[Cervico-encephalic vascular ultrasonography Doppler examination and ophthalmoscopy (author's transl)].

99 non diabetic patients whose ocular examination showed characterized retinal arteriolosclerosis, with or without arterial hypertension, or macular alterations (senile degeneration, non aphakic retinal-vitreous interlayer syndrome, colloid degeneration) have been submitted to an cervico-encephalic vascular ultrasonographic Doppler examination. 25 to 56% revealed latent cerebral arteriopathy sometimes with arterial stenoses, peculiarly carotidal. Other 58, apparently healthy and likewise aged, tested in the same way, revealed only 12% notable arteriopathy.

Roth spots seen on ophthalmoscopy: diseases with which they may be associated.

Roth spots seen on fundoscopic examination (white-centered hemorrhages) were classically described as septic emboli in patients with subacute bacterial endocarditis (SBE). Many physicians have considered them pathognomonic for this entity. More recent evidence suggests, however, that they are nonspecific and may be found in other disease states. A review of the pathogenesis of these white-centered hemorrhages will be provided, along with some diseases other than SBE in which they may occur.

Direct and indirect ophthalmoscopy for a more accurate baseline evaluation in aircrew members.

The currently required Federal Aviation Agency visual evaluation for commercial and airline pilots often does not detect quiescent retinal disease, unless there is a specific history or a current change in visual acuity which dictates the need for a dilated ophthalmoscopic evaluation. Statistics indicate that there may be a significant number of undetected retinal changes which can cause sudden and irreversible alterations in visual acuity during an airman's career. The requirements for an ophthalmoscopic examination should include, at the time of entry as an aircrew member into the aviation industry, a dilated fundus examination by the binocular indirect and direct ophthalmoscopic methods. In addition, documentary photography, visual fields, and other specific studies as indicated for these patients would be accomplished. These studies should be required by both the Federal Aviation Agency and the military services just as baseline ECG's chest films, SMA 12, and other laboratory studies are utilized.

In vivo cell tracking by scanning laser ophthalmoscopy: quantification of leukocyte kinetics.

PURPOSE: To image peripheral blood leukocyte traffic in the normal retinal and choroidal vasculature and to quantify the differences in the circulation dynamics between normal and concanavalin A (ConA)-activated leukocytes. METHODS: Normal or ConA-activated splenocytes were fluorescently labeled in vitro with 6-carboxyfluorescein diacetate (CFDA) and reinfused in vivo where they were tracked in the retinal and choroidal circulations of syngeneic rats by means of a scanning laser ophthalmoscope (SLO). Simultaneous digital and video images were captured for as long as 30 minutes, and the initial 15 seconds of image sequences and leukocyte dynamics were analyzed from digitized images by recording the velocity of trafficking cells and the number of stationary cells that accumulated with time, using a customized software package. RESULTS: Mean velocity (+/-SD) was 29.8 +/- 15.3 mm/sec in the retinal arteries, 14.7 +/- 7.2 mm/sec in the retinal veins, and 3.0 +/- 3.6 mm/sec in the retinal capillaries. Mean velocity in the choroidal vessels was 6.1 +/- 6.0 mm/sec. No significant difference in leukocyte velocity was found between activated and normal leukocytes in any of the vessel systems. However, activated leukocytes were observed to accumulate more within the choroidal vasculature (P < 0.001) and the retinal capillaries (P < 0.001) than in control animals, but not in larger retinal vessels. CONCLUSIONS: A technique to measure the kinetics of circulating leukocytes in vivo has been developed. Although leukocyte activation itself is insufficient to cause slowing of leukocyte velocity, the data indicate that leukocyte adherence to endothelium can be induced in the absence of local or systemic activating stimuli.

[Laser in situ keratomileusis (LASIK) and scanning laser ophthalmoscopy].

BACKGROUND: To determine the influence of Laser in situ keratomileusis (LASIK) on the measurements of retinal thickness and optic nerve head topography using the Retinal Thickness Analyzer (RTA). METHODS: RTA measurements were performed before and after LASIK. Forty-eight eyes of 25 healthy subjects were included. Mean age was 40.0+/-10.6 years. Mean preoperative refractive error (spherical equivalent) was -3.3+/-3.6 dpt, and 0.2+/-0.9dpt postoperatively. Correlation between ablation depth and duration and change of retinal thickness postoperatively was performed. All patients received a pachymetry preoperatively. RESULTS: Postoperatively, mean retinal nerve fiber layer thickness (MRNFL) and cross sectional area (RNFL cross section area) significantly decreased after LASIK (MRNFL preop: 0.18 mm, postop: 0.11 mm, P =0.026, RNFL cross section preop: 1.17 mm(2), postop: 0.71 mm(2), P =0.015). Ablation depth revealed a significant correlation with changes in retinal thickness measurements postoperatively (Delta MRNFL: Ablation depth, P =0.001, r=-0.5). Duration of the ablation was not significantly correlated to the MRNFL measurements postoperatively (P =0.27, r=-0.08). No correlation was found between the central corneal thickness and the change in retinal thickness after LASIK (P =0.51). CONCLUSION: Due to changes in corneal architecture after LASIK, measurements using RTA reveal a decrease in MRNFL and RNFL cross section area. These changes are likely artifacts. Further studies with a longer follow-up are desirable.
