Effectiveness of an AI-based home exercise app for rehabilitation of rotator cuff-related shoulder pain: A randomized controlled trial.

BACKGROUND: Rotator cuff-related shoulder pain contributes to disability and healthcare use. Although therapeutic exercise is first-line treatment, limited supervision and adherence may reduce its effectiveness; digital rehabilitation with real-time feedback may address these limitations. OBJECTIVES: To evaluate the effectiveness of adding a digital rehabilitation program to standard physiotherapy on pain, function, fear-avoidance beliefs, and healthcare utilization. DESIGN: Single-center, assessor-blinded, randomized controlled trial with two parallel groups. METHOD: Forty-six adults (mean age 59 years) with rotator cuff-related shoulder pain were randomized to 12 weeks of conventional physiotherapy or physiotherapy plus an AI-based digital rehabilitation program using computer vision for real-time feedback and performance monitoring. Outcomes were assessed at baseline and at 2, 4, and 12 weeks. Pain intensity (NPRS) was primary outcome; secondary outcomes included upper limb function (QuickDASH), fear-avoidance beliefs (FABQ), and post-intervention healthcare utilization. Analyses followed an intention-to-treat approach. RESULTS: Pain reduction exceeded the MCID (1.3) at 4 and 12 weeks. Between-group differences favoured the intervention at Weeks 2 and 4 (MD -0.7; 95% CI -1.13 to -0.14 and MD -1.01; 95% CI -1.8 to -0.2, respectively). Upper limb function improved more at Week 4 (MD -7.3; 95% CI -12.3 to -2.2). FABQ scores decreased more at Week 12 (MD -7.6; 95% CI -14 to -0.5). Fewer participants in the experimental group required post-intervention healthcare (3 vs 10; p = 0.02). CONCLUSION: Adding AI-based home exercise app to conventional treatment improve pain and may improve function and reduce healthcare utilization in rotator cuff-related shoulder pain.

The effect of dietetic counseling combined with digital tools intervention on hemodynamic markers in Greek adults: The GATEKEEPER Study.

BACKGROUND AND AIM: Hypertension is a leading cardiovascular risk factor with substantial global impact on morbidity, mortality, and healthcare costs. While lifestyle interventions remain central to management, mHealth technologies offer promising adjunctive support, though their clinical effectiveness remains uncertain. This study evaluated whether combining dietetic counseling with digital tools improves hemodynamic markers in adults aged ≥55 years with increased cardiometabolic risk. METHODS AND RESULTS: This 3-month RCT (NCT05031299) included 954 adults with at least one metabolic syndrome risk factor, allocated 1:1:1 to Standard Care (dietetic counseling), Platform (counseling plus web-based platform), or Platform + Devices (counseling plus platform plus wearables). Outcomes included anthropometrics, lifestyle characteristics, blood pressure, pulse pressure, and estimated pulse wave velocity, analyzed using linear mixed-effects models adjusted for age and sex. All groups improved over 3 months. Waist circumference decreased by -6.29, -4.92, and -4.69 cm across Standard Care, Platform, and Platform + Devices groups respectively, and systolic blood pressure declined by -4.84 to -7.15 mmHg across groups. The Platform + Devices group showed greater increases in physical activity (94.62 MET-min/week; 95% CI 66.49 to 122.76) and greater reductions in pulse pressure (-3.90 mmHg; -6.58 to -1.22) versus Standard Care. Weight loss was associated with lower odds of hypertension (OR 0.4; 95% CI 0.2-0.7), greater likelihood of hypertension reversal (OR 3.6; 1.2-10.3), and higher probability of achieving normal pulse pressure (OR 1.8; 1.1-3.1). CONCLUSIONS: Dietary lifestyle intervention improved cardiometabolic outcomes, with limited added benefit from digital tools. Weight loss was the primary driver of hemodynamic improvement.

BMT4me En Español: Multisite Feasibility and Usability Testing of a Spanish-Language mHealth Adherence Support App for Spanish-Speaking Caregivers of Children After Hematopoietic Stem Cell Transplantation and Cancer Treatment.

