The effect of buflomedilhydrochloride (Loftyl) on temporary hearing threshold shift.

Buflomedilhydrochloride (Loftyl, Abbott) is a vasodilating agent which has proved not only to influence peripheral blood vessels but also cerebral vascular disease. We tested the ability of Buflomedil to prevent noise-induced temporary threshold shift (TTS). Ten individuals tested the active substance and placebo 5 times each at random. In the individual analysis comparing Buflomedil and placebo sessions, there was a statistically significant decreased TTS after Buflomedil.

Correlations between the pharmacological effects and the pharmacokinetic behaviour of tetraminol.

Tetraminol (trans-2-hydroxyethylamino-3-hydroxy-5,8-dimethoxy-1,2,3, 4-tetrahydronaphthalene hydrochloride) is a newly synthesized antihypotensive agent. Its pressor activity is accompanied by a compensatory slowing down of heart rate. Changes in plasma levels after intravenous administration of 1 and 2 mg/kg b.w. to rats and rabbits can be fitted to a two-compartment open pharmacokinetic model (previous communications). Experiments were carried out with simultaneous registration of effects on blood pressure (R1) and heart rate (R2). Mathematical treatment of data for R1 and R2 revealed that changes with time can be described by biexponential equations of the type: R1i = A1i.e-a1it+B1i..e-a21t. Three different ways to correlate the data from the pharmacological and pharmacokinetic experiments were tried: 1) For the time interval during which the pharmacological response changes in the range Rimax- 20 to Rimax- 80% there exists a linear relationship with a high degree of statistical significance between the changes in the effects and plasma levels. 2) In the lambda 1-phase of the drug distribution there also exists a linear relationship between the effects and plasma levels. This is so because Tetraminol exhibits its pressor effect by direct stimulation of alpha-adrenergic receptors in the walls of the peripheral blood vessels. 3) Most suitable for expressing the relationship which exists between the pharmacological effects and the plasma concentration of Tetraminol for the entire time interval is the nonlinear function of the type: Ri/(Rimax - Ri) = QiCsi.

Conditioned adaptation-relaxation reflex in migraine therapy.

A new conditioned reflex, the adaptation-relaxation reflex, characterized by dilation of the peripheral blood vessels of the hand and arm, is associated with reduced blood flow in the area of the supraorbital and superficial temporal arteries. This reflex, learned through biofeedback training, is a central requirement for successful autonomic conditioning for migraine. In migraine patients whose conditions improve clinically, the finger-temperature feedback training is not associated with the conditioning of a single autonomic response, ie, digital vasodilation, but is associated with a general decrease of sympathetic tonic outflow.

Comparative haemodynamic effects of lidocaine, mexiletine, and disopyramide.

The effect of intravenous boluses of lidocaine (5 mg/kg), mexiletine (3.5 mg/kg), and disopyramide (5 mg/kg) on mean arterial pressure (MAP), heart rate (HR), cardiac output (CO), total peripheral resistance, left ventricular end-diastolic pressure, and peak rate of change of left ventricular pressure (peak LV dP/dt) were assessed in the conscious rabbit. Plasma levels for the three drugs corresponded to the human therapeutic range 3 min after administration. All three drugs had negative inotropic effects and reduced HR. Lidocaine reduced peak LV dP/dt by 26%, mexiletine by 41%, and disopyramide by 53%. The three drugs had quite different effects on CO as a result of differences in their actions on peripheral blood vessels: disopyramide caused a 21% fall in CO, associated with a significant vasoconstriction; mexiletine did not lower CO, as it caused a substantial vasodilation; and lidocaine did not produce any substantial change in either CO or vascular resistance. Cardiac autonomic blockade did not alter these changes. These results confirm that disopyramide has a marked cardiodepressant effect, and demonstrate that, although lidocaine depresses myocardial contractility in the conscious rabbit, it has little effect on other haemodynamic parameters. The experiments also show that mexiletine is a more profound negative inotrope than lidocaine, but that, as it produces a significant fall in MAP and thus a reduction in afterload, the CO is maintained.

Effect of mepyramine on the untoward side-effects of impromidine.

The value of impromidine, a potent histamine H2-receptor agonist, as an effective acid-secretory stimulant is limited by its tendency to cause cardiovascular side-effects. The possibility that the latter may be mediated not only by H2-receptors in the heart and peripheral blood vessels but also by overflow stimulation of H1-receptors was investigated in a double-blind study using impromidine with or without the H1-antagonist, mepyramine maleate. There was no significant difference between the two groups, suggesting that the side-effects are mediated by H2-receptors in the cardiovascular system alone.

