[Surgical strategy in oncologic diseases of organs of the pancreatobiliary zone complicated by mechanical jaundice].

In patients operated upon for oncological diseases of organs of the pancreatobiliary zone complicated by mechanical jaundice the lethality was decreased in a group of patients with purulent cholangitis and high biliary hypertension by means of using a two-stage method of operative treatment, use of the dosaged decompression of the bile ducts with a gradually increased diameter of the drain from 4 to 7-8 mm being most effective. A good effect of the application of complex antibacterial therapy was also noted when using a direct endolymphatic administration and intracholedochal perfusion of antibiotics. It allowed twice as quick to arrest the inflammatory process in the bile ducts, to normalize the biochemical indices of blood, and to prepare the patient for the main stage of operation.

[Effect of indirect endolymphatic antibiotic therapy on clinical and immunological indicators in patients with erysipelas of the lower extremities].

Two groups of patients with erysipelas of the lower extremities underwent indirect endolymphatic therapy with bicillin-3 (58 patients) and routine penicillin therapy (79 patients). Comparative clinicoimmunological examinations in the two groups revealed that lymphotropic administration of the antibiotic had a more favourable effect on the disease process, as evidenced by more rapid reverse time courses of erysipelas clinical signs, less frequent incidence of early relapses and normalization of the majority of immunological parameters by the end of the treatment. For estimation of the anti-recurrence efficacy of the antibiotic therapy in the patients with erysipelas, it was recommended to use a specific scale based on the principles of the Wald successive alternative analysis.

[The clinico-physiological validation of the use of the endolymphatic route for antibiotic administration for preventing the translocation of intestinal microflora during the treatment of sepsis patients].

One of the main mechanisms of generalization of infection in purulent-inflammatory processes of the abdominal cavity, retroperitoneal space and pelvis minor is translocation of the pathogenic microflora of the intestine in the lymphatic system via regional lymphatic nodes with subsequent rupture into the systemic circulation. Endolymphatic administration of antibacterial agents into the peripheral lymphatic vessel favours saturation of the reticuloendothelial structures of the lymph nodes by the antibiotic agent that increases their barrier function and, thus, prevents generalization of the infection and development of polyorganic insufficiency.

[Lymphotropic therapy for acute purulent odontogenic jaw periostitis].

The structure of the gingival mucosa was studied by optic microscopy in patients with acute purulent odontogenic maxillary periostitis treated traditionally and receiving lymphotropic therapy. Lymphotropic administration of the antibiotic during 2 days resulted in less pronounced dilatation of the interstitial spaces and lymph vessels adjacent to the molars and higher counts of lymphocytes, monocytes, and macrophages. This indicated high efficiency of lymphotropic therapy of acute purulent maxillary periostitis for molars. Microcirculation parameters and tissue leukocyte cytogram in gingival mucosal tissue adjacent to the canines and premolars differed negligibly in patients treated by different methods.

[Preparation of lymphatic targeting dosage form: pingyangmycin absorbed on activated carbon nanoparticles].

OBJECTIVE: To prepare anticancer nanoparticles for targeting therapy for oral cancer lymph node metastasis. METHODS: The activated carbon nanoparticles (CH-NP) were prepared for drug carrier. Pingyangmycin (PYM), a high sensitive anticancer drug for oral squamous cell carcinoma, were selected as model drug. The activated carbon nanoparticles and PYM were mixed with saline and shaken 20 minutes so that PYM was absorbed on activated carbon enough, resulting in a new formulation of PYM (PYM-CH-NP). The absorbency of PYM on activated carbon nanoparticles was evaluated. RESULTS: The diameter distribution for CH-NP ranged form 136 nm, to 540 nm, the average diameter was 176 nm. The proportion of CH-NP to PYM was increased and more absorbency of PYM on activated carbon nanoparticles was achieved. CONCLUSION: The activated carbon nanoparticles has high absorbency of PYM. The new formulation PYM-CH-NP can be used as targeting therapy of cervical lymph node metastasis by peri-cancer submucosal injection.

Locoregional administration of etoposide, but not of interleukin 2, facilitates active specific immunization in guinea pigs with advanced carcinoma.

Using a guinea pig line 10 hepatocellular carcinoma model for advanced metastatic disease, we studied the therapeutic effect of local cytotoxic drug treatment at the tumor site as compared to, and in combination with, active specific immunization. In addition, locoregional treatment with interleukin 2 (IL-2) was studied. Intratumoral administration of the cytotoxic drug etoposide (VP-16), but not of IL-2, when started in a late stage of tumor growth and continued for 3 wk, caused full regression of all intradermally implanted tumors and cured a small number of animals (14%). When the primary tumor was removed at the onset of treatment, administration of VP-16 and, to a lesser degree, IL-2 at the former tumor site led to improvement of cure rates (up to 30%). Complete cure always coincided with the induction of antitumor immunity. Since both VP-16 and IL-2, when locally administered, strongly augment T-cell-mediated immune responses, the observed therapeutic effect was partially attributed to potentiation of a T-cell-mediated antitumor response. Active specific immunization (ASI) using viable irradiated tumor cells admixed with Bacillus Calmette-Guérin also aims at induction of specific antitumor immunity. In late-stage disease, ASI alone induced cure rates of 39%. Combination of ASI with local cytotoxic drug treatment, but not with locoregional administration of IL-2 at the former primary tumor site, led to very high cure rates (up to 78%). Cured animals were always resistant to a second challenge with line 10 tumor cells. Routinely, one systemic injection with cyclophosphamide was given at the start of all treatment protocols. Omission of CY strongly reduced the cure rates obtained with ASI and locoregional VP-16 treatment. The high cure rates likely relate to the fact that locally administered cytotoxic drugs are capable of reversing immune tolerance, besides exerting direct antitumor action within tumor-draining lymphoid tissues. The present results therefore support our view that local cytotoxic drug treatment should be further explored for its incorporation in antitumor therapies such as ASI, aiming at maximal clinical benefit and minimal toxicities.

