Chemokine expression precedes inflammatory cell infiltration and chemokine receptor and cytokine expression during the initiation of murine lupus nephritis.

Lupus nephritis is characterized by immune complex deposition and inflammatory cell infiltration. Therefore, the initiation and progression of lupus nephritis in MRL/MpJ Fas(lpr/lpr) (MRL/lpr) mice were investigated, with a focus on the expression of several chemokines and chemokine receptors. Mice were monitored for proteinuria from 6 to 20 wk of age, and kidneys were examined every 2 wk by light microscopy, electron microscopy, and immunohistologic analyses. Furthermore, the expression of chemokines, chemokine receptors, and proinflammatory cytokines was analyzed in ribonuclease protection assays. MRL/lpr mice demonstrated increased expression of monocyte chemoattractant protein-1, regulated upon activation, normal T cell-expressed and -secreted protein, inducible protein of 10 kD, and macrophage inflammatory protein-1beta at week 8. At that time point, levels of circulating and glomerular immune complexes were increased, and no proteinuria or histopathologic signs of renal damage could be observed. As assessed in immunohistochemical and in situ hybridization analyses, monocyte chemoattractant protein-1 and regulated upon activation, normal T cell-expressed and -secreted protein expression was preferentially located in the glomeruli and interstitium. Mononuclear cell infiltration of the kidney was observed by weeks 10 to 12. At week 12, the renal expression of chemokine receptor 1 (CCR1), CCR2, and CCR5 was increased, mice became proteinuric, and renal damage was histologically evident. Finally, the expression of proinflammatory cytokines was detected (weeks 12 to 14). In summary, (1) chemokines are upregulated before inflammatory cell infiltration, proteinuria, and kidney damage are observed; (2) chemokine generation is restricted to sites of subsequent inflammatory cell infiltration, i.e., glomeruli and interstitium; (3) chemokine receptor expression parallels mononuclear cell infiltration; and (4) proinflammatory cytokines are upregulated later, in parallel with inflammatory cell infiltration and the onset of proteinuria. These results support the hypothesis that chemokines initiate leukocyte infiltration and precede proteinuria and renal damage in MRL/lpr mice.

Distribution of phosphorus in manure slurry and its infiltration after application to soils.

Computer models help identify agricultural areas where P transport potential is high, but commonly used models do not simulate surface application of manures and P transport from manures to runoff. As part of an effort to model such P transport, we conducted manure slurry separation and soil infiltration experiments to determine how much slurry P infiltrates into soil after application but before rain, thus becoming less available to runoff. We applied dairy and swine slurry to soil columns and after both 24 and 96 h analyzed solids remaining on the soil surface for dry matter, total phosphorus (TP), and water-extractable inorganic (WEIP) and organic (WEOP) phosphorus. We analyzed underlying soils for Mehlich-3 and water-extractable P. We also conducted slurry separation experiments by sieving, centrifuging, and suction-filtering to determine which method could easily estimate slurry P infiltration into soils. About 20% of slurry solids and 40 to 65% of slurry TP and WEIP infiltrated into soil after application, rendering this P less available to transport in runoff. Slurry separation by suction-filtering through a screen with 0.75-mm-diameter openings was the best method to estimate this slurry P infiltration. Measured quantities of manure WEOP changed too much during experiments to estimate WEOP infiltration into soil or what separation method can approximate infiltration. Applying slurries to soils always increased soil P in the top 0 to 1 cm of soil, frequently in the 1- to 2-cm depth of soil, but rarely below 2 cm. Future research should use soils with coarser texture or large macropores, and slurry with low dry matter content (1-2%).

[An experimental study of tumor cell infiltration into temporal bones--route into the inner ear from the arachnoid space].

