[Effect of a reduced diet on hyperlipoproteinemia in metabolic-alimentary obesity].

The effect of calorifically reduced diet No 8e on the lipoproteinic composition of the blood serum in patients with metabolic-alimentary adiposity was studied. Most of the patients examined demonstrated hyperlipoproteinemia, basically of the IIb type, more seldom of the IIa and IV types. Following a course of the reduced diet the frequency of hyperlipoproteinemia of the II, III and IV types declines and the content of atherogenid lipoprotein fractions in the serum goes down. In young patients the effect of a reduced diet on the lipoprotein composition of the blood serum proved most effective.

Cognitive effects of ketogenic weight-reducing diets.

OBJECTIVE: To determine whether ketogenic weight reducing diets have adverse effects on cognitive performance. SUBJECTS: 21 overweight women (mean BMI = 41 kg/m2). DESIGN: Randomized double-blinded study. METHOD: Subjects were randomized to ketogenic or nonketogenic liquid formula very low energy diets, that were comparable in energy and in protein content. Subjects remained on the diet for 28 days and were reevaluated periodically with brief measures of cognitive performance assessing attention and mental flexibility. RESULTS: Weight losses were comparable on the two diets (Mean = 8.1 kg). Performance on attention tasks did not differ as a function of the diet. However, performance on the trail making task, a neuropsychological test that requires higher order mental processing and flexibility, was adversely affected by the ketogenic diet. The worsening in performance was observed primarily between baseline and week one of the ketogenic diet. CONCLUSIONS: Further research is needed to confirm this finding and to determine whether ketogenic diets negatively affect other complex mental tasks, such as problem solving.

Diabetic ketoacidosis and rhabdomyolysis following excessive intake of a weight reducing diet.

With the awareness of health problems related to obesity, weight reducing diets have become very popular. However, if these meal supplements are not taken as recommended, they can cause considerable harm in high risk individuals. We report a case of a young obese man who developed diabetic ketoacidosis (DKA) followed by rhabdomyolysis and acute renal failure (ARF) after excessive intake of a high carbohydrate containing weight-reducing meal. DKA associated with excessive intake of weight reducing diets has not been reported previously. In people with obesity and insulin resistance improper use of these supplements can cause severe metabolic complications.

[Ultrastructure of fat cells of obese patients with diabetes mellitus before and after treatment with a reducing diet].

Ultrastructural investigation of lipocytes of obese diabetics (Diabetes mellitus) before and after treatment with a reducing diet 21 diabetics not depending on insulin with obesity of the second or third degree were given for 3 months a reducing diet containing 5110 kJ/day (86 g protein, 41 g fat, 119 g carbohydrates). After treatment the test persons had a loss of weight of 7.60 kg on average. Before and after treatment subcutaneous adipose tissue was taken by puncture biopsy and investigated by electron microscopy. After treatment extended intercellular spaces are evident in which collagenic and elastic fibres appear quantitatively enlarged. A certain plication and inequality of the cell membranes, a penetration of the fat droplet by cytoplasmatic outgrowths as well as a diminution of the lipocytes and the big fat droplet were found. The investigations show that even the relatively short treatment of 3 months exercises an influence on the structure of the lipocytes. That shows new possibilities of therapy and prophylaxis of obesity.
