The effect of progestins on the mitotic activity of uterine fibromyomas.

Previous studies have shown increased mitotic activity in uterine fibromyomas in patients using exogenous hormones. This study has compared the mitotic activity of fibromyomas in three groups of patients. The first group was using a progestin-only preparation (usually depo-medroxyprogesterone acetate). The second group of age-matched controls had never used any exogenous hormone. The third group of patients was using a combined estrogen/progestin oral contraceptive. The results show that patients using a progestin-only preparation have a significantly higher mitotic activity in fibromyomas than patients from the other two groups; results from patients using a combined estrogen/progestin preparation were the same as the controls. The highest count was 39 mitoses/100 high power fields (HPF), but focal areas containing up to 8 mitoses/10 HPF could be found. In none of the fibromyomas examined did the count exceed 10 mitoses/10 HPF, and none showed significant pleomorphism that might have been confused with malignancy.

Effect of oral contraceptives on plasma androgenic steroids and their precursors.

Plasma dehydroepiandrosterone (DHEA), its sulfate ester (DHEAS), and androstenedione (A2), delta5-pregnenolone (delta5-P), and 17 alpha-hydroxypregnenolone were measured in women using estrogen-containing oral contraceptives and in female controls of similar age. All three androgenic steroids and their two C21 precursors were reduced in the oral contraceptive users compared with the controls. It is concluded that oral contraceptives cause a decrease in plasma DHEA, DHEAS, and A2, one possible mechanism for which is the inhibition of delta5-P synthesis from cholesterol.

Metabolic effects of contraceptive steroids. I. Observations of blood lipids.

Semilongitudinal observations on fasting blood sugar, total lipids, cholesterol, phospholipids, triglycerides, nonesterified fatty acids (NEFA), and lipoproteins were carried out on 116 women taking combination type oral contraceptives for 3-42 cycles. The changes observed differed with each constituent. Total lipids showed significant increase throughout the period of observation. Cholesterol and phospholipds showed significant increases after about 12 cycles of administration whereas triglycerides showed an increase after 6 cycles of medication. NEFA showed significant decrease in the 1st-3rd cycles. The alpha and beta lipoprotein ratios showed significant changes after 13-24 cycles of treatment. The general trend of change observed is similar to that reported during pregnancy.
