Role of the argon beam coagulator in facial rejuvenation surgery.

OBJECTIVE: To compare the hemostatic effectiveness of a new electrosurgical unit, the argon beam coagulator, with current methods of electrocoagulation. DESIGN: A nonrandomized control trial of 20 patients undergoing rhytidectomy. SETTING: Two academic tertiary referral medical centers. PARTICIPANTS: Twenty male patients having moderate to major degrees of facial ptosis. INTERVENTION: All 20 patients underwent a standard rhytidectomy with hemostasis provided by the argon beam coagulator on the right side and conventional electrocoagulation on the left side. OUTCOME MEASURES: Improved hemostasis with minimal depth of injury, length of procedure, decreased blood loss, edema, and ecchymosis, and the ability to coagulate indiscriminately around and over neural tissue without damage (all listed in the literature as advantages of the argon beam coagulator). RESULTS: Poor hemostatic ability was observed with concomitant increased incidence of blood loss, edema and ecchymosis, major hematoma, and length of surgery. Also noted were increased rates of flap compromise and the potential for damage to vital neural structures. CONCLUSION: Our results do not agree with the list of advantages attributed to the argon beam coagulator in the literature.

Update on non-ablative light therapy for rejuvenation: a review.

BACKGROUND AND OBJECTIVES: Non-ablative technologies are playing an increasing role in the management of photoaging. Newer radiofrequency technologies have added to this therapeutic armamentarium. Shorter wavelength technologies are more effective in targeting pilosebaceous vascular and pigmentary alterations while longer wavelength technologies are most effective in wrinkle reduction mediated through dermal remodeling. An overview of the various technologies available to the practicing laser surgeon are outlined in the present review.

Facial photo rejuvenation using two different intense pulsed light (IPL) wavelength bands.

BACKGROUND AND OBJECTIVES: Intense pulsed light (IPL) systems are increasingly used for treatment of photo damaged skin. In the present study, we investigated the clinical efficacy and safety of two different wavelength bands generated by the same IPL device. STUDY DESIGN/MATERIALS AND METHODS: An IPL device was equipped with either a 555-950 nm filter (VL), or a 530-750 nm filter (PR). RESULTS: Fair, good or excellent clearance of visible telangiectasias was obtained in 81.8% of the patients (PR) and in 58.8% (VL). In the treatment of diffuse erythema, fair, good or excellent clearance was obtained in 72.7% (PR) and in 35.0% (VL). The PR filter was more efficient (P = 0.025) in reduction of diffuse erythema. The average number of treatments was 1.75 (PR) and 1.82 (VL). For the treatment of irregular pigmentation, fair, good or excellent clearance was obtained in 54.5% (PR) and in 61.9% (VL). Multiple treatments of irregular pigmentation were also evaluated. Using the VL filter more than two treatments did not induce further clinical improvement. The patients also scored their over-all satisfaction. Either fair, good or excellent results were reported by 66.7% (PR) and by 76.2% (VL). No skin atrophy, scarring or pigment disturbances were noted after the treatments. Swelling and erythema were registered by 2/3 (PR) and 1/3 (VL) of the patients. CONCLUSIONS: The two IPL wavelength bands were both found to be effective in the treatment of photo damaged facial skin. The clinical efficacy and safety of the two different treatment procedures were comparable to those reported in earlier studies, and finally treatment with these filter combinations required less than half the fluence, no active cooling and fewer treatments.

Minimally invasive skin rejuvenation with Erbium: YAG laser used in thermal mode.

