Analysis of the epidemiological dynamics during the 1982-1983 epidemic of foot-and-mouth disease in Denmark based on molecular high-resolution strain identification.

An epidemic of foot-and-mouth disease (FMD) causing a total of 23 cases in 1982-1983, primarily on the island of Funen, Denmark, was subjected to molecular epidemiological investigations. In an attempt to exploit the quasi-species nature of foot-and-mouth disease virus strains for molecular high-resolution strain identification in order to analyse the dynamics of this epidemic, full-length VP1 coding regions were sequenced for 17 isolates collected at different farms during the epidemic. The sequence information together with epidemiological information gathered during the epidemic suggests that the epidemic was caused by at least three introductions across Danish borders and one case of airborne transmission between two islands in Denmark over a distance of 70 km. The assortment of nucleotide markers among the three strains is indicative of common recombination events in their evolutionary history, and the prerequisite of co- or superinfection of animals with variant strains in turn implies that they have a common source or epidemiologically related sources originating from an area with endemic FMD.

Anaesthesiological airway management in Denmark: assessment, equipment and documentation.

BACKGROUND: Failed intubation remains one cause of anaesthesia-related morbidity and mortality. In a recent survey in Denmark, 20% of respondents reported preventable mishaps in airway management. METHODS: Assessment of the airway, and its documentation, as well as the availability of various equipment to manage a difficult airway, and the existence of a failed intubation plan were surveyed by mailing a questionnaire to the clinical directors of all 69 anaesthesia departments in Denmark. RESULTS: Fifty-six departments (81%) returned the questionnaire. Pre-operative airway evaluation is performed in 90% of the departments. The tests included the mouth-opening test (77%), Mallampati score (48%), lower jaw protrusion (34%), neck mobility (63%), the measurement of the thyromental (11%) and sternomental distance (4%). The result of the tests are documented by 38% of the departments in the anaesthetic chart (96%), in the record (54%), on a card given to the patient (23%), in a letter sent to the patient's general practitioner (2%) or in a database (13%). The patients are personally informed in 82% of the departments. Only 54% of the departments have a failed intubation plan readily available. CONCLUSION: The preoperative assessment of the airways and its documentation is still unsatisfactory, as is communicating with the patient after a case of a difficult/impossible intubation. The adoption of internationally recognized recommendations might improve airway management and teaching to the best standard possible in the already well-equipped Danish anaesthetic departments.

Incidence of insulin-dependent diabetes mellitus (0--29 years at onset) in Denmark.

Incidence data from Denmark on the insulin-dependent diabetes mellitus in the age group 0--29 years have been collected in two different geographical areas (West and South Jutland plus Copenhagen and North Zealand). An incidence of 13.3 per 100,000 per year (range 12.5--13.6) was registered (N = 792). No difference was found between the two areas. The male incidence exceeded the female incidence by 25.4 per cent. The age distribution showed rising values until a peak in the early puberty with a decline until a rather constant level after puberty. From year to year a seasonal variation of onset was demonstrable with maximum in the winter and minimum in the summer for males only. Ascertainment was found to be between 87.7 and 98.6 per cent and the annual number of new insulin-dependent diabetics in Denmark in the age group 0--29 years can be calculated to 310-350.

Mortality from chronic interstitial nephritis and phenacetin consumption in Denmark.

After 1950, a pronounced increase in the mortality from chronic interstitial nephritis among women was registered in Denmark. This was suspected to be caused by a contemporary increase in the consumption of drugs containing phenacetin. Restrictions were therefore imposed on the sale of such drugs in 1961 and consumption decreased sharply. This report, based upon death certificates from 1941--76, shows that the mortality rate from chronic interstitial nephritis has decreased gradually among women in Denmark since 1960, and has now reached the same level in younger women as in the 1940's.

Alcohol and suicide in Denmark 1911-24--experiences from a 'natural experiment'.

In 1916-17, prices of alcoholic beverages increased dramatically in Denmark, and alcohol consumption decreased strongly. On the basis of this 'natural experiment', the effect of variations in per capita alcohol consumption on suicide rates is estimated, and compared to Norström's analysis of Danish data from the period 1931-80, as well as similar analyses from other countries. It is concluded that per capita alcohol consumption is probably related to the suicide rate in Denmark, but to a less extent than in some other countries.

A nationwide study of the incidence and 30-day mortality rate of pyogenic liver abscess in Denmark, 1977-2002.

