[Suicidal Tachmalcor poisoning--a case report].

In a case report, a Tachmalcor intoxication with a dose of 18 mg/kg body weight is described. This dose caused a ventricular flutter in the patient which lasted for a total of 10 hours, despite intensive treatment. The treatment began approximately three hours after the intoxication and concentrated on therapy of the ventricular tachycardia. The use of Xylocitin 2%, defibrillation, glucagon and sodium chloride is recommended with such symptoms. Additionally, we used hemoperfusion for drug elimination. Despite the cardiac rhythm disorder of such duration, no neurological deficiencies were observed in the patient. Intoxications caused by these drugs in normal intensive therapies are extremely rare and for this reason treatment can often be very problematic. The following article reports on the successful therapy of one such patient.

[Acute lung injury syndrome and hemodiafiltration].

Acute lung injury syndrome is one of the most frequent components of polyorgan failure, particularly resultant from sepsis. Replacing renal therapy can correct uremic disorders in the hemostasis and gaseous exchange disorders in the lungs by eliminating excessive fluid in case of hyperhydration and by eliminating inflammation mediators. The present study demonstrated a favorable impact of hemodiafiltration on ventilation oxygen status of patients with acute lung injuries, which was confirmed by positive changes in blood oxygen pressure, intrapulmonary shunt, alveolar-arterial oxygen difference, etc. Changes in oxygen transport corresponded to those in patients with sepsis and shock without hemodiafiltration.

[Pulmonary hemosiderosis. Hemodynamic or idiopathic origin? Diagnostic contribution of imaging and review of the literature].

We report two cases of Pulmonary Hemosiderosis. The first patient (adult male) had an angiosarcoma of the left auricle and developed pulmonary arterial hypertension, was treated by surgery and chemotherapy. The other patient (adult female) suffered from Idiopathic Pulmonary Hemosiderosis (IPH) and had been treated by corticosteroids for fifteen years, with one acute episode during this period. We discuss the etiological diagnosis and the contribution of chest radiographs, CT-scan and MR Imaging in this rare illness according to literature. IPH occurs mostly in childhood with acute and fatal issue, whereas presents as a chronic form in adults. Assessment of IPH requires large investigations to eliminate hemodynamic, hemato-, immuno-, carcinological disorders, and infectious agents as well. Review.

[Use of thymic peptide thymalin in the complex treatment of therapy-resistant schizophrenia].

The thymic peptide thymaline combined with psychotropic drugs was used in the treatment of 36 therapeutically resistant patients with pronounced immune abnormalities. Such a policy favoured considerable enhancement of the treatment efficacy which manifested itself by the elimination or amelioration of psychic disorders, appreciable activation of patients and reduction of neuroleptic complications. The positive dynamics in the patients' status always correlated with the normalization of immune abnormalities, with this being seen to a greater degree in cases of the secondary "pharmacogenic" resistance and resistance because of the "pathological ground". On the one hand, the studies demonstrated wide potentialities of the use of thymaline in the treatment of the resistant patterns of schizophrenia. On the other hand, they showed the heterogeneity of immune abnormalities in patients with different varieties of resistance.

Long-chain aliphatic fatty acids and phytanic acid simultaneously measured by dual-column capillary chromatography.

A method is described for simultaneously measuring long-chain aliphatic fatty acids and phytanic acid in plasma, for the diagnosis of disorders associated with peroxisomal defects, eliminating the need for two separate chromatographic runs to resolve all of the analytes. In addition, the simultaneous, dual-column analysis allows confirmation of the identity and quantity of the individual fatty acids, thus adding a degree of certainty when results are used for interpretation and differential diagnosis of the many clinical manifestations of peroxisomal defects.

[Principles of conservative therapy of peripheral and central disorders of equilibrium].

Disturbance of equilibrium is a vestibular induced disturbance of orientation in space that is perceived subjectively as vertigo. Change of behaviour and specific medication is the optimal causal therapy, but symptomatic therapy is preferred in practice, using drugs that suppress different input activities or the activity of central vestibular structures. In this way a disordered flow of information is eliminated and the equilibrium is restored.