BACKGROUND AND OBJECTIVES: To evaluate the efficacy and safety of a thermal mode Erbium:YAG laser several in-vivo morphological as well as clinical changes were monitored in a multi-center investigation. STUDY DESIGN/MATERIALS AND METHODS: An Erbium:YAG laser was used at a thermal mode with sub-ablative fluences of 2.1 and 3.1 J/cm(2) with parallel air cooling to treat either periorbital, perioral rhytides or patients with post-traumatic or acne scars. Two treatments were applied 2 months apart, with follow-up at 1, 3, 6, and 12 months post-treatment. Photographs were taken before and at each follow-up visit and evaluated by three blinded independent reviewers. Histology and immunohistochemistry for pro-collagen expression were investigated. Optical coherence tomography (OCT) was performed before, and at 4, 14, and 28 days after single pass treatment with Erbium:YAG thermal pulses. RESULTS: The improvement of rhytides at 1-3 months follow-up was graded as excellent in 19%, good in 19%, fair in 31%, and no improvement in 31%. At the 6- to 12-month follow-up, the improvement was excellent in 40%, good in 40%, fair in 20%, and no improvement in 0%. The improvement of scars at 3-6 months follow-up was graded as excellent in 50%, good in 25%, fair in 25%, and no improvement in 0%. Intra- and post-operative discomfort was described as mild by the patients. OCT, histological sections and immunohistochemistry demonstrated production of new collagen bundles. CONCLUSIONS: Thermal Erbium:YAG pulses can induce collagen neogenesis, as proved by temperature elevation and morphological changes in the upper dermis. This leads clinically to visible and long lasting reduction of wrinkles and scars after applying multiple passes with minimal side-effects.

Rejuvenating rice proteomics: facts, challenges, and visions.

Proteomics is progressing at an unprecedented pace, as can be exemplified by the progress in model organisms such as yeast, bacteria, and mammals. Proteomics research in plants, however, has not progressed at the same pace. Unscrambling of the genome sequences of the dicotyledoneous Arabidopsis thaliana (L.) and monocotyledoneous rice (Oryza sativa L.) plant species, respectively, has made them accessible reference organisms to study plant proteomics. Study of these two reference plants is expected to unravel the mystery of plant biology. Rice, a critically important food crop on the earth, has been termed a "cornerstone" and the "Rosetta stone" for functional genomics of cereal crops. Here, we look at the progress in unraveling rice proteomes and present the facts, challenges, and vision. The text is divided into two major parts: the first part presents the facts and the second part discusses the challenges and vision. The facts include the technology and its use in developing proteomes, which have been critically and constructively reviewed. The challenges and vision deal with the establishment of technologies to exhaustively investigate the protein components of a proteome, to generate high-resolution gel-based reference maps, and to give rice proteomics a functional dimension by studying PTMs and isolation of multiprotein complexes. Finally, we direct a vision on rice proteomics. This is our third review in series on rice proteomics, which aims to stimulate an objective discussion among rice researchers and to understand the necessity and impact of unraveling rice proteomes to their full potential.

Maharishi Amrit Kalash rejuvenates ageing central nervous system's antioxidant defence system: an in vivo study.

The oxygen-free radical involvement in various deteriorative processes and in ageing is unquestionably established. In the present study age-related changes in antioxidant enzyme activity in the different regions of CNS of 10-month and 32-month-old guinea pigs were studied. Maharishi Amrit Kalash has shown promise in inhibiting the in vitro and in vivo lipid peroxidation. Therefore in the present study the effect of MAK on the activity of antioxidant enzymes was checked. Our results indicate that the activity of superoxide dismutase and glutathione peroxidase, was found to be reduced (P < 0.05) in all the regions of CNS studied, The activities of catalase declined significantly only in the cerebral cortex, hypothalamus and the cerebellum. Whereas glutathione reductase activity declined in the cerebral cortex and hypothalamus. It is concluded that the age-related decline in the activities of antioxidant enzymes is region-specific as well as enzyme-specific. The endogenous lipid peroxide was found to be increased significantly (P < 0.05) in the 32-month-old animals. Whereas the lipid peroxidation after incubating the tissue homogenate in the air was found to be decreased (P < 0.05) in the older animals. The results indicate that the accumulation of lipid peroxides take place with age but the susceptibility of lipid peroxidation decreases in the older animals. The treatment of MAK 500 mg kg-1 body wt. for 2 months could augment the activities of antioxidant enzymes (P < 0.05). The effect of MAK was more pronounced in older than younger animals. It is concluded that the MAK can be used in compensating the decline in the activities of antioxidant enzymes in CNS and thereby it reduces the risks of lipid peroxidation.

Endoscopic facial rejuvenation: endoforehead, the functional lift. Case reports.

