Parents' satisfaction with children's tooth color: fluorosis as a contributing factor.

The authors surveyed parents of 708 patients in a pediatric dental practice about their satisfaction with the color of their children's teeth and factors associated with their level of satisfaction. Overall, 43 percent of parents were dissatisfied with their children's tooth color, and 78 percent of children had a Tooth Surface Index of Fluorosis, or TSIF, score greater than 0. The worst TSIF score was the only factor associated with parent satisfaction.

Are prosthodontics a vital part of dentistry?

Prosthodontics in all of its various subspecialties is in high demand in the United States. It represents a major portion of a typical dentist's income. On the basis of data recently obtained about the projected increase in the physiological need for prosthodontic services and the extensive involvement of U.S. dentists in all areas of prosthodontics, one cannot deny the need for continued and increased predoctoral, doctoral and postgraduate education in prosthodontics.

A modified technique on the reattachment of permanent tooth fragments following dental trauma. Case report.

Fractured anterior teeth can be restored by adhesive bonding of the fractured fragment to the remaining tooth structure. One of the major challenges for the practitioner treating traumatized anterior teeth with immediate fragment reattachment is disguising the fracture line, through the correct use of masking and restorative resins to make the restorations imperceptible to the eye as well as improve the retention of the restoration. This paper discusses a modified technique for reattaching a permanent tooth fragment following dental trauma. The initial procedure involved simple reattachment using light cured composite resin between the fragment and the remnant part of the tooth, without additional preparation. The surplus resin was spread across it in an attempt to optimize marginal seal and improve the aesthetics of the restoration. Finally, after taking into account the occlusion, the lingual surfaces of the teeth were veneered with microfilled composite to improve the retention of the reattached fragments.

[Future and needs of regenerative therapy in prosthetic treatment].

Bone regeneration is currently considered to be most significant in implantology as regenerative therapy is directly involved and essential in dental prosthetics. The following factors, however, must be clarified before prosthetic treatment can be performed; 1. Necessity of bone regeneration and recovery with individual method. 2. Change of regenerated bone. 3. Analysis of dynamics which will affect bone resorption. Though these factors are important in prosthetics, dynamic analysis is not performed well even in the existing bone. Clinical follow-ups so far indicate that not only infection but also excessive loading influence most of the cases whose progress is not good. Moreover, there are many challenges to be solved for bone generated by regenerative therapy such as the timing of placing the implant, the timing of loading, conditions for loading and occlusal control.

Factors associated with subjective need for orthodontic treatment among Finnish university applicants.

Subjective orthodontic treatment need and associated factors were studied in a sample of 226 young Finnish adults. The subjects, applicants to enter the University of Helsinki, were interviewed immediately after their entrance examination. A need for treatment was felt by 14% of the subjects. For 47% of those expressing such a need their reason was appearance only, and for 25% it was both appearance and their ability to chew. Among subjects who reported a visible dental irregularity, 38% were dissatisfied with their dental appearance, and 25% felt the need for orthodontic treatment. Women were more often dissatisfied with their dentition than men. Subjective need for treatment was significantly associated with perceived visible dental irregularity and infrequent socializing but not with orthodontic treatment in childhood, pattern of dental attendance, or district of residence. Of various perceptions of anterior teeth, irregularly and wrongly positioned teeth were most often connected with treatment need.

Comparison of three indices in evaluation of orthodontic treatment outcome.

Three indices measuring outcome of orthodontic treatment were evaluated and compared in 39 specialist-treated individuals (group 1) and in 20 specialist-treated individuals who had undergone orthognathic surgery (group 2). The outcome was evaluated in accordance with the modified Indication Index, the modified ISMHB (Index of the Swedish Medical Health Board), and the PAR (Peer Assessment Rating) Index. Pre- and post-treatment need scores on the basis of the three indices were estimated. The ranking order of the individuals by the different indices was compared in the two groups separately, before and after treatment. Associations between the Indication Index and the ISMHB were consistently stronger than associations between the PAR Index and the two other indices, for which the variations were great. The reasons for the great variations seemed to be the different bases for assessment used and the different constructions of the indices: the PAR Index estimates deviations from an ideal occlusion, and the other indices evaluate treatment need. The outcome describe by the indices differed with regard to the pre- and post-treatment scores and the ranking order of the individuals. Of the three indices, the Indication Index was found to be the one most suitable for estimating treatment outcome in terms of decrease of treatment need, and the PAR Index was unsuitable for estimating treatment need.

Effect of presilanization filler decontamination on aesthetics and degradation resistance of resin composites.

Filler-matrix coupling determines, to a large extent, the mechanical strength and clinical longevity of dental composites. The aim of this study was to examine how far a methodology to decontaminate filler prior to silanization may improve aesthetic performance in addition to physico-mechanical properties such as degradation resistance. It was reported that filler particles are surrounded and wrapped by a film that consists of multiple layers of silane molecules. X-ray photoelectron spectroscopy, however, revealed that silanization of filler particles largely depended upon siloxane bridge (Si-O-Si) formation between the silica surface and the silane molecule rather than on intermolecular bonding between adjacent silane molecules. In this study, we showed that filler decontamination resulted in a higher translucency, thereby providing a better aesthetic potential. In addition, experimental composites produced following presilanization decontamination of filler revealed a higher Vickers hardness value and a diametral tensile strength that was resistant to degradation by thermo-cycling.
