Cigarette smoking during pregnancy and the occurrence of spontaneous abortion and congenital abnormality.

A multiple logistic regression analysis of 12,914 pregnancies and 10,523 live births, based on a mail survey of professional women in medicine, was carried out to determine the relationship between maternal cigarette smoking, and spontaneous abortion and congenital abnormality. After controlling for interfering variables (age, exposure to trace anesthetic gases, pregnancy history, and mailing response), a statistically significant increase in risk associated with maternal cigarette smoking was found for spontaneous abortions and congenital abnormalities. The risk of spontaneous abortion for the heavy smoker is estimated to be as much as 1.7 times that of the nonsmoker in certain risk groups. The risk for congenital abnormality for babies born of smoking mothers is estimated to be as much as 2.3 times that of the nonsmoker, depending on age, pregnancy history, and other factors.

[Quality of life at work among nursing professionals at surgical wards from the perspective of satisfaction].

This transversal and descriptive study aimed at evaluating the life quality of nursing professionals working at a surgical ward, using the Index of Work Satisfaction. Study participants were 105 workers; most of them women (82.9%), with an average age of 38. In this group, 69.5% were nursing auxiliaries and 11.4% were nurses. As to quality of life at work, scores varied between 114 and 227 (possible range of 44 to 308), with an average score of 169.7+/-25.9. The average score for the 44 items was 3.85, showing that workers were either dissatisfied or neither satisfied nor dissatisfied with the aspects mentioned in the instrument on quality of life at work. Satisfaction levels were lowest for remuneration and highest for the professional status domain. Authors found the instrument reliable for the study population, with a Cronbach's alpha of 0.81.

The operating room charge nurse: coordinator and communicator.

To achieve the potential inherent in the use of computer applications in distributed environments, we need to understand the information needs of users. The purpose of this descriptive study was to document the communication of an operating room charge nurse to inform the design of technological communication applications for operating room coordination. A data collection tool was developed to record: 1) the purpose of the communication, 2) mode of communication, 3) the target individual, and 4) the length of time taken for each occurrence. The chosen data collection categories provided a functional structure for data collection and analysis involving communication. Study findings are discussed within the context of application design.

Perioperative nursing education: the operating room as a learning climate.

Perioperative patient care presents a demand for registered nurses with competence, skills, and experience. Without a well-grounded educational program, the novice perioperative nurse is often overwhelmed at the learning demands of the perioperative practice setting. Settings in which invasive and other operative procedures are carried out present a variety of challenges in orienting the novice nurse. There must be educational resources, coaching and support for novices who have limited real-world operating room experience, and work environments that encourage and foster professional development and a learning culture. An orientation program that focuses on the tenets of learning rather than seeing and doing without understanding holds the most promise for both recruitment and retention of perioperative nurses.

Applying the nursing process to patient care in the operating room.

Using the nursing process in operating room nursing helps to assure personalized, continuous, and integrated care for the surgical patient. The steps in the nursing process clearly demonstrate the role and functions of the O.R. nurse as a professional member of the surgical team and as a component of the health care system. By this means the patient is assured of protection and quality care.