BACKGROUND: Medication nonadherence during the first 100 days after pediatric hematopoietic stem cell transplantation (HSCT) and during oncology treatment increases risk for complications. BMT4me is a caregiver-facing mobile health (mHealth) application providing medication reminders, symptom tracking, and note-taking features to support medication management. Spanish-speaking caregivers are frequently excluded from digital adherence interventions due to the lack of language-accessible tools. PROCEDURE: We conducted a multisite, mixed-methods usability testing of a Spanish-language version of BMT4me ("BMT4me en Español") with Spanish-speaking caregivers of children (ages 2-17 years) post-HSCT or with an oncology diagnosis on active treatment. Caregivers completed a facilitated, three-step usability session (unobtrusive observation, interactive observation, and debriefing), followed by a semi-structured interview, and then completed the system usability scale (SUS). Quantitative outcomes were summarized descriptively; qualitative data were analyzed using content analysis with constant comparison. RESULTS: Fifteen participants enrolled at each site for a total of 30 participants. Across both sites, the recruitment rate was 91%. All participants completed all parts of the study. The SUS score (M = 80.09; SD = 17.35) was above average (>68). Two key qualitative themes emerged: (1) the perceived positive impact of BMT4me on managing a serious illness and (2) the acceptance and sociocultural relevance of BMT4me for Spanish-speaking families. Caregivers also shared suggestions to add educational content and multiuser functionalities to BMT4me. CONCLUSIONS: The acceptance and perceived positive impact of the Spanish BMT4me app indicates that socioculturally relevant, Spanish mHealth interventions have strong potential to support Spanish-speaking caregivers in pediatric oncology and HSCT settings. [Structure-mobility relationships in thin-layer chromatography. Applications to some progestogens (author's transl)].

A study has been made on the quantitative relationship between the chromatographic mobility and the structure of progestogenic steroids. The coefficient K, corresponding to the fundamental skeleton, and the parameters of different substituents were calculated from the RM values. The results obtained with eighteen steroids tested in two solvent systems of low dielectric constant and low polarity have been improved satisfactorily.

A rapid and precise method for the determination of vitamin D3 in rat skin by high-performance liquid chromatography.

In order to develop the investigations into photobiogenesis of vitamin D3, a rapid and precise method for the determination of the vitamin in rat skin was established by using high-performance liquid chromatography (HPLC). The proposed method included saponification of small pieces of rat skin, extraction of the unsaponifiable matter and application to HPLC using "Zorbax SIL" (straight-phase) as an adsorbent and 0.5% isopropanol in n-hexane as a mobile phase. The applicable lower limit of the method was 2ng of vitamin D3/cm2 of subcutaneous tissue-removed skin and it was possible to assay a concentration higher than 2 ng/cm2. The proposed method was applied to determine the content of vitamin D3 in rat skin obtained from in vivo and in vitro irradiation experiments. In the in vitro experiment, the yield of vitamin D3 increased in proportion to the irradiation time. On the other hand, the yield in the in vivo experiment showed a proportional increase similar to the in vitro experiment until 60 min irradiation, while a nearly constant value was obtained by irradiation for longer than 60 min. When the rat skin obtained from the in vitro experiment was irradiated with monochromatic UV rays in the range s60-350 nm, the most effective wavelength for the formation of vitamin D3 was confirmed to be 303 nm, which differs from the result obtained from the experiment in a test tube (295 nm). Moreover, the yield of vitamin D3 by irradiation with UV rays below 288 nm was extremely low, which again differed from the results of a test tube experiment. These differences were thought to be due to the filter effect of the malpighian layer in the epidermis of rat skin.

External fixation. Classification and indications.

