Usefulness of color Doppler ultrasonography (CDUS) and three-dimensional spiral computed tomographic angiography (3D-CT) for diagnosis of unruptured abdominal visceral aneurysm.

A 52-year-old Japanese man was transferred to our center where screening by means of ordinal ultrasonography and color Doppler ultrasonography revealed an abdominal visceral aneurysm. Although we were unable to confirm the site of this aneurysm by means of ultrasonographic examination, thin-sliced contrast enhanced computed tomography and three-dimensional spiral computed tomographic angiography showed that the aneurysm was situated at the root of the splenic artery. Transcatheter arterial embolization of the aneurysm was performed by metallic coil with occlusion of the main route of the splenic artery because it was technically difficult to embolize the aneurysm alone while preserving the main splenic arterial flow. Angiography of the celiac trunk after transcatheter arterial embolization showed no enhancement of the aneurysm or splenic artery, and superior mesenteric arteriography after transcatheter arterial embolization showed enhancement of the spleen by collateral circulation from the superior mesenteric artery. One year after transcatheter arterial embolization, repeated contrast enhanced computed tomography revealed the enhancement of the spleen, and color Doppler ultrasonography revealed no blood flow in the aneurysm. Color Doppler ultrasonography and 3D-spiral computed tomographic angiography are both useful for diagnosis of a visceral aneurysm, for confirmation of its exact site and form, and for deciding upon a treatment procedure.

Polyarteritis nodosa: successful diagnostic imaging utilizing pulsed and color Doppler ultrasonography and computed tomography angiography.

The diagnosis of polyarteritis nodosa (PAN) is often delayed because patients present with diverse clinical symptoms, but with less disease-specific signs. However, early diagnosis and treatment of PAN is necessary to prevent serious organ damage. Herein, we present a boy in whom the diagnosis of classical PAN was established rapidly with ultrasound and computed tomography (CT) angiography. Moreover, complete disappearance of the aneurysms after one month of therapy was documented. This case is presented in order to underline the role of new imaging techniques in the diagnosis and follow-up of patients with PAN.

[Comparison of the results of conventional ultrasonography and color doppler analysis in the evaluation of the nature of adnexal tumors. II. Color duplex doppler ultrasonography].

The author evaluates the possibilities of coloured Doppler vaginosonography for prebioptic evaluation of the nature of ovarian tumours. He investigated the site of vascularization of tumours and qualitatively the blood flow by assessing the vascular resistance (PI, RI) and character of the flow velocity curve. The results revealed a statistically significant (p < or = 0.05) trend of central vascularization of malignant tumours. A new parameter with a 100% sensitivity and 89% specificity proved to be the notch at the beginning of the diastolic part of the velocity curve which was not observed in any carcinomas but was present in 89% of benign lesions. The vascular resistance was significantly lower in malignant tumours. The cut off value of malignity of the pulsatility index PI = 0.7 had a 86% sensitivity and 100% specificity. The cut off value of the resistance index RI is 0.6 with a 100% sensitivity and 75% specificity. Comparison of the predictive values of malignity of conventional and coloured duplex ultrasonography revealed that for evaluation of the nature of ovarian tumours coloured duplex sonography with several parameters is more suitable. PI and RI alone can be considered rather as indicators of active growth.

Transient arteriovenous fistulae after transrectal prostate biopsy: diagnosis with color Doppler ultrasonography.

To assess the prevalence and significance of arteriovenous fistulae after prostate biopsy, we performed color Doppler ultrasonography immediately after 136 consecutive transrectal prostate needle biopsies. Pathologic results were correlated with color Doppler ultrasonographic findings. Arteriovenous fistulae developed after 17 biopsies (13%), all closed spontaneously within 18 minutes, and none were associated with unusual bleeding. Carcinoma was noted in 25 biopsy specimens (18%), 10 (40%) of which were followed by arteriovenous fistula. The correlation between malignancy and postbiopsy arteriovenous fistula was statistically significant (P < 0.0004), consistent with hypervascularity known to be present in many prostate cancers.

