Evaluation of the fluoride release from orthodontic band cements.

PURPOSE: To compare the fluoride levels released from a polyacid-modified resin composite (Ultra BandLok), a resin-modified glass-ionomer cement (Multicure) and a conventional glass-ionomer cement (Meron). METHODS: Fluoride concentration in the deionized water was measured at 1, 2, 3, 7, 14 and 28 days. Fluoride measurement was undertaken using a fluoride ion selective electrode connected to an ion-analyzer. Friedman test was used in the repeated measurements of multiple groups. Kruskal-Wallis test was used to compare the groups. Dunn's multiple comparison test was applied for the comparison of subgroups. RESULTS: The fluoride release pattern of the three materials was similar. The highest level of fluoride was obtained at the 24-hour measurement from all samples. According to comparisons among materials, Ultra BandLok released significantly less fluoride than Multicure and Meron (P < or = 0.01) at all the measurement times. In addition, the differences between the fluoride concentrations released from Multicure and Meron materials at all measurement days were not significant (P> 0.05).

Diagnostic protocol in cases of congenitally missing maxillary lateral incisors.

Patients with congenitally missing maxillary lateral incisors may seek treatment from an orthodontist for several reasons; unpleasant smile, dental arch disharmony, and midline deviations are among the most frequent complaints. Orthodontists and prosthodontists have raised important questions about the esthetics, occlusion, periodontal health, and psychological conditions noted in these patients. The decision to open lateral incisor spaces for prosthetics or close spaces by mesial movement of the canines requires a careful diagnosis and comprehensive multidisciplinary treatment planning. The purpose of this article is to present a diagnostic protocol to assist in the decision-making process, while analyzing patients with absent maxillary lateral incisors. The basis for a sound diagnosis and an effective treatment plan resides in careful consideration of the facial and dental characteristics of each patient. All information gathered during the clinical examination should be added to data obtained with other diagnostic tools, such as cephalometric analysis and orthodontic waxups, to achieve the treatment option that best suits the patient's expectations.

Surface condition of primary teeth after approximal grinding and polishing.

Mesiodistal reduction of primary teeth is necessary in cases with mild or moderate primary crowding, in patients treated with Fränkel appliance type I or II or in those who need to keep a primary molar as long as possible due to congenitally missing premolars. In order to minimize the risks of periodontal destruction and caries, the tooth surface should be polished after grinding. A technique for stripping permanent teeth, recently proven successful in achieving smoother enamel than untreated tooth surfaces, was slightly varied and applied to primary teeth. By means of scanning electron micrographs, it could be shown that application of this technique produces smooth surfaces on primary teeth as well. The procedure is described in detail and cases in which it should be used are presented.

[Spacing correction in adult patients].

Spacing due to periodontal disease with migration of adjacent teeth to extraction space is a common problem in adult patients. Appropriate orthodontic treatment is one of the measures to prevent aggravation of periodontal structure. The objective of the article was to present basic principles of adult orthodontics comprising: establishment of mandibular position, control of tooth movement in three dimensions of space, anchorage preparation, prevention of iatrogenic effects and prevention of relapse along with 2 case reports.

[Follicular cyst. Apropos of a case].

When there's a diagnosis of follicular cyst. In front of lesions of advanced decay in deciduoud teeth about which is possible to presume chronical apical infection, we have to evaluate carefully the choice of treatment: pulpectomy or exodontia and the placement of a space maintainer. In this article there's a case in which a non-indicated pulpectomy was practised and caused a follicular cyst which is a menace to the formation and position of the unerupted permanent teeth. After the surgery we proceeded to exodontia and subsequently to place a space maintainer.

A transition space maintainer for the adolescent dentition: a case report.

Premature loss of teeth in posterior segments of the primary, mixed or permanent dentition may lead to space loss in an anterior-posterior dimension. This article illustrates how a simple, aesthetic and effective space maintainer can be used to prevent space loss in the permanent dentition.

[On eruption of mandibular third molar after extraction of mandibular first or second molar].

In the orthodontic practice, the first or second molars are extracted by reason of a necessity based on the treatment planning. In these cases, it is desirable that the third molar would be taken part in the masticatory function. This study was made to investigate the natural movement of the erupting third molars after the extraction of the first or second molar on the serial lateral cephalometric radiographs and orthopantomographs. Subjects were divided into three groups. The first is the group which the mandibular second molars were extracted. The second is the group which the first molars were extracted bringing about the successful eruption of the third molars. The third is the group which the first molars were extracted with resultant in the failure of the third molar eruption. In all cases, the orthodontic forces were not applied to the third molars. The results were as follows: 1. In the first group, all third molars were successfully erupted and a lot of the spaces after the extraction of second molars were utilized for the eruption of third molars. 2. In the second group, all third molars were successfully erupted and a lot of the spaces after the extraction of first molars were utilized for the mesial movement of the second molars. 3. In the third group, there were various patterns of the impaction of the third molars. It was suggested that the eruption of the third molar was related to the space distal to the second molar but it was not related to the anterio-posterior length of the mandibular body significantly.

[Rehabilitation after early loss of deciduous molars in mixed dentition].

Early loss of deciduous teeth in the lateral portions of an early mixed dentition usually causes shortening of the supporting zone. Secondary cramming of the teeth develops and there is not enough space for the permanent canines and premolars. In case of early losses of deciduous teeth prosthesis rehabilitation is indicated. The authors examined a group of 212 children (--111 boys and 101 girls)--from the 1st-5th form of a primary school in Prague 2. The age varied between 6 and 10 years. In 19 children early losses of deciduous teeth in the lateral portions of the dentition were found. Early losses in the lower jaw predominated. Multiple losses of deciduous teeth were recorded more frequently in both the upper and lower jaw.

[Space analysis and treatment planning in the mixed dentition].

The maintaining of the space during mixed dentition, allows the patient to obtain a good occlusion, the right growth and facial development. The orthodontist can preserve the space, using simple appliances like the lip bumper and the lingual arch.