The conclusion from the above clinical and experimental presentation is that stabilization by using external fixation in problematic cases is the method of choice because the risk of infection is better than by using the standard methods of plating and nailing. Using external fixation based on the tubular system of ASIF we can achieve rigid stability. Correct application allows early mobilization ensuring alignment even in bone defects. This method of treatment also facilitates the care of wounds. Our experimental and clinical information provides us with the opportunity to offer a systematic classification of each individual type of external fixation and its merits, as described above in types I, II, and III and its application in different situations. Our clinical experience also shows that external fixation has greatly reduced the risk of amputation in these problematic cases, but it has not solved all the problems associated with the primary injury. The advantage of the three-dimensional external fixation type III can also be seen in the case of arthrodesis of the knee joint. Here there is a better neutralization of the bending moment, than by using type II. Finally we would like to emphasize that the external fixation is not the panacea for every problematic case and each surgeon should be well aware of its methodical and correct application, as abuse of external fixation may lead to secondary complications.

Genetic differences between endotoxin-sensitive and resistant C3H mice. Intraperitoneal cell response to lipopolysaccharide and prostaglandins.

The 2 related mouse strains C3H/HeJ and C3/Han differe in their response to endotoxin. C3H/HeJ mice are endotoxin-resistant, C3H/Han mice are - like all other mice - endotoxin-sesinsitive. The uptake of fluorescent intraperitoneally injected lipopolysaccharide was similar in both mouse strains. The intraperitoneal injection of lipopolysaccharide produced a higher mobilization of neutrophil granulocytes in resistant C3H/HeJ mice than in C3H/Han mice. An opposite pattern of mobilization of neutrophil granulocytes could be observed after injection of the prostaglandins E 1, E 2, and F 1 alpha: which both mouse strains were compared, a normal mobilization was observed in C3H/Han mice. C3H/HeJ mice showed a markedly decreased mobilization. If the difference between the 2 strains resides in a single gene locus itis to be assumed that lipopolysaccharide resistance is controlled by the same gene which also effects an impaired response to neutrophil mobilization after prostaglandin application.

Quantitative immunoelectrophoretic assay for murine oncornavirus p30: noncovalent facilitation by sodium dodecyl sulfate.

Treatment of Rauscher murine leukemia virus lysates with the anionic detergent sodium dodecyl sulfate (SDS) at concentrations between 0.2 to 2.0% SDS per mg of viral protein greatly increased the anodal electrophoretic mobility of p30, the major internal polypeptide. SDS treatment did not reduce p30 antigenicity or cause nonspecific precipitation of normal serum proteins during subsequent immunoanalysis. The increased anodal electrophoretic mobility allowed assay of Rauscher murine leukemia virus p30 by Laurell rocket immunoelectrophoresis. An SDS-facilitated rocket immunoelectrophoresis assay is described that was highly reproducible (coefficient of variability, less than 3.0%) and capable of detecting 125 ng of viral protein. To our knowledge, this is the first report of a quantitative immunoelectrophoretic assay for an oncornavirus antigen. Since SDS binding is a general property of proteins, this method of noncovalently altering electrophoretic mobility appears to be applicable to other antigen-antibody systems.

Carbamates

Comparison of clofezone and diclofenac in the treatment of out-patients suffering from activated (painful) osteoarthrosis.

The effectiveness and acceptability of clofezone (Perclusone) were compared with those of diclofenac in a placebo-controlled, double-blind, cross-over trial on a group of 29 out-patients suffering from osteoarthrosis of the knee or hip. Clofezone was given at a daily dosage of 1200 mg during the first week and 600 mg during the second week; the corresponding dosages of diclofenac were 150 mg and 75 mg. The duration of the trial was 6 weeks. In order to wash out previous medication, a placebo was given for one week before and after each medication period. Eight of the patients stopped taking their drug after 1--3 weeks, namely 5 in the clofezone group and 3 in the diclofenac group, while one patient stopped taking the placebo. The principal reason for these stoppages was gastrointestinal intolerance. These 9 patients were disregarded in the evaluation of the trial results. This evaluation on the remaining 20 patients showed that clofezone brought about a significant improvement in respect of pain (both at rest and during movement), inflammation and mobility after two weeks' application. In the diclofenac group the only significant improvement after the same period was in respect of pain during movement. Statistical comparison of the two groups showed that the reduction in pain during movement and in inflammation was significantly greater under clofezone than under diclofenac and that clofezone had the longer-lasting action. These findings are discussed in the light of the kinetic characteristics of the two drugs and of the mild antidepressant properties of clofezone. There was no significant difference between the two drugs as regards tolerance.

