Good clinical research practice: what is it and is it possible in the intensive care unit?

With the growing quest for answers to vexing dilemmas in critically ill patients, more intensive care units are embarking on clinical research. This places increasing importance on Good Clinical Research Practice (GCRP), a set of guidelines drawn up by the Pharmaceutical Industry to assist investigators in conducting ethical, reliable scientific studies. GCRP is a combination of good basic management skills allied to ethical principles for scientific research. Based on the principles of the Declaration of Helsinki, it consists of three tenets: patient protection (ethics), credible data (science) and data control. This article describes GCRP specifically relating it to the intensive care situation, illustrating some of the concepts with practical examples. With a minimum of extra time and effort these basic principles can be integrated as routine into all research projects.

The significance of 2,3-DPG in red blood cell transfusions.

This review will begin by giving the highlights of the history and explain development of the basic science knowledge of hemoglobin chemistry, function, and physiology. The necessary involvement of red cell metabolism, as it pertains to the maintenance of 2,3-diphosphoglycerate (2,3-DPG) levels, both normally and under the perturbed and experimental conditions of blood storage, will be given as part of the basic science data. The clinical science and transfusion data will comprise the main critical aspects of the paper. Analysis and comment of over 20 studies will be given on the effects of animal and human transfusions with altered 2,3-DPG levels. Decreased survival and organ function have been demonstrated with transfusion of low 2,3-DPG red cells, with or without anemia, in the conditions of exercise, shock, hypotension, ischemia, cardiac surgery, hypoxia, sepsis, and acidosis. By critical analysis of these studies, recommendations on general and specific patient needs for red cell transfusions with normal or high 2,3-DPG levels are given.

Professional culture models of science and data types for computerized health records.

This paper examines the influence of professional culture and the types of science on clinical judgments and decision making. The advent of electronic records that can store richer and more complex data types will allow different professional disciplines to store data that more appropriately reflect the profession and its decisions. The results should be improved patient care.

Data Grids: a new computational infrastructure for data-intensive science.

Twenty-first-century scientific and engineering enterprises are increasingly characterized by their geographic dispersion and their reliance on large data archives. These characteristics bring with them unique challenges. First, the increasing size and complexity of modern data collections require significant investments in information technologies to store, retrieve and analyse them. Second, the increased distribution of people and resources in these projects has made resource sharing and collaboration across significant geographic and organizational boundaries critical to their success. In this paper I explore how computing infrastructures based on Data Grids offer data-intensive enterprises a comprehensive, scalable framework for collaboration and resource sharing. A detailed example of a Data Grid framework is presented for a Large Hadron Collider experiment, where a hierarchical set of laboratory and university resources comprising petaflops of processing power and a multi-petabyte data archive must be efficiently used by a global collaboration. The experience gained with these new information systems, providing transparent managed access to massive distributed data collections, will be applicable to large-scale, data-intensive problems in a wide spectrum of scientific and engineering disciplines, and eventually in industry and commerce. Such systems will be needed in the coming decades as a central element of our information-based society.

[Informatics in mental sciences: clinical data management and "expert systems"].

The role of information systems and particularly that of expert systems in medicine is analysed in brief. It is pointed out that, unlike other branches of medicine, in mental sciences the role of expert systems has so far been more limited because of certain intrinsic problems of programming on the one hand, and the scientific models of the psychiatrist on the other. An improvement in information technology and programming languages and better empirical classification of mental disturbances could provide useful bases for the realisation of computerised consultancy systems in mental sciences.

The Voyager Interstellar Mission.

The Voyager Interstellar Mission began on January 1, 1990, with the primary objective being to characterize the interplanetary medium beyond Neptune and to search for the transition region between the interplanetary medium and the interstellar medium. At the start of this mission, the two Voyager spacecraft had already been in flight for over twelve years, having successfully returned a wealth of scientific information about the planetary systems of Jupiter, Saturn, Uranus, and Neptune, and the interplanetary medium between Earth and Neptune. The two spacecraft have the potential to continue returning science data until around the year 2020. With this extended operating lifetime, there is a high likelihood of one of the two spacecraft penetrating the termination shock and possibly the heliopause boundary, and entering interstellar space before that time. This paper describes the Voyager Interstellar Mission--the mission objectives, the spacecraft and science payload, the mission operations system used to support operations, and the mission operations strategy being used to maximize science data return even in the event of certain potential spacecraft subsystem failures. The implementation of automated analysis tools to offset and enable reduced flight team staffing levels is also discussed.

