosis. The text includes full particulars of the therapy administered.

Resection of palatal tumours with the CO2 laser.

On the basis of the authors' experience with 20 patients, CO2 laser resection of palatal tumours has proved to be a good alternative to conventional surgical resection. The CO2 laser beam permits precise resection, due to only slight intra-operative bleeding coupled with use of the operating microscope. Wound healing is good and post-operative pain remarkably little.

Atypical peripheral ameloblastoma of the palate.

Clinical, radiological and histological characteristics of the peripheral ameloblastoma are briefly outlined. A case found occurring in the palate and presenting with atypical histological features is reported. The differential diagnosis of this lesion, its treatment and histogenesis are discussed.

Polymorphous low grade adenocarcinoma of the palate in a child.

We report a case of polymorphous low grade adenocarcinoma involving the palate of a 12-year-old girl, the first example of this tumour occurring in the paediatric age group. The tumour displayed infiltrative growth, neural invasion, variegated histological patterns, and minimal cytological atypia. The patient remained disease-free four years after wide local excision of the tumour. The distinction of polymorphous low grade adenocarcinoma from pleomorphic adenoma and adenoid cystic carcinoma is also discussed.

Carcinoma of the soft palate.

During a 10 year period 30 patients with carcinoma of the soft palate were treated. There were 22 cases in stages I and II. A curative dose of irradiation was used in 25 cases, four of whom required surgical salvage. Another five cases were treated by a combined modality of surgery and postoperative radiation. Seven cases had nodal metastases. A mean two-year disease-free survival of 83 per cent was obtained. The probability of five-year disease free survival was 65 per cent.

A new application of a screw-plate for prosthetic reconstruction after bilateral total maxillectomy.

Satisfactory functional and aesthetic rehabilitation of a patient after total bilateral maxillectomy is a great challenge to the surgeon and prosthodontist. A particular problem is support and retention of the obturator. In the case described, the problem was solved by attaching the prosthesis with finger clasps to a classic AO-reconstruction plate anchored rigidly with screws to both zygomata. The results in terms of mastication, swallowing, phonation and aesthetics were satisfactory for more than 2 years, when the patient died from brain metastasis and pneumonia. The technique seems promising and merits further study using titanium screws and plates.

Reverse smoking in Andhra Pradesh, India: a study of palatal lesions among 10,169 villagers.

In the district of Srikakulam in Andhra Pradesh in South India the habit of reverse smoking is widespread. In a house to house survey of oral cancer and precancerous conditions, comprising 10,169 villagers, 43·8% were reverse smokers; the female: male ratio being 1·7: 1. Ten previously undiagnosed cases of oral cancer, 9 located to palate, were found. The prevalences for leukoplakia, preleukoplakia and leukokeratosis nicotina palati were 4·9%, 2·9% and 9·5%. Of 497 leukoplakias, 422 were located to the palate and 413 of these patients had the habit of reverse smoking. Histologically, 80% of 381 palatal biopsies had a hyperorthokeratosis. Epithelial atypia was found in 15·3% of leukoplakias, in 3·6% of the preleukoplakias, and in 9·1% of leukokeratosis nicotina palati. Various explanations for the habit of reverse smoking are suggested and discussed. In addition to reverse smoking, other tobacco habits were also recorded; all habits and oral lesions were compared and have been discussed according to their apparent interrelationships and distribution by sex and age.

A correlative histocytological study of carcinoma and epithelial atypia of the palate among Indian reverse smokers.

A correlative histocytological study was made of 6 patients with palatal carcinomata and 342 patients with palatal lesions (primarily leukoplakias) associated with reverse smoking from the Srikakulam district of Andhra Pradesh. Among 6 histologically diagnosed carcinomata only 2 showed cytological findings typical of carcinoma. Of the 46 atypias diagnosed histologically among the other palatal lesions, only 6 (13%) were diagnosed cytologically. Our findings show that cytological examination of precancerous and cancerous lesions located on the hard palate, which is a highly keratinized area of the oral cavity, may not be reliable enough for revealing premalignant or malignant changes.

Investigations of tritiated menadiol sodium diphosphate (T-MNDP) as a radioactive drug.

An attempt has been made to develop tritiated derivatives of Synkavit (menadiol sodium diphosphate, MNDP) of high specific activity as a radioactive drug. This paper summarizes the preliminary biological and physical studies, with emphasis on approximate radiation dosimetry and the necessary preliminary testing, and then gives an account of the clinical investigations and the trials carried out so far, which correspond essentially to Phases I and II trials for a chemotherapeutic agent. In all, 214 patients with different sites and types of advanced and recurrent, inoperable, histologically verified malignant tumours including reticuloses have been treated with doses of at least 1 Ci of the various preparations. Among the 203 evaluable treated cases, some form of response was observed in 23 out of 151 (15·2%) receiving the drug by intravenous injections and 13 out of 52 (25%) after intra-arterial injections. For the sites and types of malignant diseases which showed responses after either intravenous or intra-arterial administration among the 55 patients surviving at least 3 months after the first injection, some form of response was observed in 32 but only 5 of these showed either a "complete" or a "partial" response. It is concluded that further investigation is desirable. It is suggested that clinical trials with randomization should be carried out for inoperable cases of carcinoma of the colon and of the pancreas.
