[An investigation on risk factors of cerebral arteriosclerosis].

OBJECTIVE: To investigate the risk factors of cerebral arteriosclerosis (CAS) in the special population--the old cadre in organizations affiliated directly under Fujian Province. METHODS: The investigation was carried out according to the case control method of epidemiology and conditional logistic regression model was used. RESULTS: Multivariate analysis showed that CAS was closely related with the clinostatic systolic blood pressure (CSBP, OR: 2.13), triglyceride (TG, OR: 2.08), diabetes mellitus (OR: 1.31), glycosylated hemoglobin (OR: 1.16), recall of portrait characteristic (OR: 0.89), Kidney-Deficiency Syndrome (OR: 1.54) and Yin-Deficiency with Yang-Excess Syndrome (OR: 1.59). And there was a quantity-effect relationship between CSBP, TG and CAS. CONCLUSION: CSBP is the most important risk factor of CAS in the special population of the old cadre, and the TCM Syndromes of Kidney-Deficiency and Yin-Deficiency with Yang-Excess are also independent risk factors of CAS.

[Clinical study on hyperthyroidism of yang hyperactivity type due to yin deficiency treated by jiakang ning capsule].

OBJECTIVE: To investigate the effect of Jiakang Ning (JKN) capsule on hyperthyroidism. METHODS: Eighty-five cases of hyperthyroidism were divided into JKN group (24 cases), JKN with low dose Tapazol group (35 cases) and Tapazol control group (26 cases) at random. The effects were evaluated by total effective rate, using principal symptoms, body weight, EKG, thyroid hormone and atrial natriuretic peptide (ANP) after 6 months. RESULTS: The total effective rates of three groups were 83.8%, 97.1%, 88.5% respectively, and there were insignificant difference among them statistically (P > 0.05). The first two groups had better effect in principal symptoms than Tapazol control group (P < 0.05). There were similar results in adjusting thyroid dysfunction and abnormal EKG, decreasing ANP and increasing body weight among those three groups (P > 0.05). CONCLUSION: The effect on hyperthyroidism treatment by JKN capsule was satisfactory. The combination of Chinese herbs with western medicine could enhance the curative effect of hyperthyroidism without obvious side-effects.

[American ginseng compound liquor on retarding-aging process].

71 cases with age over sixty were randomly divided into treated group and control group, and observed by single-blind method. 36 cases were administered with American ginseng compound liquor as a treated group, 35 cases were administered with American ginseng liquor only as a control group. The total effective rates of the treated group and the control group on symptoms associated with aging were 88.89% and 68.57% respectively (P less than 0.05). The effective rates of the treated group for Kidney-Yang, Kidney-Qi and Kidney-Yin deficiencies were 85.71%, 100% and 87.50% respectively. The effective rates of the control group were 81.82%, 100% and 61.53% respectively. The above results indicated that the symptoms of Kidney-Yang deficiency in the treated group were improved much better than those of the control group (P less than 0.05). Obviously, SOD activity of erythrocyte and SOD/LPO ratio increased remarkably and serum content of LPO decreased significantly in both groups (P less than 0.001). In the treated group, the functional months of age (physiological age) decreased from 751.77 +/- 5.215 to 743.53 +/- 5.144, the effective rate was 68.57%. It showed that these two recipes both had the efficiency on prolonging the functional age (P less than 0.05).

[Study on the regulative rule of reinforcing shen principle on sexual precocity and senescence at the molecular level].

The imbalance of Shen-yin and Shen-yang is a common pathological basis of sexual precocity and senescence. Senescence can be regarded as physiological Shen-deficiency, which is mainly the deficiency of Shen-Yang, while the essence of sexual precocity is hyperactivity of Fire due to Yin-deficiency, and deficiency of Shen-Yin is the predominance in the imbalance. The process of growth, development, maturing and aging led by Shen-qi, is mainly involving the hypothalamus-pituitary-growth hormone axis and sexual gland axis. Early initiation of these two axes appears in sexual premature children, manifesting syndrome of Fire hyperactivity due to Yin-deficiency, while in senescence, the decline of the axes appears ahead of time and manifests syndrome of Yang qi consumption. It has been shown in experimental and clinical researches that significantly good effects could be obtained by applying Chinese recipe for warmly reinforcing Shen-Yang to postpone senescence and recipe for nourishing Yin and removing Fire to inhibit sexual precocity. It also been proved by determining syndrome and speculating the damaged site according to the effects of drug that the pathologic damage of sexual precocity and senescence is sited on the hypothalamus or higher center.

[17-ketosteroid sulfates and its application to kampo-medicine].