Assessment of right hepatic artery involvement in patients with biliary tract cancer using color Doppler ultrasonography.

The accuracy of color Doppler ultrasonography (CDUS) in diagnosing right hepatic artery (RHA) involvement was preoperatively evaluated in 28 patients with biliary tract cancer. When a tumor partially or completely surrounded the RHA, the circumferential involvement was diagnosed as positive. With B-mode ultrasonography imaging alone in the 10 nonsurgical patients, the correspondence rates with angiography for the presence of invasion, absence of invasion, and overall were 33%, 0%, and 10%, respectively. However, CDUS allowed clearer visualization of the RHA than did B-mode imaging, and the correspondence rates of CDUS with angiography were 100%, 43%, and 60%, respectively. In the 13 surgical patients, the accuracy levels of B-mode imaging, CDUS, computed tomography, and angiography for the detection of RHA involvement were 46%, 77%, 58%, and 92%, respectively, when compared with surgical and histopathologic findings. Although the accuracy of CDUS was lower than that of angiography, it can be performed repeatedly and noninvasively. CDUS is expected to be useful for the initial assessment of RHA involvement.

[Evaluation of cavernous artery by color Doppler ultrasonography--significance of ultrasonic beam angle].

Penile Doppler ultrasonography is widely accepted as an essential examination in the diagnosis of impotence. However, measurement blood flow velocity using Doppler ultrasonography may be subject to some errors. We performed color Doppler ultrasonography in 63 patients with normal penile vascular function as diagnosed using positive responses to intracavernous pharmacological stimulation. We compared the Doppler measurement results of the 126 cavernous arteries and the ultrasonic beam angles. We used a Hitachi EUB 515, a sonographic probe of 7.5 MHz, a sampling width of 0.8 mm, a sampling depth of 1 mm, and a wall motion filter was not used. Ultrasonic beam angles were 5 to 77 degrees. The mean peak systolic velocity and end diastolic velocity values were 40. 0 cm/s and 3.9 cm/sec, respectively. The peak systolic velocity and end diastolic velocity values remained stable regardless of the ultrasonic beam angles (Kruskal-Wallis test, p = 0.56, p = 0.70). However, the variance of values became greater when the ultrasonic beam angles was larger than 55 degrees in the case of peak systolic velocity (F test, p < 0.05) and 50 degrees in the case of end diastolic velocity (F test, p < 0.05), indicating a reduction in reliability. Resistance index variance was significantly higher when ultrasonic beam angle exceeds 50 degrees (F test, p < 0.05). We believe that we should accept only those cavernous artery peak systolic velocity measurements as reliable when the ultrasound beam angle is less than 55 degrees.(ABSTRACT TRUNCATED AT 250 WORDS)

Color Doppler ultrasonography for distinguishing myomas from uterine contractions in pregnancy.

OBJECTIVE: To determine the applicability and reliability of color Doppler ultrasonography (US) for distinguishing a uterine myoma from a focal myometrial contraction. STUDY DESIGN: Images from 36 patients with uterine thickenings were classified as myomas when color Doppler US demonstrated no centralized flow with a circumscribed vessel pattern at the border. Thickenings were classified as focal myometrial contractions when there was demonstrable vascular flow throughout the thickening. RESULTS: Using these diagnostic criteria, images from 36 patients were reliably characterized as representing myomas or contractions. The diagnosis was made more reliable by using the lowest velocity settings and exclusion of power Doppler US in nonretroplacental lesions. CONCLUSION: Color Doppler US is a sensitive and reliable tool for distinguishing uterine myomas from focal myometrial contractions.

Image-directed and color Doppler ultrasonography in the diagnosis of postbiopsy arteriovenous fistulas of native kidneys.