BACKGROUND: Pyogenic liver abscess is a life-threatening disease. Accurate data on incidence and prognosis are important, but scarce. AIM: To examine changes in the incidence and 30-day mortality rate of patients with pyogenic liver abscess in Denmark. METHODS: Using nationwide administrative registers, we identified all patients diagnosed with pyogenic liver abscess in Denmark, 1977-2002, and their dates of death. We computed annual standardized incidence and 30-day mortality rates, and used Poisson regression to adjust gender-specific mortality rates for year-by-year differences in age at diagnosis. RESULTS: We identified 1448 patients with pyogenic liver abscess, of whom 54% were men. The crude incidence rate for the entire study period was 11.8 per 1,000,000 for men and 9.7 per 1,000,000 for women. Between 1977 and 2002, the incidence rate increased from 6 to 18 per 1,000,000 for men and from 8 to 12 per 1,000,000 for women. The cumulative 30-day mortality rate was 15% for men and 23% for women. The adjusted 30-day mortality rate decreased from 40% for men and 50% for women to around 10% for both genders. CONCLUSIONS: In this large nationwide study spanning a 26-year period, we found an increasing incidence rate and a decreasing mortality rate of pyogenic liver abscess. We believe that these changes are primarily explained by more sensitive diagnostic tools.

Iodine intake and incidence of thyroid cancer in Denmark.

OBJECTIVE: Iodine intake is suspected to be a risk factor for thyroid cancer. Eastern Denmark is characterized by mild and western Denmark by moderate iodine deficiency, and this difference is associated with a 50% difference in the occurrence of goitre and thyrotoxicosis. The objective of the study was to determine whether the incidence of thyroid cancer differs between these two regions, as any difference would have important safety implications for the national iodine supplementation programme. DESIGN AND METHODS: We studied all thyroid cancers notified to the Danish Cancer Registry in the period 1973-1997, focusing on the four most frequent subtypes: papillary, follicular, anaplastic and medullary thyroid cancer. A Poisson regression model was used with models of goodness-of-fit for age, period, sex. RESULTS: No regional difference was found in the overall incidence of follicular (0.3% 100 000 person-years) or papillary (0.7% 100 000 person-years) thyroid cancer. A slight but nonsignificant increase in total incidence, resulting mainly from a significant increase in the incidence for the papillary subtype, was observed in both regions. CONCLUSION: The results suggest that modest differences in iodine intake do not affect thyroid cancer incidence or the distribution of subtypes.

Register data on Assisted Reproductive Technology (ART) in Europe including a detailed description of ART in Denmark.

The European Society for Human Reproduction and Embryology (ESHRE) has monitored Assisted Reproduction in Europe since 1997. In 2001, 579 clinics from 23 countries reported 289,690 treatment cycles with: in vitro fertilization (IVF) 120,946, intracytoplasmatisk sperm injection (ICSI) 114,378, frozen embryo replacements (FER) 47,195 and egg donation (ED) 7,171. Intrauterine inseminations are less consistently reported, but in 2001, 15 countries reported 67,124 cycles (IUI-H 52 949 and IUI-D 14 185). Data from 2001 showed that in 12 countries, where all clinics reported the in vitro techniques to the register, 829 treatment cycles were performed per million inhabitants. The availability was highest in the Nordic countries (range 975-1,923 cycles). The proportion of infants born after ART with in vitro techniques ranged from 0.2% in Latvia to 3.9% in Denmark. In general, the impact of the in vitro techniques on the birth rate is highest in the Nordic countries (range 2.2-3.9%). Denmark is the only country with an established National ART reporting system involving all ART treatments both with the in vitro techniques and intrauterine inseminations. In the year 2002, a total of 20,837 treatment cycles were performed. The result was that 6.2% of all infants were born after assisted reproduction.

Increased prevalence of insulin-treated diabetes mellitus in Funen County, Denmark.

OBJECTIVE: To determine the prevalence of insulin-treated diabetes mellitus in Funen County, Denmark as of 1 July 1987 and compare the findings with those of a similar study from 1973. DESIGN: The population was identified by insulin prescriptions collected during a 1 year period. The identity of the prescription holders, the diagnosis and the date of diagnosis were verified from medical records and public registers. RESULTS: The estimated degree of ascertainment of the material is 0.95-0.97. The prevalence rates (per 1000 of the population) of insulin-treated diabetes mellitus as of 1 July 1987 are: 4.74 (95% Cl 4.66-4.82) for all cases, 2.08 (95% Cl 2.01-2.15) for the group with onset of diabetes before age 30 years and 2.66 (95% Cl 2.59-2.73) for the group with onset after age 30 years. These figures correspond to increases of 39, 26 and 51% (P < 0.001) on data collected in 1973. CONCLUSIONS: There was a significant increase in the prevalence of insulin-treated diabetes mellitus in Funen County, Denmark from 1973 to 1987. Analysis of the data shows that an important factor for this increase is a liability to start insulin treatment of type 2 (non-insulin-dependent) diabetes mellitus at an earlier stage than previously. Also, the data suggest that improved prognosis for diabetic patients as well as an increasing incidence of diabetes mellitus are factors involved in the increase.

