Delayed hypersensitivity response to Dinitrochlorobenzine (DNCB), Purified Protein Derivative (PPD) and Candida albicans abstract (Dermatophytin 'O') was observed in 60 patients of squamous cell carcinoma of the oral cavity and 40 normal controls. The response to all these three antigens were significantly impaired in cancer patients as compared to normal controls. PPD and Dermatophytin 'O' response showed progressive decline with increasing tumour burden. Patients with well differentiated tumours responded well to all these three antigens as compared to those with poorly differentiated tumours.
The therapeutic approach to intransit metastasis of melanoma has been surveyed in 52 patients admitted to this institute during the past ten years with a clinical history of intransit lesions of melanoma. Ten of these patients have been long term survivors. Two additional patients died at 23 and 28 months, respectively, after surgical removal of the lesions, and at autopsy, no evidence of recurrence was noted. All long term survivors belong to the group of 35 patients that received intensive local management, while all 17 patients given systemic chemotherapy or immunotherapy only died from progression of the disease. Every trial of system chemotherapy or system immunotherapy, aiming to control these lesions of intransit metastasis, failed. Systemic chemotherapy or systemic immunotherapy should not be used alone in the treatment of intransit metastasis. Such regional modalities as hyperthermic perfusion, local immunotherapy or excisions of wide strips of skin, subcutaneous fat and fascia around these lesions, in addition to systemic treatment, should be utilized persistently.
The biological classification system provides a useful clinical distinction between proliferative and biologically stable vascular anomalies. Hemangiomas demonstrate the ability to proliferate, then involute, and the majority still are followed conservatively. Intervention usually is reserved for life- or vision-threatening lesions. Surgical reconstruction may be indicated for the 10% to 30% of hemangiomas that do not regress adequately. The use of lasers to treat early hemangiomas is controversial but may gain greater clinical application as favorable data accumulate. In the future, biological suppression with antiangiogenic agents may become the preferred mode of therapy. Vascular malformations represent developmental errors in embryogenesis. They are present at birth, do not proliferate, and do not regress spontaneously. Lasers, interventional radiology, and surgical therapy may offer significant palliation and possible cures. It is important that the physician understand the difference between these two groups to better provide appropriate prognostic and therapeutic advice to the parents of children with these anomalies.
Squamous cell carcinoma of the nail bed is a rare disorder. Metastasis is extremely unusual. A case of primary squamous cell carcinoma of the nail bed with metastasis to the regional lymph nodes is reported. The tumor was successfully treated by amputation of the involved finger, block dissection of the regional lymph nodes, postoperative irradiation of the involved axilla, and systemic chemotherapy. Early diagnosis by biopsy, especially in patients with recurrent and persistent disease of the nail bed, is recommended, so that treatment can be instituted early while the tumor is confined to its primary site.
A group of 79 patients with soft tissue sarcomas and 46 with primary malignant bone tumors underwent lymphangiography as part of the initial diagnostic workup. In the group of soft tissue sarcomas, the overall incidence of metastases was 22 of 79 (28%). According to the site of origin, the highest incidence was found in tumors originating from the buttocks (57%), followed by those from the inferior limbs (31%). Considering the histology, the incidence ranges from 50% in anaplastic sarcoma, to 43% in rhabdomyosarcoma, to 23% for liposarcoma and fibrosarcoma. The incidence of lymphographically proven metastases in bone tumors was 8 to 46 (17%), with 1 out of 4 in reticulum cell sarcoma, 1/1 in chordoma, and 22% in Ewing's sarcoma. A radiographic/histologic correlation on the lymph nodes was obtained in 19/79 (24%) soft tissue sarcomas and in 4/46 (9%) bone tumors. In the 12 radiographically negative and 11 radiographically positive cases, this correlation was always correct. The results of this study suggest a larger use of lymphography in these tumors.
PURPOSE: To evaluate the accuracy of positron emission tomography (PET) with 2-deoxy-2-[fluorine-18] fluoro-D-glucose (FDG) in the detection of osseous and soft-tissue metastases of prostate cancer. MATERIALS AND METHODS: Thirty-four patients (mean age, 71 years) with biopsy-proved prostate cancer and known or suspected metastatic disease were examined. Blinded interpretation of the PET images was compared with bone scan, CT, and clinical follow-up findings. RESULTS: In 202 untreated osseous metastases in 22 patients, the sensitivity of FDG PET was 65% (131 of 202 metastases), with a positive predictive value of 98% (131 of 133 positive findings). The estimated standardized uptake value in metastases was 2.1-5.7. Soft-tissue metastases to the lymph nodes or liver were identified, but evaluation of pelvic lymph node metastases was severely limited because of bladder tracer activity. CONCLUSION: FDG PET can help identify osseous and soft-tissue metastases of prostate cancer with a high positive predictive value but is less sensitive than bone scintigraphy in the identification of osseous metastases.