[Combined surgical and physical treatment in traumatic painful syndromes of the cervical spine].

Clinical observations suggest the need for changing therapeutic management to a more active one in cases of cervical spine injury with damage to the spinal cord and nerve roots or brachial plexus. In 248 patients with these injuries treated initially conservatively the incidence of cervicobrachial pain was analysed. Neuralgic pains were present in 31.5% of cases, causalgic pains in 2.4% and sympathalgic pains in 2%. Conservative treatment conducted in these patients (89 cases) during many months after trauma had no effect on return of mobility. Long-term application of physioterapy prevented only temporarily the development of trophic changes and only partially relieved pains. Only surgical decompression of the spinal cord or spinal nerves with stabilization of damaged vertebrae caused disappearance of painful syndromes and improvement in the motor activity of the extremities. These observations show that early surgical intervention for decompression of the spinal cord, roots or brachial plexus should be advocated in these cases.

Application of teem-test for investigations of cellular immune phenomena in multiple sclerosis.

Some evidence is presented for usableness of tanned erythrocytes in electrophoretic mobility (TEEM)-test. Two possibilities of application of TEEM-test for immunological investigation in multiple sclerosis are discussed: detection of lymphocyte sensitization against a soluble antigen (3 M KCl extracted) derived from a normal brain and measurement of mixed lymphocyte reactin (MLR) after a short-time lymphocyte culture. Results show some connection between autosensitization (TEEM-test using normal tissue antigen) and predisposition (TEEM-MLR-test).

Noninvasive recording of His-Purkinje activity in man by ORS-triggered signal averaging.

Mobile instrumentation and a clinically applicable method have been developed for external His bundle recording. High gain signal amplification (10)(5) filtering (30--300 HZ) and averaging (128 or 256 consecutive cycles) are used. Acquisition of signals arising in the P-R interval is triggered by the patient's QRS signal at the end of that interval. The precordial bipolar electrodiogram is digitized at 5k HZ with 8 bit resolution and transferred to a 1,024 word, 18 bit signal averager. The averaged signal is then displayed on an oscilloscope and photographed. Good correlations were obtained between direct intracardiac and precordial recordings in experimental animals and in humans. Noise level after averaging was below 0.3 microV, and there was good elimination of asynchronous atrial and ectopic ventricular activity. With averaging of 128 or 256 consecutive cycles, the signal attenuation after propagation to the chest wall was in the range 1:2000 to 1:4000 in comparison with the directly recorded His bundle activity deflections. The noninvasive method may be of value in follow-up of acute and chronic disturbances of atrioventricular conduction, as well as in studies of effects of pharmacologic interventions.

Letterer-Siwe disease: report of case.

Initial oral symptoms of Letterer-Siwe disease reveal the importance of a dentist in establishing this diagnosis. This case is important in that the patient was five months old, at which time an early diagnosis was made and treatment had begun. Because most cases occur in young children, a high level of suspicion is necessary, because the oral manifestations and radiographic findings in children may often resemble the periapical, periodontal or cystic involvements frequently seen first by the dentist. The oral lesions were necrotic, hemorrhagic, and degenerative gingival tissues in the maxillary molar area. Loss of alveolar bone was evident. There was precocious eruption of seven primary teeth, which were slightly mobile. Dentally, prophylaxis and fluoride applications were supportive measures.

Estimation of size of infarcted focus by spatial quantitative vectorcardiography in patients with acute anterior myocardial infarction.