In humans, metastatic tumors which invaded the temporal bones have been studied in regard to the relationship between histopathologic findings and clinical symptoms. On the other hand, there is no experimental study using an animal model for tumor infiltration into the temporal bone. This study was designed to establish such an animal model and examine the temporal bones histopathologically. Rat thymic lymphoma cell (FTL-A2) were inoculated into the cisterna magna of Wistar rats. The animals were decapitated under deep anesthesia with pentobarbital sodium from the 1st to 8th day after inoculation. Their heads were fixed with Heiden-hain SuSa solution, decalcified, dehydrated, embedded in celloidin, and sectioned horizontally at a thickness of 25 microns. These were stained with hematoxylin and eosin, and histologically examined by light microscopy. Inoculated tumor cells showed active viability in the arachnoid space at a rate of 98%. Two major routes of tumor cell infiltration into the inner ear were found: the cochlear aqueduct and the internal auditory canal. At the early stage after the inoculation, tumor cells infiltrated the scala tympani through the cochlear aqueduct. Invading the fiber of the cochlear nerve, tumor cells infiltrated Rosenthal's canal via the tractus spiralis foraminosus, and passed through Rosenthal's canal and the osseous spiral lamina into the scala tymani. However, tumor cells did not infiltrate the organ of Corti through the habenula perporata. Tractus spiralis foraminosus and habenula perforata functioned as a barrier against tumor infiltration. In a few cases, tumor cells infiltrated over the macula cribrosa into the subepithelial space of the utricule and saccule. The macula cribrosa functioned as a barrier.

[Is therapy with local infiltrations feasible in primary care consultations?].

OBJECTIVE: To study the feasibility of local infiltration in primary care consultations. DESIGN: Before-and-after intervention study. SETTING: Two clinics at an urban health centre. PARTICIPANTS: Patients diagnosed with pathology of tender areas and treated with corticosteroid infiltration between May 1997 and May 1998. INTERVENTION: Corticoid infiltration plus local anaesthetic. Weekly check-up. Analysis of the variables: sex, age, diagnosis, time elapsed between indication and start of treatment, subjective assessment of pain before and after treatment (VRS scale), number of infiltrations per patient, side-effects. Evaluation of overall and individual effectiveness. RESULTS: 41 patients were infiltrated. Average age was 58. Most common pathologies were: rotary joint tendinitis (48.7%), anserine bursitis (24.4%), plantar fasciitis (7.3%). Average number of infiltrations per pathology: 1.3. Mean waiting time: 3.5 days. Comparison of pain by means of VRS (range 0-5) before and after treatment used the Wilcoxon test, with a statistically significant difference and p < 0.001 (z = -5.5109). For 35 patients (85.4%), pain was solved very well (values 0 and 1 on the VRS). For 3 patients (7.3%), improvement was moderate; and for 3 (7.3%) there was no improvement. CONCLUSIONS: 1. Treatment with local infiltration of corticosteroids is effective in dealing with pain, and is an alternative to treatment with NSAIDs. 2. It is feasible in primary care, and there are many advantages if the general practitioner employs this therapeutic technique.

Macrophage infiltration and heme oxygenase-1 expression correlate with angiogenesis in human gliomas.

Macrophages are key participants in angiogenesis. In this study on human brain tumors, we first investigated whether macrophage infiltration is associated with angiogenesis and malignant histological appearance. Immunostaining of macrophages and small vessels in resected glioma specimens indicated that numbers of infiltrating macrophages and small vessel density were higher in glioblastomas than in astrocytomas or anaplastic astrocytomas. Macrophage infiltration was closely correlated with vascular density in human gliomas. Heme oxygenase-1 (HO-1), which is the rate-limiting enzyme in heme catabolism, was also associated with activated macrophages. Expression of mRNA encoding HO-1 was correlated with macrophage infiltration and vascular density in human glioma samples. Infiltrating macrophages were positively stained with anti-HO-1 antibody by immunohistochemical analysis, and in situ hybridization for HO-1 indicated that HO-1 was expressed in infiltrating macrophages in gliomas. HO-1 gene may be a useful marker for macrophage infiltration as well as neovascularization in human gliomas.

Enhanced local intracoronary delivery of heparin with the Infiltrator catheter: a comparative study.