Endoscopic techniques have only very recently been applied to aesthetic plastic surgery procedures, especially in carpal tunnel release, in forehead plasties, and in breast augmentation operations. The author briefly outlines his experiences with endoscopic forehead lifts, first reported at surgical conferences in Los Angeles and Buenos Aires in 1992, and now totaling 61 cases performed in the 11 months just prior to the submission of this article. This endoscopic approach to forehead lifting has provided similar and comparable results to conventional coronal forehead lift operations, by means of small incisions made in the scalp area with minimal and fewer complications.

Bone expansion in facial rejuvenation.

Soft tissues and bone structures of the face are both affected by aging, but the bone is the more important element in the three-dimensional contour and suspension of the soft tissue. Bone expansion augments bone volume in the midface. A lamellar split osteotomy of the midface is used for bone expansion, and cranial bone grafts fill spaces created by the expansion. These grafts, which resorb less than onlay grafts located on the resorption zones, are placed in contact with the apposition zone. This technique changes the three-dimensional contour of the outer lamella of the midface while maintaining its morphology. Bone expansion restores bone mass lost to resorption. It is performed along with a facelift. Yielding a face with a younger appearance, not just a face with less wrinkles.

Endoscopic techniques in facial rejuvenation: an overview. Part I.

The use of endoscopic techniques in orthopedic, gynecological, and general surgery has had a significant effect on the traditional techniques in those specialties. The introduction of those techniques to plastic surgery, particularly to aesthetic surgery, has been very slow. However, recent interest in using endoscopic techniques in forehead plasty, corrugator-procerus resection, and breast augmentation has opened up countless possibilities in these and other areas of aesthetic and reconstructive surgery.

Different technical approaches for blepharoplasty in eyelid rejuvenation surgery.

The authors have reviewed their clinical experiences with 92 cases of blepharoplasties treated during the past 10 years and discuss the principal approaches used, depending on the age of the patient, the amount of adipose tissue present, the degree of lower lid tarsal and muscle laxity present, and the quality of the skin excess encountered. On the upper eyelid, skin excision is combined with an orbicular muscle strip and in older patients a suspension of the lateral canthus. On the lower eyelid the transconjunctival approach is emphasized in young and or colored patients, using an undermined musculocutaneous flap in the classical technique. A good choice between these different approaches avoids the rare complications of ectropion and scleral show because the procedure used is considered to be a variable "geometric" operation.

Rejuvenating health systems for aging communities.

Nowadays, about the half of Swiss women die after their 84th birthday. This unprecedented proportion of the population reaching an old age, or even a very old age (25% of women die after 89 years, and 5% after 95 years) is a novel aspect of human demographics, and represents the very last stage of the epidemiological transition, a term coined to describe the transformation of the prevailing health burden in the population, shifting from infectious and communicable pathologies to chronic and degenerative diseases. In developed countries, this epidemiological transition has been well documented during the last century; worldwide, a similar transition is taking place, with some countries still at mid or early stages of transition. A striking aspect of the current transition is its speed. In India, the mean duration of life since 1947 has increased from 32 to 62 years. As a result, India, like many other developing countries, is facing a double burden of disease, i.e., an upsurge of degenerative diseases while the burden from the old agenda (i.e., malaria, tuberculosis) still reaches devastating proportions in the population. This double burden is certainly a crucial problem in developing countries, and probably is the most important health challenge for the coming century. A similar accelerated pace of change is observed with the decline of mortality at old age. Worldwide, the current estimate of centenarians is 100000, i.e., ten time more centenarians than the number estimated in 1960. The downward trend in mortality, which is steeper with increasing age, is now the leading factor to Increase the life expectancy in developed countries. In the United Kingdom, life expectancy increased by 2.5 years between 1971 and 1991; this is equivalent to the increase observed between 1851 and 1961. This accelerated increase will influence public health in two different ways. The first will be the absolute increase in the number of older persons, with a corresponding increase in degenerative diseases. A second consequence will be the need for a substantial and rapid adaptation of the health care system. Three selected aspects are addressed below: 1) the increase of resources, 2) the improvement of performance, and 3) the reduction of demand through preventive strategies.