Using image-directed and color Doppler ultrasonography (ICDUS), we examined 65 patients with single kidney biopsy and diagnosed one arteriovenous fistula (AVF) in each of 8 kidneys. Three of them were associated with pseudoaneurysms. Three of the patients with AVF who presented with macrohematuria underwent angiography. Therapeutic percutaneous embolization was performed in 2 of them. The remaining 6 patients were followed up with ICDUS. All the lesions had disappeared at the end of a 6-month period. We conclude that ICDUS is an easy and noninvasive imaging technique in the diagnosis of postbiopsy native renal AVFs.

[Diagnosis and treatment of subcutaneous cavernous hemangioma with color Doppler ultrasonography].

OBJECTIVE: To discuss clinical application of the color Doppler ultrasonography in diagnosis and treatment of cavernous hemangioma in deep subcutaneous tissue. METHODS: From 1996, 15 cases of cavernous hemangioma were diagnosed and located with color Doppler ultrasonography and were embolized under monitoring of the ultrasonography or resected by operation before re-examination of the hemangioma via the color Doppler ultrasonography after the intervention. RESULTS: Direct embolization was achieved in 10 cases after pinpoint location of the hemangioma by the ultrasonography, and guided embolization was performed successfully in 2 cases via the monitoring of ultrasonography, and operation had to be adopted to remove the focus. No reoccurrence of the hemangioma was observed in all the cases. CONCLUSION: Cavernous hemangioma in deep subcutaneous tissue could be easily diagnosed and located with color Doppler ultrasonography, and could be removed by embolization under monitoring of the ultrasonography successfully.

Color Doppler ultrasonography in the identification of communicating vessels in twin-twin transfusion syndrome and acardiac twins.

This study evaluates the role of color Doppler ultrasonography in the identification of the communicating placental vessels in pregnancies with twin-twin transfusion syndrome and acardiac twins. In 18 pregnancies with twin-twin transfusion syndrome and two with an acardiac twin, color Doppler studies of the placental vasculature were performed before fetoscopy for laser coagulation of the communicating vessels. In six cases of twin-twin transfusion syndrome the placental attachment of the intertwin membrane could be visualized. Pulsatile arterial blood flow was observed from the donor to the recipient twin that disappeared after laser therapy. In both cases of acardiac twins, one communicating vessel with pulsatile and another vessel with nonpulsatile blood flow in the opposite direction could be identified. Color Doppler imaging is unlikely to play a major role in assisting endoscopic laser separation of chorioangiopagus in patients with acute polyhydramnios, but it may prove to be useful in the early identification of pregnancies at risk of developing twin-twin transfusion syndrome.

Color Doppler ultrasonography in patients with subacute thyroiditis.

We studied the utility of color Doppler ultrasonography in patients with subacute thyroiditis. Eighteen patients with subacute thyroiditis (SAT) with painful goiter and thyrotoxicosis underwent color Doppler ultrasonography during the acute and recovery stages of the disease. Thyroid vascularization in these patients was compared with that of 15 untreated patients with Graves' disease and 17 control subjects. During the acute stage of subacute thyroiditis, color Doppler ultrasonography showed low echogenicity without increased tissue vascularity in the affected swollen thyroid. In the recovery stage, color Doppler ultrasonography showed isoechogenicity with slightly increased vascularization. Vascularization became normal at 1 year follow-up time. In contrast, marked by increased vascularization was observed in patients with untreated Graves' disease. Color Doppler ultrasonography showed clear differences between SAT and Graves' disease patients. Vascularity was significantly correlated with serum free thyroxine (FT4) and thyrotropin (TSH) concentrations in the recovery stage (3 months after the initial ultrasonography). Color Doppler ultrasonography accurately visualized lesions without increased vascularity in the acute stage of SAT and lesions of slightly increased vascularity in the recovery stage. Color Doppler ultrasonography may be a useful, noninvasive, and rapid method for differentiating SAT from Graves' disease and for evaluating and monitoring the location and activity of lesions in SAT.

[Kidney obstruction: potential use of ultrasonography and Doppler color ultrasonography].

