[Computed tomography of obturator foramen hernia].

A 89 year-old female was admitted with the complaint of right hypochondralgia. And also she complained of right thigh pain after admission. On hospital day 4, she underwent CT examination in order to evaluate right thigh with severe pain. The preoperative diagnosis of obturator foramen hernia was performed.

Obturator foramen approach. I. A new technique for reconstruction of the vas deferens after extensive resections.

A new approach for restoring continuity of the vas deferens after long segment resection is presented. The technique was used in six patients. It consists of passing the pelvic segment of the vas through the obturator foramen for anastomosis with the scrotal vasal segment. No complications were encountered, and the results were excellent. The vasal length was studied in 37 cadavers of different ages. The portion of the vas that could be spared by the obturator approach was calculated and found to represent as much as one third of the total vasal length. The indications for the technique are outlined.

Intrapelvic dislocation of the head of femur through the obturator foramen associated with ipsilateral fracture femur.

We describe a case of traumatic anterior dislocation of the hip in a 14-year-old boy with associated intrapelvic displacement of the femoral head and ipsilateral fractures of the shaft of the femur and greater trochanter. There was a delay in presentation of eight days. At operation the femoral head was reduced into the acetabulum after enlarging the obturator foramen by performing an osteotomy of the superior pubic ramus.

Arterial grafting through the obturator foramen.

This paper reviews the indications for obturator bypass and adds five more cases to the 29 reported in the literature. The indications were groin sepsis, fear of sepsis, severe groin scarring and false femoral aneurysm. The graft material was variously alone or in composite fashion saphenous vein, arm vein, umbilical vein, dacron and PTFE. All patients are alive but one graft occluded at 12 months and another required ligation at 5 days because of further infection and haemorrhage. The other three are patent at 5, 4 and 2 1/2 years.

Anatomical study of the obturator foramen and dorsal nerve of the clitoris and their relationship to minimally invasive slings.

OBJECTIVES: The objective of the study is to clarify potential risks to the dorsal nerve of the clitoris (DNC) and obturator canal using different minimally invasive slings. STUDY DESIGN: Ten embalmed hemipelves were dissected to demonstrate the course of the DNC and the obturator canal. On each cadaver, tension-free vaginal tape (TVT), transobturator in-out (TVT-O) and transobturator out-in (Monarc) procedures were performed. Distances between the DNC and the obturator canal to the different devices were measured. RESULTS: The DNC passes beneath the pubic bone at a distance of 14.3 +/- 4.7 mm of the midline. The distances of the different devices to the DNC were similar. The distance to the obturator canal was significantly different, with TVT being the furthest (40.1 +/- 3.7 mm) and TVT-O the closest (19.3 +/- 3.1 mm; p < 0.0001). CONCLUSION: Given the course of the DNC along the medial aspect of the ischiopubic ramus, the out-in technique may be safer. The in-out technique is the closest to the obturator canal.