[Ultrasound examination of the breast. Experience with a high resolution 'real-time'-scanner (author's transl)].

A new high resolution real-time ultrasound scanner for diagnosis of the breast is presented. The scanner is designed for the examination of peripheral blood vessels, all superficial soft-tissue organs, such as the thyroid, breast, salivary glands, testes as for well as pediatric examinations. It is a mechanical linear scanner with electronic and mechanical transducer focusing. Its high resolution as such and also the sonographic appearance of benign and malignant breast lesions are demonstrated, general problems of ultrasound examinations of the breast are discussed. The equipment under evaluation is of considerable value in the differentiation of palpable or otherwise localised breast tumors. It is less useful for a general application in the screening for breast cancer.

Inhibition of the early circulatory effects of triiodothyronine in rats by propranolol.

In two identical experiments, A and B, we studied the effect of the simultaneous l.v. injection of propranolol (Inderal, 0.5 mg/kg) on the circulatory effects of triiodothyronine (T3 500 microgram/kg i.v. 3 hours before measuring). The two substances act at different rates and so the blocking effect of propranolol preceded the development of the circulatory effects of T3. Cardiac output was measured by the Evans blue dilution method, the heart rate was calculated from the ECG recording and blood pressure was measured with a mercury manometer; stroke volume and total peripheral vascular resistance were also calculated. The isolated injection of T3 was followed by a significant increase in cardiac output (experiment A: 129%, B: 118%) and stroke volume (A: 125%, B: 118%) and by a drop in total peripheral vascular resistance (A: 82%, B: 85%). There was no change, in this early phase, in the heart rate or blood pressure. No changes were found 3 hours after the isolated administration of Inderal (the maximum effect of propranolol is attained in 30-60 min). During the same period, the above initial effects of T3 were completely suppressed by the simultaneous injection of Inderal. These results were probably related to the experimental conditions (early and not very marked changes after T3), but they demonstrate that the initial effects of T3 on cardiac performance and on the peripheral blood vessels can be completely suppressed by a block of beta receptors. From this it can be concluded that beta-adrenergic regulation is an important part of the mechanism of the early haemodynamic action of T3.

Leukotriene D4 inhibits cardiovascular responses to sympathetic stimulation, angiotensin and vasopressin in the pithed spontaneously hypertensive rats.

We have previously shown that high doses of leukotriene D4 (LTD4) induces hypotension in conscious and in pithed spontaneously hypertensive rats. The present study was designed to determine whether the hypotensive effect of LTD4 in spontaneously hypertensive rats is due to reduced sensitivity of peripheral blood vessels to pressor stimuli and/or reduced responsiveness of the sympatho-adrenomedullary system. To test these possibilities we examined the effect of spinal cord stimulation, vasopressin, norepinephrine and angiotensin II on blood pressure, heart rate and plasma catecholamines of pithed spontaneously hypertensive rats, before and after treatment with LTD4 (20 micrograms/kg i.v.), a dose which produces hypotension in conscious and pithed rats. LTD4 suppressed the pressor responses to spinal cord stimulation (50 V, 1 msec, for 1 min) at 0.3 and 3.0 Hz by 64% (P less than .001) and 71% (P less than .001), respectively, and the pressor and cardiac accelerating effects of systemic injections of norepinephrine. Plasma norepinephrine response to spinal cord stimulation at 3.0 Hz was significantly (P less than .05) reduced after administration of LTD4. The evoked release of epinephrine from the adrenal medulla was enhanced (+139%, P less than .05) by LTD4, but the tachycardia was blocked. The pressor responses to 0.1 and 1.0 microgram/kg of angiotensin II were blocked by 73% (P less than .05) and 70% (P less than .01), respectively. The pressor effects produced by vasopressin (0.03 and 0.1 microgram/kg) were also attenuated (by 83 and 80%, respectively) after LTD4 administration. The depressor effect of sodium nitroprusside (0.01-0.1 mg/kg) was also attenuated by LTD4. These data suggest that LTD4-induced hypotension involves severe interference with vascular smooth muscle responsiveness to all pressor agents.(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in various indicators of orthostatic stability in humans on long-term low-caloric diets].