Traditional oriental medicine tells us that everything is interrelated and consists of combinations of opposing factors, "Yin-Yang". So we need to evaluate the normal state and pathologic condition from the perspective of dynamic hormone balance between cortisol and DHEA which are opposing and physiologically very important. Cortisol has a more functional side and is considered Yang, while DHEA has a more material side and is considered Yin. We tried to explain the concepts of KAMPO which include SHO (Yin-Yang. Excess-Deficiency), the 6 Stages of Disease and Deficiency of the Kidney, by measuring urinary 17-OHCS and 17-KS-S, metabolites of cortisol and DHEA. We think our effort may develop a good objective indicator of KAMPO's diagnosis and be useful for selecting of treatment.

[Effect of Yang-warming and Qi-tonifying natural products on neuroendocrine of deficiency-cold rats].

The rat fed with the compound drug Zhi Mu and Shi Gao was made into an animal model of deficiency cold syndrome. The deficiency cold state was kept for more than 7 days after stopping the drug. So the animal model is good for studing the therapeutic effect of products of heat nature. After 3 therapeutic days with Fu Jiang and Shen Qi compounds, the D beta H activity in serum, cortisone in adrenal gland and NE, DA amount in brain were increased, but serotonin was decreased, and after 7 days the effect was increased significantly. Both compounds could excite the sympathetic nerve and endocrine system in the peripheral and central nervous systems. Shen Qi compound acts quickly, but does not last as long as Fu Jiang. The effect will be better if the two compounds are used together.

N-cadherin is not essential for limb mesenchymal chondrogenesis.

The cell adhesion molecule N-cadherin is implicated in many morphogenetic processes, including mesenchyme condensation during limb development. To further understand N-cadherin function, we characterized a new N-cadherin allele containing the lacZ reporter gene under the regulation of the mouse N-cadherin promoter. The reporter gene recapitulates the expression pattern of the N-cadherin gene, including expression in heart, neural tube, and somites. In addition, strong expression was observed in areas of active cellular condensation, a prerequisite for chondrogenic differentiation, including the developing mandible, vertebrae, and limbs. Previous studies from our laboratory have shown that limb buds can form in N-cadherin-null embryos expressing a cardiac-specific cadherin transgene, however, these partially rescued embryos do not survive long enough to observe limb development. To overcome the embryonic lethality, we used an organ culture system to examine limb development ex vivo. We demonstrate that N-cadherin-deficient limb buds were capable of mesenchymal condensation and chondrogenesis, resulting in skeletal structures. In contrast to previous studies in chicken using N-cadherin-perturbing antibodies, our organ culture studies with mouse tissue demonstrate that N-cadherin is not essential for limb mesenchymal chondrogenesis. We postulate that another cell adhesion molecule, possibly cadherin-11, is responsible for chondrogenesis in the N-cadherin-deficient limb.

A murine model for human sepiapterin-reductase deficiency.

Tetrahydrobiopterin (BH(4)) is an essential cofactor for several enzymes, including all three forms of nitric oxide synthases, the three aromatic hydroxylases, and glyceryl-ether mono-oxygenase. A proper level of BH(4) is, therefore, necessary for the metabolism of phenylalanine and the production of nitric oxide, catecholamines, and serotonin. BH(4) deficiency has been shown to be closely associated with diverse neurological psychiatric disorders. Sepiapterin reductase (SPR) is an enzyme that catalyzes the final step of BH(4) biosynthesis. Whereas the number of cases of neuropsychological disorders resulting from deficiencies of other catalytic enzymes involved in BH(4) biosynthesis and metabolism has been increasing, only a handful of cases of SPR deficiency have been reported, and the role of SPR in BH(4) biosynthesis in vivo has been poorly understood. Here, we report that mice deficient in the Spr gene (Spr(-/-)) display disturbed pterin profiles and greatly diminished levels of dopamine, norepinephrine, and serotonin, indicating that SPR is essential for homeostasis of BH(4) and for the normal functions of BH(4)-dependent enzymes. The Spr(-/-) mice exhibit phenylketonuria, dwarfism, and impaired body movement. Oral supplementation of BH(4) and neurotransmitter precursors completely rescued dwarfism and phenylalanine metabolism. The biochemical and behavioral characteristics of Spr(-/-) mice share striking similarities with the symptoms observed in SPR-deficient patients. This Spr mutant strain of mice will be an invaluable resource to elucidate many important issues regarding SPR and BH(4) deficiencies.

Does periconceptional multivitamin use reduce the risk for limb deficiency in offspring?