Ultrasonography is the first imaging approach in evaluation of patients with urinary obstruction. Presence of hydronephrosis, urinomas and inflammatory complications can be assessed. Moreover, the level and the cause of obstruction are often identified. The major limits of ultrasonography are poor visibility of the lumbar portion of the urinary tract and lack of specificity in detection of pyelocaliectasis. These limits are partially overcome with Doppler studies. Several factors can bias diagnostic accuracy of colour Doppler ultrasonography in evaluating renal obstruction; in particular, partial or intermittent obstruction cause minimum hemodynamic changes in patients with renal colic. Sensitivity of colour Doppler ultrasonography to detect obstruction decreases following medical treatment with NSAID and increases following hydration and diuretics administration. Diuretic ultrasound, colour Doppler evaluation of ureteral jets and evaluation of the twinkling artefact of stones are useful imaging techniques as well. Tissue harmonic imaging is a new imaging technique available in most latest generation US equipments. Evaluation of the kidney and of the urinary tract improves using this technique due to artefact reduction and increased contrast resolution.

[Ultrasonography and Doppler color ultrasonography in kidney neoplasms: the demands of the clinician].

The high number of diagnostic methods for images (ultrasound, computer tomography--CT, magnetic nuclear resonance--MNR) has allowed the urologist diagnosis more precocious of renal masses contributing to increase the survival rate and quality of life of patients affected by renal carcinoma. Although many are the advantages that the ultrasound scan gives to the urologist, still today in presence of ultrasound scan results showing expansive renal masses, probably malignant, many perform a CT or MNR for diagnostic confirmation and tumor extension.

Utility of color Doppler ultrasonography predicting tips dysfunction.

OBJECTIVES: Color Doppler ultrasonography (CDUS) has been proposed as an alternative to portal pressure gradient (PPG) measurement to detect transjugular intrahepatic portosystemic shunt (TIPS) dysfunction but with inconsistent results. This study aimed at developing and validating CDUS criteria to assess TIPS dysfunction. METHODS: A total of 117 consecutive follow-up simultaneous CDUS and hemodynamic evaluations in 34 patients with TIPS were analyzed. TIPS dysfunction was defined as a PPG >12 mmHg. A predictive model was obtained with logistic regression and was validated in an independent, prospective sample of 119 consecutive paired CDUS/hemodynamic evaluations in 55 patients. RESULTS: TIPS dysfunction was present in 57 of the 117 studies in the retrospective series. At multivariate analysis, mean maximum flow velocity at the portal vein (mVPmax) and direction of flow in the intrahepatic portal vein branches (FD) were the only independent predictors of TIPS dysfunction. The prediction rule for TIPS dysfunction derived from the model (mVPmax <28 cm/s when flow is hepatofugal or mVPmax <39 cm/s when flow is hepatopetal) had 90% sensitivity, 45% specificity, and negative likelihood ratio of 0.23. This prediction rule was validated both in patients with bare stents and in patients with polytetra fluoroethylene (PTFE)-covered stents, showing an overall 87% sensitivity, 57% specificity, and 0.23 negative likelihood ratio. CONCLUSIONS: The combination of two CDUS parameters correlate with TIPS dysfunction with high sensitivity and low specificity but with a good negative likelihood ratio. TIPS catheterization can be safely avoided in half of the patients using this predictive rule.

Transitional cell carcinoma involving the distal ureter: assessment with transrectal and color Doppler ultrasonography.

