Otoscleroma.

Otoscleroma is a new term used by us to describe the occurrence of scleroma in the middle ear. Although no cases have been reported in the literature, we describe here two cases.

Experimental scleroma. A histopathological study.

Klebsilla rhinoscleromatis suspension was experimentally inoculated into the nose and maxillary sinus of eight adult healthy rabbits. Two normal litter-mates served as controls. The animals were killed at varying intervals up to five months following the inoculation. Histopathological examination of the upper and lower respiratory tracts revealed a consistent pulmonary peribronchial and perivascular granulomatous reaction with a Mikulic cell-like appearance. The reaction was present in all the inoculated rabbits and increased in severity as the survival period was prolonged. The significance of these findings in the aetiopathogenesis of scleroma and to scleroma in humans is discussed.

Surgical management of cicatricial post-scleromatous sub-glottic stenosis.

In Egypt, sub-glottic stenosis is commonly caused by scleromatous affection. The countries in which scleroma is endemic have been reviewed. The incidence of scleroma, laryngeal scleroma, and sub-glottic affection, and their sex ratios have been discussed. Radiological study of the sub-glottic extension has been done. The operative procedures are detailed and the results are presented. The use of the Montgomery T-tube is an essential part of all these operations. The excision of scar tissue from the sub-glottic region was done in children, and this accords with recent trends. Twenty cases have been operated upon, with airway improvement in all of them.

Clofazimin in the treatment of scleroma.

Scleroma is a chronic progressive granulomatous disease predominantly affecting the mucous membrane of the upper respiratory tract, endemic in temperate and tropical zone countries including Egypt. Many patients in early stages respond to medical treatment while others do not. In these patients an antileprotic agent Clofazimin (Lamprene) was tried in 76 patients and the therapeutic results are very promising, especially for the early stages of the disease.

Laryngotracheal resection for post scleromatous laryngeal stenosis.

Scleroma is an endemic disease in Egypt, which frequently results in subglottic laryngeal stenosis. Six patients suffering from laryngoscleroma are represented. They all underwent laryngotracheal resection of the stenosed segment, followed by direct anastomosis. The results were satisfactory in four of the patients. Surgery was abandoned in one patient who was believed to suffer from perichondritis following previous surgery. Restenosis occurred in a 14-year-old girl two weeks after surgery. This was attributed to recurrence, possibly due to residual activity of the disease which was missed before surgery.

Scleroma of the upper respiratory passages: a CT study.

Scleroma is a chronic specific granulomatous disease endemic in Egypt. The nasal cavities were all affected in the 37 patients examined. The paranasal sinuses and nasopharynx were involved in 17 patients, while the larynx and upper trachea were affected in five patients, four of whom were females. Twenty-three patients were in the granulomatous stage; the rest of the patients were in the atrophic and fibrotic stages. The main nasal and nasopharyngeal CT findings were soft tissue masses of variable sizes. The lesions were characteristically homogenous, non-enhancing and had distinct edge definition; adjacent fascial planes were not invaded. The subglottic area was involved in laryngeal and tracheal scleroma. The lesions were mainly in the form of concentric irregular narrowing of the airway. In the trachea, crypt-like irregularities were diagnostic of scleroma. Intra-orbital, intra-cranial and infratemporal parapharyngeal scleromatous masses were detected in one patient.

Scleroma of the lower respiratory tract: case report and review of literature.

The case of a 46-year-old man who presented with acute respiratory distress due to a tracheal mass is reported. The patient underwent endoscopic resection of the mass and the histopathology revealed scleroma. No other sites of involvement by scleroma were found. This case is reported because laryngotracheal involvement by scleroma as the only manifestation of this disease is extremely rare.

Brush cytology and its comparison with histopathological examination in cases of diseases of the nose.

Nasal cytology is a simple, easy, bloodless and quite reliable investigation. Although its utility was recognized as early as 1927 by Eyerman, it has not yet gained much popularity. Here, an attempt is made to assess its efficacy for the diagnosis of various inflammatory, benign and malignant conditions in 151 cases of common rhinological problems in the M.Y. Group of Hospitals, Indore MP, India. This method can be used as an adjuvant to clinical diagnosis and histopathological diagnosis as an outpatient department (OPD) procedure with the help of a nasal endoscope, as it was found to be a simple, reliable and time-saving procedure for further management. It can also be used as a screening test for the detection of symptomless patients particularly in precancerous lesions.

Endoscopy of lower respiratory tract scleroma.

Eleven cases of scleroma with lower respiratory tract affection were examined by the flexible and rigid bronchoscopes. The patterns of laryngeal, tracheal and bronchial scleroma as seen through the flexible scope are presented. In the larynx, diffuse and localized forms are found. The trachea was affected by granular or atrophic lesions. Stenosis of the main bronchi was seen in two patients. The present-day treatment of lower respiratory tract scleroma is discussed.

Experimental intravenous inoculation of Klebsiella rhinoscleromatis bacilli in albino rats: a histopathological and bacteriological study.

Scleroma, chronic specific granuloma of the nose and upper respiratory tract, is endemic in Egypt and many other countries. The exact pathogenesis of the disease as regards the aetiological role of Klebsiella rhinoscleromatis is contradictory. This work investigated the effect of experimental intravenous injection of K. rhinoscleromatis in albino rats to demonstrate that the micro-organism can fulfil Koch's postulates. Micro-organisms were isolated from biopsy specimens taken from nasal lesions of 10 patients in the granulomatous stage of scleroma. Specimens were subjected to bacteriological and histopathological examinations to confirm the diagnosis. A 100 microl volume of freshly prepared bacterial inoculum containing 10(8) cfu/ml was injected weekly in the tail vein of each of 30 albino rats for 5 consecutive weeks. Biopsy specimens were taken from sacrificed animals and subjected to bacteriological and histopathological examinations. Positive histopathological diagnosis of scleroma was reported in the nose of 66.7% of rats, the larynx of 46.7%, the lungs of 26.7% and liver of 20% of rats. Bacteriological techniques were successful in revealing K. rhinoscleromatis from the nose of 36.7% of rats, the larynx of 30% and the lungs of 20% of rats. Various techniques were carried out to demonstrate the micro-organisms in tissue sections. Two histochemical stains for bacteria were employed: silver and Periodic Acid Schiff (PAS) stains. Immunoperoxidase technique using Klebsiella capsular type 3 antiserum was applied. It gave positive results in 66.7% of the 6 stained liver sections in spite of negative bacteriological cultures. The histiocytic nature of the Mikulicz cells was confirmed using alpha-1 antitrypsin, an immunohistochemical marker of histiocytes, and by studying the ultrastructural features of Mikulicz cells using the transmission electron microscope.