The efficacy of local drug delivery in the treatment of coronary artery disease is limited by the relatively low delivery efficiency of the available devices. A unique local drug delivery device, the Infiltrator catheter (InterVentional Technologies, Inc.), has been designed specifically to enhance efficiency by injecting drugs directly into the arterial wall through microports mounted on the balloon surface. The purpose of this study was to assess the efficiency of delivery of this device in the porcine coronary model and to compare it to a previously validated device, the hydrogel balloon (Boston Scientific, Maple Grove, Minnesota). Studies were also performed to assess the pattern of intramural heparin deposition following delivery with the Infiltrator catheter and to assess the effect of the microports on vascular integrity. The efficiency of delivery was significantly greater with the Infiltrator catheter than with the hydrogel balloon (4.5% vs. 0.08%; p = 0.02). Similarly, the absolute amount of intramurally deposited heparin was greater with the Infiltrator (111.3 +/- 38.5 units vs. 2.4 +/- 0.85 units; p = 0.02) despite the fact that more heparin was delivered with the hydrogel catheter. Histologic studies revealed characteristic discrete puncture channels in the vessel wall due to penetration of the microports. Other than this histologic finding, there was no significant difference in the extent of architectural disruption between the Infiltrator and conventional balloon inflations. Fluorescein-labeled heparin studies revealed heparin to be diffusely distributed throughout the vessel wall immediately following delivery with the Infiltrator. We conclude that the Infiltrator catheterOs unique mechanism of delivery improves the efficiency of local drug delivery without excessive vessel wall trauma.

[The potencies of substance P, substance P fragments, and compound 48/80 for granulocyte infiltration in mouse skin].

To determine whether the N-terminal or C-terminal peptide of substance P (SP) induces granulocyte infiltration in mouse skin, we examined the potencies of SP, the N-terminal peptides SP1-4 and SP1-9, the C-terminal peptides SP4-11 and SP6-11, and a mast cell degranulating agent compound 48/80 in inducing granulocyte (neutrophil and eosinophil) infiltration in the skin of BALB/c mice. The subcutaneous administration of SP (10(-7)-10(-5) M) caused granulocyte infiltration in mouse skin in a concentration-dependent fashion 6 h after the injection. SP1-9 (10(-5)-10(-4) M) also caused granulocyte infiltration in the skin which was associated with mast cell degranulation. However, SP1-4, SP4-11 and SP6-11 (up to 10(-4) M) induced neither granulocyte infiltration nor mast cell degranulation. In addition, compound 48/80 (0.5-50 micrograms/ml) also induced granulocyte infiltration of mouse skin with a concentration-dependent increase in mast cell degranulation. These results indicate that SP induces granulocyte infiltration of mouse skin through mast cell degranulation induced by the N-terminal peptide.

Effects of tumor necrosis factor, lipopolysaccharide, and IL-4 on the expression of vascular cell adhesion molecule-1 in vivo. Correlation with CD3+ T cell infiltration.

We have injected human TNF, LPS, and IL-4 into the skin of baboons to examine regulation of endothelial leukocyte adhesion molecules (ELAM) in vivo and to determine which endothelial adhesion molecules correlate temporally and spatially with cytokine-induced T cell infiltration. The expression of adhesion molecules ELAM-1 (E-selectin), VCAM-1, and ICAM-1 (CD54) were quantified by immunocytochemical staining of frozen sections obtained from skin biopsies; T cell infiltration was measured by immunocytochemical staining of CD3+ T cells in serial sections. We found that injection of TNF causes late (24 to 48 h) T cell infiltration whereas injection of LPS, in doses that do not cause tissue necrosis, does not. The ability of TNF (but not LPS) to recruit T cells correlates with the ability of TNF to cause sustained endothelial cell adhesion molecule expression. Expression of VCAM-1 on post-capillary venules showed the highest degree of spatial localization with infiltrates. IL-4, although not proinflammatory by itself, can cause T cell infiltration in combination with an ineffective dose of TNF. The ability of IL-4 to augment TNF-induced inflammation best correlates with the ability of the combination of IL-4 and TNF to increase endothelial VCAM-1 expression. In contrast, IL-4 does not promote T cell infiltration or endothelial VCAM-1 expression in combination with LPS. In cytokine-injected tissues, VCAM-1 is also expressed on connective tissue cells other than endothelium, including smooth muscle and perineural cells, where it is induced by cytokines in parallel with endothelial VCAM-1. Overall, our data support the hypothesis that endothelial VCAM-1 expression contributes to T cell extravasation at sites of inflammation. Furthermore, we find that IL-4, a product a Ag-activated T cells, can interact with TNF to selectively promote VCAM-1 expression and the development of T cell-rich infiltrates, characteristic of Ag-induced inflammatory reactions.

[Influence of dendritic cell infiltration on prognosis and biologic characteristics of progressing gastric cancer].