[Pseudocoronary syndrome in reflux esophagitis].

The authors have shown that among 173 patients with the insufficient closing function of the cardia there were 18 patients with pseudo-coronary syndrome related with reflux-esophagitis. Operations were performed on 14 of them with good effects. The surgical correction of insufficiency of cardia functions in most cases gives elimination of cardio-vascular disorders resulting from reflux-esophagitis.

Eruptive vellus hair cysts.

Umbilicated and nonumbilicated papules were present on the presternal area of a patient with eruptive vellus hair cysts. Histopathological examination of umbilicated papules showed penetration of vellus hairs through cyst walls, surrounding granulomatous foreign body reaction, and communication to the surface of the skin with released vellus hairs present in the pore. This suggests that resolution of some of the cysts in this disorder may occur by transepithelial elimination.

Light and ultrastructural study of reactive perforating collagenosis.

In 1967, Mehregan et al described a new and distinct clinicopathologic entity that they called reactive perforating collagenosis (RPC). This rare disease is classified with a group of disorders that involve the transepithelial elimination of histochemically altered dermal tissue. In RPC, minor trauma such as an insect bite, scratch, or pilosebaceous infection alters the collagen fibers in the papillary dermis. The histochemically altered but ultrastructurally intact collagen elicits both a dermal and epithelial response. For the case presented, it is our purpose to demonstrate transepithelial elimination of collagen fibers by ultrastructural study, thereby supporting the recognized histochemical observations.

Progressive dementia: strategies to manage new problem behaviors.

The primary care physician plays a key role in the diagnosis and treatment of progressive dementia and in coordinating supportive care with the patient's caregivers. When a patient exhibits a new symptom such as wandering or verbal abuse, your initial workup should eliminate acute medical illness, psychiatric disorders, and drug reactions that may present as behavioral symptoms. In the home, the main focus of care is to provide a safe and secure environment, using preventive measures to minimize the risk of injuries. When caregivers become exhausted from dealing with nocturnal wakenings, a short course of sleep medication for the dementia patient is often necessary. To improve communication, avoid stressful and confusing situations that compound the patient's difficulty in understanding messages or expressing thoughts.

Age factor in human alveolar bone repair.

This was a retrospective review study based on histological and histochemical observations of 69 biopsy specimens of human extraction wounds, screened for elimination of medical or nutritional disorders that might have interfered with normal healing. Initially, for the first 10 post-extraction days, tissues from the younger individuals (second decade or less) and those from older individuals (sixth decade or over) appeared to heal at the same rate. After 10 post-extraction days, the younger tissues continued with progressive healing and osteoid formation, while the older tissues remained in a resting (lag) phase. Following approximately three weeks, the older tissues entered into an accelerated growth cycle, and by 30 to 40 days equaled the healing pattern of the younger tissues, describing a sigma-type curve.

[Mega-dolicho carotid arteries. Role of non-invasive imaging. Apropos of 3 clinical cases].

Three cases of aberrant (elongated and tortuous) carotid arteries of the neck are reported. In 2 patients the carotid artery abnormalities were discovered at diagnostic work-up for: 1) sudden deafness, 2) persistent pharyngeal discomfort. Both patients presented a pulsating bulge in a pharyngeal wall with no lesion of the mucosa. In the third patient, the arterial abnormality was a peroperative discovery. All patients presented factors of risk for vascular disease. In the 2 symptomatic patients, Doppler U.S. and magnetic resonance imaging made it possible to confirm the diagnosis and to eliminate other vascular or tumoral disorders. The indications and limitations of the various available diagnosis imaging procedures are assessed.

[The clinico-pathogenetic characteristics of neurosis-like states in the participants in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station].

A clinical and paraclinical examination was done in 120 individuals with neurosis-like disorders, who had taken part in the elimination of the aftermath of the Chernobyl Nuclear Power Plant breakdown. A complex of etiological factors in the disorder has been identified. The most important syndrome was cerebral asthenia associated with depressive disorders and apparent vegetovascular dysfunction. Cerebral influences were implicated in the origination and development of neurosis-like disorders, with functional inadequacies having been disclosed in the patients' bodily catecholamine and immune systems.

