Pancreatic tuberculosis.

Pancreatic tuberculosis is a rare clinical entity. It most often is associated with miliary tuberculosis or occurs in the immunocompromised patient and is extremely uncommon in nonendemic geographic regions. Described is a case of isolated infection of the pancreas by Mycobacterium tuberculosis occurring in an otherwise healthy individual from North America who presented with recurrent hyperamylasemia and fevers. Computerized tomography revealed a multiloculated lesion in the head of the pancreas. Intraoperative biopsies demonstrated granulomas. The diagnosis was confirmed by positive cultures. Triple drug therapy was effective in alleviating symptoms, and a follow up CT revealed a normal pancreas. One year following operation the patient remains well.

Tuberculous pancreatic abscess: an unusual manifestation of HIV infection.

Pancreatic tuberculosis is unusual occurring in only 2.7% of autopsy studies of persons with miliary disease. Reports of focal pancreatic tuberculosis are rare, even in patients with the acquired immunodeficiency syndrome (AIDS) in whom abdominal tuberculosis is a frequent feature. We describe two patients infected with the human immunodeficiency virus (HIV) who developed tuberculous pancreatic abscesses. In both this was their AIDS-defining illness.

AIDS-Related Opportunistic Infections↗

Addisonian crisis following pericardiectomy.

In developing countries tuberculosis is a common cause of constrictive pericarditis and its involvement of other organs may sometimes complicate the surgical management of such patients. A case is reported of constrictive pericarditis in which the patient developed protracted hypotension which was unresponsive to inotropic therapy, following pericardiectomy. On investigation the cause of hypotension was diagnosed as acute adrenocortical insufficiency, secondary to adrenal tuberculosis. The patient was successfully treated with corticosteroids. Low cardiac output after pericardiectomy, which is not uncommon, should not necessarily be attributed to a 'myocardial factor'. Patients with tuberculous pericarditis may have co-existent Addison's disease.

[A case of tuberculosis of the thyroid gland].

A 49-year-old woman was admitted to our university hospital for examination of a neck mass. Computed tomography and ultrasonography revealed a nodule in her thyroid gland and a swollen cervical lymph node. They were removed surgically; a subsequent histological study revealed an epithelioid cell granuloma. A diagnosis of localized tuberculosis of the thyroid gland and a cervical lymph node was thus confirmed. The data of the present case were discussed with reference to the characteristics of thyroid tuberculosis described in the literature.

Female↗

[Tuberculosis of the pancreatic region--sonographic and CT findings].

In the case reports of two black men of 29 and 32 years of age the rare condition of tuberculous involvement of the pancreas is described. The diagnosis was established by exploratory laparotomy and by the culture of mycobacterium tuberculosis. Ultrasonography demonstrated inhomogeneous, hypoechoic masses in and around the pancreas. Contrast-enhanced computed tomography of the abdomen revealed enlargement of the pancreas and the presence of lesions of decreased attenuation or cystic collections with involvement of the peripancreatic area.

Adult↗

[Tuberculosis of the breast].

Tuberculosis of the breast is a rare modern disease or a forgotten diagnosis. During the last 15 years (1982-1996), 28 cases of mammary tuberculosis were reported in Japan. The initial diagnosis of almost all were carcinoma. Physical sign, mammography and USG are not so usefull for the diagnosis. It is important for the early and certain diagnosis to detect acid-fast bacilli on smear or culture and to perform pathological examination on biopsied specimen.

Association of precipitating anti-adrenal anti-adrenal antibodies with moniliasis-polyendocrinopathy syndrome.

The association of precipitating anti-adrenal antibodies with different subgroups of idiopathic Addison's disease were studied. We had previously found these antibodies in patients with the moniliasis-polyendocrinopathy syndrome. Sera of 36 adult patients suffering from different froms of Addison's disease were examined for the presence of adrenal antibodies demonstrable either by immunofluorescence (IFL) or by gel diffusion. 3 of the 17 patients with tuberculous and 17 of 19 patients with idiopathic Addison's disease had IFL antibodies but only one had precipitating antibodies. There was one typical case of Schmidt's syndrome, and four additional cases with Addison's disease combined with diabetes or thyroiditis, who may later develop the syndrome. None of htese patients had precipitating anti-adrenal antibodies. The only patients with precipitating adrenal antibodies had the moniliasis-polyendocrinopathy syndrome. He was not typical as Addison' disease appeared unusually late and he did not have hypoparathyroidism. The presence of precipitating anti-adrenal antibodies in this patient, and the absence of these in other groups of Addison's disease, is further evidence for the association of precipitating antibodies with the moniliasis-polyendocrinopathy syndrome.