BACKGROUND: Matrix metalloproteinases (MMPs) are implicated in several pathological conditions, including cancer invasion and metastases. Among the MMPs associated with the tumor cell surface, gelatinase A (MMP2) is believed to be particularly important for cancer invasion. MATERIAL/METHODS: The immunoexpression of matrix metalloproteinase MMP2 and its inhibitor TIMP2 was investigated in 40 laryngeal carcinomas, in order to estimate the prognostic significance of these factors. The tissue sections were studied morphometrically using computer image analysis. RESULTS: The mean value of MMP2 immunoexpression in squamous cell carcinoma in patients without lymph node involvement was 7.36I7.3, and the mean value of MMP2 immunostaining in patients with lymph node metastases was 23.24I14.9. The difference between groups was statistically significant (P<0.001). Statistical analysis did not reveal a significant difference in TIMP2 expression between groups with metastases (mean: 11.38I7.7) and without (mean: 11.47I9.1). The survival time of patients with tumors with higher MMP2 expression (greater than the median, 9.6) was significantly shorter (P<0.004) than those with tumors showing low MMP2 expression (<9.6). The survival analysis revealed no significant difference between the survival time of patients in groups with high (> the median, 10.6) and low (<10.6) expression of TIMP2. CONCLUSIONS: Our results suggest that MMP2 expression may be of prognostic importance in laryngeal cancer. However, imbalances in the extracellular activities of matrix metalloproteinases and tissue inhibitors of matrix metalloproteinases, linked to pathological tissue destruction, are more complex and need further investigation.
PURPOSE: Enhanced cisplatin (Pt) retention using transcatheter arterial chemoembolization (TAE) with Gelfoam particles was studied in rabbit uterine tumors. METHODS: Ten rabbit uteri were inoculated with 5 x 10(7) cells of VX2 carcinoma. Three to four weeks later cisplatin, 1 mg/kg, was injected, either with (TAE group) or without (IA group) being mixed with small Gelfoam particles, into the aortic bifurcation over 5 s. Blood and tissue concentration of cisplatin were determined. RESULTS: Slower arterial blood clearance of Pt was observed in the TAE group compared with the IA group, whereas the venous blood Pt clearance curves were similar for both groups. The uterine tumor Pt concentration at 80 min was found to be 2.52-fold higher after TAE compared with IA (p < 0.01). In the pelvic metastatic lymph nodes, the Pt concentration was 4.63 times higher after TAE than after IA (p < 0.01). CONCLUSION: These data indicate that TAE is an effective means of increasing tissue concentration in uterine tumors.

Material comprising 569 cancer patients in clinical stages I-III is presented; recurrence, factors influencing recurrence and survival after recurrence have been studied. The 10-year recurrence rate for the whole material was 52%. Frequency of recurrence correlated with the clinical stage, the axillary biopsy finding and with the malignancy grade of infiltrating ductogenic carcinoma. The incidence ratio of local recurrence and distant metastases was 1:2; the most common site of the former being the chest wall and of the latter the viscera. After diagnosis of the first local recurrence, subsequent 5- and 10-year survival rates were 42% and 24% and after diagnosis of the first distant metastasis correspondingly 15% and 13%. Survival after the detection of axillary and supraclavicular metastases was better than after the detection of chest wall recurrences. Prognosis for distant metastases was best for soft tissue metastases and poorest for visceral and multiple metastases.
Tumors of endothelial cell origin are relatively common. Soft tissue tumors and numerous subtypes of benign and malignant vascular tumors have been described; the histogenesis of many of these tumors is uncertain, and distinguishing between benign and malignant vascular tumors, some of which express lymphatic endothelial cell markers, can be problematic. In the present study, immunophenotypic expression of a novel hyaluronan receptor (LYVE-1), which is expressed by endothelial cells of normal lymphatic vessels but not blood vessels, was determined in benign and malignant vascular tumors. It was found that, except in lymphangiomas, intramuscular hemangiomas, and Masson's hemangiomas, endothelial cells in benign blood vessel tumors (including capillary and cavernous hemangiomas, glomus tumors, pyogenic granulomas, and epithelioid hemangiomas) were negative for LYVE-1, and that all angiosarcomas and Kaposi's sarcomas were positive for LYVE-1. Expression of LYVE-1 and other lymphatic endothelial cell markers in relatively few vascular neoplasms has implications for the histogenesis of these lesions, and may prove useful in distinguishing angiosarcoma and Kaposi's sarcoma from most common benign vascular tumors.
We measured antigen levels of 2 kinds of plasminogen activator, tissue-type plasminogen activator (t-PA) and urokinase-type plasminogen activator (UK), as well as those of their primary inhibitors, type-I plasminogen activator inhibitor (PAI-1) and type-2 plasminogen activator inhibitor (PAI-2), in tissue extracts from benign and malignant breast tumors. Tumor tissue samples from 40 fibroadenomas and 40 breast cancers were examined. t-PA antigen levels were the same in the 2 groups. Malignant tumors contained higher levels of UK antigen than did benign tumors. In the case of breast cancer, UK antigen levels of tumors with axillary lymph-node involvement were significantly higher than those of tumors without lymph-node involvement. PAI-1 and PAI-2 antigen levels of breast-cancer tissue samples were higher than those of fibroadenoma samples. PAI-1 antigen levels of carcinomas with lymph-node involvement were also significantly higher than those of carcinomas without node involvement. PAI-2 antigen levels, on the contrary, were higher in carcinomas without node involvement. UK, PAI-1 and PAI-2 antigen levels are potentially excellent independent factors for prediction of the metastatic potential of breast cancers.