The pharmacotherapy of panic disorder.

Panic disorder is common and associated with significant morbidity and dysfunction. The pharmacologic treatment of panic disorder is aimed at reducing or eliminating panic attacks, avoidance behavior, anticipatory anxiety, and comorbid conditions--and substantially improving and normalizing overall function and quality of life. Antidepressants and benzodiazepines remain the current mainstays of pharmacotherapy for panic disorder, although other novel agents and strategies are becoming available and may add effective alternatives to the therapeutic armamentarium.

Granuloma annulare with transfollicular perforation.

In recent years the phenomenon of transepithelial elimination has been found to be the underlying mechanism in a variety of skin disorders. "Perforating dermatoses" may show transepidermal or transfollicular elimination or a combination of the two. A case of papular granuloma annulare is reported, where in one lesion the pilary apparatus actively participated in the elimination of necrobiotic collagen from the dermis. To our knowledge, perforating granuloma annulare with a transfollicular pathway has not previously been described. Also, a classification of perforating dermatoses is presented.

Clonality analysis of hematopoiesis and thrombopoietin levels in patients with essential thrombocythemia.

Essential thrombocythemia (ET) is a myeloproliferative disorder, characterized by sustained thrombocytosis. Diagnosis requires the elimination of all known causes of thrombocytosis. ET is believed to be a clonal disorder, and we investigated the frequency of a clonal hematopoiesis in this disease with the aim of using this as a positive diagnostic criterion. However, a non-random inactivation pattern can be encountered in normal females which mimics clonal hematopoiesis. In addition, the percentage of normal females with skewed lyonization seems higher using techniques based on the difference in DNA methylation, compared to G6PD enzyme polymorphism. Recently, new techniques based on transcript analysis have been developed. We report here the results of clonality studies of hematopoiesis in 53 ET patients using two different techniques based on DNA and RNA polymorphisms, and T-lymphocytes as a control tissue of lyonization. The majority of ET patients showed monoclonal hematopoiesis in the presence of polyclonality of T-lymphocytes. Because all ET patients did not show the same clonal pattern of hematopoiesis, we searched for inappropriate secretion of thrombopoietin (TPO) in patients with polyclonal disease. This assay was performed in 48 patients, of whom 9 showed polyclonal hematopoiesis and 27 monoclonal hematopoiesis. We found no difference in TPO levels between ET patients and normal controls, nor between patients with polyclonal hematopoiesis and those with monoclonal hematopoiesis. Our results confirm the high frequency of monoclonal hematopoiesis in ET, the usefulness of RNA markers, and the possibility of using T-lymphocytes as a control tissue for X-chromosome inactivation patterns. On the other hand, TPO levels are not decreased even in ET patients with high platelet counts, suggesting an increased production or decreased clearance of TPO in this disease.

Perforating disorder caused by salt-water application and its experimental induction.

BACKGROUND: Perforating disorders are uncommon diseases characterized by transepidermal elimination histopathologically and include reactive perforating collagenosis, elastosis perforans serpiginosa, Kyrle's disease and perforating folliculitis. In addition, perforating disorders can develop in patients with diabetes mellitus, renal failure and even by accidental exposure of calcium salts. METHODS: We report two cases of perforating disorder caused by chemical burn with commercially available salt-water application for self-treatment of chronic dermatitis or pruritus. RESULTS: The commercially used salt water for making bean curd was analyzed and it consisted of calcium and other salts without harmful heavy metals. We induced a similar phenomenon by experimental application of commercial salt water on guinea pigs. CONCLUSION: Bean curd is used as a food commonly in the Far-East, allowing a greater chance of exposure to salt water accidentally or occupationally. Bean curd is becoming more popular even in Western countries. To confirm causation, we induced a similar phenomenon in guinea pigs by experimental application of commercial salt water.