OBJECTIVE: To study the relation between dendritic cell (DC) infiltration and clinicopathologic parameters, biologic characteristics and prognosis of progressing gastric cancer. METHODS: The development of apoptotic cell death (apoptotic index, AI) in 61 progressing gastric carcinoma tissues was analyzed by terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate biotin nick end labeling (TUNEL) method. The PCNA labeling index (PCNA-LI), density of dendritic cells in the tumor were detected by immunohistochemical method by the LSAB kit using antibody against S-100 protein and PC-10. RESULTS: DC infiltration was negatively correlated with lymph node metastasis, clinical stage and PCNA-LI, but positively with AI. The DCs in gastric cancer groups with and without lymph node metastasis were (5.63 +/- 4.37)/HPF and (8.51 +/- 5.57)/HPF with difference significant (P < 0.05). The DC infiltration in I, II, III stage lesions were (11.23 +/- 6.05)/HPF, (6.28 +/- 4.37)/HPF and (5.53 +/- 5.19)/HPF also with differences significant (P < 0.01). The PCNA-LI was significantly higher in the low DC group (57.10% +/- 14.18%) than that of high DC group (48.15% +/- 10.59%, P < 0.01). AI findings were 3.77% +/- 1.26% and 2.95% +/- 1.07% in the high and low DC groups (P < 0.01). A positive correlation was observed between DC infiltration and AI (r = 0.39, P < 0.01) whereas a negative correlation between DC infiltration and PCNA-LI (r = -0.47, P < 0.01). The prognosis of high DC infiltration patients was significantly better than those with low ones. CONCLUSION: The infiltrating dendritic cells in and around tumor, representing the local immune status of the host, may play an important role in immunological defense mechanism of host versus tumor. Dendritic cells may inhibit the proliferation and induce the apoptosis of the tumor cells, thus affecting the clinical features and improve the prognosis of gastric carcinoma.

Pulmonary infiltrates during chemotherapy-induced febrile neutropenia: incidence, patterns and outcomes.

OBJECTIVE: To analyze the incidence, etiologies, radiographic patterns, and clinical outcomes of adult leukemics with prolonged febrile neutropenia and pneumonia. METHODS: A retrospective study was conducted at a tertiary care hospital. The medical records of adult patients with acute myeloid leukemia diagnosed between January 1989 and June 2000 and undergoing induction chemotherapy were included. Only the patients who presented with a pulmonary infiltrate, secondary leukemia (e.g., transformed chronic myeloid leukemia underlying myelodysplastic syndrome, or disease following alkylating agent therapy) were included and those developing infiltrates following consolidation chemotherapy were excluded. RESULTS: A total of 124 patients were admitted to the hospital with a diagnosis of AML during the study period. Thirty-one patients were excluded; 93 patients received induction chemotherapy and were included in the study analysis. The median age was 36 years (15 - 70 years); 58 males and 35 females. Sixty two percent patients received Cytosine Arabinoside (Ara-C), 17% received Etoposide, 11% received Ara-C and Mitoxantrone, and 6% received All-trans-retinoic Acid. The mean onset and duration of neutropenia were 5 and 15 days, respectively. Pulmonary infiltrates were identified during 45% of neutropenic episodes. A presumptive causative organism was isolated from 50% of patients with an infiltrate: Gram-positive bacteria were most common (47%) followed by Gram-negative bacilli (33%) and fungi (20%). Survival data were available for 88 patients; median disease free survival for the entire cohort was 7 months. Male sex (p=0.015), onset of neutropenia (p=0.02) and bilateral distribution of an infiltrate (p=0.03) were statistically significant predictors of early mortality. For patients with and without pneumonia, the median disease-free interval and overall survival were 2.5 and 4.6 months and 9 and 13 months (p=0.038 and p=0.095) respectively. CONCLUSION: Neutropenia occurred at a mean of 5.0 after initiation of induction chemotherapy. The majority of patients had bilateral pulmonary infiltrates. Male sex, onset of neutropenia and bilateral distribution of an infiltrate were found to be statistically significant predictors of early mortality.

Magnetic resonance imaging of pelvic and femoral bones for detection of bone marrow infiltration in patients with non-Hodgkins lymphoma.

