[Results of cervix neoplasm prophylaxis in women examined during the health promotion program in the Central Railway Hospital].

In the program of cervical cancer prophylaxis in CRH in 1997 simultaneous cytology and colposcopy evaluation of the uterine cervix were performed. Among 1479 women examined there were 6 cases (0.4%) of CIN 3, 34 cases (2.79%) of CIN 1 and CIN 2 and no case of invasive cervical cancer. The results of these studies point at high effectiveness of simultaneous use of colposcopy and cytology as complementary methods in prophylaxis of cervical cancer.

[Role of endoscopic studies and cytogenetic indices in diagnosis of preclinical variants of cervix neoplasms].

The results of colpocervicoscopic examination of more than 15,000 females suggested cervical carcinoma in 345 cases, which was confirmed histologically in 319 patients. A detailed morphological and cytogenetic investigation carried out in 36 patients with preclinical form of TIaNXMO invasive carcinoma of cervix uteri established a relationship of mitotic activity, pathological mitosis count and fraction of cells with X-chromatin, on the one hand, and the depth of tumor invasion, on the other. The stages of complex examination of patients with pathology of cervix uteri are discussed.

[Prognosis in radiotherapy of patients with local recurrence of cervix neoplasms].

An analysis of the results of treatment in 231 patients with recurrences of cervical cancer is presented in relation to 5-year survival criterion ("dynamic analysis"). An actual 5-year survival rate was 16.0%. The most important factors determining the prognosis include the stage of the primary process, localization of the secondary tumor, terms of the recurrence and the absorbed dose of radiation given to the recurrence. Standardized rates showed a 3-5 fold improvement of the results of treatment in the presence of favourable prognostic factors. In combinations of favourable prognostic factors the 5-year survival rate of patients with recurrent cervical cancer reached 67.0%. The amount of the absorbed local dosage of radiation is of practical value for improvement of the results of treatment. Radiotherapy is the method of choice in treatment of patients with local recurrences of uterine cervix cancer.

[Laparoscopy as part of the evaluation and management of ovarian and cervix neoplasms].

INTRODUCTION: Surgery is the treatment of choice in most early stages of cervical cancer and advanced stages of ovarian cancer. Failing to preoperatively diagnose para-aortic and parametrial metastases in cervical cancer or a non-resectable, ovarian cancer, may results in a superfluous laparotomy. AIM: To evaluate the advantage of using laparoscopy in cervical and ovarian cancer. PATIENTS AND METHODS: Study population includes patients with ovarian or cervical cancer referred between 1997-1999. A CT scan and a trans-vaginal sonography were used to detect involvement of pelvic and para-aortic lymph nodes, parametrium and other metastases. In patients with cervical cancer, para-aortic lymph node dissection was laparoscopically performed. When para-aortic nodes were negative and parametrium was clear, radical hysterectomy and pelvic lymph nodes dissection was conducted through laparotomy. When para-aortic lymph nodes or parametrium were positive, patients were referred for radiation therapy. In ovarian cancer patients, the upper abdomen and the pelvis were examined laparoscopically to evaluate the possibility of optimal debulking surgery. Staging was done for patients who were not candidates for optimal debulking surgery and second debulking surgery was considered. RESULTS: Nine patients with stage 1b-11a cervical cancer were included. Preoperatively, 2 of them were suspected for lymph node involvement. Following laparoscopy one was confirmed to have para-aortic lymph node involvement and the other did not. In the remaining 8 patients, one was found to have parametrial involvement and laparotomy was avoided, while the rest were treated surgically. Eighteen ovarian cancer patients were included in this study. Laparoscopy revealed an extensive disease in 7 patients and therefore staging laparoscopy was completed. In the remaining 11 patients, laparotomy was performed, operable disease was found and complete debulking surgery was conducted. In only one patient of the 11 complete debulking surgeries was not possible. CONCLUSIONS: In accordance with the experience and skills of the surgical team, we propose utilizing laparoscopy in cases where laparotomy may be avoided.

[Experience with radical surgery for tumor persistence or recurrence after primary irradiation of cervix neoplasms].

At first the general problems of operations in the x-ray treated tissue are discussed. After that 11 cases of patients, who were x-rayed, were demonstrated. With these patients we carried out a radical operation, because the cancer of cervicis was insufficiently affected by a primary radiological therapy. The bad turn of disease could not be stopped at 6 cases. The women patients got either uretero-vaginal-fistulae (2) or recto-vaginal-fistulae (2) or combined vesico-recto-vaginal-fistulae (1) and in less than one year they died of recidiv of cancer. Two women patients are still tended on account of their vesico-recto-vaginal fistulae. Only three women had been cured satisfactorily and they are till now free of recidivs for 4 to 7 years.

[Changes of antioxidant parameters correlating with clinical regression of uterine cervix neoplasms].

In order to identify processes associated with clinically-proven regression, we analyzed variations of the indices of the antioxidant system in 151 patients with cervical carcinoma (T3N3Mo) who had received remote gamma-therapy or endolymphatic administration of large doses of cytostatic and/or immunomodulating drugs. The study established significant disturbances in the function of the enzymatic and non-enzymatic components of the antioxidant system, the extent of which was determined by therapy effectiveness. A close link was found between regression and the structural and functional condition of cellular membranes of formed elements of the blood. Our investigation identified variations in the basal levels of superoxide dismutase in tumor tissue as well as those in the course of treatment, depending on variations in response.