Neural network modeling of risk assessment in child protective services.

The advantages of using neural network methodology for the modeling of complex social science data are demonstrated, and neural network analysis is applied to Washington State Child Protective Services risk assessment data. Neural network modeling of the association between social worker overall assessment of risk and the 37 separate risk factors from the State of Washington Risk Assessment Matrix is shown to provide case classification results superior to linear or logistic multiple regression. The improvement in case prediction and classification accuracy is attributed to the superiority of neural networks for modeling nonlinear relationships between interacting variables; in this respect the mathematical framework of neural networks is a better approximation to the actual process of human decision making than linear, main effects regression. The implications of this modeling advantage for evaluating social science data within the framework of ecological theories are discussed.

[The meaning of statistical data in medical science and their examination--true and false analysis of statistical data].

The subjects which are often encountered in the statistical design and analysis of data in medical science studies were discussed. The five topics examined were: Medical science and statistical methods So-called mathematical statistics and medical science Fundamentals of cross-tabulation analysis of statistical data and inference Exploratory study by multidimensional data analyses Optimal process control of individual, medical science and informatics of statistical data In I, the author's statistico-mathematical idea is characterized as the analysis of phenomena by statistical data. This is closely related to the logic, methodology and philosophy of science. This statistical concept and method are based on operational and pragmatic ideas. Self-examination of mathematical statistics is particularly focused in II and III. In II, the effectiveness of experimental design and statistical testing is thoroughly examined with regard to the study of medical science, and the limitation of its application is discussed. In III the apparent paradox of analysis of cross-tabulation of statistical data and statistical inference is shown. This is due to the operation of a simple two- or three-fold cross-tabulation analysis of (more than two or three) multidimensional data, apart from the sophisticated statistical test theory of association. In IV, the necessity of informatics of multidimensional data analysis in medical science is stressed. In V, the following point is discussed. The essential point of clinical trials is that they are not based on any simple statistical test in a traditional experimental design but on the optimal process control of individuals in the information space of the body and mind, which is based on a knowledge of medical science and the informatics of multidimensional statistical data analysis.

Application of micro- and nano-electromechanical devices to drug delivery.

Micro- and nano-electromechanical systems (MEMS and NEMS)-based drug delivery devices have become commercially-feasible due to converging technologies and regulatory accommodation. The FDA Office of Combination Products coordinates review of innovative medical therapies that join elements from multiple established categories: drugs, devices, and biologics. Combination products constructed using MEMS or NEMS technology offer revolutionary opportunities to address unmet medical needs related to dosing. These products have the potential to completely control drug release, meeting requirements for on-demand pulsatile or adjustable continuous administration for extended periods. MEMS or NEMS technologies, materials science, data management, and biological science have all significantly developed in recent years, providing a multidisciplinary foundation for developing integrated therapeutic systems. If small-scale biosensor and drug reservoir units are combined and implanted, a wireless integrated system can regulate drug release, receive sensor feedback, and transmit updates. For example, an "artificial pancreas" implementation of an integrated therapeutic system would improve diabetes management. The tools of microfabrication technology, information science, and systems biology are being combined to design increasingly sophisticated drug delivery systems that promise to significantly improve medical care.

Earth science: microseismicity data forecast rupture area.

On 28 September 2004 there was an earthquake of magnitude 6.0 at Parkfield, California. Here we show that the size distribution of the micro-earthquakes recorded in the decades before the main shock occurred allowed an accurate forecast of its eventual rupture area. Applying this approach to other well monitored faults should improve earthquake hazard assessment in future.

Current status of cellular therapy for ischemic heart disease.

Cellular therapy for acute myocardial infarction and ischemic cardiomyopathy has entered clinical trials across the globe. Early promising results have now provided the justification for larger randomized and blinded trials to address the efficacy of cellular therapy. A variety of fresh or cultured autologous cells have been delivered by catheter-guided endocardial, catheter-guided intracoronary, catheter-guided transvenous, and direct epicardial routes. This review will summarize the clinical data and highlight salient basic science data that support the ongoing efforts to identify the optimal cellular therapy both for acute myocardial infarction and chronic ischemic cardiomyopathy patients.