OBJECTIVE: Detection of the residual bone marrow (BM) involvement is essential in treatment of patients with non-Hodgkin's lymphoma (NHL). Bilateral iliac crest BM biopsies appear to have low sensitivity for BM infiltration, while magnetic resonance imaging (MRI) presents more global view of BM. This study was conducted to determine the advantage of femoral marrow MRI as a non-invasive technique over bilateral iliac crest BM biopsies in detecting BM infiltration before treatment and residual disease after completion of treatment. METHODS: Over a period of 27 months from January 2002 to March 2004, a total of 30 patients with newly diagnosed NHL were included in the study. Magnetic resonance imaging of 26 patients were conducted in King Fahd Hospital of the University, Al-Khobar, Kingdom of Saudi Arabia. Magnetic resonance imaging of 4 patients were referred from different hospitals in the Eastern Province. Twenty-five patients were of B-cell type and 5 patients were of T-cell type. Coronal MRI of the pelvis and femoral marrow were obtained by the T1, T2-weighted spin echo sequences and short TI inversion recovery technique. RESULTS: Magnetic resonance images showed BM infiltration in 17 cases (56.7%) before treatment and positive biopsy results were found in 9 cases (30%); all had abnormal MR images. There was a significant difference between both methods in the detection of infiltration (p=0.037). Magnetic resonance imaging showed that 58.8% of cases have scattered pattern, 23.5% were uniform and 17.6% of cases have nodular patterns. Magnetic resonance images after completion of treatment showed residual BM infiltration in 6 out of 17 cases who previously had positive MR images and only one case of them had a positive BM biopsy with a significant difference between both methods and all of them relapsed within 6 months (p=0.034). CONCLUSION: Magnetic resonance images of the pelvis and femoral marrow were superior than BM biopsy on detection of BM infiltration before treatment and residual infiltration after treatment.

[Dilated cardiomyopathy--a chronic myocarditis? Immunohistological characterization of lymphocytic infiltrates].

Experimental and clinical data suggest a relationship between myocarditis and dilated cardiomyopathy. One postulated pathomechanism is a viral infection triggering a host response with autoimmune features directed against the heart, resulting in an initial myocarditis which is followed by a dilated cardiomyopathy. Until now, the importance of an undetected myocarditis as an etiological factor in the pathogenesis of idiopathic cardiomyopathy is unknown. This investigation was undertaken to determine the frequency of lymphocytic infiltrations in endomyocardial biopsies of patients with dilated cardiomyopathy by immunohistological methods and to analyze T lymphocytic subsets and other immunological features in order to search for possible relationships between immunohistological-documented myocarditis and dilated cardiomyopathy. In our study, 48 of 130 biopsies (37%) from patients with clinically proven dilated cardiomyopathy contained lymphocytic infiltrates when stained by the immunoperoxidase method with lymphocyte surface markers (Table 1). 23% (n = 30) of these biopsies contained more than 2.0 (range: 2.0 to 13.8) T lymphocytes per high power light microscopy field (x 400), in 14% (n = 18) 1.5 to 2.0 cells/HPH were seen (Table 2). 82 biopsies (63%) with less than 0.8 cells/HPF were regarded as negative (dilated cardiomyopathy). By histological analysis, only seven cases (5%) were classified as borderline myocarditis by conventional histological evaluation according to the Dallas classification. In the other patients, our results were consistent with the clinical diagnosis of dilated cardiomyopathy (Table 1). 71% of biopsies with lymphocytic infiltrates contained activated T cells when analysed with activation markers in serial sections (Table 2). Activated macrophages were seen in 52% of biopsies with T cell infiltrations, but only in 31% of tissues containing normal numbers of lymphocytes (Table 2). In biopsy specimens with lymphocytic infiltrates HLA-class I and II antigen expression was increased. An enhanced HLA-DR antigen staining was seen in 80% on interstitial cells and vascular endothelium while HLA class I was found at an increased level in 67% (Table 3). In negative biopsies, an enhanced class I staining was seen in only 14%, class II in 30%. Because of the specific identification of lymphocytes by immunocytochemical methods, lymphocytic infiltrates are easily detected, even if the infiltrate is sparse or focal (Figure 1). The considerable interobserver variability in the quantitation of lymphocyte counts, which is a main problem in hematoxylin and eosin stained sections is negligible when lymphocyte quantitation is performed by immunoperoxidase staining. Thus, our data indicate that in about 37% of patients with clinical suspected idiopathic cardiomyopathy an ongoing or reactivated myocarditic process is involved.(ABSTRACT TRUNCATED AT 400 WORDS)

Evaluation of the L2 spinal nerve root infiltration as a diagnostic tool for discogenic low back pain.

