[Surgery of hydatid cysts of the liver: 581 patients, 952 cysts].

Over a period of 20 years, surgery was carried out in 581 patients for 952 hydatid cysts of the liver. Patients included 372 females and 209 males, aged from 6 to 70 years, with a mean age of 34 years. 52% of patients were aged between 20 and 40 years. The cyst was single in 324 patients, double in 102, triple in 22 and multiple in 133. Treatment most often consisted of resection of the projecting dome of the cyst (78%), and more rarely pericystectomy (5.5%), hepatic resection (3.5%) or external drainage (4.2%) via a median incision in 412 cases and a subcostal incision in 126 cases. Capitonnage of the residual cavity was combined with resection of the projecting dome, 176 cases included a cystobiliary fistula with migration occurring in 30 cases. Morbidity was 20% and included, in particular, 45 cases of subphrenic abscess. 19 patients showed signs of recurrence and 16 underwent further surgery. Overall mortality was 2.06% i.e. 12 patients. In endemic countries, resection of the projecting dome remains a method which gives good results and has a low mortality and morbidity.

Dermoid cyst of the tongue: an association of dermoid cyst with bronchogenic epithelium.

A congenital dermoid together with a bronchogenic cyst of the tongue is extremely rare. The diagnosis made in this case of a 1-year-old boy was "teratoid cyst of the tongue". A surgical exploration was performed under general anaesthesia via a midline sagittal glossotomy. The tumour was completely dissected and excised, the microscopical examination of the surgical specimen revealed multiple cystic cavities lined by a keratinized squamous epithelium with skin appendages and fatty tissue. Others were composed of cylindrical, ciliated epithelial cells of respiratory type. This is the third reported case in the world literature. All cases were reviewed and compared with this case.

[Aneurysmal bone cyst and solitary cyst of bone].

Results of treatment of 7 cases of aneurysmal bone cysts and 12 cases of solitary bone cysts in 19 patients treated in our Department between 1991 and 1996 are presented. Changes were found in the humerus, femur, tibia and fibula. Curettage or excision was the mode of treatment. Recurrence of the condition has been noted in 4 cases. Excision of the change and filling of the defect with autologous or homologous bone graft is more efficient than curettage alone.

[Placental protein 12 (PP 12) in cyst and peritoneal fluids and serum samples of patients with ovarian cysts and cystic ovarian tumors].

Placental Protein 12 (PP 12) concentrations were measured by a radioimmunoassay in cyst and peritoneal fluids and in serum samples respectively of patients with tumor like conditions and benign and malignant cystic ovarian tumours. PP 12 can not recommended as a serum tumour marker in ovarian cancer. It must be proved the value of PP 12 in follow up of ovarian cancer with help of more extensive sample material. There are many cysts in tumour like conditions with measurable PP 12-concentrations.

Serous cysts of the aging guinea pig ovary. III. Permeability of the cyst epithelium to lanthanum and horseradish peroxidase.

The permeability of the epithelium lining serous cysts of the guinea pig ovary was examined using lanthanum and horseradish peroxidase (HRP). Both lanthanum and HRP introduced at the luminal surface of the cyst penetrated to the basal region filling caveolae in both lateral and basal cell surfaces. Within three minutes of vascular infusion of HRP, the tracer was detected between epithelial cells and in caveolae on their lateral and basal surfaces but not associated with intracellular organelles. There was no change in the intracellular distribution of HRP after ten minutes. It was concluded that the epithelium was permeable to the tracers within this time period but that pinocytosis and transport of these tracers through the epithelial cell were not demonstrated.

Multiple pineal cysts associated with an ependymal cyst presenting with infantile spasm.

We present a unique case of infantile spasm in a patient with multiple pineal cysts associated with an ependymal cyst. Such a morphological etiology, to our knowledge, has not been described for patients with infantile spasm. Although postoperative neuroradiological studies demonstrated satisfactory surgical results, the patient continued to have seizures.

Functional mediastinal parathyroid cyst. Dynamics of parathyroid hormone secretion during cyst aspirations and surgery.

A patient presented with a large neck mass, hypercalcemia, and elevated serum parathyroid hormone levels. Aspiration of the cystic mass yielded 100 ml of fluid that contained a low concentration of thyroxine, but large amounts of parathyroid hormone, as measured by bioassay or with three different radioimmunoassays. After each of four aspirations, the serum calcium level declined significantly. Serial measurements showed that serum amino-terminal and mid-region parathyroid hormone levels and urinary cyclic adenosine monophosphate values declined after aspiration. Surgical resection of the cervical-mediastinal cyst restored normal serum calcium and parathyroid hormone levels, and these have been maintained for nine months. Calculations suggest that parathyroid hormone traversing the cyst lumen might contribute significantly to the excess circulating parathyroid hormone.

Traumatic bone cyst, aneurysmal bone cyst, and central giant cell granuloma--pathogenetically related lesions?

Three central bone lesions, a traumatic bone cyst, an aneurysmal bone cyst, and a central giant cell granuloma are reported. Comparisons of the clinical, radiographic, and histological features were made. Although these lesions are recognized as separate and distinct pathological entities, a common pathogenesis is suggested by similarities in certain features. Diagnostic aids are suggested to assist in a differential diagnosis.

Neuroectodermal cyst may be a rare differential diagnosis of fetal sacrococcygeal teratoma: first case report of a prenatally observed neuroectodermal cyst.

In spite of the fact that most anomalies can be accurately diagnosed prenatally, one may be confronted with an extremely rare fetal abnormality that has never been described before. We report a case of a neuroectodermal cyst arising from the posterior neuropore. After 21 weeks of gestation a fetal anechoic intra- and extra-abdominal tumor was detected. The mass grew to 75 x 41 x 33 mm at 35 weeks. Initially it was believed to be a sacrococcygeal teratoma. Serial scans and Doppler ultrasound examinations were performed, which demonstrated fetal well-being. Color Doppler imaging failed to demonstrate increased tumoral perfusion. After elective Cesarean section, the tumor was excised. The postoperative course was complicated by recurrent infections of the urinary tract due to neurological damage to the bladder. There was also impaired function of the anal sphincter. The histological finding of a monolayer of neuroepithelial cells and melanocytes led to the diagnosis of a neuroectodermal cyst.

Heterotopic gastrointestinal cyst partially lined with dermoid cyst epithelium.

We report a rare heterotopic gastrointestinal cyst located in the right submandibular/submental area with histopathologic features that included portions resembling a dermoid cyst. Some theories of pathogenesis are discussed, and an origin of this lesion in entrapped undifferentiated endodermal cells is suggested.

Mullerian duct cyst (utricular cyst): treatment with the transvesical, transtrigonal approach.

The diagnosis of mullerian duct or utricular cyst should be considered in a child with urinary difficulties and a palpable midline, anterior rectal mass. Endoscopic cannulation of the cyst has been found to be the most useful diagnostic test. Infection should be treated with the appropriate antibiotics before definitive treatment. Surgical excision offers the best result. The transperitoneal and posterior parasacral approaches have been described but we favour the transvesical, transtrigonal approach which we find highly satisfactory. The risk of malignancy at a later age is an added indication for surgery.
