The paper is concerned with the results of open field gamma-beam therapy and gamma-therapy using gamma-beam forming agents. Besides, a method of simultaneous chemo- and radiotherapy was used for 175 nasopharyngeal cancer patients. Of them 2 had Stage II tumors, 22 Stage III tumors, 151 (first admitted for treatment as well as those with recurrences and tumor metastases), by local spread and regional and distant metastatic spread, had Stage IV tumors. A single focal dose was 1.4-2.5 Gy, the summary dose 55-70 Gy. In simultaneous chemo- and radiotherapy using gamma-beam forming agents the values of a single and the summary focal dose did not lessen; a single and the summary dose of chemotherapeutic drugs got less and equaled 1/2-2/3 of standard dosages. The chemotherapeutic component of multimodality chemotherapy was indicated with regard to a tumor histological structure. The best results were achieved in the group of patients on multimodality chemo- and radiotherapy. Of 75 patients, a complete or partial tumor resorption was noted in 71. With the use of all the methods of radio- and multimodality therapy the 3-year survival was 41.1%, the 5-year survival 19.4% (34 patients out of the 175).

Patients with adult onset otitis media with effusion are generally subjected to an examination and biopsy of the nasopharynx under a general anaesthetic to exclude a neoplasm of the post-nasal space. The likelihood in Caucasian countries of a tumour being detected has not been clearly defined. The clinical features of the 55 patients with nasopharyngeal malignancy presenting to the Glasgow hospitals since 1970 have been analysed, and the incidence of malignancy in all adults presenting with middle ear effusions has been assessed in order to define the association of the 2 conditions. The study confirmed that while an effusion is commonly associated with a nasopharyngeal tumour (33%), in only 2% was it the sole clinical manifestation at the initial consultation. In addition, adults with effusions, but without other symptoms and signs suggestive of a tumour, are unlikely to harbour a tumour, the incidence in this review being 0.4%. Therefore, an examination and biopsy of the nasopharynx under general anaesthesia is likely to give a very low yield of additional information in cases of isolated middle ear effusions in adults, and the cost effectiveness of the procedure should be questioned.

Nasopharyngeal carcinoma (NPC) is the primary cancer of the head and neck localisations in the Salah Azaiz Institute (Tunisia). From 1970 to 1987, 80 patients with histologically proven T4 N0 NPC, were treated with exclusive radiation (70-75 Gy to the primary lesion and 50-55 Gy to cervical lymph nodes). The T4 N0 represents 7% of all NPC and 16% of the T4 treated in our Institute. Ninety percent of the patients are over 20 years old with a mean age of 52 years. The sex-ratio was 4:1. Extension to the brain was observed in 55% of the cases. Local control was 70% at 2 months after the end of irradiation. The actuarial survival at 5 years was 47%. Distant metastasis represent 13% (30% of all NPC). The main failure of treatment was local recurrence. The T4 N0 is probably a particular entity concerning the age, the response to radiotherapy and the low rate of distant metastasis.
A case of phenytoin sodium-induced lymphadenopathy appeared as a nasopharyngeal malignant neoplasm with regional metastases. Cervical lymphadenopathy is a lesser-known drug reaction that is produced by anticonvulsant therapy; under some circumstances, this condition may be confused with head and neck malignant neoplasms. To date, phenytoin-induced nasopharyngeal lymphadenopathy has not previously been reported in the literature. A review of the medical literature provides some interesting facts about the syndrome. A familiarity with this potential complication of anticonvulsant therapy may expedite early diagnosis and appropriate patient treatment.
A group of 58 patients with nasopharyngeal malignancies are described and subjected to statistical analysis. The programme of diagnostic management and results of its realization are presented. The following was subjected to analysis: age and sex of the patients, complaints reported by them, and the clinical progression of the disease determined by physical and X-ray examinations. Taking into account the U.I.C.C. directives, the histological structure of the tumours was assessed as well as the incidence and character of metastases in regional lymph nodes. In the discussion attention is paid to the causes of delayed diagnosis of these tumours, while in the conclusions the factors are presented whose observation exerts a beneficial effect on further fates of patients.