OBJECTIVE: The purpose of this study was to compare the feasibility of transrectal ultrasonography (TRUS) and color Doppler ultrasonography (CDUS) with transabdominal ultrasonography (TAUS) for the diagnosis of transitional cell carcinoma (TCC) involving the distal ureter. METHODS: Our study group consisted of 12 patients having TCC involving the distal ureter who had undergone TAUS and TRUS. Verification of the TCC involving the distal ureter was made by surgery (n = 9) or urine cytologic examination combined with the imaging findings (n = 3). The gray scale images of TAUS and TRUS were retrospectively evaluated to determine whether the ureteral mass was visualized. The CDUS findings were compared with the T stage, the histopathologic grade, and the mean microvessel density of histopathologically proved distal ureteral TCCs (n = 8). RESULTS: Transabdominal ultrasonography showed hypoechoic ureteral dilatation in 6 patients, a ureteral mass in 5, and anechoic ureteral dilatation in 1. Transrectal ultrasonography showed a ureteral mass in all 12 patients. Color Doppler ultrasonography could show blood flow in the ureteral mass in all patients. The tumor vascularity on CDUS showed no statistically significant relationship with the T stage, the histopathologic grade, and the mean microvessel density. CONCLUSIONS: Compared with TAUS, TRUS seems to improve the ability to detect TCC involving the distal ureter. In conjunction with TRUS, CDUS shows blood flow in the ureteral mass, and this may be helpful for the diagnosis of TCC involving the distal ureter.

[Diagnostic value of high frequency color Doppler ultrasonography for epididymitis].

OBJECTIVES: To evaluate the diagnostic value of color Doppler ultrasonography for epididymitis. METHODS: High frequency color Doppler ultrasonography was performed in 42 patients with epididymitis and 21 health volunteers. RESULTS: Compared with control group, epididymis enlarged significantly all in head, body and tail in 92% of patients with epididymitis. Bilateral epididymitis occurred in 73% of patients. Color Doppler flow imaging (CDFI), in patient group, showed the markedly increased flow signals bright in bundle in epididymis. Mild hydroccele of testis and orchitis were companied in most patients(93%). Orchitis was often complicated in the patients with long history. CONCLUSIONS: High frequency color Doppler ultrasonography can be a preferred exam method and has diagnostic value for epididymitis.

[Periampullar tumors: role of intraoperative color-Doppler ultrasonography in the evaluation of vascular invasion. Methods available to the surgeon and personal experience].

BACKGROUND: Tumors of the head of the pancreas and of the periampullary region, similar nosologic entity, represent the fourth most common cause of cancer death. These tumors are characterized bt high mortality rate associated with a low percentage of surgical resectability. Vascularly involvement is, in our experience, one of the criteria of exclusion for curative surgical resection. For this reason regional pancreasectomy don't show, in several case records, comforting datas about survival. METHODS: From 1995 to 1998 we introduced in the operative practice the color--doppler ultrasonography to diagnose vasculary involvement. We evaluated twenty patients that previously were candidated for surgical therapy consisting in a duodenal-pancreasectomy. All the patients were staged by preoperative abdominal ultrasound, E.R.C.P., CT, and angio M.R.I.. In no one of these patients were founded signs of vascularly involvement. We recognized by intraoperative color-doppler ultrasound three microscopic invasions of spleno-mesenteric-portal vasculary axis. RESULTS: U.S. I.O. C.D. in our case reports has underlined a sensibility and specificity of 100%. It has discovered, also, a false positive for neoplastic thrombosis, and a case of false negative. CONCLUSIONS: Intraoperative ultrasound shows a high sensibility about the intraoperative staging for the bibliopancreatic tumors. The study about the possible vascularly involvement is helped by the application of the colour in terms of an high sensibility and diagnostic care.

Prenatal diagnosis of cerebral arteriovenous malformation using color Doppler ultrasonography: case report and review of the literature.

Aneurysms of the vein of Galen are rare, representing less than 1% of all intracranial arteriovenous malformations. In the fetus, however, almost all cerebral arterial malformations involve the vein of Galen, usually presenting in the neonatal period with severe cardiac failure leading to neonatal death. In this report we present a case of arteriovenous malformation involving the vein of Galen detected prenatally at 35 weeks with the use of color Doppler ultrasonography. The literature is reviewed in an attempt to delineate the role of prenatal ultrasonography in the diagnosis and its impact on perinatal outcome.
