[Ossification of the transverse ligament of the atlas. A report on three new cases (author's transl)].

Following the publication of the first two cases reported of ossification of the transverse ligament, the authors describe three recent observations of this lesion. In contrast to the first two cases, in which ossification was reported at the level of the cervico-occipital articulation, which was normal, these three new cases were noted to have associated acquired or congenital abnormalities of the craniovertebral junction.

Birth defects involving the spine.

There is a multiplicity of birth defects of the spine occurring as isolated anomalies or as aspects of multifaceted syndromes. The clinical significance of these anomalies varies from the asymptomatic to the life threatening. Symptoms and signs may present at birth or not until years later. The optimal clinical result for the patient requires a careful history and physical examination, appropriate routine and specialized roentgenograms, laboratory data, and a high index of suspicion for signs or symptoms of neurological dysfunction. When the diagnosis is made, conservative or surgical treatment may be instituted as appropriate.

The pathological anatomy of fatal atlanto-occipital dislocations.

Nine atlanto-occipital dislocations were found in postmortem examinations of 112 victims of multiple trauma. Axial traction facilitated roentgenographic identification of the injury. A hyperextension mechanism of injury was suggested by the associated injuries, including submental lacerations and mandibular fractures. Atlanto-occipital dislocations were more frequent in children than in adults. A pure dislocation injury without fracture was identified.

[Double malformation discovered during investigations for hypertension; single pelvic kidney and occipitovertebral joint malformation (author's transl)].

A young Portuges eman with a spastic gait was found to have a kidney lying below the bladder. This was the only kidney present and its blood supply was derived from a renal artery which was also the only one present. This renal abnormality was associated with a major malformation of the occipitovertebral joint (occipitalization of the atlas, total occipito-atlanto-odontoid dissociation above the line of the digastric). The possible relationship between these two affections is discussed.

Acute fractures and dislocations of the cervical spine. An analysis of three hundred hospitalized patients and review of the literature.

Of 300 patients who were hospitalized for acute cervical injuries, 216 lived, fifty-one died within four months of injury, and thirty-three were lost to follow-up. The important findings in a retrospective review were that laminectomy resulted in a high mortality rate and loss of motor function and that steroids did not improve neural recovery in quadriplegics and their use was associated with gastrointestinal hemorrhage. Closed or open reduction followed by posterior fusion for subluxations or dislocations, and anterior decompression and fusion for vertebral compression fractures, offered the best chance for recovery of neural function and restoration of stability. Massive epidural hemorrhage was found only in patients with ankylosing spondylitis.

[Pathomechanisms of cervical headaches].

Various types of disorders of the cervical region may produce headache. In many of these increased tension of the neck musculature plays an important role. The following mechanisms underlying headache are described and differentiated: 1. Headache due to faulty muscle pattern (stereotype) resulting in overstrain of the upper fixators of the shoulder girdle (upper part of the m.trapezius and levator scapulae); faulty respiration with the aid of the upper auxillary muscles even at rest is pointed out. 2. A forward drawn head position producing static overstrain in the posterior neck muscles and compensatory retroflexion of the cranio-cervical junction resulting in blockage in this region. 3. Anteflexion and ligament pain mainly due to faulty position at work and jolting. 4. Static disturbance in the frontal plain due to obliquity producing asymmetrical strain in the neck musculature. 5. Increased muscular tension due to psychological stress. 6. Blockage in the regions of the cervical spine, shoulder girdle and upper ribs with reflex muscular spasm. 7. Reflex spasm of the neck musculature in visceral disorders causing in addition blockage of the cervico-thoracic junction (heart, gall bladder). 8. Pain arising from the posterior arch of the atlas (here described for the first time). 9. Headache due to vertebral artery involvement. The type of headache seems to be determined rather by the individual mode of reaction than by the mechanism underlying it. As a rule a combination of mechanisms is actually found.

[Bony malformations of the cranio-vertebral junction and its clinical relevance (author's transl)].

A statistical analysis is performed in 86 cases suffering from osseous malformation of the cranio-vertebral junction. The material consists of 71 cases with basilar impression, 8 cases with hypoplasia or absence of the odontoid process, and 7 cases with a congenital atlanto-epistropheal dislocation. The patients are clinically asymptomatic in 23 cases, and are marked by a para-respectively tetraparesis in 28 cases, by bulbar-pontine-cerebellar disorders in 24 cases, and by syringomyelia in 11 cases. A control group is included consisting of 30 cases without neurological symptoms and with a normal radiological finding of the cranio-vertebral region. The following measured values are taken in consideration to perform a statistical calculation using the simple variance analysis: The height of the odontoid process above of McGregor's line, the distance between the back of the odontoid process and the inner limit of the arch of C1, and, concerning the groups with hypoplasia of the odontoid process process and with atlanto-axial dislocation, the narrowing of the spinal canal at the level of C1 dependent on movements of the head. The measured values are referred to the different clinical syndromes and are described and divided according to its statistical significance related to the clinical groups.