There is accumulating evidence that periconceptional multivitamin use may prevent the occurrence of some birth defects other than neural tube defects. Using data from the population-based Atlanta Birth Defects Case-Control Study, we investigated the possible association between periconceptional multivitamin use and the occurrence of limb deficiency. We examined the periconceptional use of multivitamins among mothers of 117 babies with nonsyndromic limb deficiency who were liveborn or stillborn to residents of metropolitan Atlanta from 1968 to 1980 and among mothers of 3,029 control babies born without birth defects who were randomly selected through birth certificates. We found that children whose mothers were periconceptional multivitamin users had a lower risk of having a limb deficiency [odds ratio (OR) = 0.47; 95% confidence interval (CI) = 0.23-0.97]. This protective effect, however, was mostly seen for transverse limb deficiency (OR = 0.30; 95% CI = 0.07-1.32) and not for longitudinal deficiency (including preaxial and postaxial deficiencies; OR = 1.03; 95% CI = 0.17-4.30). Adjustment for potential confounding factors did not change these findings. We found a trend of decreasing risk for all transverse limb deficiencies with earlier vitamin use. These data indicate that mothers' periconceptional multivitamin use may reduce the risk for some types of limb deficiency among their offspring. In addition, because we did not find the protective effect for all types of limb deficiency, the data may also indicate causal heterogeneity of limb deficiencies.

Phenotype analysis of aquaporin-8 null mice.

Aquaporin-8 (AQP8) is a water-transporting protein expressed in organs of the mammalian gastrointestinal tract (salivary gland, liver, pancreas, small intestine, and colon) and in the testes, heart, kidney, and airways. We studied the phenotype of AQP8-null mice, and mice lacking AQP8, together with AQP1 or AQP5. AQP8-knockout mice lacked detectable AQP8 transcript and protein, and had reduced water permeability in plasma membranes from testes. Breeding of AQP8 heterozygous mice yielded AQP8-null mice, whose number, survival, and growth were not different from those of wild-type mice. Organ weight and serum/urine chemistries were similar in wild-type and AQP8-null mice, except for increased testicular weight in the null mice (4.8 +/- 0.7 vs. 7.3 +/- 0.3 mg/g body wt). Urinary concentrating ability in AQP8-null mice was unimpaired as assessed by urine osmolality (3,590 +/- 360 mosmol/kgH(2)O) and weight loss (22 +/- 2%) after 36-h water deprivation; urinary concentrating ability was similarly impaired in AQP1-null mice vs. AQP8/AQP1 double-knockout mice. Agonist-driven fluid secretion in salivary gland was not different in AQP8 vs. wild-type mice ( approximately 1 microl.min(-1).g body wt(-1)) or in AQP5-null mice vs. AQP8/AQP5 double-knockout mice. Closed intestinal loop measurements in vivo indicated unimpaired osmotically driven water transport, active fluid absorption, and cholera toxin-driven fluid secretion in AQP8-null mice. After 21 days on a 50% fat diet, wild-type and AQP8-null mice had similar weight gain ( approximately 15 g), with no evidence of steatorrhea or abnormalities in blood chemistries, except for mild hypertriglyceridemia in the null mice. The mild phenotype of AQP8-null mice was surprising in view of the multiple phenotype abnormalities found in mouse models of AQP1-5 deficiency.

[Study on stroke syndrome of Chinese medicine and its relationship with blood hypercoagulative status and insulin resistance in diabetic ischemic stroke patients].

OBJECTIVE: To study the relationship of TCM Syndromes, involving Viscera Syndrome and involving Meridian Syndrome, with blood hypercoagulative state and insulin resistance in patients with diabetic ischemic stroke. METHODS: Insulin sensitivity index (ISI) could reflect the insulin resistance, and those reflecting blood hypercoagulative state parameters such as platelet agglutination test (PAgT), fibrinogen (FG), in vitro thrombus length (VTL) and activating partial thrombinogen time (APTT), were used to expound and prove the relationship with the stroke syndrome. RESULTS: The symptom scores, PAgT, VTL, FG levels in patients involved with Viscera Syndromes (including those of Wind Syndrome, Phlegm-Dampness Syndrome, Heat-Fire Syndrome and Yin-deficiency with Yang-excess Syndrome) were significantly higher than those in patients involved with Meridian Syndromes (P < 0.05 or P < 0.01), while the ISI level in the former was lower than that in the latter (P < 0.01). CONCLUSION: There is close relationship between blood hypercoagulative state, insulin resistance and TCM Stroke Syndromes in patients with diabetic ischemic stroke.

[Relationship of the A1166C polymorphism of ATI R gene with TCM syndrome and efficacy of Chinese hypotensor in patients with essential hypertension].

