[The adaptation of the blood circulation to the long-term microgravity: ballistic-cardiographic investigation during 14 month space flight].

Specificity of the use of ballistocardiography (BCG) in space flight is considered. In particular, the contactless method makes it possible to continue recording during the night hours and study the ultradian rhythms reflective of the state of higher autonomous centers. As was stated, on months 7 to 8 of the mission by cosmonaut-physician Polyakov the BCG amplitude rose in concert with activation of the higher autonomous centers that were apparently involved in maintaining the established cardiovascular homeostasis. By months 11 to 13 on mission, the BCG amplitude decline resulted in an increase in the SCG/BCG index representing the amount of myocardium energy spent on blood transport.

Entropy is more resistant to artifacts than bispectral index in brain-dead organ donors.

OBJECTIVE: To evaluate the usefulness of entropy and the bispectral index (BIS) in brain-dead subjects. DESIGN AND SETTING: A prospective, open, nonselective, observational study in the university hospital. PATIENTS AND PARTICIPANTS: 16 brain-dead organ donors. INTERVENTIONS: Time-domain electroencephalography (EEG), spectral entropy of the EEG, and BIS were recorded during solid organ harvest. MEASUREMENTS AND RESULTS: State entropy differed significantly from 0 (isoelectric EEG) 28%, response entropy 29%, and BIS 68% of the total recorded time. The median values during the operation were state entropy 0.0, response entropy 0.0, and BIS 3.0. In four of 16 organ donors studied the EEG was not isoelectric, and nonreactive rhythmic activity was noted in time-domain EEG. After excluding the results from subjects with persistent residual EEG activity state entropy, response entropy, and BIS values differed from zero 17%, 18%, and 62% of the recorded time, respectively. Median values were 0.0, 0.0, and 2.0 for state entropy, response entropy, and BIS, respectively. The highest index values in entropy and BIS monitoring were recorded without neuromuscular blockade. The main sources of artifacts were electrocauterization, 50-Hz artifact, handling of the donor, ballistocardiography, electromyography, and electrocardiography. CONCLUSION: Both entropy and BIS showed nonzero values due to artifacts after brain death diagnosis. BIS was more liable to artifacts than entropy. Neither of these indices are diagnostic tools, and care should be taken when interpreting EEG and EEG-derived indices in the evaluation of brain death.

TERVA: system for long-term monitoring of wellness at home.

Long-term monitoring of physiological and psychosocial variables in out-hospital conditions would be beneficial for investigating changes in wellness status of an individual or to understand interaction between physiological and behavioral processes. We aimed to design a personal wellness monitoring system (TERVA), which would allow monitoring of wellness-related variables at home for several weeks or even months. The designed TERVA system runs on a laptop computer and interfaces with different measurement devices through a serial interface. Measured variables include beat-to-beat heart rate, motor activity, blood pressure, weight, body temperature, respiration, ballistocardiography, movements, and sleep stages. In addition, self-assessments of daily well-being and activities are stored by keeping a behavioral diary. To test the system, one healthy man used the system for 10 weeks. The system was successfully applied in out-hospital conditions. The success rate of the measurements was 70-91%, depending on the variable under consideration. The pilot study indicated that the recorded data accurately reflected the health status of the subject. The TERVA system provides a method to record and investigate wellness-related data over several weeks, or even months, outside the hospital among subjects capable of using a personal computer. Several applications of the system are discussed.

Focal epileptiform activity in the brain: detection with spike-related functional MR imaging--preliminary results.

The purpose of this study was to develop a spike-related functional magnetic resonance (MR) imaging method to detect epileptic brain activity. Correlations between simultaneous spike-related functional MR imaging and electroencephalographic (EEG) recordings were performed in 10 patients with focal epilepsy. Postprocessing techniques were implemented to eliminate contamination of the EEG recording from ballistocardiography and the echo-planar MR imaging sequence. A diagnostic EEG recording was achieved during functional MR imaging. Spike location correlated with the site of blood oxygen level-dependent signal increase. Spike-related functional MR imaging is a promising technique for detecting focal epileptic brain activity.

[The development of cardiology in the last 40 years].

In order to correctly appreciate the way cardiology developed in the last 40 years, the "state-of-the-art" balance when the first World Congress took place, September-1950, in Paris, is made. The most current diagnostic methods were relatively scarce at the time-ECG, chest X ray, phonocardiography, pulse wave tracings, coupled with vectorcardiography and ballistocardiography, but in the well equipped hospitals right ventricular catheterization was already performed. The therapeutics of the important morbid situations-like congestive heart failure, arterial hypertension, myocardial infarction and ischemia was disappointing, but closed heart surgery was already taking place. Since then, things had changed suddenly. Over the last 40 years a marked scientific and technology explosion has emerged, that had benefited cardiology science, encompassing several broad areas, namely, patient approaching, pathophysiology understanding, the emergence of new drugs and clinical use, interventional cardiology and remarkable progress in open heart surgery. Imaging techniques development and other technology lead to a simple and accurate cardiac diagnosis. 24 hours ECG and blood pressure recording, and cardiac output measurement, greatly improves our medical knowledge. Computers development, representing the most significant technology advances, has given to cardiology non-predicted advances.(ABSTRACT TRUNCATED AT 250 WORDS)

[Past and present medical and biological studies in Italy on microgravity].

Since 1957, Italian workers have been carrying out experiments on the physiological and pathological effects of the weightlessness encountered during space travel. Most of this work has been done at the Air Force Aviation Medicine Study and Research Centre, Rome, while other research has been conducted at the Department of Human Physiology, University of Milan. Various original devices (axis subgravitis tower, triaxial clinostats, etc.) have been devised and constructed to simulate partial and total weightlessness. They have been employed well ahead of actual space flights in investigations of the effects of weightlessness on vestibular function, sensory and muscle coordination, the position of the diaphragm and its adjacent viscera, locomotion, plant growth, rapid adaptation phenomena, etc. One experiment conducted during orbital flight was called Orbiting Frog Otolith. This investigated vestibular function under statis and dynamic conditions in the absence of weight. An Italian three-dimensional ballistocardiography experimental project will be conducted during the first Spacelab flight.
