[Some thoughts on the adaptation of technical aids for children with cerebral palsy (author's transl)].

The technical aids used by children undergoing inpatient treatment at a centre for the cerebral palsied were critically assessed over a one-year period. Of the evaluated technical aids 50% were optimal for the child under consideration, the remainder was inappropriate for one or more reasons. The most frequent amongst the non-optimal were either not medically prescribed or adequately adapted and tested. The children in question are hence threatened with unnecessary deformations, an insufficient utilisation of their development potential and a lower degree of independence. Also, the financially responsible agencies have to cope with considerably larger expenditures.

Aids for speech-handicapped.

An improved electrolarynx is described. It provides handcontrolled fundamental frequency which enables the user to approximate natural intonation patterns, thus overcoming the monotony of speech with conventional aids. Training experiments with normal and with laryngectomized subjects have given promising results. A voice amplifier has been developed. Special care has been taken to minimize acoustic feedback problems. It is used for compensation of any weak voice, including esophageal speech and as a supplement to the electrolarynx for increasing the sound output and improving the signal-to-noise ratio. The use of synthetic speech in aids for anarthric patients is discussed.

Cosmetic, functional, independent: self-help aids.

Self-help orthotics have been developed to assist functional activities for high-level spinal cord injured patients. The goal of rehabilitation has been to do this with as little intrusion on independence and cosmesis as possible. This paper describes devices to help a patient with C6 quadriplegia independently place his own utensils and eat with an almost normal appearance. The new self-help aids are made from standard silverware, a rivet, and two one-eighth-inch Kydex loops. The loops are riveted centrally on the utensil, allowing for free rotation. The device was evaluated on a patient with C6, complete quadriplegia. The subject demonstrated independence in donning and doffing the device and in eating with an almost normal appearance.

["Mobile occupational therapy" for rheumatic patients: development of the service, utilization and management with aids and braces].

The development and utilization of occupational therapy as a home care service for patients with rheumatic diseases is examined. This new service was implemented in Schleswig-Holstein by local branch of the German League against Rheumatism, an organization devoted to the promotion of help and self-help for patients. The data for this study are based on standardized documentations within a comprehensive evaluation of model-intervention, and also accounting records. From July 1, 1986 until June 31, 1991, there were 1120 prescriptions issued by 293 physicians at 86 locations. 529 patients could be served with a total amount of 6778 time-units of care. The development of the service shows a slowly but steady and region wide growing acceptance by the practitioners and, accordingly, an increasing utilization associated with an increasing inclusion of non-rheumatic diagnoses. The supplies of support devices (included in 55% of all prescriptions) and braces (included in 26% of all prescriptions) are described separately, since patient counseling and assistance--particularly in connection with support devices and aids for everyday life--are of great importance.

[Treatment of fractures of the neck and trochanteric regions of the femur in middle-aged and aged persons].

To obviate the necessity of a continuous plaster dressing immobilization and to provide early mobility of a patient, it is believed to be rational to perform osteosynthesis of the femoral neck and acetabulum in their fractures without displacement. It is felt that the whole period of therapy in aged persons should be managed in collaboration with a physician. According to the author's data an application of functional devices during the process of rehabilitation would allow an improved management and self-help in this category of patients.

[Computer-assisted speech training for aphasic patients--STACH and WEGE--in a self-help group].

Reported is our experience with two computer programmes for aphasic people (language loss after stroke). The STACH and WEGE programmes combine data banks of written language, pictures, and auditory speech output (delta modulation/Audicard 300 E by Speech Design, Munich) using MS-DOS. Special attention is given to the social environment, the patients' families and the context of self-help groups, who show great interest in this new facility for speech-training.
