Vitamin D metabolites in spasmophilia.

The comparison of 20 untreated spasmophilic patients (2 men, 18 women) with 25 healthy subjects revealed no significant differences in vitamin D metabolite plasma levels or in other laboratory indexes of calcium metabolism except for ionized and cytosolic free calcium levels which were lower in the former group (P less than 0.01; P less than 0.001). After 3 months treatment with calcitonin and 25-hydroxyvitamin D there was a clear improvement in patient symptoms, no significant modification of 1,25-dihydroxyvitamin D levels and significant increases in 25-hydroxyvitamin D, cytosolic free calcium and urinary calcium (P less than 0.001, P less than 0.001 and P less than 0.01, respectively). We conclude that high plasma levels of 25-hydroxyvitamin D play an important role in restoring normal cytosolic free calcium levels and reducing clinical manifestations in spasmophilia.

[Spasmophilia or panic attack?].

For many years, the symptoms grouped under the label "spasmophilia" have been differently evaluated in France by psychiatrists, who ascribe them to hysteria or anxiety, and by endocrinologists and general practitioners for whom they are all due to neuromuscular hyperexcitability, the cause of which must be sought in the biochemistry of calcium. Recent studies on anxiety should reduce the gap between these discordant views. The concept of anxiety itself has changed owing to the discriminant effects of psychotropic drugs and to a classification based on precise diagnostic criteria without theoretical presuppositions. Since anxiety can be biochemically induced, the hypotheses put forward by "spasmophilologists", such as disorders of calcium metabolism or hyperventilation, can now be tested in the laboratory.

[Symptomatology in 162 patients with spasmophilia (chronic idiopathic or constitutional tetany). A statistical analysis using a computer (author's transl)].

A computer was used for a statistical analysis of the results of treatment in 162 patients (130 women and 32 men with an average age of 29 years), with chronic idiopathic tetany. Symptoms leading to diagnosis were mainly feelings of ill-health (27.2 %), motor disorders (19.7 %), and sensory problems (19.1 %). The onset of acute attacks is related, more especially, to psychogenic factors. The frequency of the various clinical manifestations, and their symptomatic grouping were analyzed. The clinical profile is similar in both sexes apart from 7 of the 89 items: lipothymia, abnormal feelings of warmth, pseudo-contraction of the larynx, abnormalities of the exoskeleton, and digestive disorders are more frequent in women, while precordalgia is more often present in men. Factorial analysis of correspondence between 38 variables reveals a very large dispersion of the variables, as the first three factors, which are the most significant, supply only 30% of the total information. Furthermore, very few variables share significant relationships in each factor. In spite of the amount of information collected, this dispersion of the symptomatology does not permit the definition of one or more profile-types of spasmophilia.

[A therapeutic trial in patients with spasmophilia].

As the clinical manifestations of spasmophilia are characterized by their striking polymorphism, quantitative assessment, which is requisite in order to evaluate therapeutic effectiveness, is a difficult task. The authors have attempted to analyze clinical, electrical and biological data from 30 patients. Each of these patients, given only manganese-cobalt oligosol and magnesium oligosol for two months, in two daily doses, had at least two groups of symptoms. Analysis of results shows that the most significant improvement was clinical: improvement was satisfactory or very satisfactory in 83% of patients tested, with muscular relaxation being recorded first, In relation with these clinical results, is the 59% improvement rate in EMG controls. Improvement in overall laboratory data was conclusive in only 24% of patients; however, if only the changes in intraerythrocytic magnesium, which is the only parameter with real clinical value (60% of abnormal results before treatment) are considered, improvement or return to normal values was recorded in 83% of cases.

The functional status of the parathyroid in adult spasmophilia patients estimated by plasma parathormone assay.

In 25 cases of spasmophilia the parathyroid function was assessed by complete exploration of the P-Ca metabolism and radioimmunoassay (RIA) of parathormone (PTH). Concomitantly the bone mineralization status was assessed by induced hypercalciuria tests and administration of 30 mg vitamin D2. RIA of PTH revealed increased values in 50% of the cases. The values fall within the range of secondary hyperparathyroidism, but this percentage varies with the values of the parathyroid function obtained by P-Ca metabolism explorations. The increased PTH values do no correlate with the level of plasma total calcium, ionic calcium, and magnesemia. Nor do they correlate with the bone mineralization disorders or the duration of the disease and the age of the patients.

[Tetany, spasmophilia, hyperventilation syndrome: theoretical and therapeutic synthesis].

The hyperventilation syndrome (HVS), characterised by multiple somatic symptoms induced by inappropriate hyperventilation, constitutes the physiopathological manifestation of a common disorder in general medicine. As a synonym of spasmophilia or tetany, it has the advantage of offering diagnostic criteria, even though the latter are still vaguely defined. But its definition allows for objective measurements: indeed, a decrease in PCO2 during a hyperventilation provoking test and an abnormally low PCO2 rate at rest can be easily quantified. Moreover, the HVS concept offers a treatment which is both structured (respiratory reeducation, psychotherapy and pharmacology) and efficient. Yet, a number of scientific uncertainties still exist. There is no general agreement regarding the criteria which should be taken into account in a hyperventilation provoking test in order to diagnose an hyperventilation syndrome; the specificity of such a test is weak and a placebo can induce as many symptoms as can a HVS. Respiratory reeducation has good results but does not necessarily have an effect on PCO2. Some therapists see in it no more than a mechanism of relaxation and a rational explanation of frightening symptoms. This has led some authors to reject the term "hyperventilation syndrome" and to prefer the expression "chronic hyperventilation of unknown origin".
