Family adjustment following disclosure of homosexuality by a member: themes discerned in narrative accounts.

Disclosure of homosexuality by a family member frequently creates a crisis within the family. Previous research has focused primarily on families' response to this initial crisis and how they cope with and transform initial negative reactions. Little attention has been given to how families respond to disclosure over time and to how families integrate a gay or lesbian family member--and his or her relationships--once they have come to accept his or her homosexuality. Using a narrative approach and interviewing whole families, this project explores these questions. We present 12 themes we found consistently recurring in family members' narratives. We discuss the relationship between these themes and current models in the literature within the context of heterosexism and suggest that this approach may represent a first step in developing a model of successful adaptation subsequent to disclosure.

Patients' knowledge of their condition and treatment: how it might be improved.

A study was carried out in which patients were asked questions to assess their knowledge of their condition and treatment, the questions relating particularly to care before siting an intravenous infusion and preoperative care. A high proportion of patients knew little about the procedure they were about to undergo. It is recommended that patients should be told more about matters relating to their condition and treatment. Written information could be given to supplement that given verbally by staff. A check list might be drawn up to ensure that patients receive all relevant information.

The disclosure of the diagnosis of cancer.

In summary, the consensus today is that full disclosure of the diagnosis of cancer to competent individuals (including children) is morally, ethically, legally, and therapeutically the appropriate policy. Unfortunately, this task is one that most physicians still find awkward, in part because of the continued de-emphasis of "soft" clinical skills in the medical education. Thus, doctors have little opportunity to address their own beliefs and emotions. By being cognizant of clinician unease and using well-described communication skills, however, disclosure can be satisfactorily accomplished. As American health care becomes increasingly ambulatory in nature, primary care physicians will have even more need to hone this skill.

HIV testing: an ethical analysis.

As the epidemic of the human immunodeficiency virus continues to grow, the relationship between patients and health care workers is being tainted with fear and mistrust. The demand for mandatory testing of health care providers has caused many of us to assess our basic values of autonomy, beneficence, nonmaleficence, and justice. Is our "right to privacy" more important than our "right to know"? This article will analyze some of the legal, moral, and ethical issues to be considered in identifying possible alternatives and a justified solution to this dilemma.