Borrelia burgdorferi infection in Ixodes ricinus from habitats in Denmark.

A total of 2647 ticks of the genus Ixodes was sampled by flagging the vegetation in thirty-one sites in eastern Jutland, Denmark. All ticks were identified as Ixodes ricinus Linnaeus. A total of 317 ticks (202 nymphs and 115 adults) from three different sites were examined for the spirochaete Borrelia burgdorferi Johnson et al. by indirect fluorescent antibody staining; the frequency of infection varied from 7% to 22%. It is concluded that I. ricinus, known to be the most common tick in Denmark, is the vector largely responsible for the transmission of B. burgdorferi in this country.

Can subjects with a positive allergen skin test be selected by a short questionnaire? The Glostrup Allergy Study, Denmark.

The objective was to evaluate a postal questionnaire screening procedure for selection of subjects with positive reactions to skin prick tests with common allergens. The project consisted of a screening, with subsequent skin prick test of two selected groups. The setting was the Glostrup Population Studies institute in Copenhagen, Denmark. Participants in the screening included 8000 subjects, aged 15-69 years. The subjects were randomly selected from the population of western Copenhagen County, Denmark. From the 6998 respondents (87.5%), 793 subjects were randomly selected (Random Group), and 788 subjects were chosen on the basis of their answers to the questionnaire (Symptom Group). Both groups were invited to take skin prick tests. Attendance rates were 75.5% (Random Group) and 80.6% (Symptom Group). The main outcome measures were responses (yes or no) to the specific questions and the subjects' skin reaction (positive or negative). The association between symptoms and skin reactivity, adjusted for the effects of sex and age, was summarized by odds ratios. Symptoms on exposure to allergens were highly associated with positive skin reactivity. In the Symptom Group the percentage of subjects with at least one positive skin reaction was 57.7%, which was twice as much (28.4%) as in the Random Group. The results show that it was possible to select a group with high skin reactivity on the basis of the symptoms reported in the screening. Questions about exposure to allergens were the most appropriate for selection of this group.

Survival of rectal cancer patients in Denmark during 1994-99.

OBJECTIVE: As survival from rectal cancer in Denmark is below the European average, we analysed survival during the period of 1994-99 focusing upon improvement strategies. METHOD: All patients with a first-time rectal cancer were registered in a national database during this 5-year period. In the observational cohort study, data on patient age and gender, tumour stage, surgical procedures, adjuvant radiotherapy, anastomotic leakage, 30-day mortality and long-term survival were evaluated. RESULTS: The database comprised 5021 patients. Sixty-four percent had a localized tumour. Less than a third of patients with fixed tumours had pre-operative radiotherapy and curative surgery was achieved in 70%. Anastomotic leakage occurred in 13%, and 30-day mortality was 4% following abdominoperineal or anterior resection and 11% following a Hartmann's procedure. The relative 5-year survival in the entire series was 39% in males and 47% in females, respectively. Following curative surgery the relative 5-year survival was 55% in males and 63% in females, respectively. Survival was 71% in the subset of patients receiving curative total mesorectal excision. CONCLUSION: The average tumour stage upon diagnosis was probably more advanced compared to the other Nordic countries and pre-operative radiotherapy was administered to a minority of patients with fixed tumours. The anastomotic leakage rate was relatively high, whereas the 30-day mortality was comparable to other studies. Survival from rectal cancer in Denmark is still less favourable compared to the other Nordic and several European countries but improved from 1996 and onwards.

Public attitudes toward epilepsy in Denmark.

Social acceptance of persons with epilepsy very often constitutes a considerable problem for patients and their relatives. Nationwide opinion pools on the knowledge of and public attitude toward epilepsy have been taken in several countries, but never in Denmark. We report a Gallup survey of the general knowledge of and attitude toward epilepsy. A representative population of 1,500 persons aged greater than or equal to 15 years was selected in a four-state proportional sampling procedure. Ninety-seven percent of respondents had heard or read about epilepsy, 60% of these knew a person with epilepsy, and 50% had seen an epileptic seizure. The attitudes toward social acceptance and employment of persons with epilepsy were generally favorable, but 7% had objections to social contact between their children and persons with epilepsy in the playground and at school and 7% had objections to equal employment. Familiarity with persons with epilepsy was correlated to questions about attitudes and general knowledge of epilepsy. Such knowledge and public attitude in Denmark are mainly positive, but we believe that a continuous information campaign about epilepsy is essential, especially among youth.

Utilization of antiparkinson drugs in Norway, Sweden, Denmark and Finland 1975-1979.