Ten male volunteers received a low-caloric diet (LCD) (under 1200 kcal) containing 49.4 g protein, 28 g fat and 181 g carbohydrates for 3 weeks, with energy expenditure amounting to about 2900 kcal/day. During the time indicated, the body mass of the test subjects dropped by 7.1%. According to subjective data the well-being, physical and mental working ability remained unchanged in the test subjects. During feeding with the LCD and making the active orthostatic test, the increment of the heart rate upon the change of the body position appeared the same as that during the comparison period. Meanwhile on the 6th day of the recovery period the increment was significantly higher than at the comparison period. The systolic, pulse and mean arterial pressure, with the test subjects fed the LCD being in the vertical position and at the recovery period, was lower than at the comparison period. The data obtained attest to a definite lowering of the orthostatic stability of man kept on the LCD for a long time and at the recovery period, with this lowering taking place chiefly at the expense of changes in the tone of peripheral blood vessels.

[Experimental evaluation of two-sided velour vascular prostheses of Polish production].

The experiments were performed on nine mongrel dogs. The authors used straight crenated grafts of diameters 6 and 12 mm. In 6 dogs were implanted into abdominal aorta below an origin of renal arteries and above its bifurcation. Autopsy was performed after 49, 62, 98 days. The authors evaluated the implant examining it with two methods: macroscopic and histopathologic. In addition were 6 experiments made with three dogs. The grafts were placed extracorporealy between canulated artery and femoral vein. The authors measured the volume of transudated blood, the time of transudation and degree of grafts flexibility marked by the alongation. In was stated that tested double veloured grafts of Polish production are softer and more flexible than those produced so far. The edges of the tested grafts have higher shred resistance during the sewing procedure. Their more regular texture results in smaller degree of transudation: after initial typical preclotting the grafts "sweat" evenly and quickly stops the transudation. In authors opinion the grafts should be clinically evaluated in treatment of peripheral blood vessels diseases.

Effect of calcium membrane blockers on in vivo vasoconstrictor properties of norepinephrine, angiotensin II and vasopressin.

In the present study the in vivo role of transcellular calcium influx in humorally-mediated vasoconstriction was examined in conscious rats using two chemically dissimilar inhibitors of cellular calcium uptake, namely verapamil (Ver, 50 micrograms/kg/min) and nifedipine (Nif, 7.5 micrograms/kg/min). The pressor effect of arginine vasopressin (AVP, 10 mU/kg/min), angiotensin II (AII, 0.3 micrograms/kg/min) and norepinephrine (NE, 4 micrograms/kg/min) was virtually abolished by Ver and Nif. None of the pressors when infused alone increased mean arterial pressure (MAP) by increasing cardiac index (CI). During the administration of the calcium antagonists and AII or NE, CI was slightly but not significantly higher than with AII or NE alone. A modest fall in CI occurred with AVP and Ver but not with AVP and Nif. Thus, the effect of calcium antagonists to block the pressor responses of AVP, NE and AII was primarily by preventing the increase in peripheral vascular resistance rather than depressing CI. In summary, transcellular calcium influx appears to constitute the common in vivo pathway for constriction of peripheral blood vessels by AVP, NE and AII. These findings have substantial pathogenetic and therapeutic implications in hypo- and hypertensive states.

The cardiovascular responses of conscious newborn lambs treated in utero with 6-hydroxydopamine.

Fourteen fetal sheep were treated in utero with a single intramuscular injection or three to four injections of 6-hydroxydopamine. After birth the cardiovascular responses of these lambs to adrenergic drugs were studied and compared with the cardiovascular responses of untreated newborn lambs. Lambs which received three to four injections of 6-hydroxydopamine were more sensitive to the pressor actions of noradrenaline and phenylephrine than were control lambs or lambs which received a single injection of 6-hydroxydopamine. The supersensitivity to noradrenaline persisted in multiple treatment lambs until at least 5 months of age. Lambs which received multiple injections of 6-hydroxydopamine were also subsensitive to the pressor action of tyramine. The sensitivity to the pressor actions of noradrenaline and phenylephrine in lambs which received a single injection of 6-hydroxydopamine lay between that of control and multiple treatment groups. Despite the pharmacological evidence of impairment of sympathetic innervation of peripheral blood vessels plus a lack of responsiveness of these lambs to alpha-adrenergic blockade with phentolamine, treated lambs had arterial pressures and heart rates similar to control lambs. As well, treated lambs showed no impairment in their ability to reflexly regulate heart rate in response to changes in arterial pressure, which is consistent with the recently reported vagal control of the cardiac baroreceptor reflex.

[Mitogenic effect of erythropoietin on cultured aortic myocytes].