BACKGROUND: To assess whether unilateral L2 infiltration with local anesthetic can be used to identify patients who will have negative discograms and thus eliminate the need for the discogram. Discogenic low-back pain is considered to have afferent pathways in the sinuvertebral nerves, mainly originating from the ventral rami of the spinal nerves. There is evidence that pain arising from the lower lumbar intervertebral discs may be transmitted through the sympathetic afferent fibers contained in the L2 spinal nerve root. Provocative discography, within the context of other clinical data, is the current "gold standard" by which to diagnose discogenic low-back pain, but a far more invasive procedure than L2 infiltration. OBJECTIVE: To evaluate the correlation between unilateral second lumbar (L2) spinal nerve root infiltration with local anesthetic and provocative discography in patients with chronic low back pain. STUDY DESIGN: A prospective, observational study. METHODS: All patients scheduled for discography were asked to participate in having local anesthetic infiltration of the L2 spinal nerve root at least two weeks prior to discography, until forty subjects were enrolled. Discography was performed after the patient's pain level returned to baseline. RESULTS: Local anesthetic infiltration of the L2 spinal nerve root was predictive of provocative discography results in only 46.5% of the subjects (26% true positives, and 20.5% true negatives). In 53.5% of the subjects, L2 infiltration was not predictive of discography results (20.5% false positives, and 33% false negatives). CONCLUSIONS: The results showed that unilateral L2 infiltration is not predictive of discogenic low-back pain when compared to discography, the current "gold-standard" for diagnosis.

Association of leptomeningeal carcinomatosis in carcinoma of the breast with infiltrating lobular carcinoma. An autopsy study.

We analyzed the autopsy records of 261 patients with breast carcinoma, who died at The Institute of Oncology, Ljubljana, Yugoslavia, for the presence of metastases to the central nervous system, particularly for the presence of leptomeningeal carcinomatosis. Metastases to the central nervous system were found in 41 (18.1%) of 226 patients who had metastatic cancer. In 24 cases (10.6%), metastases were seen in brain parenchyma; in 11 cases (4.9%), only dura mater was involved, and in six cases (2.6%), leptomeningeal carcinomatosis was found. All patients with leptomeningeal carcinomatosis showed some symptoms of central nervous system involvement while alive. The patients died between 2 weeks and 2 months after the onset of central nervous system symptoms. Four of six patients with such a complication presented with locally advanced or disseminated carcinoma on the first admission. With regard to histologic findings, three patients had infiltrating lobular carcinoma, two had infiltrating ductal carcinoma, and one had mixed infiltrating lobular and ductal carcinoma, with a metastasizing lobular component. Metastases to the brain parenchyma were found exclusively in infiltrating ductal carcinoma. We stress that infiltrating lobular carcinoma represents a distinctive type of breast cancer with an unusual propensity to disseminate into leptomeninges, as this occurred in almost 14% of all cases of infiltrating lobular carcinoma in our series in contrast to a 1% incidence in cases of infiltrating ductal carcinoma.

Prognostic significance of lymphoid infiltration in cerebral malignant gliomas.

Data concerning a study on lymphoid infiltration in 61 cerebral malignant gliomas are reported. Lymphoid cellular infiltrates were found in 28 cases (45.9%): 8 (13.1%) with marked and 20 (32.8%) with slight infiltration; the remaining 33 cases (54.1%) did not exhibit lymphoid infiltration. The mean survival time (+/- standard deviation) of patients harboring gliomas with marked lymphoid infiltration was 20.5 (+/-19.9) months, and that of patients with slight lymphoid infiltration in their glioma was 10.3 (+/- 7.5) months; those patients having gliomas without lymphoid cellular infiltrates showed a mean survival time (+/- standard deviation) of 7.2 (+/-6.1) months.

[Monoclonal antibody analysis of glomerular, tubulo-interstitial infiltrating immune cells in various glomerulonephritis].