OBJECTIVE: To investigate the polymorphic distribution of the A1166C, the angiotensin II type I receptor (AT1R) gene, in the Chinese people with essential hypertension (EH), and to study the relationship of A1166C with the TCM syndrome types and the efficacy of hypotensor. METHODS: Blood pressure (BP), body weight index (BWI), fasting blood glucose (FBG), cholesterol (Ch), concentration of triglyceride, levels of serum plasma Ang II , endothelin (ET) and calcitonin-gene related peptide (CGRP) in 206 patients with EH and 86 health subjects of Chinese nationality were determined. DNA of peripheral white blood cells were extracted by salting out method and the polymorphism of AT1R gene A1166C was assessed by PCR and limited enzymatic segmentation. The 206 EH patients were differentiated into different types according to TCM syndrome differentiation, and the gene distribution appeared in each TCM syndromes was compared. Qingxin Capsule (QC) or captopril was used respectively to treat 34 and 32 patients with hypertension of Yin-deficiency and excessive Yang type to observe the relationship between the hypotensive effect of treatment and the distribution of gene types. RESULTS: (1) No significant difference was found in BWI, FBG, blood lipid or serum related hormones, only the difference in systolic and diastolic pressure was shown between the hypertension group and the control group. (2) The rate of AT1R AC plus CC gene type in EH patients was 0.126, which was significantly higher than that in the normal control group (0.047, P < 0.01). The frequency of C allele distribution was obviously higher than that in the normal control group (0.068 vs 0.023, P<0.05). No significant difference was found in AT1R gene types or frequency of allele distribution between different sexes (male and female). (3) Analysis on relationship between gene types and clinical phenotype showed that no significant difference was found in systolic pressure, diastolic pressure, blood glucose, blood lipid, plasma Ang II, ET, or CGRP between patients with AA gene type and with AA plus CC gene type by t test (P > 0.05). (4) Analysis on relationship between gene type and TCM syndrome type showed that the distribution of AA gene type and AA plus CC type was insignificantly different in patients with various TCM syndrome types. (5) Analysis on relationship between gene type and hypotensive effect of treatments showed that both QC and captopril were effective to patients with hypertension of different gene types. The hypotensive effect of either was similar in spite of what gene types the patients with. CONCLUSION: A1166C gene type is related to EH in the Chinese, C allele might a susceptible gene to hypertension, while TCM syndrome type of EH patients might not be related to whether C allele was borne. The hypotensive effect of QC or captopril was not related to whether C1166 allele was borne. No significant difference of blood pressure, blood glucose, blood lipid, plasma Ang II, ET, or CGRP in patients with different gene types, which suggested the effect of gene types on hypertension might not be by path of glucose or lipid metabolism, ET, CGRP or plasma Ang II system.

[Relation between traditional Chinese medicine differential types and plasma levels of renin, angiotension II, aldosterone, atrial natriuretic factor in patients with essential hypertension].

130 cases of patients with essential hypertension (EH) and 70 cases of normal subjects were researched for correlation between TCM differential types and plasma levels of renin, angiotension II, aldosterone, atrial natriuretic factor (ANF) in patients with EH. Results indicated that: (1) basic level of renin was lower in patients with EH than that in normal subject. There were significant differences of plasma levels of renin between different TCM types. Plasma renin level of excessive Yang patients was higher than that in normal subject group and groups of deficiency of Yin essence combined with excessive Yang as well as deficiency of both Yin and Yang (P less than 0.01-0.001). Plasma level of angiotension II was significantly higher in group of excessive Yang than that in normal subject and other two groups (P less than 0.01-0.001). It was indicated that there were correlation between plasma basic level of renin, angiotension II and TCM types. (2) Plasma ANF level in patients with EH was significantly lower than that in group of normal subject (P less than 0.01). There were significant differences between groups of three different TCM types (P greater than 0.05). The result suggested that lower plasma ANF level was general character in three groups with EH. The prognosis of these patients was discussed.

The liver in traditional Chinese medicine.

Medical thinkers in China visualized the liver in microcosmal and macrocosmal terms. An anatomical tradition did not exist, hence the liver was described grossly in broad outline. It was recognized as being functionally important in the movement of qi (vital energy) and storage of xue ('blood'). The liver corresponded to various phenomena in both the natural and social orders, according to the scheme of yin yang and five phases. These interrelationships provided the basis for the diagnosis and treatment of liver dysfunctions. The disorders fell into three general groups: (i) hepatic qi stasis; (ii) hepatic yang excess with yin deficiency; and (iii) hepatic yin insufficiency. The signs and symptoms represented the logical outcomes of the disturbed physiology. Acupuncture, moxibustion and herbal drugs were used to redress the imbalance of hepatic qi and yin-yang. The impact of Western medicine led traditional authors to recognize the hepatobiliary role in bile secretion and in jaundice. The exchange between the Western and Chinese medical traditions revealed that active agents were included in the Chinese formulary, such as glycyrrhizin, which has recently been shown to be beneficial in chronic viral hepatitis.