The administration of recombinant erythropoietin (rHuEpo) to anemic chronic renal failure patients may be associated with an increase in blood pressure, possibly by direct effects on peripheral blood vessels. In the present study, experiments were designed to explore the hypothesis that rHuEpo could enhance vascular resistance through mitogenic effect on vascular smooth muscle cells (VSMCs), and that preexisting hypertension might be a predisposing condition. Cultured VSMCs from the thoracic aortae of spontaneously hypertensive (SHR) and normotensive Wistar-Kyoto (WKY) rats were studied for DNA synthesis, phospholipase C activity, and cell growth related proto-oncogene expression in the presence of rHuEpo. In cells from both strains, rHuEpo dose-dependently increased DNA synthesis and stimulated phospholipase C activity, as indicated by 3H-thymidine incorporation and 3H-inositol phosphate formation, respectively (EC50 approximately 4 U/ml). Exposure of VSMCs to rHuEpo for various times gradually increased the levels of c-myc and junB and transiently induced c-fos expression, as determined by Northern analysis. rHuEpo-induced DNA synthesis was markedly enhanced in VSMCs from SHR compared to those from WKY. In contrast, rHuEpo-induced phospholipase C activity and proto-oncogene expression did not differ between the two strains. Taken together, these results suggest that rHuEpo may function as a vascular smooth muscle cell growth promoting factor through activation of the phospholipase C cascade and modulation of proto-oncogene expression. It could thereby contribute to vascular hypertrophy and arterial hypertension.

Sensory functions of the heart.

Stretch receptors are situated in the walls of the atria, left ventricle and coronary arteries. Atrial receptors are stimulated by atrial distension due to increased filling as the result, amongst other things, of an increased blood volume. Their reflex responses are increases in heart rate and in urine flow. Ventricular receptors may be stimulated by chemical agents, mechanical stimuli, or both. They are responsible for the Bezold-Jarisch reflex of bradycardia and hypotension, but their normal physiological role, if any, is uncertain. The proximal part of the left coronary artery contains baroreceptors which are excited by increases in blood pressure. Their role, like other baroreceptors, is to control blood pressure by regulation of the diameter of the peripheral blood vessels.

Saralasin and SQ 20881 : their effects on central nervous system norepinephrine.

The effects of the infusion of Saralasin and SQ 20881, two drugs that inhibit the renin-angiotensin system, on the norepinephrine (NE) concentration in hypothalamus and medulla oblongata were studied in male Wistar rats. NE content increased in hypothalamus in response to both drugs, without changes in medulla oblongata catecholamine concentration. These results showed that the NE concentration of certain areas of the central nervous system can be modified, in a short time, by the inhibition of the renin-angiotensin system. Results observed after the infusion of Saralasin could indicate that the receptors, on which the angiotensin acts to produce this alteration, are similar to those of the peripheral blood vessels.

[Effect and effectiveness of iodine brine baths in a spa].

On the occasion of closing the research department of the Paracelsus Institute in the iodine salt spa, Bad Hall, the results of more than 30 years' research work in this institute are summarized. This work concerned the therapeutic effects of iodine on circulatory illnesses, especially hypertension, bronchitis and degenerative eye diseases. Controlled studies in patients showed significant cure effects of the iodine springs. Other studies demonstrated an influence of iodine on peripheral blood vessels, blood viscosity, connective tissue and vision and demonstrated the scavenger function of iodine, too. The different therapeutic forms of iodine administration (baths, sprays, iontophoresis, drinking cures) are described and the potential side effects of a cure in an iodine spa, notably temporary hypothyroidism, are mentioned.

[Glaucoma with low intraocular pressure--diagnosis and treatment].

The author examined 23 patients (46 eyes) with peripheral blood vessels diseases and non-characteristic ocular complaints, which has been lasted for 3-7 years (mean 4.3) and were treated as conjunctivitis. Diagnosis of low-tension glaucoma was made basing on the following signs: normal intraocular pressure, open angle, distinctly abnormal outflow facility coefficient, typical for glaucoma changes in visual field and glaucomatous excavation of the optic disc. The typical treatment with timolol and pilocarpine was applied and additionally resistant cases calcium channel blockers. The control examination after mean follow-up of 22 months revealed that in all patients, subjective symptoms subsided and a statistically significant lowering of the intraocular pressure, as well as an improvement of the outflow facility coefficient and visual field were observed. In no patient the state of the fundus was deteriorated. These results are comparable with the data from literature.