Defined daily doses per day and 10(3) inhabitants [Formula: see text] of the different types of antiparkinson agents have been compared in the period 1975-1979 for Norway, Sweden, Denmark and Finland. A marked increase in the utilization of L-DOPA containing drugs has taken place especially in Sweden following the introduction of the "combined drugs" (L-DOPA + decarboxylase inhibitor). Statistically, nearly all patients with parkinsonism in Sweden have since 1977 been treated with L-DOPA, while in the other nordic countries about 10% of the patients are not on this therapy. These data are discussed in relation to the current concept of pharmacotherapy of parkinsonism. Further, many patients treated with neuroleptic drugs do in addition take anticholinergic agents. Drug utilization figures indicate this method of treatment to be most common in Denmark and Sweden.

Epidemiology of multiple sclerosis: incidence and prevalence rates in Denmark 1948-64 based on the Danish Multiple Sclerosis Registry.

The Danish Multiple Sclerosis Registry (DMSR) is a national register based upon the ethnically homogeneous Danish population of about 5 millions. The DMSR was founded in 1956 following a nationwide Danish prevalence survey of MS in 1949 and a continuous registration of incident cases of MS since January 1, 1948. Included in DMSR are all Danish cases of MS (or suspicion of MS) diagnosed by a neurologist or a department of neurology. The sources of notification are the 22 neurological departments in Denmark, the National Patient Registry, the neuropathological departments, the Registry of Causes of Death, and, up to 1975, the Disablement Insurance Court. Notified cases which do not comply with the standardized diagnostic criteria of the DMSR are excluded. An estimate of the completeness of the DMSR is 90-95% and the validity is around 94%. Age- and sex-specific incidence rates of MS for the interval 1948-64 are presented. The crude annual incidence rate of MS in Denmark was in the year 1948-64 4.42 per 100,000 population, 22% higher in females than males. There was a significant geographical variation of incidence rates and a significant downward trend in incidence rates during the interval, whereas the prevalence rates showed a slight increase.

Trends in utilization of antiepileptic drugs in Denmark.

OBJECTIVES: To describe utilization of antiepileptic drugs (AEDs) in Denmark during 1993-2002, with special emphasis on oxcarbazepine, and to assess probable indications for AED use. MATERIALS AND METHODS: We retrieved prescription data from Odense University Pharmacoepidemiological Database, in Funen County, Denmark (population in 2002: 472,869). Within each calendar year we estimated period prevalence, incidence rate and monotherapy rate. Based on co-medication we defined 'epilepsy' when only AEDs were prescribed, 'pain' with co-prescription of opioids, and 'mood disorder' with co-prescription of antipsychotics or antidepressants. RESULTS: We identified 15,604 AED users. The prevalence of using AED increased from 9.3 (95% CI, 9-9.5) to 12.1 (11.8-12.4)/1000 persons. The incidence rate increased from 1.4 (1.3-1.6) to 1.7 (1.6-1.9)/1000 personyears. The monotherapy rate was 79-82%. AED use for 'epilepsy' declined by 19.7%, whereas the proportion of 'pain' and 'mood disorder' treatment increased by 11.2% and 8.4% respectively. CONCLUSIONS: Antiepileptic drug utilization increased during the study period, the increase probably caused by expanding use in areas other than epilepsy.

Screening for diabetic retinopathy in Denmark: the current status.

PURPOSE: To investigate the current status of screening for diabetic retinopathy in Denmark, focussing on organization, methods of screening and regional differences. METHODS: A questionnaire was sent out in 2002 to 14 departments of ophthalmology and one ophthalmology practice, covering all 15 counties in Denmark. RESULTS: Six counties reported having systematic screening, defined as organized screening, including a database; three reported having plans for systematic screening; two reported having undetermined plans for systematic screening and four reported having no plans for systematic screening. In counties with systematic screening, both the organization and the method of screening varied. CONCLUSION: Approximately 43% of Danish patients with diabetes currently live in a county without systematic screening for diabetic retinopathy. Should all counties contemplating systematic screening implement their plans, this proportion will be reduced to approximately 17%.

Suicide and social integration. A pilot study of the integration levels in Norway and Denmark.

A model for comparison of levels of social integration in each of the Nordic countries was developed from Durkheim's theories and then tested on Norwegian and Danish data to determine whether the difference in the frequency of suicide between Norway and Denmark could be "explained" by a corresponding difference in degree of social integration. The results confirmed Durkheim's theory in that the general level of social integration was found to be considerably lower in Denmark than in Norway. Danish middleaged women, whose rate of suicide is more than three times that of their Norwegian counterparts, were especially poorly integrated by comparison. Furthermore, the level of integration among young Norwegian men was found to be in marked decline and their suicide rate on the increase. The necessity of testing and refining the method is stressed.