The study included 113 patients with acute myocardial infarction of the left ventricular anterior wall. The patients were repeatedly followed up from the first hours of the affection onward in the course of the individual stages of treatment (stage of intensive monitoring and therapy; intermediate stage; initial rehabilitation with application of active therapy; early mobilization, activation and rehabilitation). A new approach to the estimation of the size of the infarcted focus is described, namely, the method using spatial quantitative vectorcardiography. It was found that the spatial localization of the 40-ms vector, especially its azimuth, is a specific indicator, with the aid of which the localization, extent and depth of the lesion in the left ventricular anterior wall can be assessed with a high probability in most instances. The compensatory adaptive and reparative capacity of the affected myocardium, estimated on the basis of spatial quantitative VCG, is closely correlated with the size of the infarcted focus.

Responses of motoneurones to electrophoretically applied dopamine.

1. The effect of electrophoretically applied dopamine upon motoneurone excitability has been investigated. Field potentials originating from antidromically activated motoneurones were recorded from the ventral horn of the rat lumbar spinal cord. 2. Field potentials showed an increase in amplitude following electrophoretic application of dopamine. Dopamine was shown to be less potent than noradrenaline and 5-hydroxytryptamine in producing these changes. 3. Measurement of the transport number of dopamine suggest that the relatively low potency of dopamine cannot be attributed to differences in ionic mobilities between the amines. 4. Electrophoretic application of alpha-flupenthixol was shown to discriminate between dopamine and 5-hydroxytryptamine responses. Dopamine responses were profoundly reduced. 5. Electrophoretically applied alpha-flupenthixol also discriminated between dopamine and noradrenaline. Noradrenaline responses were consistently potentiated by alpha-flupenthixol. The possibility is discussed that dopamine may not merely be a precursor for noradrenaline in the rat spinal cord.

[Effect of podophyllin resine on the mobility of langerhans cells (author's transl)].

The application of an alcoholic solution of podophyllin resin at concentrations above 10 p. 100 on guinea-pig udders, produces in epidemic Langerhans cells the same cytological effects as those by colchicine and vinca alcaloid in culture cells. The Langerhans cells are a suitable experimental model in vivo because of its easy ultrastructural identification, its migratory character and its preferential localization in epidermis.

Mobile health apps improve Health-Related Quality of Life in Type 2 Diabetes Mellitus by enhancing medication adherence: A multicentre randomised controlled trial with mediation analysis.

AIMS: This study evaluated whether a gamified mHealth application (CareAide&#xae;) improves Health-Related Quality of Life (HRQoL) in Type 2 Diabetes Mellitus (T2DM) and whether this effect is mediated by medication adherence. METHODS: Prespecified secondary analysis of the T2DM cohort from a 6-month multicentre RCT (NCT06068309; N&#x202f;=&#x202f;663; three Malaysian hospitals). Participants were randomised 1:1 to standard care or CareAide&#xae;. Adherence (MMAS-8), EQ-5D-5L utility (Malaysian value set), and AQoL-6D were assessed at baseline and 6 months. Simple mediation analysis (PROCESS Model 4; 5000 bootstraps) adjusted for baseline HRQoL. RESULTS: CareAide&#xae; significantly predicted higher MMAS-8 scores (mean difference +1.756; d = 1.638; p&#x202f;<&#x202f;0.001). Higher MMAS-8 scores significantly predicted improved AQoL-6D utility (b = 0.024; p&#x202f;<&#x202f;0.001). The direct effect on AQoL-6D was non-significant (p&#x202f;=&#x202f;0.248). Bootstrapped indirect effect confirmed full mediation via AQoL-6D (0.042; 95% CI [0.024, 0.060]). A sensitivity analysis adjusting for baseline HbA1c confirmed full mediation (indirect = 0.034; 95% CI [0.015, 0.052]; n&#x202f;=&#x202f;563). EQ-5D-5L utility showed a significant direct between-group difference at 6 months (p&#x202f;=&#x202f;0.012) but did not operate as a mediation outcome. CONCLUSIONS: Medication adherence fully mediates the AQoL-6D HRQoL benefit of a gamified mHealth intervention in T2DM, as confirmed by both the primary and HbA1c-adjusted sensitivity analyses. These findings support integration of behaviourally informed digital adjuncts into routine primary diabetes care.