[Effect of preoperative lymphatic chemotherapy on Bcl-2 and Bax expression in axillary metastasis of breast cancer].

OBJECTIVE: To observe the expression of Bcl-2 and Bax proteins and cell apoptosis induced by preoperative lymphatic chemotherapy with epirubicin-activated carbon suspension (Epi-CH) in the cells of axillary metastatic lymph node of breast cancer and investigate the mechanism. METHODS: Sixty patients with breast cancer of stages II-III were randomly divided into two groups. Forty patients in Epi-CH group were injected with 10 mg Epi-CH in the tissue around the primary tumor or biopsy excision 72 h before operation. Twenty patients in the control group were injected with 10 mg epirubicin solution in the same region. The stained lymph nodes full of tumor cells in Epi-CH group and the non-stained nodes in the control group were selected for apoptotic detection by TUNEL method. The expression of Bcl-2 and Bax proteins were examined by SP immunohistochemistry. RESULTS: The apoptotic index of the metastatic cancer cells in Epi-CH group was increased remarkably in comparison with that in the control group [(9.5+/-2.7) % vs (3.8+/-1.4) %, P<0.01). Compared with the control group, the expression of Bcl-2 and Bax proteins were up-regulated significantly in Epi-CH group (P<0.05 and P<0.01, respectively), resulting in decreased ratio of Bcl-2/Bax. CONCLUSION: Lymphatic chemotherapy can promote cell apotosis in axillary metastasis of breast cancer, which may result from decreased ratio of Bcl-2/Bax.

[Impact of preoperative lymphatic chemotherapy on relapse and metastasis of breast cancer and its mechanism].

BACKGROUND & OBJECTIVE: Although it is reported that lymphatic chemotherapy could raise drug concentrations in local lymph nodes and prolong survival time of patients with gastrointestinal tumors, its effect on breast cancer has not been explored. This study was to explore the impact of lymphatic chemotherapy on relapse and metastasis of breast cancer, and to investigate the mechanism. METHODS: Sixty patients with breast cancer of stage II-III were randomized into 2 groups: 40 patients in Epi-CH (carbon activated absorbing epirubicin) group were injected with 10 mg of Epi-CH in the tissue around primary tumor 72 h before modified radical resection; 20 patients in control group were injected with 10 mg of aqueous epirubicin in the same region. The stained nodes full of tumor cells in Epi-CH group and non-stained nodes in control group were selected. The apoptotic index (AI) of cancer cells in metastatic axillary lymph node was calculated by TUNEL method; the expression of Fas/Fas-L proteins was examined by SP immunohistochemistry; the relapse and metastatic rate was compared. RESULTS: The AI of cancer cells in metastatic axillary lymph node was significantly higher in Epi-CH group than in control group [(9.5+/-2.7)% vs. (3.8+/-1.4)%, P<0.01]. The expression of Fas protein was significantly higher in Epi-CH group than in control group (P<0.05), but the expression of Fas-L protein had no difference between the 2 groups (P>0.05). No chemotherapy-related local and whole body reaction occurred in both groups. The relapse and metastatic rate was significantly lower in Epi-CH group than in control group (P<0.05). CONCLUSION: Preoperative Epi-CH lymphatic chemotherapy could suppress relapse and metastasis of breast cancer, which might through up-regulating expression of Fas protein and inducing apoptosis of axillary metastasis cells.

[The level of cyclic nucleotides and the activity of their phosphodiesterases in the peripheral blood lymphocytes in lung diseases during endolymphatic antibiotic therapy].

Fifty patients with chronic bronchitis, 30 with lingering pneumonias and 60 with lung abscesses were examined. The status of the system of cyclic nucleotides and functional properties of T lymphocytes depending on different administrations of antibiotics into the patients' body (endolymphatically or intravenously) were studied over time. The degree of the recovery to normal of the functional status of the T system immunity and cyclic nucleotides turned out significantly higher in patients receiving antibiotics endolymphatically than in those given intravenous injections. This attests to the high efficacy and advantages of endolymphatic antibiotic therapy in patients with inflammation in the lungs and bronchi.

[Characterization of an experimental model of monosodium- glutamate-induced convulsions in the amphibian Bufo spp].

In previous reports, Feria-Velasco et al. characterized an experimental model of convulsions in rats induced by monosodium glutamate (MSG) with evaluation of the motor behavior and neurochemical parameters. In the present work, MSG injected in toads (Bufo spp) reproduced the model of convulsions with some peculiarities. The electrocorticographic recordings in toads after MSG injection were similar to those obtained in rats after administration of convulsant agents. Most of the toads injected with MSG (81.8%) showed convulsions preceded by an episode of stereotyped movements and signs of hyperexcitability. Latency for convulsions and frequency of convulsive episodes were similar to what has been reported in rats injected with MSG. However, the duration of convulsive period was larger than that seen in rats, and no deaths were recorded in toads. The peculiar feature of amphibia regarding their cerebral structures and their blood-brain barrier (BBB) make the amphibian model, an interesting and valuable one in studying experimentally induced convulsions, as well as the role of BBB in these phenomena.

[Complex therapy of acute and exacerbated chronic nonspecific parotitis including mexidol and non-coherent infrared therapy].

The task in view to reveal efficiency of complex application mexidol and in coherent infra-red therapy of inflammatory diseases of salivary glands. On the basis of clinical, laboratory and functional methods it is revealed, that combined application of mexidol and in coherent infra-red therapy leads to clinical effect that is expressed in reduction of terms of treatment an increase the terms of remission.