To investigate the role of cell-mediated immunity (CMI) in glomerulonephritis (GN), we identified the infiltrating immune cells both within the glomerulus and in the interstitium. Frozen sections from 103 patients with various forms of GN: 10 with minor glomerular abnormality (MGA) as control, 10 with minimal change nephrotic syndrome (MCNS), 10 with membranous nephropathy (MN), 9 with focal glomerulosclerosis (FGS), 30 with IgA nephropathy (IgAN), 22 with acute post streptococcal glomerulonephritis (APSGN), and 2 with rapidly progressive glomerulonephritis (RPGN) were examined using monoclonal antibodies (MoAb) by indirect immunoalkaline-phosphatase labelling. In most glomerulonephritis, monocyte/M phi and helper/inducer T cells were predominantly infiltrating in the interstitium, but intraglomerular infiltration was rare, except for APSGN. This interstitial infiltration increased proportionally to the level of serum creatinine, and was most prominent in RPGN. Apparently different distribution was seen in APSGN, that is, prominent increase in total number of intra-glomerular monocyte/M phi infiltration with slightly increased T cells. The change was correlated with time after onset; namely the more leucocytic infiltration was observed when the tissue was taken earlier. These data suggest that in APSGN, monocyte/M phi accumulate in glomeruli via cell mediated immunity in addition to humoral immune mechanism resulting in glomerular hypercellularity, whereas in most chronic glomerulonephritis interstitial leucocyte infiltration, particularly helper T cells and monocyte/M phi may play an important role in the progression of glomerulonephritis.

[Antigen-induced biphasic eosinophil infiltration in the airways of actively sensitized guinea pigs and its inhibition by PAF antagonist and cyclosporin A].

Bronchial eosinophilia is a characteristic of asthma. To elucidate the mechanisms of eosinophil accumulation in the airways, the time course of eosinophil infiltration in the airway mucosa after antigen inhalation was examined in actively sensitized and passively sensitized guinea pig models of asthma. The lungs and tracheae were removed at intervals after antigen challenge, fixed and stained. The eosinophil infiltration was quantitated in the tracheal walls by counting the number of cells per square millimeter. Guinea pigs sensitized by intraperitoneal injection of ovalbumin (OA) responded to a single exposure to aerosolized OA with biphasic infiltration of eosinophils in the tracheal walls; a striking early-phase which peaked at 6 hr and a delayed-phase which peaked at 24 hr and persisted for as long as 5 days. These kinetics were different from those observed with passively sensitized animals which showed only early-phase infiltration. Administration of CV-6209, a specific PAF antagonist, before and 12 hrs after antigen challenge significantly (p less than 0.01) inhibited the early-phase but not the delayed-phase eosinophil infiltration in actively sensitized animals. In contrast, when guinea pigs were treated with Cyclosporin A, a T lymphocyte-selective immunosuppressive agent, throughout the immunization period, the delayed-phase but not the early-phase infiltration was significantly (p less than 0.01) inhibited. These results suggest that PAF may contribute to early-phase and T cell factor(s) may contribute to delayed-phase eosinophil infiltration of the airways.

Tumor-infiltrating cells and their possible prognostic significance.

1. The introduction presents the most important recent knowledge of tumor immunity. All the known concepts of tumor-infiltrating cells are summarized including the historical background of stromal reaction. Its possible significance in an assessment of prognosis of cancer patients is emphasized. 2. The method of evaluation of this reaction in bioptic preparations is proposed and verified in seven series of carcinomas. The evaluation was directed to quantity, quality and topography of tumor-infiltrating cells. The method is recommended to routine practice for rapid performance, easy applicability, and reproducibility. 3. Cell reaction was evaluated by standard methods in carcinoma of the oral cavity, stomach, larynx, lung, breast, uterine cervix, and kidney. Individual parameters of cell reaction were confronted with clinical data on survival of patients with the aim to determine the possibilities and limits of interpretation of this reaction as a useful prognostic marker. 4. A special attention has been paid to the role of mast cells infiltrating tumors in the course of cancerogenesis in both experimental and human systems. The changes of individual parameters of reaction of tumor-infiltrating cells after regional chemotherapy were studied. Application of histochemical enzymatic reactions in identification of tumor-infiltrating cells is also reported. 5. The final chapter appraises further prospects of the investigation of tumor-infiltrating cells. The factors affecting the variability of this reaction, mechanisms of interactions between tumor-infiltrating cells and the tumor, and utilization of immunohistologic methods employing monoclonal antibodies are discussed.
